The purpose of this study was to evaluate time until therapeutic intervention with POCT HbA1c versus venipuncture, where venipuncture was considered standard of care. The primary outcome was time (hours) to implementation of a therapeutic intervention based on POCT HbA1c result, as compared with most recent venipuncture HbA1c before the study and its associated therapeutic intervention. A total of 94 POCT HbA1c tests were included in the primary analysis. For the POCT HbA1c, the mean time to therapeutic intervention was 1.6 ± 3.14 hours. For the previous venipuncture HbA1c, the mean time to therapeutic intervention was 1376.66 ± 3356.6 hours (P < .001). Overall, this trial showed that POCT HbA1c results in a significantly faster time to therapeutic intervention than venipuncture in a primary care clinic that serves a rural population. Full Article

The expiratory phase of airway pressure release ventilation (APRV) may provide physiologic information about lung volume based on the expiratory flow-time slope, angle, and time to approach a no-flow state (expiratory time [TE]). We hypothesized that expiratory flow would correlate with estimated lung volume (ELV) as measured using a modified nitrogen washout/washin technique in a large-animal lung injury model.METHODS:Eight pigs (35.2 ± 1.0 kg) were mechanically ventilated using an Engström Carescape R860 on the APRV mode. All settings were held constant except the expiratory duration, which was adjusted based on the expiratory flow curve. Abdominal pressure was increased to 15 mm Hg in normal and injured lungs to replicate a combination of pulmonary and extrapulmonary lung injury. ELV was estimated using the Carescape FRC INview tool. The expiratory flow-time slope and TE were measured from the expiratory flow profile.RESULTS:Lung elastance increased with induced lung injury from 29.3 ± 7.3 cm H2O/L to 39.9 ± 15.1cm H2O/L, and chest wall elastance increased with increasing intra-abdominal pressures (IAPs) from 15.3 ± 4.1 cm H2O/L to 25.7 ± 10.0 cm H2O/L in the normal lung and 15.8 ± 6.0 cm H2O/L to 33.0 ± 6.2 cm H2O/L in the injured lung (P = .39). ELV decreased from 1.90 ± 0.83 L in the injured lung to 0.67 ± 0.10 L by increasing IAP to 15 mm Hg. This had a significant correlation with a TE decrease from 2.3 ± 0.8 s to 1.0 ± 0.1 s in the injured group with increasing insufflation pressures (ρ = 0.95) and with the expiratory flow-time slope, which increased from 0.29 ± 0.06 L/s2 to 0.63 ± 0.05 L/s2 (ρ = 0.78).CONCLUSIONS:Changes in ELV over time, and the TE and flow-time slope, could be used to demonstrate evolving lung injury during APRV. Using the slope to infer changes in functional lung volume represents a unique, reproducible, real-time, bedside technique that does not interrupt ventilation and may be used for clinical interpretation. Rarely, however, are methods used to objectively determine the model that best characterizes the data. Additionally, the uncertainty associated with the resultant TIAC is generally not evaluated. As part of the MIRDsoft initiative, MIRDfit has been developed to offer a biodistribution fitting software solution that provides the following essential features and advantages for internal dose assessment: nuclear medicine–appropriate fit functions; objective metrics for guiding best-fit selection; TIAC uncertainty calculation; quality control and data archiving; integration with MIRDcalc software for dose calculation; and a user-friendly Excel-based interface. For demonstration and comparative validation of MIRDfit’s performance, TIACs were derived from serial imaging studies involving 18F-FDG and 177Lu-DOTATATE using MIRDfit. These TIACs were then compared with TIAC estimates obtained using other software. In most cases, the TIACs agreed within approximately 10% between MIRDfit and the other software. Herein, we present our preliminary findings on the evaluation of a deep learning–optimized sequence using T1-weighted imaging. MATERIALS AND METHODS: Clinical and optimized deep learning–based image reconstruction T1 3D Sampling Perfection with Application optimized Contrast using different flip angle Evolution (SPACE) were evaluated, comparing noncontrast sequences in 10 healthy controls and postcontrast sequences in 5 consecutive patients. Images were reviewed on a Likert-like scale by 4 fellowship-trained neuroradiologists. Scores (range, 1–4) were separately assigned for 11 vessel segments in terms of vessel wall and lumen delineation. Additionally, images were evaluated in terms of overall background noise, image sharpness, and homogeneous CSF signal. Segment-wise scores were compared using paired samples t tests. RESULTS: The scan time for the clinical and deep learning–based image reconstruction sequences were 7:26 minutes and 5:23 minutes respectively. Deep learning–based image reconstruction images showed consistently higher wall signal and lumen visualization scores, with the differences being statistically significant in most vessel segments on both pre- and postcontrast images. Deep learning–based image reconstruction had lower background noise, higher image sharpness, and uniform CSF signal. Depiction of intracranial pathologies was better or similar on the deep learning–based image reconstruction. CONCLUSIONS: Our preliminary findings suggest that deep learning–based image reconstruction–optimized intracranial vessel wall imaging sequences may be helpful in achieving shorter gradient times with improved vessel wall visualization and overall image quality. These improvements may help with wider adoption of intracranial vessel wall imaging in clinical practice and should be further validated on a larger cohort. Full Article

Internal moonlighting opportunities for faculty were offered by 70% of divisions, with a median pay of $250 per hour. The median annual academic fund for a full-time neuroradiology faculty member was $6000, typically excluding license fees but including American College of Radiology and American Board of Radiology membership, leaving $4000 for professional expenses. This survey calls for further dialogue on adapting and innovating academic institutions to meet evolving needs in neuroradiology. Full Article

But do you really need to do every single test?Engineers at NXP have developed a machine-learning algorithm that learns the patterns of test results and figures out the subset of tests that are really needed and those that they could safely do without. The NXP engineers described the process at the IEEE International Test Conference in San Diego last week.NXP makes a wide variety of chips with complex circuitry and advanced chip-making technology, including inverters for EV motors, audio chips for consumer electronics, and key-fob transponders to secure your car. These chips are tested with different signals at different voltages and at different temperatures in a test process called continue-on-fail. In that process, chips are tested in groups and are all subjected to the complete battery, even if some parts fail some of the tests along the way.Chips were subject to between 41 and 164 tests, and the algorithm was able to recommend removing 42 to 74 percent of those tests.“We have to ensure stringent quality requirements in the field, so we have to do a lot of testing,” says Mehul Shroff, an NXP Fellow who led the research. But with much of the actual production and packaging of chips outsourced to other companies, testing is one of the few knobs most chip companies can turn to control costs. “What we were trying to do here is come up with a way to reduce test cost in a way that was statistically rigorous and gave us good results without compromising field quality.”A Test Recommender SystemShroff says the problem has certain similarities to the machine learning-based recommender systems used in e-commerce. “We took the concept from the retail world, where a data analyst can look at receipts and see what items people are buying together,” he says. “Instead of a transaction receipt, we have a unique part identifier and instead of the items that a consumer would purchase, we have a list of failing tests.”The NXP algorithm then discovered which tests fail together. Of course, what’s at stake for whether a purchaser of bread will want to buy butter is quite different from whether a test of an automotive part at a particular temperature means other tests don’t need to be done. “We need to have 100 percent or near 100 percent certainty,” Shroff says. “We operate in a different space with respect to statistical rigor compared to the retail world, but it’s borrowing the same concept.”As rigorous as the results are, Shroff says that they shouldn’t be relied upon on their own. You have to “make sure it makes sense from engineering perspective and that you can understand it in technical terms,” he says. “Only then, remove the test.”Shroff and his colleagues analyzed data obtained from testing seven microcontrollers and applications processors built using advanced chipmaking processes. Depending on which chip was involved, they were subject to between 41 and 164 tests, and the algorithm was able to recommend removing 42 to 74 percent of those tests. Extending the analysis to data from other types of chips led to an even wider range of opportunities to trim testing.The algorithm is a pilot project for now, and the NXP team is looking to expand it to a broader set of parts, reduce the computational overhead, and make it easier to use.
