ADAM10 and ADAM17 proteases mediate proinflammatory cytokine-induced and constitutive cleavage of endomucin from the endothelial surface [Membrane Biology]

Contact between inflammatory cells and endothelial cells (ECs) is a crucial step in vascular inflammation. Recently, we demonstrated that the cell-surface level of endomucin (EMCN), a heavily O-glycosylated single-transmembrane sialomucin, interferes with the interactions between inflammatory cells and ECs. We have also shown that, in response to an inflammatory stimulus, EMCN is cleared from the cell surface by an unknown mechanism. In this study, using adenovirus-mediated overexpression of a tagged EMCN in human umbilical vein ECs, we found that treatment with tumor necrosis factor α (TNF-α) or the strong oxidant pervanadate leads to loss of cell-surface EMCN and increases the levels of the C-terminal fragment of EMCN 3- to 4-fold. Furthermore, treatment with the broad-spectrum matrix metalloproteinase inhibitor batimastat (BB94) or inhibition of ADAM metallopeptidase domain 10 (ADAM10) and ADAM17 with two small-molecule inhibitors, GW280264X and GI254023X, or with siRNA significantly reduced basal and TNFα-induced cell-surface EMCN cleavage. Release of the C-terminal fragment of EMCN by TNF-α treatment was blocked by chemical inhibition of ADAM10 alone or in combination with ADAM17. These results indicate that cell-surface EMCN undergoes constitutive cleavage and that TNF-α treatment dramatically increases this cleavage, which is mediated predominantly by ADAM10 and ADAM17. As endothelial cell-surface EMCN attenuates leukocyte–EC interactions during inflammation, we propose that EMCN is a potential therapeutic target to manage vascular inflammation.

Endorepellin evokes an angiostatic stress signaling cascade in endothelial cells [Glycobiology and Extracellular Matrices]

Endorepellin, the C-terminal fragment of the heparan sulfate proteoglycan perlecan, influences various signaling pathways in endothelial cells by binding to VEGFR2. In this study, we discovered that soluble endorepellin activates the canonical stress signaling pathway consisting of PERK, eIF2α, ATF4, and GADD45α. Specifically, endorepellin evoked transient activation of VEGFR2, which, in turn, phosphorylated PERK at Thr980. Subsequently, PERK phosphorylated eIF2α at Ser51, upregulating its downstream effector proteins ATF4 and GADD45α. RNAi-mediated knockdown of PERK or eIF2α abrogated the endorepellin-mediated up-regulation of GADD45α, the ultimate effector protein of this stress signaling cascade. To functionally validate these findings, we utilized an ex vivo model of angiogenesis. Exposure of the aortic rings embedded in 3D fibrillar collagen to recombinant endorepellin for 2–4 h activated PERK and induced GADD45α vis à vis vehicle-treated counterparts. Similar effects were obtained with the established cellular stress inducer tunicamycin. Notably, chronic exposure of aortic rings to endorepellin for 7–9 days markedly suppressed vessel sprouting, an angiostatic effect that was rescued by blocking PERK kinase activity. Our findings unravel a mechanism by which an extracellular matrix protein evokes stress signaling in endothelial cells, which leads to angiostasis.

Pre-eclamptic Fetal Programming Alters Neuroinflammatory and Cardiovascular Consequences of Endotoxemia in Sex-Specific Manners [Neuropharmacology]

Pre-eclampsia (PE)-induced fetal programming predisposes offspring to health hazards in adult life. Here, we tested the hypothesis that pre-eclamptic fetal programming elicits sexually dimorphic inflammatory and cardiovascular complications to endotoxemia in adult rat offspring. PE was induced by oral administration of L-NAME (50 mg/kg per day for seven consecutive days) starting from day 14 of conception. Cardiovascular studies were performed in conscious adult male and female offspring preinstrumented with femoral indwelling catheters. Compared with non-PE male counterparts, intravenous administration of lipopolysaccharide (LPS, 5 mg/kg) to PE male offspring caused significantly greater 1) falls in blood pressure, 2) increases in heart rate, 3) rises in arterial dP/dtmax, a correlate of left ventricular contractility, and 4) decreases in time- and frequency-domain indices of heart rate variability (HRV). By contrast, the hypotensive and tachycardic actions of LPS in female offspring were independent of the pre-eclamptic state and no clear changes in HRV or dP/dtmax were noted. Measurement of arterial baroreflex activity by vasoactive method revealed no sex specificity in baroreflex dysfunction induced by LPS. Immunohistochemical studies showed increased protein expression of toll-like receptor 4 in heart as well as in brainstem neuronal pools of the nucleus of solitary tract and rostral ventrolateral medulla in endotoxic PE male, but not female, offspring. Enhanced myocardial, but not neuronal, expression of monocyte chemoattractant protein-1 was also demonstrated in LPS-treated male offspring. Together, pre-eclamptic fetal programming aggravates endotoxic manifestations of hypotension and autonomic dysfunction in male offspring via exacerbating myocardial and neuromedullary inflammatory pathways.

SIGNIFICANCE STATEMENT

Current molecular and neuroanatomical evidence highlights a key role for pre-eclamptic fetal programming in offspring predisposition to health hazards induced by endotoxemia in adult life. Pre-eclampsia accentuates endotoxic manifestations of hypotension, tachycardia, and cardiac autonomic dysfunction in male offspring via exacerbating myocardial and central inflammatory pathways. The absence of such detrimental effects in female littermates suggests sexual dimorphism in the interaction of pre-eclamptic fetal programming with endotoxemia.

A Mechanistic and Translational Pharmacokinetic-Pharmacodynamic Model of Abicipar Pegol and Vascular Endothelial Growth Factor Inhibition [Drug Discovery and Translational Medicine]

Abicipar pegol (abicipar) is a novel DARPin therapeutic and highly potent vascular endothelial growth factor (VEGF) inhibitor intended for the treatment of neovascular age-related macular degeneration (nAMD). Here we develop a translational pharmacokinetic/pharmacodynamic (PK/PD) model for abicipar to guide dosing regimens in the clinic. The model incorporated abicipar-VEGF binding kinetics, VEGF expression levels, and VEGF turnover rates to describe the ocular and systemic PK data collected from the vitreous, aqueous humor (AH), choroid, retina, and serum of rabbits after a 1-mg abicipar intravitreal (IVT) dose. The model was translated to humans using human-specific mechanistic parameters and refitted to human serum and AH concentrations from patients with diabetic macular edema and nAMD. The model was then used to simulate 8-, 12- (quarterly), and 16-week dosing intervals in the clinic. Simulations of 2 mg abicipar IVT at 8-week or quarterly dosing in humans indicates minimum steady-state vitreal concentrations are maintained above both in vitro IC50 and in vivo human IC50 values. The model predicted virtually complete VEGF inhibition for the 8-week and quarterly dosing schedule during the 52-week treatment period. In the 16-week schedule, clinically significant VEGF inhibition was maintained during the 52-week period. The model quantitatively described abicipar-VEGF target engagement leading to rapid reduction of VEGF and a long duration of VEGF inhibition demonstrating the clinical feasibility of up to a 16-week dosing interval. Abicipar is predicted to reduce IVT dosing compared with other anti-VEGF therapies with the potential to lessen patient treatment burden.

SIGNIFICANCE STATEMENT

Current anti-VEGF treatments for neovascular age-related macular degeneration require frequent (monthly) intravitreal injections and monitoring, which increases patient burden. We developed a mechanistic pharmakinetic/pharmadynamic model to describe the interaction between abicipar (a novel VEGF inhibitor) and VEGF to evaluate the duration of action. The model demonstrates extended abicipar-VEGF target engagement leading to clinical feasibility of up to a 16-week dosing interval. Our model predicted that abicipar 8-week and quarterly dosing schedules maintain virtually complete VEGF inhibition during the 52-week period.

Imaging DNA Damage Repair In Vivo After 177Lu-DOTATATE Therapy

Molecular radiotherapy using 177Lu-DOTATATE is a most effective treatment for somatostatin receptor–expressing neuroendocrine tumors. Despite its frequent and successful use in the clinic, little or no radiobiologic considerations are made at the time of treatment planning or delivery. On positive uptake on octreotide-based PET/SPECT imaging, treatment is usually administered as a standard dose and number of cycles without adjustment for peptide uptake, dosimetry, or radiobiologic and DNA damage effects in the tumor. Here, we visualized and quantified the extent of DNA damage response after 177Lu-DOTATATE therapy using SPECT imaging with 111In-anti-H2AX-TAT. This work was a proof-of-principle study of this in vivo noninvasive biodosimeter with β-emitting therapeutic radiopharmaceuticals. Methods: Six cell lines were exposed to external-beam radiotherapy (EBRT) or 177Lu-DOTATATE, after which the number of H2AX foci and the clonogenic survival were measured. Mice bearing CA20948 somatostatin receptor–positive tumor xenografts were treated with 177Lu-DOTATATE or sham-treated and coinjected with 111In-anti-H2AX-TAT, 111In-IgG-TAT control, or vehicle. Results: Clonogenic survival after external-beam radiotherapy was cell-line–specific, indicating varying levels of intrinsic radiosensitivity. Regarding in vitro cell lines treated with 177Lu-DOTATATE, clonogenic survival decreased and H2AX foci increased for cells expressing high levels of somatostatin receptor subtype 2. Ex vivo measurements revealed a partial correlation between 177Lu-DOTATATE uptake and H2AX focus induction between different regions of CA20948 xenograft tumors, suggesting that different parts of the tumor may react differentially to 177Lu-DOTATATE irradiation. Conclusion: 111In-anti-H2AX-TAT allows monitoring of DNA damage after 177Lu-DOTATATE therapy and reveals heterogeneous damage responses.

Defining an embryonal rhabdomyosarcoma endotype [RESEARCH ARTICLE]

Rhabdomyosarcoma (RMS) is the most common childhood soft-tissue sarcoma. The largest subtype of RMS is embryonal rhabdomyosarcoma (ERMS) and accounts for 53% of all RMS. ERMS typically occurs in the head and neck region, bladder, or reproductive organs and portends a promising prognosis when localized; however, when metastatic the 5-yr overall survival rate is ~43%. The genomic landscape of ERMS demonstrates a range of putative driver mutations, and thus the recognition of the pathological mechanisms driving tumor maintenance should be critical for identifying effective targeted treatments at the level of the individual patients. Here, we report genomic, phenotypic, and bioinformatic analyses for a case of a 3-yr-old male who presented with bladder ERMS. Additionally, we use an unsupervised agglomerative clustering analysis of RNA and whole-exome sequencing data across ERMS and undifferentiated pleomorphic sarcoma (UPS) tumor samples to determine several major endotypes inferring potential targeted treatments for a spectrum of pediatric ERMS patient cases.

    China’s and Russia’s Interests in Central Asia: Connecting the Dots in Kazakhstan


    Visiting Astana, the modernistic capital of Kazakhstan, last week, I couldn't help feeling that I was at, or at least close to, the center of the universe. 

    Consider this:  On September 7, the president of Kazakhstan, Nursultan Nazarbayev, having just returned from attending the G-20 Summit in St. Petersburg at the invitation of President Putin of Russia, welcomed President Xi Jinping of China for an official visit in Astana. President Xi gave a speech that day at Nazarbayev University, in which he unabashedly borrowed a turn of phrase from former U.S. Secretary of State Hillary Clinton  by proposing a “New Silk Road” to serve as an “economic belt” of Eurasia, connecting “3 million people from the Pacific to the Baltic Sea” with Kazakhstan as a key partner along the way. 

    On September 10, President Nazarbayev opened the Eurasian Emerging Markets Forum in Astana, at which he addressed some 800 participants, including high-level dignitaries and representatives from 87 countries.  In his keynote speech, he laid out his plans to catapult Kazakhstan into the ranks of the top 30 developed countries in the world by 2050.  The rest of the forum was devoted to exploring the ways in which this ambitious vision could be achieved and how economic integration of the Eurasian supercontinent—i.e., Europe plus Asia, with Kazakhstan at its center—would be a driver of regional and global prosperity. 

    Finally, on September 13, President Nazarbayev joined the leaders of China, Russia and the five Central Asian republics in Bishkek for a summit of the Shanghai Cooperation Organization (SCO), which was also attended by a number of other regional leaders with observer status, including from Afghanistan, India, Iran and Pakistan.  Besides the usual pledges of good neighborly relations within the group, the leaders weighed in with a chorus of statements about current geopolitical trouble spots, including Afghanistan, Iran and Syria, many of them directed critically at the United States.

    While the president and people of Kazakhstan might have felt at the center of global action this week, there is little doubt that China and Russia are the key external actors on the Central Asian stage.  Europe and the United States are far away and hardly visible, and everybody expects that, with the imminent end of NATO’s engagement in Afghanistan, their attention to Central Asia will slip even further.  In contrast, the leaders of China and Russia are clearly focused on this region.  

    Central Asian leaders, while perhaps privately worried about the long-term consequences of too tight an embrace by China, welcome the low-key approach of their big neighbor...

    If there had been any doubt, President Xi’s speech in Astana showed that China is now concerned with Central Asia at the highest level.  While China faces its neighbors in the Pacific region in an assertive pose designed to counter what it sees as encirclement by unfriendly countries led by the U.S., it evidently feels no threat in Central Asia and projects an image of itself as benevolent and modest senior partner.  No doubt sensing opportunities to create a stable backyard, to secure access to energy resources and to build a land bridge to European and Middle Eastern markets while also gently wresting influence away from Russia, China has a strong incentive to push westward.  The substantial energy supply deals  that President Xi signed in Kazakhstan, Turkmenistan and Uzbekistan this past week and the stress Xi placed in his Astana speech on measures to open up transport links throughout Eurasia reflect China’s growing engagement in this region.  Central Asian leaders, while perhaps privately worried about the long-term consequences of too tight an embrace by China, welcome the low-key approach of their big neighbor, which promises to strengthen their own hand economically and politically at least in the short term.

    At the same time, there is also a new dynamic between Central Asia and Russia.  Since Mr. Putin resumed the Russian presidency in 2012, Russia has breathed new life into a long-dormant regional grouping, the Eurasian Economic Community (EurAsEC), by pushing hard to create a customs union  (and eventually an economic union) that, in Russia’s view, would encompass most of the republics of the former Soviet Union. Although only a fraction of the geographic space of continental Eurasia (Europe + Asia), the reference to “Eurasia” harks back to a long-standing Russian ideological vision.  Under this vision, Russia and its former Soviet neighbors are endowed with a unique combination of European and Asian values and, led by Russia, with a mission to dominate the land bridge between Europe and Asia. 

    In the pursuit of establishing a unified economic “Eurasian” space, Russia has not only successfully pushed for the full implementation of the current customs union between Russia, Kazakhstan and Belorussia, but is also vigorously pursuing the expansion of the union in Ukraine, Central Asia (specifically targeting the Kyrgyz Republic and Tajikistan) and Armenia in the South Caucasus.  In the case of Armenia and Ukraine, this pursuit has taken on a decidedly anti-European Union tone, as Russia seems to spare no effort to ensure that these countries will join its own economic orbit, rather than associating with the EU.  In Central Asia, the Russian campaign of expanding the customs union has been more low key, but nonetheless persistent with the quiet support of Kazakhstan.  Interestingly, this effort to create a unified economic space has not been cast by Russia as a move to counteract the growing influence of China in Central Asia, even though it is undoubtedly one of the underlying long-term motives for Russian diplomacy in the region.  

    Much more important for China will be whether the “Eurasian” economic union can create safe, low-cost and high-speed transit routes to China’s key trading partners in Europe, South Asia and the Middle East.

    Indeed, for Central Asia in general and for Kazakhstan in particular, the important questions for the future will be how China and Russia shape their mutual relations overall and how they will seek to accommodate their overlapping interests in the region.  For the moment, a common geopolitical front vis-à-vis the U.S., evident in their joint positions at the U.N. Security Council and at the SCO summit last week, is an overarching priority for China and Russia.  Moreover, they share the common interest of establishing a stable and prosperous political and economic sphere in Central Asia.  For now and the foreseeable future, China’s thirst for energy is large enough to allow both Russia and Central Asian countries to pursue opportunities for major oil and gas supply deals with China without undue competition. Finally, whatever protectionist effects an expansion of the Russian-led customs union may have in limiting trade between China and Central Asia will likely be temporary and will hardly be noticed in China’s huge overall trade account.  Much more important for China will be whether the “Eurasian” economic union can create safe, low-cost and high-speed transit routes to China’s key trading partners in Europe, South Asia and the Middle East. This priority strongly resonated in President Xi’s speech, in which he not only staked out an interest in Eurasian economic integration, but also promised greater cooperation between the SCO and EurAsEC.

    What does all of this mean in practical terms for Central Asia and for Kazakhstan?  As President Nazarbayev indicated in his speech at the Eurasian Emerging Markets Forum, he sees Kazakhstan as playing a key role in supporting the economic integration of larger Eurasia.  This presumably should mean: investing in regional infrastructure, such as the major East-West Highway through Kazakhstan as a link from China to Europe; assuring that the customs union pursues open, rather than protectionist, policies; and convincing the other Central Asian countries, including Uzbekistan and Turkmenistan, to participate in an effort to increase the region’s connectivity both internally and with the rest of the world. 

    In addition, there are a number of institutional options for promoting these goals and for turning China’s and Russia’s engagement in Central Asia into a pragmatic partnership.  One option would be to have China join the Eurasian Development Bank (EADB), the financial arm of EurAsEC.  Another would be for Russia to join the Central Asian Regional Economic Cooperation Program (CAREC), in which China has teamed up with Central Asian countries (now also including Afghanistan, Mongolia and Pakistan) and with six international financial organizations (including the Asian Development Bank and the World Bank) with the goal of improving regional cooperation and investment in trade, transport and energy.  Either or both of these two options could then offer SCO a financial and technical institutional platform to pursue economic integration between China, Russia and Central Asia (and, ultimately, even South Asia), a goal that has eluded SCO up until now. 

    Kazakhstan is a member of EurAsEc, EADB, CAREC and SCO, and is therefore in a unique position to promote institutional changes along some or all of these lines.  One place to start would be the next ministerial conference of CAREC, to be held in Astana on October 24-25.  Of course, it is by no means clear that China and Russia will see it in their interest to dilute their lead roles in EADB and CAREC, the regional organizations that they now respectively dominate.  However, establishing a strong and meaningful institutional capacity that would support the economic integration process in Central Asia and in the larger Eurasia would be of great benefit for Kazakhstan, since it would help turn the country from being “land-locked” to being “land-linked” with the world’s largest and most dynamic economies.

    The lull in incidents involving doctors and relatives of patients has been broken once again. An indefinite strike called by resident doctors of the civic-run JJ Hospital entered its second day on the weekend. The doctors held a candlelight march to protest the attack on their colleagues and to pressure the management to take action to ensure their safety.

    Doctors from other hospitals were supporting the protest as well. It started with over 400 resident doctors from JJ Hospital going on strike over the weekend after two were attacked by the family of a deceased patient.

    We had seen these flare ups and thought that a long lull may spell the end of the doc-patients' fracas, which has been a very worrisome aspect of the health service sector in the country. It was not to be and the latest incident shows that measures have to be in place to tackle these incidents before they spiral out of control.
    Doctors must have adequate security at hospitals. Fears for safety can inadvertently crop up in medical decisions or diagnosis, where sometimes a professional's judgement may be compromised if they think that their workplace does not care enough to protect them.

    Violence is not the answer even if the relatives think that the doctors have neglected them or are shocked by the death of their family member. Our civic hospitals have too many patients, but staff is small is comparison. Awareness campaigns, an intermediary between doctors and patients' family, security measures, counselling, and somehow bringing the sense that patients' relatives and doctors are on the same side, could defuse volatile situations. The time for talk, discussion and clucking in disappointment is over. This is a medical emergency at our hospitals. An antidote needs to be found.

    Sachin Tendulkar reveals interesting McGrath anecdote from '99 Adelaide Test

    Former India cricketer Sachin Tendulkar spoke regarding an interesting story from the 1999 Adelaide Test featuring former Australia speedster Glenn McGrath. Tendulkar revealed how he managed to win a game of chess on the field against the player of McGrath's quality.

    The 'Master Blaster' explained how he first remained calm and patient when McGrath was in full flow but cut loose the next morning and put pressure back on the bowler.

    "In 1999, in our first match at Adelaide... the first innings there was hardly 40 minutes to go in the day's play. Glenn McGrath comes and bowls five or six maiden overs to me," Tendulkar told Star Sports during a video chat that was tweeted by BCCI.

    "That was their strategy... 'let's frustrate Sachin. 70 per cent of the ball should go to (wicket-keeper) Adam Gilchrist and 10 per cent close to his bat. If he plays or chases deliveries outside the off-stump we have succeeded'.

    "So I kept leaving as many deliveries as possible. There were some good deliveries where I was beaten as well. But I said 'well bowled and now go back and bowl again as I am still here'," he added.

    "I remember hitting him for a few boundaries the next morning because it was a fresh day and we both were at the same level. They had a strategy but I knew their strategy was to frustrate me.

    "I thought this evening I am patient but tomorrow morning I am gonna play the way I want. You won't control how I want to play but I would control where you are going to bowl."

    Tendulkar also had a word of advice for current cricketer as to how to keep themselves mentally fit during lockdown. Due to coronavirus pandemic, all cricket across the world has been postponed for the time being and Tendulkar stated sometimes it is good to stay away from the game for a brief period.

    "First of all I would like to tell them to recharge their respective batteries. It is important to have some off time. When you are playing regularly it is not easy to be at the top of your game. It is good to move away from cricket for a while to recharge your batteries," he said.
