Up to this year, phubb sent HTTP requests (GET + POST) with file_get_contents() and a HTTP stream context - see my previous example. But then I needed a 100% correct way of detecting a page's Hub URL, and copied the code from phinde, my blog search engine. With that I introduced a dependency to PEAR's good old HTTP_Request2 library and I decided to use that library for all requests. (No idea if the cURL one has it; I do not want to rely on php-curl). Finding a new HTTP library was the only option. New HTTP library The PHP Framework Interop Group has several HTTP-related proposals; one of them PSR-18: HTTP Client. Now that we have a standardized way to send HTTP requests in 2020, I should use a library that implements it. The psr-18 topic on Github listed some clients: Symfony's HTTP client was among them, and it provides a mock client for unit tests! Unfortunately, it also introduces a million dependencies. There were two others that looked ok-ish on first sight (diciotto and http-client-curl) but both of them had no mock client, and the latter was even curl only. Again nothing for me. Then I found PHP-HTTP that promises a standard interface for HTTP clients in PHP, and it supports PSR-18! So I'm just starting right off here, who can actually do an RFC? Is it anybody on the internet? Derick Rethans 1:38 Yeah, pretty much. In order to be able to do an RFC, what you would need is you need to have an idea. And then you need access to our wiki system to be able to actually start writing that, well not to write them, to publish it. The RFC process is open for everybody. In the last year and a half or so, some of the podcasts that I've done have been with people that have been contributing to PHP for a long time. But in other cases, it's people like yourself that have an idea, come up, work together with somebody to work on a patch, and then create an RFC out of that. And that's then goes through the whole process. And sometimes they get accepted, and sometimes they don't. Henrik Gemal 2:16 How technical are the RFCs? Is it like coding? Or is it more like the idea in general? Derick Rethans 2:23 The idea needs to be there, it needs to be thought out. It needs to have a good reason for why we want to add or change something in PHP. The motivation is almost as important as what the change or addition actually is about. Now, that doesn't always get us here at variable. In my opinion, but that is an important thing. Now with the idea we need to talk about what changes it has on the rest of the ecosystem, whether they are backward compatible breaks in there, how it effects extensions, or sometimes how it effects OPCache. Sometimes considerations have to be taken for that because it's, it's something quite important in the PHP ecosystem. And it is recommended that it comes with a patch, because it's often a lot easier to talk about an implementation than to talk about the idea. But that is not a necessity. There have been quite some RFCs where the idea was there. But it wasn't a patch right away yet. Larry Garfield 1:34 As you said, there's been a lot of discussion around improving PHP's general user experience of working with objects in PHP. Where there's definitely room for improvement, no question. And I found a lot of these to be useful in their own right, but also very narrow and narrow in ways that solve the immediate problem but could get in the way of solving larger problems later on down the line. So I went into this with an attitude of: Okay, we can kind of piecemeal and attack certain parts of the problem space. Or we can take a step back and look at the big picture and say: Alright, here's all the pain points we have. What can we do that would solve not just this one pain point. But let us solve multiple pain points with a single change? Or these two changes together solve this other pain point as well. Or, you know, how can we do this in a way that is not going to interfere with later development that we've talked about. We know we want to do, but isn't been done yet. So how do we not paint ourselves into a corner by thinking too narrow? Derick Rethans 2:41 It's a curious thing, because a more narrow RFC is likely easier to get accepted, because it doesn't pull in a whole set of other problems as well. But of course, as you say, if the whole idea hasn't been thought through, then some of these things might not actually end up being beneficial. Because it can be combined with some other things to directly address the problems that we're trying to solve, right? George Banyard 0:52 Currently when you do a float to string conversion, so all casting or displaying a float, the conversion will depend on like the current local. So instead of always using like the decimal dot separator. For example, if you have like a German or the French locale enabled, it will use like a comma to separate like the decimals. Derick Rethans 1:14 Okay, I can understand that that could be a bit confusing. What are these locales exactly? George Banyard 1:20 So locales, which are more or less C locales, which PHP exposes to user land is a way how to change a bunch of rules on how string and like stuff gets displayed on the C level. One of the issues with it is that like it's global. For example, if you use like a thread safe API, if you use the thread safe PHP version, then set_locale() is not thread safe, so we'll just like impact other threads where you're using it. Derick Rethans 1:50 So a locale is a set of rules to format specific things with floating point numbers being one of them in which situations does the locale influence the display a floating point numbers in every situation in PHP or only in some? George Banyard 2:06 Yes, it only impacts like certain aspects, which is quite surprising. So a string cast will affect it the strval() function, vardump(), and debug_zval_dump() will all affect the decimal locator and also printf() with the percentage lowercase F, but that's expected because it's locale aware compared to the capital F modifier. Derick Rethans 2:32 But it doesn't, for example, have the same problem in the serialised function or say var_export(). George Banyard 2:37 Yeah, and json_encode() also doesn't do that. PDO has special code which handles also this so that like all the PDO drivers get like a constant treat like float string, because that could like impact on the databases. ARID1A is a core member of the polymorphic BRG/BRM-associated factor chromatin remodeling complex. ARID1A mutations occur in human cancers and drive cancer development. Here, we studied the molecular, cellular, and clinical impact of ARID1A aberrations on cancer immunity. We demonstrated that ARID1A aberrations resulted in limited chromatin accessibility to IFN-responsive genes, impaired IFN gene expression, anemic T cell tumor infiltration, poor tumor immunity, and shortened host survival in many human cancer histologies and in murine cancer models. Impaired IFN signaling was associated with poor immunotherapy response. Mechanistically, ARID1A interacted with EZH2 via its carboxyl terminal and antagonized EZH2-mediated IFN responsiveness. Thus, the interaction between ARID1A and EZH2 defines cancer IFN responsiveness and immune evasion. Our work indicates that cancer epigenetic driver mutations can shape cancer immune phenotype and immunotherapy. Full Article

In this issue of the JCI, Li et al. suggest an answer to this vexing problem, while exploring why the Elp1 gene that is mutated in familial dysautonomia (FD) causes peripheral neuropathy. They describe a distinctive function of Elp1 as a protein that is required to sustain NGF signaling by blocking the activity of its phosphatase that shuts off those signals. This finding helps explain the innervation deficits prominent in FD and reveals a unique role for Elp1 in the regulation of NGF-dependent TrkA activity. Full Article
ph GPR160 de-orphanization reveals critical roles in neuropathic pain in rodents By www.jci.org Published On :: Treating neuropathic pain is challenging and novel non–opioid-based medicines are needed. Using unbiased receptomics, transcriptomic analyses, immunofluorescence, and in situ hybridization, we found that the expression of the orphan GPCR Gpr160 and GPR160 increased in the rodent dorsal horn of the spinal cord following traumatic nerve injury. Genetic and immunopharmacological approaches demonstrated that GPR160 inhibition in the spinal cord prevented and reversed neuropathic pain in male and female rodents without altering normal pain response. GPR160 inhibition in the spinal cord attenuated sensory processing in the thalamus, a key relay in the sensory discriminative pathways of pain. We also identified cocaine- and amphetamine-regulated transcript peptide (CARTp) as a GPR160 ligand. Inhibiting endogenous CARTp signaling in spinal cord attenuated neuropathic pain, whereas exogenous intrathecal CARTp evoked painful hypersensitivity through GPR160-dependent ERK and cAMP response element–binding protein (CREB). Our findings de-orphanize GPR160, identify it as a determinant of neuropathic pain and potential therapeutic target, and provide insights into its signaling pathways. CARTp is involved in many diseases including depression and reward and addiction; de-orphanization of GPR160 is a major step forward understanding the role of CARTp signaling in health and disease. LNSC-produced laminin α5 (Lama5) regulates CD4+ T cells but the underlying mechanisms of its functions are poorly understood. Here we show that depleting Lama5 in LNSCs resulted in decreased Lama5 protein in the LN cortical ridge (CR) and around high endothelial venules (HEVs). Lama5 depletion affected LN structure with increased HEVs, upregulated chemokines, and cell adhesion molecules, and led to greater numbers of Tregs in the T cell zone. Mouse and human T cell transendothelial migration and T cell entry into LNs were suppressed by Lama5 through the receptors α6 integrin and α-dystroglycan. During immune responses and allograft transplantation, depleting Lama5 promoted antigen-specific CD4+ T cell entry into the CR through HEVs, suppressed T cell activation, and altered T cell differentiation to suppressive regulatory phenotypes. Enhanced allograft acceptance resulted from depleting Lama5 or blockade of T cell Lama5 receptors. Lama5 and Lama4/Lama5 ratios in allografts were associated with the rejection severity. Overall, our results demonstrated that stromal Lama5 regulated immune responses through altering LN structures and T cell behaviors. ph Phase I trial of donor-derived modified immune cell infusion in kidney transplantation By www.jci.org Published On :: BACKGROUND Preclinical experiments have shown that donor blood cells, modified in vitro by an alkylating agent (modified immune cells [MICs]), induced long-term specific immunosuppression against the allogeneic donor.METHODS In this phase I trial, patients received either 1.5 × 106 MICs per kg BW on day –2 (n = 3, group A), or 1.5 × 108 MICs per kg BW on day –2 (n = 3, group B) or day –7 (n = 4, group C) before living donor kidney transplantation in addition to post-transplantation immunosuppression. The primary outcome measure was the frequency of adverse events (AEs) until day 30 (study phase) with follow-up out to day 360.RESULTS MIC infusions were extremely well tolerated. During the study phase, 10 treated patients experienced a total of 69 AEs that were unlikely to be related or not related to MIC infusion. No donor-specific human leukocyte antigen Abs or rejection episodes were noted, even though the patients received up to 1.3 × 1010 donor mononuclear cells before transplantation. Group C patients with low immunosuppression during follow-up showed no in vitro reactivity against stimulatory donor blood cells on day 360, whereas reactivity against third-party cells was still preserved. Frequencies of CD19+CD24hiCD38hi transitional B lymphocytes (Bregs) increased from a median of 6% before MIC infusion to 20% on day 180, which was 19- and 68-fold higher, respectively, than in 2 independent cohorts of transplanted controls. The majority of Bregs produced the immunosuppressive cytokine IL-10. ph It’s not all about muscle: fibroadipogenic progenitors contribute to facioscapulohumeral muscular dystrophy By www.jci.org Published On :: Facioscapulohumeral muscular dystrophy (FSHD) results from expression of the full-length double homeobox 4 (DUX4-FL) retrogene in skeletal muscle. However, even in cases of severe FSHD the presence of DUX4 is barely detectable. In this issue of the JCI, Bosnakovski et al. used an inducible, muscle-specific human DUX4 to reproduce the low-level, sporadic DUX4 expression of human FSHD muscle as well the myopathology seen in human FSHD disease. Notably, dysregulated fibroadipogenic progenitors accumulated in affected muscles, thus providing a mechanism for the replacement of muscle by fibrosis and fat. Full Article
ph Dysfunctional polycomb transcriptional repression contributes to lamin A/C–dependent muscular dystrophy By www.jci.org Published On :: Lamin A is a component of the inner nuclear membrane that, together with epigenetic factors, organizes the genome in higher order structures required for transcriptional control. Mutations in the lamin A/C gene cause several diseases belonging to the class of laminopathies, including muscular dystrophies. Nevertheless, molecular mechanisms involved in the pathogenesis of lamin A–dependent dystrophies are still largely unknown. The polycomb group (PcG) of proteins are epigenetic repressors and lamin A interactors, primarily involved in the maintenance of cell identity. Using a murine model of Emery-Dreifuss muscular dystrophy (EDMD), we show here that lamin A loss deregulated PcG positioning in muscle satellite stem cells, leading to derepression of non–muscle-specific genes and p16INK4a, a senescence driver encoded in the Cdkn2a locus. This aberrant transcriptional program caused impairment in self-renewal, loss of cell identity, and premature exhaustion of the quiescent satellite cell pool. Genetic ablation of the Cdkn2a locus restored muscle stem cell properties in lamin A/C–null dystrophic mice. Our findings establish a direct link between lamin A and PcG epigenetic silencing and indicate that lamin A–dependent muscular dystrophy can be ascribed to intrinsic epigenetic dysfunctions of muscle stem cells. ph Cytotoxic CD4+ T lymphocytes may induce endothelial cell apoptosis in systemic sclerosis By www.jci.org Published On :: Systemic sclerosis (SSc) is an autoimmune fibrotic disease whose pathogenesis is poorly understood and lacks effective therapies. We undertook quantitative analyses of T cell infiltrates in the skin of 35 untreated patients with early diffuse SSc and here show that CD4+ cytotoxic T cells and CD8+ T cells contribute prominently to these infiltrates. We also observed an accumulation of apoptotic cells in SSc tissues, suggesting that recurring cell death may contribute to tissue damage and remodeling in this fibrotic disease. HLA-DR–expressing endothelial cells were frequent targets of apoptosis in SSc, consistent with the prominent vasculopathy seen in patients with this disease. A circulating effector population of cytotoxic CD4+ T cells, which exhibited signatures of enhanced metabolic activity, was clonally expanded in patients with systemic sclerosis. These data suggest that cytotoxic T cells may induce the apoptotic death of endothelial and other cells in systemic sclerosis. Cell loss driven by immune cells may be followed by overly exuberant tissue repair processes that lead to fibrosis and tissue dysfunction. Full Article
ph Leaking chemokines confuse neutrophils By www.jci.org Published On :: The physical integrity of endothelial cells (ECs) lining the blood vessels regulates the inflammatory response. Both innate immunity and inflammatory disorders hinge on the EC-neutrophil interaction. Neutrophil binding, rolling, and migrating along and between ECs is associated with vascular permeability. In this issue of the JCI, Owen-Woods et al. tracked neutrophils in vivo in venules of mouse striated muscle and revealed how endothelial permeability can affect neutrophil trafficking. Strikingly, many neutrophils that migrated between EC junctions were able to rejoin the blood circulation. Further, the chemokine and neutrophil chemoattractant, CXCL1, drove this reverse transendothelial migration (rTEM). This paradigm-shifting study provides a mechanism for distal organ damage as well as an explanation for sepsis-associated acute respiratory distress syndrome. Full Article
ph Donor glucose-6-phosphate dehydrogenase deficiency decreases blood quality for transfusion By www.jci.org Published On :: BACKGROUND Glucose-6-phosphate dehydrogenase (G6PD) deficiency decreases the ability of red blood cells (RBCs) to withstand oxidative stress. Refrigerated storage of RBCs induces oxidative stress. We hypothesized that G6PD-deficient donor RBCs would have inferior storage quality for transfusion as compared with G6PD-normal RBCs.METHODS Male volunteers were screened for G6PD deficiency; 27 control and 10 G6PD-deficient volunteers each donated 1 RBC unit. After 42 days of refrigerated storage, autologous 51-chromium 24-hour posttransfusion RBC recovery (PTR) studies were performed. Metabolomics analyses of these RBC units were also performed.RESULTS The mean 24-hour PTR for G6PD-deficient subjects was 78.5% ± 8.4% (mean ± SD), which was significantly lower than that for G6PD-normal RBCs (85.3% ± 3.2%; P = 0.0009). None of the G6PD-normal volunteers (0/27) and 3 G6PD-deficient volunteers (3/10) had PTR results below 75%, a key FDA acceptability criterion for stored donor RBCs. As expected, fresh G6PD-deficient RBCs demonstrated defects in the oxidative phase of the pentose phosphate pathway. During refrigerated storage, G6PD-deficient RBCs demonstrated increased glycolysis, impaired glutathione homeostasis, and increased purine oxidation, as compared with G6PD-normal RBCs. In addition, there were significant correlations between PTR and specific metabolites in these pathways.CONCLUSION Based on current FDA criteria, RBCs from G6PD-deficient donors would not meet the requirements for storage quality. ph Marked and rapid effects of pharmacological HIF-2α antagonism on hypoxic ventilatory control By www.jci.org Published On :: Hypoxia-inducible factor (HIF) is strikingly upregulated in many types of cancer, and there is great interest in applying inhibitors of HIF as anticancer therapeutics. The most advanced of these are small molecules that target the HIF-2 isoform through binding the PAS-B domain of HIF-2α. These molecules are undergoing clinical trials with promising results in renal and other cancers where HIF-2 is considered to be driving growth. Nevertheless, a central question remains as to whether such inhibitors affect physiological responses to hypoxia at relevant doses. Here, we show that pharmacological HIF-2α inhibition with PT2385, at doses similar to those reported to inhibit tumor growth, rapidly impaired ventilatory responses to hypoxia, abrogating both ventilatory acclimatization and carotid body cell proliferative responses to sustained hypoxia. Mice carrying a HIF-2α PAS-B S305M mutation that disrupts PT2385 binding, but not dimerization with HIF-1β, did not respond to PT2385, indicating that these effects are on-target. Furthermore, the finding of a hypomorphic ventilatory phenotype in untreated HIF-2α S305M mutant mice suggests a function for the HIF-2α PAS-B domain beyond heterodimerization with HIF-1β. Although PT2385 was well tolerated, the findings indicate the need for caution in patients who are dependent on hypoxic ventilatory drive. Full Article
ph Local microvascular leakage promotes trafficking of activated neutrophils to remote organs By www.jci.org Published On :: Increased microvascular permeability to plasma proteins and neutrophil emigration are hallmarks of innate immunity and key features of numerous inflammatory disorders. Although neutrophils can promote microvascular leakage, the impact of vascular permeability on neutrophil trafficking is unknown. Here, through the application of confocal intravital microscopy, we report that vascular permeability–enhancing stimuli caused a significant frequency of neutrophil reverse transendothelial cell migration (rTEM). Furthermore, mice with a selective defect in microvascular permeability enhancement (VEC-Y685F-ki) showed reduced incidence of neutrophil rTEM. Mechanistically, elevated vascular leakage promoted movement of interstitial chemokines into the bloodstream, a response that supported abluminal-to-luminal neutrophil TEM. Through development of an in vivo cell labeling method we provide direct evidence for the systemic dissemination of rTEM neutrophils, and showed them to exhibit an activated phenotype and be capable of trafficking to the lungs where their presence was aligned with regions of vascular injury. Collectively, we demonstrate that increased microvascular leakage reverses the localization of directional cues across venular walls, thus causing neutrophils engaged in diapedesis to reenter the systemic circulation. This cascade of events offers a mechanism to explain how local tissue inflammation and vascular permeability can induce downstream pathological effects in remote organs, most notably in the lungs.
