One year later when Bach was thirty-seven in 1722 he married another woman. This woman's name was Anna Magdalina. Anna had thirteen children. ... Bach wrote 300 religious and nonreligious pieces called cantatas. Some people think that bach was the best composer of all time. Bach was a very religious man. Bach showed his religions in his music. He was of German blood and speech, and shared some characteristics of his people- strength of body, courage of spirit, pride of race, and a crude simplicity many centuries apart from the urbane polish of the modern French. He had little book learning; read only a few books- but good ones; tried in his old age to learn writing, but never quite succeeded; yet he could speak old Teutonic and literary Latin, and understood Greek. In 771 Carloman II died, and Charles at twenty-nine became sole king. Two years later he received from Pope Hadrian II an urgent appeal for aid against the Lombard Desiderius, who was invading the papal states. Charles hastened up from Rome, built fleets and forts on shores and rivers, and stationed garrisons at danger points. In 810 the king of Jutland invaded Frisia and was repulsed; but shortly thereafter, if we may follow the chronicle of the Monk of St. Gall, Charlemagne, from his palace at Narbonne, was shocked to see Danish pirate vessels in the Gulf of Lyons. Perhaps because he foresaw, like Diocletian, that his overreaching empire needed quick defense at many points at once, he divided it in 806 among his three sons- Pepin, Louis, and Charles. But Pepin died in 810, Charles in 811; only Louis remained, so absorbed in piety as to seem unfit to govern a rough and treacherous world. Nevertheless, in 813, at a solemn ceremony, Louis was elevated from the rank of king to that of emperor, and the old monarch uttered his nunc dimittis: "Blessed be Thou, O Lord God, Who hast granted me the grace to see with my own eyes my son seated on my throne!" -- Death: Four months later, wintering at Aachen, he was seized with a high fever, and developed pleurisy. He tried to cure himself by taking only liquids; but after an illness of seven days he died, in the forty-seventh year of his reign and the seventy-second year of his life (814 A.D.). He was buried under the dome of the cathedral at Aachen, dressed in his imperial robes. Soon all the world called him Carolus Magnus, Karl der Grosse, Charlemagne; and in 1165 A.D., when time had washed away all memory of his mistresses, the Church which he had served so well enrolled him among the blessed.

Saint John the Hermit and thirty-five companions lived in Egypt, but took ship as a group for Cyprus in order to practice ascesis in exile. At Cyprus they met a party of thirty-nine others who also sought to live the ascetic life more fully, and the two groups joined. "In order that these ascetics, too, might taste of the graces of voluntary exile" (Synaxarion), they travelled to Attalia in Pamphylia. There, twenty-four more monks joined them, so that their company now numbered ninety-nine. (This number was ordained by God, so that Christ Himself, their Head, would complete their number at one hundred.) After some time they took ship again for Crete, where they lived in two caves in a remote, deserted area, living only on the plants that grew wild there.   Saint John sought his brothers' blessing to live as a hermit. On the day of his departure they prayed that they all might repose on the day of John's death, and enter together into the Kingdom of God. John's asceticism was so severe that after awhile he could no longer walk, but crawled from his cave to gather the small quantities of food he allowed himself. A shepherd, seeing him from a distance, thought that he was some animal, and shot him with an arrow. Finding the dying John, the shepherd was stricken with horror and grief, and threw himself at the hermit's feet, begging forgiveness. The saint only lived long enough to give the young man his pardon and blessing before surrendering his soul to God. The prayer of his ninety-eight brethren was mysteriously granted: between the third and seventh hour of that day, they all, one after another, fell asleep in peace.

AlphaFold-assisted structure determination of a bacterial protein of unknown function using X-ray and electron crystallography

Macromolecular crystallography generally requires the recovery of missing phase information from diffraction data to reconstruct an electron-density map of the crystallized molecule. Most recent structures have been solved using molecular replacement as a phasing method, requiring an a priori structure that is closely related to the target protein to serve as a search model; when no such search model exists, molecular replacement is not possible. New advances in computational machine-learning methods, however, have resulted in major advances in protein structure predictions from sequence information. Methods that generate predicted structural models of sufficient accuracy provide a powerful approach to molecular replacement. Taking advantage of these advances, AlphaFold predictions were applied to enable structure determination of a bacterial protein of unknown function (UniProtKB Q63NT7, NCBI locus BPSS0212) based on diffraction data that had evaded phasing attempts using MIR and anomalous scattering methods. Using both X-ray and micro-electron (microED) diffraction data, it was possible to solve the structure of the main fragment of the protein using a predicted model of that domain as a starting point. The use of predicted structural models importantly expands the promise of electron diffraction, where structure determination relies critically on molecular replacement.

New insights into the domain of unknown function (DUF) of EccC5, the pivotal ATPase providing the secretion driving force to the ESX-5 secretion system

Type VII secretion (T7S) systems, also referred to as ESAT-6 secretion (ESX) systems, are molecular machines that have gained great attention due to their implications in cell homeostasis and in host–pathogen interactions in mycobacteria. The latter include important human pathogens such as Mycobacterium tuberculosis (Mtb), the etiological cause of human tuberculosis, which constitutes a pandemic accounting for more than one million deaths every year. The ESX-5 system is exclusively found in slow-growing pathogenic mycobacteria, where it mediates the secretion of a large family of virulence factors: the PE and PPE proteins. The secretion driving force is provided by EccC5, a multidomain ATPase that operates using four globular cytosolic domains: an N-terminal domain of unknown function (EccC5DUF) and three FtsK/SpoIIIE ATPase domains. Recent structural and functional studies of ESX-3 and ESX-5 systems have revealed EccCDUF to be an ATPase-like fold domain with potential ATPase activity, the functionality of which is essential for secretion. Here, the crystal structure of the MtbEccC5DUF domain is reported at 2.05 Å resolution, which reveals a nucleotide-free structure with degenerated cis-acting and trans-acting elements involved in ATP binding and hydrolysis. This crystallographic study, together with a biophysical assessment of the interaction of MtbEccC5DUF with ATP/Mg2+, supports the absence of ATPase activity proposed for this domain. It is shown that this degeneration is also present in DUF domains from other ESX and ESX-like systems, which are likely to exhibit poor or null ATPase activity. Moreover, based on an in silico model of the N-terminal region of MtbEccC5DUF, it is hypothesized that MtbEccC5DUF is a degenerated ATPase domain that may have retained the ability to hexamerize. These observations draw attention to DUF domains as structural elements with potential implications in the opening and closure of the membrane pore during the secretion process via their involvement in inter-protomer interactions.

In the crystal, the mol­ecules are connected into dimers by C—H⋯O hydrogen bonds with R22(8) ring motifs, forming zigzag ribbons along the b-axis direction. According to a Hirshfeld surface analysis, H⋯H (68.2%) and O⋯H/H⋯O (25.9%) inter­actions are the most significant contributors to the crystal packing. The contribution of some disordered solvent to the scattering was removed using the SQUEEZE routine [Spek (2015). Acta Cryst. C71, 9–18] in PLATON. The solvent contribution was not included in the reported mol­ecular weight and density.

Evaluating the Utility of 18F-FDG PET/CT in Cancer of Unknown Primary

Cancer of unknown primary (CUP) represents a heterogeneous group of metastatic tumors for which standardized diagnostic work-up fails to identify the primary site. We aimed to describe the Peter MacCallum Cancer Centre experience with 18F-FDG PET/CT in extracervical CUP with respect to detection of a primary site and its impact on management. A secondary aim was to compare overall survival (OS) in patients with and without a detected primary site. Methods: CUP patients treated between 2014 and 2020 were identified from medical oncology clinics and 18F-FDG PET/CT records. Information collated from electronic medical records included the suspected primary site and treatment details before and after 18F-FDG PET/CT. Clinicopathologic details and genomic analysis were used to determine the clinically suspected primary site and compared against 2 independent masked reads of 18F-FDG PET/CT images by nuclear medicine specialists to determine sensitivity, specificity, accuracy, and the rate of detection of the primary site. Results: We identified 147 patients, 65% of whom had undergone molecular profiling. The median age at diagnosis was 61 y (range, 20–84 y), and the median follow-up time was 74 mo (range, 26–83 mo). Eighty-two percent were classified as having an unfavorable CUP subtype as per international guidelines.18F-FDG PET/CT demonstrated a primary site detection rate of 41%, resulted in a change in management in 22%, and identified previously occult disease sites in 37%. Median OS was 16.8 mo for all patients and 104.7 and 12.1 mo for favorable and unfavorable CUP subtypes, respectively (P < 0.0001). Median OS in CUP patients when using 18F-FDG PET/CT, clinicopathologic, and genomic information was 19.8 and 8.5 mo when a primary site was detected and not detected, respectively (P = 0.016). Multivariable analysis of survival adjusted for age and sex remained significant for identification of a potential primary site (P < 0.001), a favorable CUP (P < 0.001), and an Eastern Cooperative Oncology Group status of 1 or less (P < 0.001). Conclusion: 18F-FDG PET/CT plays a complementary role in CUP diagnostic work-up and was able to determine the likely primary site in 41% of cases. OS is improved with primary site identification, demonstrating the value of access to diagnostic 18F-FDG PET/CT for CUP patients.

Summary: Appropriate Use Criteria for the Use of Nuclear Medicine in Fever of Unknown Origin

The diagnostic work-up of patients with fever of unknown origin (FUO) begins with a thorough history and physical examination, complete blood count with differential, chest x-ray, urinalysis and culture, electrolyte panel, liver enzymes, erythrocyte sedimentation rate, and C-reactive protein level. Additional imaging procedures, including nuclear medicine tests, are generally used as second-line procedures, with 18F-FDG PET and PET/CT assuming increasingly important roles in the diagnostic work-up. The Society of Nuclear Medicine and Molecular Imaging, the Infectious Diseases Society of America, and the American College of Nuclear Medicine convened an autonomous expert work group to comprehensively review the published literature for nuclear imaging in adults and children with FUO and establish appropriate use criteria (AUC). This process was performed in accordance with the Protecting Access to Medicare Act of 2014, which requires that all referring physicians consult AUC by using a clinical decision support mechanism before ordering advanced diagnostic imaging services. The complete findings and discussions of the work group were published on January 8, 2023, and are available at https://www.snmmi.org/ClinicalPractice/content.aspx?ItemNumber=15666. The AUC in the final document are intended to assist referring health care providers in appropriate use of nuclear medicine imaging procedures in patients with FUO. The work group noted limitations in the current literature on nuclear medicine imaging for FUO, with the need for well-designed prospective multicenter investigations. Consensus findings from published data and expert opinions were used to create recommendations in common clinical scenarios for adults and children. Included in the complete document is a discussion of inflammation of unknown origin (IUO), a recently described entity. In view of the fact that the criteria for FUO and IUO are similar (except for fever > 38.3°C [100.9°F]) and that the most common etiologies of these 2 entities are similar, it is the expert opinion of the work group that the recommendations for nuclear medicine imaging of FUO are also applicable to IUO. These recommendations are included in the full guidance document. This summary reviews rationale, methodology, and main findings and refers the reader to the complete AUC document.
