Clustering of Risk Factors: A Simple Method of Detecting Cardiovascular Disease in Youth

Cardiovascular risk factors predict the development of premature atherosclerosis. As the number of risk factors increases, so does the extent of these lesions. Assessment of cardiovascular risk factors is an accepted practice in adults but is not used in pediatrics.

In this study, the authors discuss how the presence of ≥2 cardiovascular risk factors is associated with vascular changes in adolescents. The findings were compared with the Patholobiological Determinants of Atherosclerosis in Youth risk score to demonstrate that a simple method of clustering is a reliable tool to use in clinical practice. Serotonin Transporter Role in Identifying Similarities Between SIDS and Idiopathic ALTE

Literature about polymorphic expression of an apparent life-threatening event (ALTE), particularly that concerning discrimination between ALTE with evident cause and idiopathic ALTE, is scarce. Relationships between SIDS and ALTEs have been supposed but data are still controversial and no genetic data are available.

Genetic analysis (5HTT and MAOA) on ALTEs and idiopathic ALTEs discriminated the 2 syndromes and found a link between the idiopathic form and SIDS. Consequently, we hypothesized that the 2 latter syndromes could be different phenotype expressions of a common genetic base. (Read the full article)

Interns' Success With Clinical Procedures in Infants After Simulation Training

Pediatric training programs use simulation for procedural skills training. Research demonstrates student satisfaction with simulation training, improved confidence, and improved skills when retested on a simulator. Few studies, however, have investigated the clinical impact of simulation education.

This is the first multicenter, randomized trial to evaluate the impact of simulation-based mastery learning on clinical procedural performance in pediatrics. A single simulation-based training session was not sufficient to improve interns’ clinical procedural performance. (Read the full article)

Use of a Metronome in Cardiopulmonary Resuscitation: A Simulation Study

The frequency of cardiac arrest is significantly lower in children than in adults, rendering the delivery of high-quality cardiopulmonary resuscitation more difficult. Metronome-based studies in adults showed improvement in adequate compression rate, with a detrimental effect on the depth of chest compressions.

This is the first pediatric study to confirm that the use of a metronome during cardiopulmonary resuscitation significantly improves the delivery of adequate rate without affecting the compression depth. This effect was more prominent among medical students and pediatric residents and fellows than nurses. (Read the full article)

Novel peptide from commensal Staphylococcus simulans blocks MRSA quorum sensing and protects host skin from damage [Mechanisms of Action]

Recent studies highlight the abundance of commensal coagulase-negative staphylococci (CoNS) on healthy skin. Evidence suggests that CoNS actively shape the skin immunological and microbial milieu to resist colonization or infection by opportunistic pathogens, including methicillin resistant Staphylococcus aureus (MRSA), in a variety of mechanisms collectively termed colonization resistance. One potential colonization resistance mechanism is the application of quorum sensing, also called the Accessory Gene Regulator (agr) system, which is ubiquitous among staphylococci. Common and rare CoNS make autoinducing peptides (AIPs) that function as MRSA agr inhibitors, protecting the host from invasive infection. In a screen of CoNS spent media we found that Staphylococcus simulans, a rare human skin colonizer and frequent livestock colonizer, released potent inhibitors of all classes of MRSA agr signaling. We identified three S. simulans agr classes, and have shown intraspecies cross-talk between non-cognate S. simulans agr types for the first time. The S. simulans AIP-I structure was confirmed, and the novel AIP-II and AIP-III structures were solved via mass spectrometry. Synthetic S. simulans AIPs inhibited MRSA agr signaling with nanomolar potency. S. simulans in competition with MRSA reduced dermonecrotic and epicutaneous skin injury in murine models. Addition of synthetic AIP-I also effectively reduced MRSA dermonecrosis and epicutaneous skin injury in murine models. These results demonstrate potent anti-MRSA quorum sensing inhibition by a rare human skin commensal, and suggest that cross-talk between CoNS and MRSA may be important in maintaining healthy skin homeostasis and preventing MRSA skin damage during colonization or acute infection.

The ODD Protocol for Describing Agent-Based and Other Simulation Models: A Second Update to Improve Clarity, Replication, and Structural Realism

Volker Grimm, Steven F. Railsback, Christian E. Vincenot, Uta Berger, Cara Gallagher, Donald L. DeAngelis, Bruce Edmonds, Jiaqi Ge, Jarl Giske, Jürgen Groeneveld, Alice S.A. Johnston, Alexander Milles, Jacob Nabe-Nielsen, J. Gareth Polhill, Viktoriia Radchuk, Marie-Sophie Rohwäder, Richard A. Stillman, Jan C. Thiele and Daniel Ayllón: The Overview, Design concepts and Details (ODD) protocol for describing Individual- and Agent-Based Models (ABMs) is now widely accepted and used to document such models in journal articles. As a standardized document for providing a consistent, logical and readable account of the structure and dynamics of ABMs, some research groups also find it useful as a workflow for model design. Even so, there are still limitations to ODD that obstruct its more widespread adoption. Such limitations are discussed and addressed in this paper: the limited availability of guidance on how to use ODD; the length of ODD documents; limitations of ODD for highly complex models; lack of sufficient details of many ODDs to enable reimplementation without access to the model code; and the lack of provision for sections in the document structure covering model design rationale, the model’s underlying narrative, and the means by which the model’s fitness for purpose is evaluated. We document the steps we have taken to provide better guidance on: structuring complex ODDs and an ODD summary for inclusion in a journal article (with full details in supplementary material; Table 1); using ODD to point readers to relevant sections of the model code; update the document structure to include sections on model rationale and evaluation. We also further advocate the need for standard descriptions of simulation experiments and argue that ODD can in principle be used for any type of simulation model. Thereby ODD would provide a lingua franca for simulation modelling.

Verification of the Lane Adapter FSM of a USB4 Router Design Is Not Simple

Verifying lane adapter state machine in a router design is quite an involved task and needs verification from several aspects including that for its link training functionality.

The diagram below shows two lane adapters connected to each other and each going through the link training process. Each training sub-state transition is contingent on conditions for both transmission and reception of relevant ordered sets needed for a transition. Until conditions for both are satisfied an adapter cannot transition to the next training sub-state.

As deduced from the lane adapter state machine section of USB4 specification, the reception condition for the next training sub-state transition is less strict than that of the transmission condition. For ex., for LOCK1 to LOCK2 transition, the reception condition requires only two SLOS symbols in a row being detected, while the transmission condition requires at least four complete SLOS1 ordered sets to be sent.

From the above conditions in the specification, it is a possibility that a lane adapter A may detect the two SLOS or TS ordered sets, being sent by the lane adapter B on the other end, in the very beginning as soon as it starts transmitting its own SLOS or TS ordered sets. On the other hand, it is also a possibility that these SLOS or TS ordered sets are not yet detected by lane adapter A even when it has met the condition of sending minimum number of SLOS or TS ordered sets.

In such a case, lane adapter A, even though it has satisfied the transmission condition cannot transition to the next sub-state because the reception condition is not yet met. Hence lane adapter A must first wait for the required number of ordered sets to be detected by it before it can go to the next sub-state. But this wait cannot be endless as there are timeouts defined in the specification, after which the training process may be re-attempted.

This interlocked way of operation also ensures that state machine of a lane adapter does not go out of sync with that of the other lane adapter. Such type of scenarios can occur whenever lane adapter state machine transitions to the training state from other states.

Cadence has a mature Verification IP solution for the verification of various aspects of the logical layer of a USB4 router design, with verification capabilities provided to do a comprehensive verification of it.
