Best practices of mobile health in clinical care: The development and evaluation of a competency-based provider training program.

This article describes the development and evaluation of a training program for behavioral health providers on the integration of mobile health applications (health apps) in clinical care. The purpose was to train military and Department of Veterans Affairs (VA) providers in the use and current research of various behavioral health apps. The training focused on 5 core competencies: gaining knowledge on the evidence base for using mobile apps in behavioral health practice, effective integration of mobile technologies into traditional treatment settings, security and privacy issues involved with digital data, ethical issues, and cultural considerations. During a 3-year span (October 2014–September 2017), the training program delivered 20 one-day workshops at 15 military or VA locations, reaching 760 Department of Defense and VA clinicians (psychologists, social workers, physicians, and nurses) on best practices for health apps in clinical care. Prior to the training, less than half of the providers who attended the workshop (41.1%) reported that they were using health apps during treatment. Just following the training, 93.7% of the trainees indicated their intent to use health apps during patient care. At the 3-month posttraining interval, provider-reported use of health apps in treatment was 90.8%. Results demonstrated the impact of a mobile apps provider training program to deliver a relevant, feasible, and impactful approach to educating providers regarding mobile health best practices. Clues from the digital world: A survey of clinicians’ reliance on social media as collateral data in forensic evaluations.

Access to and use of social media has increased throughout the United States. In parallel, information gleaned from social media is often available as part of discovery packets provided to clinicians conducting forensic evaluations. As social media continues to be a primary mode of communication, forensic evaluators are likely to grapple with decisions to use information from these sites to inform psycholegal opinions. However, professional commentary on obtaining, interpreting, and integrating data from social networking sites (SNSs) in forensic practice began only recently. There have been few empirical efforts thus far to better understand whether and how forensic mental health evaluators use SNSs to inform their opinions about various psycholegal issues. Thus, the current study sought to address this gap in the professional literature by surveying practicing forensic evaluators (N = 102) regarding their use of SNS data in civil and criminal legal contexts. Quantitative and qualitative findings from our survey are presented and discussed to provide an overview of current practices and concerns among forensic clinicians. Overall, SNSs are used with some frequency in forensic mental health assessments and are generally perceived as a useful source. Findings are integrated with previous professional commentary about Internet-based data to facilitate greater understanding of how SNS data may be best incorporated into assessments and to identify emergent legal and ethical issues when such data are included. Adolescents’ perceptions of their therapists’ social media competency and the therapeutic alliance.

Adolescents’ lives are now experienced in sophisticated digital environments, potentially warranting their consideration as a distinct cultural group. As such, clinicians who work with this population may be expected to be conversant with their clients’ digital habits to establish an efficacious therapeutic alliance and bolster treatment outcomes. We investigated linkages between adolescents’ social media habits and therapeutic alliance ratings based on perceptions of their therapists’ social media competency and traditional multicultural competency (MCC). Seventy-seven 14- to 17-year-olds currently receiving therapy services completed an online survey assessing their social media habits, perceptions of their therapists’ social media competency and traditional MCC, and ratings of therapeutic alliance. Adolescents’ perceptions of therapists’ social media competency positively predicted therapeutic alliance ratings, regardless of variations in participants’ social media use. A positive relationship was also found between perceived traditional MCC and therapeutic alliance ratings. Finally, preliminary validation for a newly adapted measure of therapists’ perceived social media competency was demonstrated. These findings may be among the first to establish social media competency as a contemporary form of MCC, which may inform new avenues for training and professional development for psychologists working with adolescents. As clinical records continue to become digitized, technological competence is increasingly part of clinical competence. Clinicians must be mindful of how they are storing client data and consult with information technology professionals to best protect client data stored online. Clinicians have offline, local computer storage options that require minimal technological competence, but it is essential for clinicians and practices to be thoughtful in storing client data in ways that both suit their clinical needs and best protect client data. A practice without compelling reasons for storing client data in an online environment exposes client data to undue risk, because of the multitude of security vulnerabilities to which such data are exposed. Given the increasing reliance on electronic records, it is important for clinicians to understand exactly how such records are stored, accessed, and transferred. Although all technology can fail, clinicians who carefully consider their own unique data storage needs and have robust reasons for how they store their data can best uphold the ethical standards of privacy and confidentiality with an understanding of how client data is being stored. In this article, we comment on the articles in this special section based on the Summit on Master’s Training in Psychological Practice. We begin with some general remarks highlighting the increased urgency on the topic of the master’s degree in psychology as detailed in the opening article, and provide a few comments on the four areas discussed in the Summit and the special section papers: scope of practice, marketplace and workforce, quality assurance, and regulation and licensure. We then review the general areas of consensus that the Summit participants came to, summarize critical concerns that the issue of licensing individuals with master’s degrees raise, and report on the preliminary discussion held by the Council of Representatives, which will make the final decision about if and how to proceed on this important policy decision for APA and for psychology. The regulation and licensure of master’s level practitioners in psychology has been a discussion within the American Psychological Association for many years. This article entitled Regulatory and Licensure Issues Derived from the Summit on Master’s Level Training in Psychological Practice reviews the licensure and regulatory considerations of independent master’s level practice in psychology, identifies the concerns, potential actions, impact on the profession, areas of consensus, and future considerations. Prominent among the topics were (a) the impact upon doctoral level practice in psychology and (b) the challenge of differentiating master’s level scope of practice, competencies, and title from doctoral level psychologists. A review of the licensure status of other health professions indicates that doctoral level licensure is the standard for health professions and the master’s level licensure is standard for mental health professions. This factor is discussed in the context of the goal of maintaining the doctoral level status for psychologists while recognizing a new master’s level licensure under the rubric of psychology. The article describes the discussions that occurred at the American Psychological Association’s Minority Fellowship Summit on Master’s Level Training for Psychological Practice. Four core areas of impact were considered by experts/participants, 1 of which was scope of practice. The other 3 were: (a) marketplace and workforce issues, (b) regulatory factors and licensure, and (c) quality assurance and accreditation. This article provides a brief description of the topic of scope of practice, and the perspectives that were reflected in the discussion at the Summit are presented. This is followed by a review of proposed action steps that were based on consensus of Summit participants. Master’s summit: Quality assurance and accreditation.

Universities across the United States are granting master’s degrees in many areas of psychology, including areas in which graduates provide behavioral health services. In this growing market, counseling psychology graduates comprise more than one third of all psychology master’s graduates (American Psychological Association [APA], 2017c). Given the role graduates are playing in service provision to the public, the Quality Assurance and Accreditation Working Group considered (a) the current context of master’s education and training in APA, and (b) APA’s potential role in quality assurance. Without challenging APA’s position on the doctoral standard for independent practice, there was consensus among group members that the legal and ethical responsibilities inherent to serving the public meant that programs resulting in master’s-level health service provision should be subject to quality assurance and public protection afforded by the process of accreditation. Master’s level practice: Introduction, history, and current status.

The master’s degree has been an elusive training and practice component of psychology as early as 1947, generating well over 45 conferences and many working groups charged with determining the function and role of the degree within the field. In 2016, a Master’s Summit was held for the same purpose and to ask the same questions including, “Should APA embrace the training of psychological practitioners at the master’s level?” This article introduces the Special Section on Master’s Training in Psychological Practice and reviews the early major conferences, taskforces, and reports which, over the last 70 years, have investigated the master’s issue and identified the recurring topics, findings, and recommendations. Periodic, internal APA work groups on the issue have conferred in recent years but few publications have been issued since the early attention to the topic. This article first provides a brief overview of the history and context of each of four master’s topics that were themes of the 2016 Master’s Summit and were recurring topics of master’s discussions over the years (American Psychological Association Minority Fellowship Program, 2017). These topics are (a) quality assurance and accreditation, (b) scope of practice, (c) regulation and licensure, and (d) marketplace and workforce and each of these is presented in an article in this special section. Past barriers to resolution of the master’s issue are reviewed, why questions could not be answered earlier, and reasons why they may be answered now are proposed.
