She describes her victory as a testament to resilience, determination and the realization of the American dream. Standing in the State Capitol for the first time on the morning of her orientation, Hiltsley told VOA she was overwhelmed with emotions and eager to start her journey as an elected official. “I am super excited,” Hiltsley said. “Today is orientation day for new legislators, and to be standing in this Capitol as an African immigrant woman is a tremendous honor. I’m just overexcited right now.” She said this milestone did not come easily. Her path to the Minnesota State Capitol was marked by struggles, including a fight against an immigration system that nearly led to her family’s deportation. She credits much of her success to the community support and the intervention of the late U.S. Senator Paul Wellstone, whose advocacy she said ultimately secured her family’s green cards and, later, citizenship. “Getting to this moment honestly is just a testament to the struggles that my family has gone through to be in this country,” she said. There has been a media frenzy surrounding Hiltsley’s victory and it has captured the attention of Kenyan media, with celebrations taking place in her hometown, Nyamemiso village, which is eight hours from Nairobi, Kenya's capital. Despite the attention, Hiltsley said she remains humble. “I’m that little girl from that little village somewhere in the middle of Kenya, and now I’m in the spotlight of this media frenzy. And so, I’m still trying to really appreciate the magnitude of the moment,” she said. Hiltsley said she has a desire to inspire others, particularly young girls in Kenya and the United States. "It is still surreal,” she said, adding that “if I can make it to this point, I can be a role model to somebody to remind them that it is possible that our wildest dreams are possible. And that would be something that I would look back and say, wow, I’ve made a difference in somebody’s life.” Her legislative priorities Looking ahead, Hiltsley said she is committed to championing issues that matter to her constituents in Minnesota’s Legislative District 38A. Her priorities include community-centered public safety policies, affordable housing options, workers’ rights and support for small businesses — many of which are run by African immigrants. “The resources are out here,” she said, promising to empower her community. “It is my job to go back to my community and tell them, hey, there are resources here. This is how this system works. Let’s work together to mobilize and make sure that we are also taking a piece of the pie,” she said. As the first Kenyan American woman in Minnesota's Legislature, Hiltsley said she recognizes the weight and responsibility of her position. Vietnam's government and local businesses have expressed concern about the impact of Chinese online platforms on local markets due to deep discounting. The trade ministry has also said it is worried about the potential for the sale of counterfeit items. Nguyen Hoang Long, Vietnam's deputy trade minister, told a government meeting at the weekend that the ministry had worked with both Shein and Temu on the licensing matter. "After the ministry's notification, if these platforms do not comply, the Ministry of Industry and Trade will coordinate with relevant agencies to implement technical measures such as blocking applications and domains," Long said in a government statement. Shein and Temu did not respond immediately to a request for comment. Fast-fashion retailer Shein has been selling into Vietnam for at least two years, while Temu, owned by Chinese e-commerce giant PDD Holdings, started allowing users in Vietnam to shop last month. Vietnam allows imported goods of up to $40 to be exempt from a value-added tax. The finance ministry said most items benefiting from this tax break are imported via e-commerce platforms and it is considering terminating the tax break. Both Temu and Shein are also facing increased scrutiny and legal challenges elsewhere. Last month, Indonesia requested Apple and Google block Temu from their app stores to protect small merchants from competing with ultra-cheap items. Vietnam's e-commerce market has grown 18% this year to be worth $22 billion, the third-largest in Southeast Asia behind Indonesia and Thailand, according to a report by Google, Temasek and Bain & Company released last week. This study investigated patient clinical characteristics associated with exacerbation frequency. Methods A cross-sectional cohort study of patients ≥18 years with bronchiectasis enrolled in the US Bronchiectasis and Nontuberculous Mycobacteria (NTM) Research Registry (BRR) September 2008–March 2020. Patients were stratified by exacerbation frequency in their 2 years before enrolment. Patient demographics, respiratory symptoms, healthcare resource utilisation, microbiology, modified bronchiectasis severity index (mBSI) and select comorbidities were collected at enrolment. Patient characteristics associated with exacerbation frequency were assessed using a negative binomial model. Results The study included 2950 patients (mean age 65.6 years; 79.1% female). Frequency of moderate to severe airway obstruction (forced expiratory volume in 1 s (FEV1) % predicted <50%; most recent measure) was 15.9%, 17.8%, and 24.6% in patients with 1, 2, and ≥3 exacerbations versus 8.9% in patients with 0 exacerbations; severe disease (mBSI) was 27.8%, 24.2% and 51.1% versus 13.2%; respiratory hospitalisation was 24.5%, 33.0% and 36.5% versus 4.1%; and Pseudomonas aeruginosa infection was 18.8%, 23.4% and 35.2% versus 11.9%. In multivariable model analysis, respiratory hospitalisation, cough, haemoptysis, P. aeruginosa, younger age, lower FEV1% predicted, asthma, and gastro-oesophageal reflux disease were associated with more exacerbations. Conclusions These findings demonstrate a high disease burden, including increased respiratory symptoms, healthcare resource utilisation, and P. aeruginosa infection in patients with bronchiectasis and multiple exacerbations. gis The Updated Registry of Fast Myocardial Perfusion Imaging with Next-Generation SPECT (REFINE SPECT 2.0) By jnm.snmjournals.org Published On :: 2024-11-01T04:25:31-07:00 The Registry of Fast Myocardial Perfusion Imaging with Next-Generation SPECT (REFINE SPECT) has been expanded to include more patients and CT attenuation correction imaging. We present the design and initial results from the updated registry. Methods: The updated REFINE SPECT is a multicenter, international registry with clinical data and image files. SPECT images were processed by quantitative software and CT images by deep learning software detecting coronary artery calcium (CAC). Patients were followed for major adverse cardiovascular events (MACEs) (death, myocardial infarction, unstable angina, late revascularization). Results: The registry included scans from 45,252 patients from 13 centers (55.9% male, 64.7 ± 11.8 y). Correlating invasive coronary angiography was available for 3,786 (8.4%) patients. CT attenuation correction imaging was available for 13,405 patients. MACEs occurred in 6,514 (14.4%) patients during a median follow-up of 3.6 y (interquartile range, 2.5–4.8 y). Patients with a stress total perfusion deficit of 5% to less than 10% (unadjusted hazard ratio [HR], 2.42; 95% CI, 2.23–2.62) and a stress total perfusion deficit of at least 10% (unadjusted HR, 3.85; 95% CI, 3.56–4.16) were more likely to experience MACEs. Patients with a deep learning CAC score of 101–400 (unadjusted HR, 3.09; 95% CI, 2.57–3.72) and a CAC of more than 400 (unadjusted HR, 5.17; 95% CI, 4.41–6.05) were at increased risk of MACEs. Conclusion: The REFINE SPECT registry contains a comprehensive set of imaging and clinical variables. It will aid in understanding the value of SPECT myocardial perfusion imaging, leverage hybrid imaging, and facilitate validation of new artificial intelligence tools for improving prediction of adverse outcomes incorporating multimodality imaging. gis Automated Volumetric Software in Dementia: Help or Hindrance to the Neuroradiologist? [RESEARCH] By www.ajnr.org Published On :: 2024-11-07T15:14:12-08:00 BACKGROUND AND PURPOSE: Brain atrophy occurs in the late stage of dementia, yet structural MRI is widely used in the work-up. Atrophy patterns can suggest a diagnosis of Alzheimer disease (AD) or frontotemporal dementia (FTD) but are difficult to assess visually. We hypothesized that the availability of a quantitative volumetric brain MRI report would increase neuroradiologists’ accuracy in diagnosing AD, FTD, or healthy controls compared with visual assessment. MATERIALS AND METHODS: Twenty-two patients with AD, 17 with FTD, and 21 cognitively healthy patients were identified from the electronic health systems record and a behavioral neurology clinic. Four neuroradiologists evaluated T1-weighted anatomic MRI studies with and without a volumetric report. Outcome measures were the proportion of correct diagnoses of neurodegenerative disease versus normal aging ("rough accuracy") and AD versus FTD ("exact accuracy"). Generalized linear mixed models were fit to assess whether the use of a volumetric report was associated with higher accuracy, accounting for random effects of within-rater and within-subject variability. Post hoc within-group analysis was performed with multiple comparisons correction. Residualized volumes were tested for an association with the diagnosis using ANOVA. RESULTS: There was no statistically significant effect of the report on overall correct diagnoses. The proportion of "exact" correct diagnoses was higher with the report versus without the report for AD (0.52 versus 0.38) and FTD (0.49 versus 0.32) and lower for cognitively healthy (0.75 versus 0.89). The proportion of "rough" correct diagnoses of neurodegenerative disease was higher with the report than without the report within the AD group (0.59 versus 0.41), and it was similar within the FTD group (0.66 versus 0.63). Post hoc within-group analysis suggested that the report increased the accuracy in AD (OR = 2.77) and decreased the accuracy in cognitively healthy (OR = 0.25). Residualized hippocampal volumes were smaller in AD (mean difference –1.8; multiple comparisons correction, –2.8 to –0.8; P < .001) and FTD (mean difference –1.2; multiple comparisons correction, –2.2 to –0.1; P = .02) compared with cognitively healthy. CONCLUSIONS: The availability of a brain volumetric report did not improve neuroradiologists’ accuracy over visual assessment in diagnosing AD or FTD in this limited sample. Post hoc analysis suggested that the report may have biased readers incorrectly toward a diagnosis of neurodegeneration in cognitively healthy adults. Full Article

The purpose of this study was to assess the trends and distributions of industry payments to neuroradiologists. MATERIALS AND METHODS: Neuroradiologists were identified using a previously-validated method based on Work Relative Value Units and Neiman Imaging Types of Service classification. Data on payments from industry were obtained from the Open Payments database from the Centers for Medicare & Medicaid Services, from 2016 to 2021. Payments were grouped into 7 categories, including consulting fees, education, gifts, medical supplies, research, royalties/ownership, and speaker fees. Descriptive statistics were calculated. RESULTS: A total of 3019 neuroradiologists were identified in this study. Between 2016 and 2021, 48% (1440/3019) received at least 1 payment from industry, amounting to a total number of 21,967 payments. Each year, among those receiving payments from industry, each unique neuroradiologist received between a mean of 5.49–7.42 payments and a median of 2 payments, indicating a strong rightward skew to the distribution of payments. Gifts were the most frequent payment type made (60%, 13,285/21,967) but accounted for only 4.1% ($689,859/$17,010,546) of payment value. The greatest aggregate payment value came from speaker fees, which made up 36% ($6,127,484/$17,010,546) of the total payment value. The top 5% highest paid neuroradiologists received 42% (9133/21,967) of payments, which accounted for 84% ($14,284,120/$17,010,546) of the total dollar value. Since the start of the coronavirus 2019 (COVID-19) pandemic, the number of neuroradiologists receiving industry payments decreased from a mean of 671 neuroradiologists per year prepandemic (2016–2019) to 411 in the postpandemic (2020–2021) era (P = .030). The total number of payments to neuroradiologists decreased from 4177 per year prepandemic versus 2631 per year postpandemic (P = .011). CONCLUSIONS: Industry payments to neuroradiologists are highly concentrated among top earners, particularly among the top 5% of payment recipients. The number of payments decreased during the COVID-19 pandemic, though the dollar value of payments was offset by coincidental increases in royalty payments. Further investigation is needed in subsequent years to determine if the postpandemic changes in industry payment trends continue. Full Article
