This systematic review and meta-analysis aims to quantify lung function in preterm-born children and identify risk factors for a compromised lung function. Methods We searched MEDLINE, Embase, Cochrane Library, Web of Science and Scopus for relevant studies published on preterm cohorts born since 1990. Studies comparing forced expiratory volume in 1 s (FEV1) in preterm-born children aged ≥5 years to term-born controls or normative data were included. Study quality was assessed using the Newcastle–Ottawa Scale for cohort studies. Standardised mean differences in FEV1 and secondary spirometry outcomes per study were pooled using meta-analysis. The impact of different demographic and neonatal variables on studies’ FEV1 effect sizes was investigated by meta-regression analyses. Certainty of evidence was assessed using the Grading of Recommendations, Assessment, Development, and Evaluations framework. Results We identified 42 studies with unique cohorts including 4743 preterm children and 9843 controls. Median gestational age in the studies was 28.0 weeks and age at assessment ranged from 6.7 to 16.7 years. Preterm children had lower FEV1 than controls (–0.58 sd, 95% CI –0.69– –0.47 sd, p<0.001) resulting in a relative risk of 2.9 (95% CI 2.4–3.4) for abnormal outcome, with high certainty of evidence. FEV1 was significantly associated with gestational age, birthweight, bronchopulmonary dysplasia and invasive mechanical ventilation in univariate meta-regression analyses (R2=36–96%). Conclusion This systematic review shows robust evidence of impaired lung function in preterm-born children with a high certainty of evidence. Full Article

Interchromosomal (trans) contacts have received much less attention, and tools for interrogating potential biologically relevant trans structures are lacking. Here, we develop a computational framework that uses Hi-C data to identify sets of loci that jointly interact in trans. This method, trans-C, initiates probabilistic random walks with restarts from a set of seed loci to traverse an input Hi-C contact network, thereby identifying sets of trans-contacting loci. We validate trans-C in three increasingly complex models of established trans contacts: the Plasmodium falciparum var genes, the mouse olfactory receptor "Greek islands," and the human RBM20 cardiac splicing factory. We then apply trans-C to systematically test the hypothesis that genes coregulated by the same trans-acting element (i.e., a transcription or splicing factor) colocalize in three dimensions to form "RNA factories" that maximize the efficiency and accuracy of RNA biogenesis. We find that many loci with multiple binding sites of the same DNA-binding proteins interact with one another in trans, especially those bound by factors with intrinsically disordered domains. Similarly, clustered binding of a subset of RNA-binding proteins correlates with trans interaction of the encoding loci. We observe that these trans-interacting loci are close to nuclear speckles. These findings support the existence of trans-interacting chromatin domains (TIDs) driven by RNA biogenesis. Trans-C provides an efficient computational framework for studying these and other types of trans interactions, empowering studies of a poorly understood aspect of genome architecture. Full Article

Several human CRX variants cause degenerative retinopathies, but most are variants of uncertain significance. We performed a deep mutational scan (DMS) of nearly all possible single amino acid substitutions in CRX using a cell-based transcriptional reporter assay, curating a high-confidence list of nearly 2000 variants with altered transcriptional activity. In the structured homeodomain, activity scores closely aligned to a predicted structure and demonstrated position-specific constraints on amino acid substitution. In contrast, the intrinsically disordered transcriptional effector domain displayed a qualitatively different pattern of substitution effects, following compositional constraints without specific residue position requirements in the peptide chain. These compositional constraints were consistent with the acidic exposure model of transcriptional activation. We evaluated the performance of the DMS assay as a clinical variant classification tool using gold-standard classified human variants from ClinVar, identifying pathogenic variants with high specificity and moderate sensitivity. That this performance could be achieved using a synthetic reporter assay in a foreign cell type, even for a highly cell type-specific TF like CRX, suggests that this approach shows promise for DMS of other TFs that function in cell types that are not easily accessible. The aim of this study was to ascertain the beliefs of academic pulmonary and critical care providers regarding the use of race as a variable in spirometry prediction equations.METHODS:We report data from 151 open-ended responses from a voluntary, nationwide survey (distributed by the Association of Pulmonary Critical Care Medicine Program Directors) of academic pulmonary and critical care providers regarding the use of race in PFT prediction equations. Responses were coded using inductive and deductive methods, and a thematic content analysis was conducted.RESULTS:There was a balanced distribution of opinions among respondents supporting, opposing, or being unsure about the incorporation of race in spirometry prediction equations. Responses demonstrated a wide array of understanding related to the concept and definition of race and its relationship to physiology.CONCLUSIONS:There was no consensus among providers regarding the use of race in spirometry prediction equations. Concepts of race having biologic implications persist among pulmonary providers and will likely affect the uptake of the Global Lung Function Initiative per the American Thoracic Society guidelines. OLX702A-075-16 is a novel N-acetylgalactosamine conjugated asymmetric small-interfering RNA (GalNAc-asiRNA) used for the treatment of an undisclosed liver disease. Its unique 16/21-mer asymmetric structure reduces nonspecific off-target effects without compromising efficacy. We investigated the plasma concentration, tissue distribution, metabolism, and renal excretion of OLX702A-075-16 following a subcutaneous administration in mice and rats. For bioanalysis, high-performance liquid chromatography with fluorescence detection was used. The results showed rapid clearance from plasma (0.5 to 1.5 hours of half-life) and predominant distribution to the liver and/or kidney. Less than 1% of the liver concentration of OLX702A-075-16 was detected in the other tissues. Metabolite profiling using liquid chromatography coupled with high-resolution mass spectrometry revealed that the intact duplex OLX702A-075-16 was the major compound in plasma. The GalNAc moiety was predominantly metabolized from the sense strand in the liver, with the unconjugated sense strand of OLX702A-075-16 accounting for more than 95% of the total exposure in the rat liver. Meanwhile, the antisense strand was metabolized by the sequential loss of nucleotides from the 3'-terminus by exonuclease, with the rat liver samples yielding the most diverse truncated forms of metabolites. Urinary excretion over 96 hours was less than 1% of the administered dose in rats. High plasma protein binding of OLX702A-075-16 likely inhibited its clearance through renal filtration. SIGNIFICANCE STATEMENT This study presents the first comprehensive characterization of the in vivo pharmacokinetics of GalNAc-asiRNA. The pharmacokinetic insights gained from this research will aid in understanding toxicology and efficacy, optimizing delivery platforms, and improving the predictive power of preclinical species data for human applications. We present the experiences and lessons learned from implementing the no-scalpel vasectomy (NSV) program in Davao City, showcasing its potential for increasing male engagement in family planning decisions. Launched in 2008, the program aimed to address gender disparity by promoting NSV as a safe and effective contraceptive alternative to female-centric methods. Through the use of culturally sensitive information campaigns and couple-focused counseling, the program challenged traditional notions of masculinity and encouraged shared decision-making. Strong local government commitment and public-private partnerships played key roles in driving the program’s success. Results showed an average annual increase of 80% in NSV clients over the past 3 years compared to before the COVID-19 pandemic, underscoring its effectiveness. The program presents a compelling intervention model for similar initiatives, highlighting how overcoming cultural barriers, infrastructure limitations, and budgetary constraints through policy advocacy, strategic partnerships, and tailored approaches can significantly boost male involvement in family planning and improve reproductive health outcomes within communities. Yet, the private sector remains fragmented, making it difficult for health system actors to support and ensure the availability of quality health care services. In global health programs, social franchising is one model used to engage and organize the private health care sector. Two social franchise networks, ProFam in West Africa and Tunza in East and Central Africa, provide health care through branded networks of facilities. However, these social franchise networks include a limited number of private health care facilities, and in fragile contexts, like Burundi and Mali, they have faced challenges in integrating with national health systems. The MOMENTUM Private Healthcare Delivery (MPHD) project in Burundi and Mali sought to expand the number of health facilities it engaged beyond the existing ProFam and Tunza networks. The expansion aimed to help improve service quality in more private facilities while advancing localization and reducing fragmentation for improved stewardship by health system actors. MPHD achieved this expansion by removing barriers for private health facilities to join inclusive, nonbranded networks and engaging local partners to build and maintain these networks. We share lessons learned regarding the growing role of local organizations as actors within mixed health systems and provide insights on strengthening stewardship of the increasingly heterogeneous private health care delivery sector in LMICs, particularly in fragile settings. GBM has a poor prognosis despite aggressive treatment, in part due to the lack of adequate drug permeability at the BBB. Standard of care GBM therapies include radiation and cytotoxic chemotherapy that lead to DNA damage. Subsequent activation of DNA damage response (DDR) pathways can induce resistance. Various DDR inhibitors, targeting the key regulators of these pathways such as ataxia telangiectasia mutated and Rad3-related (ATR), are being explored as radio- and chemosensitizers. Elimusertib, a novel ATR kinase inhibitor, can prevent repair of damaged DNA, increasing efficacy of DNA-damaging cytotoxic therapies. Robust synergy was observed in vitro when elimusertib was combined with the DNA-damaging agent temozolomide; however, we did not observe improvement with this combination in in vivo efficacy studies in GBM orthotopic tumor-bearing mice. This in vitro–in vivo disconnect was explored to understand factors influencing central nervous system (CNS) distribution of elimusertib and reasons for lack of efficacy. We observed that elimusertib is rapidly cleared from systemic circulation in mice and would not maintain adequate exposure in the CNS for efficacious combination therapy with temozolomide. CNS distribution of elimusertib is partially limited by P-glycoprotein efflux at the BBB, and high binding to CNS tissues leads to low levels of pharmacologically active (unbound) drug in the brain. Acknowledging the potential for interspecies differences in pharmacokinetics, these data suggest that clinical translation of elimusertib in combination with temozolomide for treatment of GBM may be limited. SIGNIFICANCE STATEMENT This study examined the disconnect between the in vitro synergy and in vivo efficacy of elimusertib/temozolomide combination therapy by exploring systemic and central nervous system (CNS) distributional pharmacokinetics. Results indicate that the lack of improvement in in vivo efficacy in glioblastoma (GBM) patient-derived xenograft (PDX) models could be attributed to inadequate exposure of pharmacologically active drug concentrations in the CNS. The role of the free-to-total prostate-specific antigen (PSA) ratio (FPSAR) in posttreatment prostate cancer, specifically in the PSMA PET/CT era, remains unknown. Our aim in this study was to determine the association of FPSAR in patients referred for 18F-DCFPyL PSMA PET/CT in the BCR setting and assess the correlation between FPSAR and 18F-DCFPyL PSMA PET/CT positivity (local recurrence or distant metastases). Methods: This prospective study included 137 patients who were referred for 18F-DCFPyL PSMA PET/CT and had BCR with a total PSA of less than 1 ng/mL after radical prostatectomy (RP) (including adjuvant or salvage radiotherapy). Blood samples were collected on the day of 18F-DCFPyL PSMA PET/CT. FPSAR was categorized as less than 0.10 or as 0.10 or more. A positive 18F-DCFPyL PSMA PET/CT scan was defined by a PROMISE classification lesion score of 2 or 3, irrespective of the site of increased tracer uptake (e.g., prostate, pelvic nodes, bone, or viscera). Results: Overall, 137 blood samples of patients with BCR after RP were analyzed to calculate FPSAR. The median age at 18F-DCFPyL PSMA PET/CT was 68.6 y (interquartile range, 63.0–72.4 y), and the median PSA at 18F-DCFPyL PSMA PET/CT was 0.3 ng/mL (interquartile range, 0.3–0.6 ng/mL). Eighty-six patients (62.8%) had an FPSAR of less than 0.10, whereas 51 patients (37.2%) had an FPSAR of 0.10 or more. An FPSAR of 0.10 or more was identified as an independent predictor of a positive 18F-DCFPyL PSMA PET/CT scan, with an odds ratio of 6.99 (95% CI, 2.96–16.51; P < 0.001). Conclusion: An FPSAR of 0.10 or more after RP independently correlated with increased odds of a positive 18F-DCFPyL PSMA PET/CT scan among BCR post-RP patients. These findings may offer an inexpensive method by which to triage access to 18F-DCFPyL PSMA PET/CT in jurisdictions where availability is not replete. Full Article

This study was conducted to demonstrate predictors and long-term outcomes of periprocedural intracranial hemorrhage after stent placement for symptomatic intracranial atherosclerotic stenosis. MATERIALS AND METHODS: We retrospectively analyzed patients with symptomatic intracranial atherosclerotic stenosis stent placement in a prospective cohort at a high-volume stroke center. Clinical, radiologic, and periprocedural characteristics and long-term outcomes were reviewed. Periprocedural intracranial hemorrhage was classified as procedure-related hemorrhage (PRH) and non-procedure-related hemorrhage (NPRH). The long-term outcomes were compared between patients with PRH and NPRH, and the predictors of NPRH were explored. RESULTS: Among 1849 patients, 24 (1.3%) had periprocedural intracranial hemorrhage, including PRH (4) and NPRH (20). The postprocedural 30-day mRS was 0–2 in 9 (37.5%) cases, 3–5 in 5 (20.8%) cases, and 6 in 10 (41.7%) cases. For the 14 survivors, the long-term (median of 78 months) mRS were 0–2 in 10 (76.9%) cases and 3–5 in 3 (23.1%) cases. The proportion of poor long-term outcomes (mRS ≥3) in patients with NPRH was significantly higher than those with PRH (68.4% versus 0%, P = .024). Anterior circulation (P = .002), high preprocedural stenosis rate (P < .001), and cerebral infarction within 30 days (P = .006) were independent predictors of NPRH after stent placement. CONCLUSIONS: Patients with NPRH had worse outcomes than those with PRH after stent placement for symptomatic ICAS. Anterior circulation, severe preprocedural stenosis, and recent infarction are independent predictors of NPRH. Full Article

While conventional techniques are effective, cyclic aspiration (CyA) shows potential for better recanalization rates. We aim to investigate factors affecting CyA and compare them with static aspiration (StA). MATERIALS AND METHODS: StA setup consisted of an aspiration pump connected to pressure transducer. CyA was tested with 5 subsequent iterations: single solenoid valve with air plus saline (i1) or saline alone (i2) as aspiration medium; 2 solenoid valves with air plus saline (i3) as aspiration medium; complete air removal and saline feeding (i4); and pressurized saline feeding (i5). To assess the efficacy of clot ingestion, the pressure transducer was replaced with a distal aspiration catheter. Moderately stiff clot analogs (15 mm) were used to investigate the ingestion quantified as clot relative weight loss. Additionally, the aspiration flow rate was assessed for each setup. RESULTS: With CyA i1, the amplitude of the achieved negative pressure waves declined with increasing frequencies but progressively increased with each subsequent iteration, achieving a maximum amplitude of 81 kPa for i5 at 1 Hz. Relative clot weight loss was significantly higher with i5 at 5 Hz than with StA (100% versus 37.8%; P = .05). Aspiration flow rate was lower with CyA than with StA (i5 at 5 Hz: 199.8 mL/min versus StA: 311 mL/min; P < .01). CONCLUSIONS: CyA with the appropriate setup may represent an encouraging innovation in mechanical thrombectomy, offering a promising pathway for improving efficacy in clot ingestion and recanalization. This study linked a nationally representative sample of family physicians (FPs) with Medicaid claims data to explore the relative contributions to care of Medicaid populations by FP race and ethnicity. METHODS This descriptive cross-sectional study used 2016 Medicaid claims data from the Transformed Medicaid Statistical Information System and from 2016-2017 American Board of Family Medicine certification questionnaire responses to examine the diversity and Medicaid participation of FPs. We explored the diversity of FP Medicaid patient panels and whether they saw ≥150 beneficiaries in 2016. Using logistic regression models, we controlled for FP demographics, practice characteristics, and characteristics of the communities in which they practiced. RESULTS Of 13,096 FPs, Latine, Hispanic, or of Spanish Origin (LHS) FPs and non-LHS Black FPs saw more Medicaid beneficiaries compared with non-LHS White and non-LHS Asian FPs. The patient panels of URiM FPs had a much greater proportion of Medicaid beneficiaries from racial and ethnic minority groups. Overall, non-LHS Black and LHS FPs had greater odds of seeing ≥150 Medicaid beneficiaries in 2016. CONCLUSIONS These findings clearly show the critical role URiM FPs play in caring for Medicaid beneficiaries, suggesting physician race and ethnicity are correlated with Medicaid participation. Diversity in the health care workforce is essential for addressing racial health inequities. Policies need to address problems in pathways to medical education, including failures to recruit, nurture, and retain URiM students. Full Article

The Nintendo Switch has now sold an estimated 34.30 million units lifetime in Japan. The PlayStation 5 sold an estimated 49,056 units to bring its lifetime sales to 6.12 million units. The Xbox Series X|S sold 5,862 units to bring their lifetime sales to 0.64 million units. The PlayStation 4 sold an estimated 162 units to bring its lifetime sales to 9.68 million units. PS5 sales compared to the same month for the PS4 in 2017 are down by nearly 53,000 units, while the Xbox Series X|S compared to the same month for the Xbox One are up by over 5,000 units. PS4 sold 101,851 units for the month of October 2017 and Xbox One sales were at 563 units. Nintendo Switch sales compared to the same month a year ago are down by 41,368 units (-14.6%). PlayStation 5 sales are down by 17,208 (-26.0%) and Xbox Series X|S sales are down by 2,294 units (-28.1%). The PlayStation 4 is down by 5,362 units (-97.1%) year-over-year. The Xbox Series X|S was the second best-selling console for the month, followed by the Nintendo Switch in third. The Xbox Series X|S has also surpassed the Switch in year-to-date sales. Sales for the PS5 are up 27 percent month-on-month and down 18 percent year-on-year. Sales for the Switch are up 13 percent month-on-month and down 18 percent year-on-year. Sales for the Xbox Series X|S are down seven percent month-on-month and down 17 percent year-on-year. Overall, there were just under 144,000 video game consoles sold (NielsenIQ/GfK panel data) in the UK in October 2024. This is down 18 percent year-on-year. GSD data shows there were 2.9 million games sold in October 2024, which is up 3.2 percent year-on-year. Call of Duty: Black Ops 6 debuted in first place on the software charts (digital and physical combined). Sales are 11 percent lower than the launch month of 2023's 2023's Call of Duty: Modern Warfare III, however, this is most likely due to the number of people playing the game via Xbox Game Pass. Sales of Call of Duty: Black Ops 6 are up 24 percent year-on-year on PlayStation consoles, while sales dropped on Xbox consoles and PC. 75 percent of total sales were on PlayStation consoles, followed by 15 percent on PC, and just under 11 percent on Xbox consoles. Dragon Ball: Sparking! Zero debuted in third place with the first four week of sales up 80 percent compared to 2020's Dragon Ball Z: Kakarot. Sales are 11 percent higher than 2018's Super Mario Party, but are four percent lower than 2021's Mario Party Superstar. It should be noted Nintendo doesn't share digital sales with these charts. Sonic X Shadows Generations debuted in 10th place. Sales for the game are 150 percent compared to 2023's Sonic Superstars, but are down 21 percent compared to 2022's Sonic Frontiers. Metaphor: Refantazio debuted in 12th place and the new version of Until Dawn debuted in 30th place. EA Sports FC 25 came in second place with sales down 14 percent year-on-year. There were over 681,000 accessories and add-on products sold in the UK in October, which is up nearly 16 percent year-over-year, but is down over eight percent month-on-month. The White DualSense controller was the best-selling accessory, followed by the Midnight Black DualSense controller. The PlayStation Portal Remote Player is up four spots to take fourth place. cto How a Robot Is Grabbing Fuel From a Fukushima Reactor By spectrum.ieee.org Published On :: Mon, 07 Oct 2024 12:00:02 +0000 Thirteen years since a massive earthquake and tsunami struck the Fukushima Dai-ichi nuclear power plant in northern Japan, causing a loss of power, meltdowns and a major release of radioactive material, operator Tokyo Electric Power Co. (TEPCO) finally seems to be close to extracting the first bit of melted fuel from the complex—thanks to a special telescopic robotic device. Despite Japan’s prowess in industrial robotics, TEPCO had no robots to deploy in the immediate aftermath of the disaster. Since then, however, robots have been used to measure radiation levels, clear building debris, and survey the exterior and interior of the plant overlooking the Pacific Ocean. It will take decades to decommission Fukushima Dai-ichi, and one of the most dangerous, complex tasks is the removal and storage of about 880 tons of highly radioactive molten fuel in three reactor buildings that were operating when the tsunami hit. TEPCO believes mixtures of uranium, zirconium and other metals accumulated around the bottom of the primary containment vessels (PCVs) of the reactors—but the exact composition of the material is unknown. The material is “fuel debris,” which TEPCO defines as overheated fuel that has melted with fuel rods and in-vessel structures, then cooled and re-solidified. The extraction was supposed to begin in 2021 but ran into development delays and obstacles in the extraction route; the coronavirus pandemic also slowed work.While TEPCO wants a molten fuel sample to analyze for exact composition, getting just a teaspoon of the stuff has proven so tricky that the job is years behind schedule. That may change soon as crews have deployed the telescoping device to target the 237 tons of fuel debris in Unit 2, which suffered less damage than the other reactor buildings and no hydrogen explosion, making it an easier and safer test bed.“We plan to retrieve a small amount of fuel debris from Unit 2, analyze it to evaluate its properties and the process of its formation, and then move on to large-scale retrieval,” says Tatsuya Matoba, a spokesperson for TEPCO. “We believe that extracting as much information as possible from the retrieved fuel debris will likely contribute greatly to future decommissioning work.”How TEPCO Plans to Retrieve a Fuel SampleGetting to the fuel is easier said than done. Shaped like an inverted light bulb, the damaged PCV is a 33-meter-tall steel structure that houses the reactor pressure vessel where nuclear fission took place. A 2-meter-long isolation valve designed to block the release of radioactive material sits at the bottom of the PCV, and that’s where the robot will go in. The fuel debris itself is partly underwater. The robot arm is being preceded by a smaller telescopic device. The telescopic device, which is trying to retrieve 3 grams of the fuel debris without further contamination to the outside environment, is similar to the larger robot arm, which is better suited for the retrieval of larger bits of debris.Mitsubishi Heavy Industries, the International Research Institute for Nuclear Decommissioning and UK-based Veolia Nuclear Solutions developed the robot arm to enter small openings in the PCV, where it can survey the interior and grab the fuel. Mostly made of stainless steel and aluminum, the arm measures 22 meters long, weighs 4.6 tons and can move along 18 degrees of freedom. It’s a boom-style arm, not unlike the robotic arms on the International Space Station, that rests in a sealed enclosure box when not extended. The arm consists of four main elements: a carriage that pushes the assembly through the openings, arm links that can fold up like a ream of dot matrix printer paper, an arm that has three telescopic stages, and a “wand” (an extendable pipe-shaped component) with cameras and a gripper on its tip. Both the arm and the wand can tilt downward toward the target area. After the assembly is pushed through the PCV’s isolation valve, it angles downward over a 7.2-meter-long rail heading toward the base of the reactor. It continues through existing openings in the pedestal, a concrete structure supporting the reactor, and the platform, which is a flat surface under the reactor. Then, the tip is lowered on a cable like the grabber in a claw machine toward the debris field at the bottom of the pedestal. The gripper tool at the end of the component has two delicate pincers (only 5 square millimeters), that can pinch a small pebble of debris. The debris is transferred to a container and, if all goes well, is brought back up through the openings and placed in a glovebox: A sealed, negative-pressure container in the reactor building where initial testing can be performed. It will then be moved to a Japan Atomic Energy Agency facility in nearby Ibaraki Prefecture for detailed analysis.While the gripper on the telescopic device currently being used was able to reach the debris field and grasp a piece of rubble—it’s unknown if it was actually melted fuel—last month, two of the four cameras on the device stopped working a few days later, and the device was eventually reeled back into the enclosure box. Crews confirmed there were no problems with signal wiring from the control panel in the reactor building, and proceeded to perform oscilloscope testing. TEPCO speculates that radiation passing through camera semiconductor elements caused electrical charge to build up, and that the charge will drain if the cameras are left on in a relatively low-dose environment. It was the latest setback in a very long project. “Retrieving fuel debris from Fukushima Daiichi Nuclear Power Station is an extremely difficult task, and a very important part of decommissioning,” says Matoba. “With the goal of completing the decommissioning in 30 to 40 years, we believe it is important to proceed strategically and systematically with each step of the work at hand.”This story was updated on 15 October, 2024 to clarify that TEPCO is using two separate tools (a smaller telescopic device and a larger robot arm) in the process of retrieving fuel debris samples.
