Clinical and Pathophysiologic Correlates of Basilar Artery Measurements in Fabry Disease [CLINICAL PRACTICE]

BACKGROUND AND PURPOSE:

Alterations of the basilar artery (BA) anatomy have been suggested as a possible MRA feature of Fabry disease (FD). Nonetheless, no information about their clinical or pathophysiologic correlates is available, limiting our comprehension of the real impact of vessel remodeling in FD.

MATERIALS AND METHODS:

Brain MRIs of 53 subjects with FD (mean age, 40.7 [SD, 12.4] years; male/female ratio = 23:30) were collected in this single-center study. Mean BA diameter and its tortuosity index were calculated on MRA. Possible correlations between these metrics and clinical, laboratory, and advanced imaging variables of the posterior circulation were tested. In a subgroup of 20 subjects, a 2-year clinical and imaging follow-up was available, and possible longitudinal changes of these metrics and their ability to predict clinical scores were also probed.

RESULTS:

No significant association was found between MRA metrics and any clinical, laboratory, or advanced imaging variable (P values ranging from –0.006 to 0.32). At the follow-up examination, no changes were observed with time for the mean BA diameter (P = .84) and the tortuosity index (P = .70). Finally, baseline MRA variables failed to predict the clinical status of patients with FD at follow-up (P = .42 and 0.66, respectively).

CONCLUSIONS:

Alterations of the BA in FD lack of any meaningful association with clinical, laboratory, or advanced imaging findings collected in this study. Furthermore, this lack of correlation seems constant across time, suggesting stability over time. Taken together, these results suggest that the role of BA dolichoectasia in FD should be reconsidered.

Slitterhead review: body-hopping action horror that's best left dispossessed

I was excited for Slitterhead, an action adventure game by Bokeh Studio, a studio founded by none other than your boy Keiichiro Toyama: the creator of Silent Hill, Gravity Rush, and the Siren series. And within that first hour, Slitterhead's body-possessing and Hong Kong-inspired streets had me thinking, "Is this it, the sleeper hit of 2024?!"

No, sadly not. It's no doubt built a compelling universe filled with brain-sucking aliens that masquerade as humans, and it attempts plenty else besides: bouncing between bodies as you stealth around dingy apartment blocks, fighting with blood katanas, and gorging on pools of red plasma to refuel skills, many of which require more body-flitting. Thing is, they are ultimately just attempts, attempts that fall victim to an emptiness and jitteriness that quickly reveals Slitterhead's true, irritating form.

When a family member we hold near and dear to our hearts passes on to the next life, everything they leave behind holds astronomically more sentimental value—especially those who had special bonds with this family member. We can only hope that this family member can live on in the life they lived with and without us, and the items they pass down to us stick with us for as long as we are here, too.

That's the thing about passing on—this event really makes certain individuals show their true colors. Family members who have been nice to you all of your life now turn sour. Others creep out of the woodwork and assume they were grandma's favorite (which means they get everything she left in the will, despite her last wishes.) It's baffling that some folks use somber life events like this to gain something instead of grieving, going so far as to try and rip family heirlooms away from those who have the legal right to keep them.

The woman in this story is dealing with an entitled cousin who, despite not having a close, personal relationship with their grandmother, automatically assumes she is entitled to a necklace grandma left behind. Their grandmother personally gave the woman this necklace in hopes that she'd "treasure" it, and also wrote it into her will before she left them. Their family is divided, though it doesn't make sense as to why. Going against someone's last wishes in order to "keep the peace" is an entitlement that knows no bounds.
