How host immune responses are orchestrated after alphaviral infections and lead to musculoskeletal inflammation remains poorly understood. Here, we show that myositis induced by Ross River virus (RRV) infection is driven by CD11bhi Ly6Chi inflammatory monocytes and followed by the establishment of a CD11bhi Ly6Clo CX3CR1+ macrophage population in the muscle upon recovery. Selective modulation of CD11bhi Ly6Chi monocyte migration to infected muscle using immune-modifying microparticles (IMP) reduced disease score, tissue damage, and inflammation and promoted the accumulation of CX3CR1+ macrophages, enhancing recovery and resolution. Here, we detail the role of immune pathology, describing a poorly characterized muscle macrophage subset as part of the dynamics of alphavirus-induced myositis and tissue recovery and identify IMP as an effective immunomodulatory approach. Given the lack of specific treatments available for alphavirus-induced pathologies, this study highlights a therapeutic potential for simple immune modulation by IMP in infected individuals in the event of large alphavirus outbreaks. IMPORTANCE Arthritogenic alphaviruses cause debilitating inflammatory disease, and current therapies are restricted to palliative approaches. Here, we show that following monocyte-driven muscle inflammation, tissue recovery is associated with the accumulation of CX3CR1+ macrophages in the muscle. Modulating inflammatory monocyte infiltration using immune-modifying microparticles (IMP) reduced tissue damage and inflammation and enhanced the formation of tissue repair-associated CX3CR1+ macrophages in the muscle. This shows that modulating key effectors of viral inflammation using microparticles can alter the outcome of disease by facilitating the accumulation of macrophage subsets associated with tissue repair. Despite its frequent and successful use in the clinic, little or no radiobiologic considerations are made at the time of treatment planning or delivery. On positive uptake on octreotide-based PET/SPECT imaging, treatment is usually administered as a standard dose and number of cycles without adjustment for peptide uptake, dosimetry, or radiobiologic and DNA damage effects in the tumor. Here, we visualized and quantified the extent of DNA damage response after 177Lu-DOTATATE therapy using SPECT imaging with 111In-anti-H2AX-TAT. This work was a proof-of-principle study of this in vivo noninvasive biodosimeter with β-emitting therapeutic radiopharmaceuticals. Methods: Six cell lines were exposed to external-beam radiotherapy (EBRT) or 177Lu-DOTATATE, after which the number of H2AX foci and the clonogenic survival were measured. Mice bearing CA20948 somatostatin receptor–positive tumor xenografts were treated with 177Lu-DOTATATE or sham-treated and coinjected with 111In-anti-H2AX-TAT, 111In-IgG-TAT control, or vehicle. Results: Clonogenic survival after external-beam radiotherapy was cell-line–specific, indicating varying levels of intrinsic radiosensitivity. Regarding in vitro cell lines treated with 177Lu-DOTATATE, clonogenic survival decreased and H2AX foci increased for cells expressing high levels of somatostatin receptor subtype 2. Ex vivo measurements revealed a partial correlation between 177Lu-DOTATATE uptake and H2AX focus induction between different regions of CA20948 xenograft tumors, suggesting that different parts of the tumor may react differentially to 177Lu-DOTATATE irradiation. Conclusion: 111In-anti-H2AX-TAT allows monitoring of DNA damage after 177Lu-DOTATATE therapy and reveals heterogeneous damage responses. Full Article

There is a subgroup of patients with poor response despite sufficient expression of PSMA in their tumors. The aim of this work was to characterize PSMA-TAT–nonresponding lesions by targeted next-generation sequencing. Methods: Of 60 patients treated with 225Ac-PSMA-617, we identified 10 patients who presented with a poor response despite sufficient tumor uptake in PSMA PET/CT. We were able to perform CT-guided biopsies with histologic validation of the nonresponding lesions in 7 of these nonresponding patients. Specimens were analyzed by targeted next-generation sequencing interrogating 37 DNA damage-repair–associated genes. Results: In the 7 tumor samples analyzed, we found a total of 15 whole-gene deletions, deleterious or presumably deleterious mutations affecting TP53 (n = 3), CHEK2 (n = 2), ATM (n = 2), and BRCA1, BRCA2, PALB2, MSH2, MSH6, NBN, FANCB, and PMS1 (n = 1 each). The average number of deleterious or presumably deleterious mutations was 2.2 (range, 0–6) per patient. In addition, several variants of unknown significance in ATM, BRCA1, MSH2, SLX4, ERCC, and various FANC genes were detected. Conclusion: Patients with resistance to PSMA-TAT despite PSMA positivity frequently harbor mutations in DNA damage-repair and checkpoint genes. Although the causal role of these alterations in the patient outcome remains to be determined, our findings encourage future studies combining PSMA-TAT and DNA damage-repair–targeting agents such as poly(ADP-ribose)-polymerase inhibitors. When he took office almost two years ago, Secretary McDonald inherited an organization in crisis: too many veterans faced shockingly long wait-times before they received care, VA officials had allegedly falsified records, and other allegations of mismanagement abounded. Photo: Paul Morigi Since he was sworn into office, Secretary McDonald has led the VA through a period of ambitious reform, anchored by the MyVA program. He and his team have embraced three core strategies that are securing meaningful change. They are important insights for all government leaders, and private sector ones as well. 1. Set bold goals Secretary McDonald’s vision is for the VA to become the number one customer-service agency in the federal government. But he and his team know that words alone won’t make this happen. They developed twelve breakthrough priorities for 2016 that will directly improve service to veterans. These actionable short-term objectives support the VA’s longer term aim to deliver an exceptional experience for our veterans. By aiming high, and also drafting a concrete roadmap, the VA has put itself on a path to success. 2. Hybridize the best of public and private sectors To accomplish their ambitious goal, VA leadership is applying the best practices of customer-service businesses around the nation. The Secretary and his colleagues are leveraging the goodwill, resources, and expertise of both the private and public sector. To do that, the VA has brought together diverse groups of business leaders, medical professionals, government executives, and veteran advocates under their umbrella MyVA Advisory Committee. Following the examples set by private sector leaders in service provision and innovation, the VA is developing user-friendly mobile apps for veterans, modernizing its website, and seeking to make hiring practices faster, more competitive, and more efficient. And so that no good idea is left unheard, the VA has created a "shark tank” to capture and enact suggestions and recommendations for improvement from the folks who best understand daily VA operations—VA employees themselves. 3. Data, data, data The benefits of data-driven decision making in government are well known. As led by Secretary McDonald, the VA has continued to embrace the use of data to inform its policies and improve its performance. Already a leader in the collection and publication of data, the VA has recently taken even greater strides in sharing information between its healthcare delivery agencies. In addition to collecting administrative and health-outcomes information, the VA is gathering data from veterans about what they think . Automated kiosks allow veterans to check in for appointments, and to record their level of satisfaction with the services provided. The results that the Secretary and his team have achieved speak for themselves: 5 million more appointments completed last fiscal year over the previous fiscal year 7 million additional hours of care for veterans in the last two years (based on an increase in the clinical workload of 11 percent over the last two years) 97 percent of appointments completed within 30 days of the veteran’s preferred date; 86 percent within 7 days; 22 percent the same day Average wait times of 5 days for primary care, 6 days for specialty care, and 2 days for mental health are 90 percent of veterans say they are satisfied or completely satisfied with when they got their appointment (less than 3 percent said they were dissatisfied or completely dissatisfied). The backlog for disability claims—once over 600,000 claims that were more than 125 days old—is down almost 90 percent. Thanks to Secretary McDonald’s continued commitment to modernization, the VA has made significant progress. Problems, of course, remain at the VA and the Secretary has more work to do to ensure America honors the debt it owes its veterans, but the past two years of reform have moved the Department in the right direction. Relations between the two were once solid with strong support from AU member countries. The ICC has had support from African countries because of the court’s ability to prevent Rwandan genocide-type atrocities and ease power differentials between small and large African nations in international trial situations. Brookings nonresident scholar, John Mukum Mbaku, discusses the rising tensions between the ICC and African countries in his contribution to Foresight Africa, a collection of short briefs on the top challenges and opportunities for Africa in 2014. Read the related paper » According to John Mbaku, some African countries see the ICC as quite problematic because of the perceived bias that the court brings mostly African criminal cases to trial. Other countries feel their sovereignty is threatened by the court. The African Union has gone so far as to ask member countries not to comply or cooperate with the ICC and has attempted (unsuccessfully) to withdraw from The Hague. A major trigger for these the recent issues with the ICC and African countries was the election of Uhuru Kenyatta as president of Kenya. Kenyatta was indicted by the ICC for crimes against humanity and for allegedly inciting ethnic violence following the highly disputed 2007 elections in Kenya. John argues that repairing the ties between African countries and the ICC will be difficult, but might be possible through reforms to the ICC process and commitment to stronger judicial systems in African nations. The aircraft was owned by the UP government and after it was involved in an accident some time ago, it was bought by UY Aviation (Pvt) Ltd, a three-year-old company, which rents out aircraft to ferry patients from all over the country, an insider told mid-day. The company had got the King C90 repaired by Indamer Aviation Private Ltd and, on Thursday, it was being taken on its first test flight. "Around 1.08 pm, the ATC towers at Mumbai and Juhu both lost contact with the pilot and suddenly an SOS alert was sent out by the Air Traffic Control. Finally, it was learnt that the plane had crashed," said Juhu Aerodrome Director Ashok Kumar Verma. Verma said the aircraft took off from the Juhu runway a little after 12.20 pm as per ATC Juhu records. Pilot, Captain Pradeep Rajput, and co-pilot, Maria Zuberi (who had joined UY Aviation barely a month ago), were in constant touch with the Juhu ATC and later with the Mumbai main ATC as it passed through their radar. The charter was barely a mile from Juhu aerodrome, on its way back after completing the test flight, when it crashed. When asked if the aircraft was on a wrong flying line, as usually passenger flights pass through the airway used by the charter, Verma replied in the negative, stating that there was no air space violation and that the charter was on its own scheduled air space." The investigating team from the Aircraft Accident Investigation Bureau (AAIB) will be recording the statements of the ATC officers who were in dialogue with the pilots of the charter, which will be compared with the black box that has been recovered. Speaking to mid-day soon after landing in Mumbai from Delhi on Thursday evening, Captain Vinod MRSK, a former air force pilot, who is the Chief of Flight Safety at UY Aviation (Pvt) Ltd, clarified that the charter was still with Indamer Aviation Private Ltd (an aviation repair and maintenance organisation), and two of their engineers Manish Pandey and Surbhi, along with Rajput and Zuberi were to conduct the airworthiness test profile and had obtained necessary permissions from DGCA. "The air profile testing was crucial as only then could UY Aviation have approached DGCA to complete other formalities to officially take the charter into its inventory. The charter was flying on a Non-Scheduled Operator's Permit (NSOP) for testing. At present UY Aviation has four charters, two in Mumbai and two in Delhi, and this eight-seater King C90, would have made it to its inventory in Mumbai," the insider said. Captain Vinod said, "I am here to assist Aircraft Accident Investigation Bureau (AAIB) team under DGCA to understand the circumstances under which the accident happened. By www.mid-day.com Published On :: 28 Apr 2020 02:00:54 GMT With most areas in the city marked as COVID-19 hotspots and no transport available, residents feel that the Brihanmumbai Municipal Corporation's (BMC) circular allowing pre-monsoon repair work and waterproofing in buildings before the onset of monsoon, is nothing but an eyewash. They are of the opinion that if the lockdown continues beyond May 3, it will be practically difficult to complete work before the rains, as no shops selling cement, sand, brick and waterproofing chemicals will be open. Speaking to mid-day, Somasunderan Nair, director, Intercons Tectonic (Pvt) Ltd, a Vashi-based firm doing engineering and civil construction work, said, "It is just impossible for the labourers to go to different locations with no transportation available. They can't travel between construction sites located in different areas of Mumbai and Navi Mumbai." Rekhi Sai Daffodils has been undergoing repairs since January 'All going to waste'"Also, I had stored raw materials (cement and sand) worth a few lakhs of rupees at different buildings undergoing repairs, but all of it got stalled due to the lockdown," added Nair. He further said, "At one of our Kharghar site, the Pimpri-Chinchwad Municipal Corporation (PCMC) staff had asked the site supervisor to wind up all repair work, and at that time, a ready-mix cement mortar consignment was being offloaded from a Gujarat registered truck. Since the municipal staff warned of '15,000 fine per staff, the workers had no option but to leave the site. Nearly 700 bags of ready-mix cement mortar were offloaded, but I fear that all of it will go to waste. They have been lying unused for over one month and if the lockdown continues, we might have to dispose of the raw materials." While the BMC has come out with a circular allowing work to be carried out, the NMMC and PCMC are yet to provide any clarity on such repair works. Exposed beams of Rekhi Sai Daffodils in Kharghar A Ghatkopar West resident, whose building is under repairs, said, "We have stored some materials in our building premises and have allowed some workers to stay on the compound. Even the contractor is ready to buy construction materials like bricks, cement etc from the local market, but for that we need to know where such outlets are open." 'Apply for online permissions'When contacted, a senior official from BMC's Development Plan department said, "We have issued the circular which clearly states the directives. As per the directions of the state government, certain activities are being allowed during lockdown. However, the said guidelines were stayed by the government vide GR dated April 21, 2020 for Mumbai Metropolitan Region and Pune Metropolitan Region. However, to save some existing structures/buildings and prevent flood like situations during monsoon, certain construction activities within MCGM limits may be allowed in non-containment zones." Navre Premises Cooperative society in Sion West When asked that if the entire city is under lockdown, then how will the circular be implemented, the official said, "It is a fact that most of the areas in Mumbai are containment zones and no work will be allowed there. If the situation is under control after May 3, then as and when the lockdown is lifted, work can be started. Meanwhile, the contractors/societies concerned should apply for online permissions." And if monsoon sets in by then, the official said, "The contractors can cover the open terraces with tarpaulin sheets and permission can be given for repairs post monsoon." Expert speakWhen contacted, senior lawyer, Vinod Sampat, said, "As far as the BMC is concerned, everyone is interested in protecting their own skin. There is no coordination between government departments. A simple solution can be easily worked out by designating one responsible official for the job. He would ensure that the residents' requirements are easily met and the construction materials are made available to those whose buildings are getting repaired. Secondly, there is no accountability among government officials and they are unpredictable. As far as possible, repair work should not be undertaken immediately. A better way would be to put tarpaulin sheets on the buildings and postpone repair work to the post-monsoon time." He further said, "Moreover, it is very difficult to get skilled labourers now as most of them want to return home. And lastly, if any of the contractors or their workers test positive for COVID-19, the society managing committee members may be held responsible for not taking proper precaution."
