I had a version preformed to flip her 3. She flipped back When I last left off, I was waiting on our planned c-section. I had a list of things to do and by the Thursday before her Saturday arrival, nearly everything was finished. All of our school work for the semester was complete and all of the laundry in my house was washed, folded, and put away. The only major family thing I had left to do was packing. I never got around to packing. Thursday afternoon I noticed I was having about 4 contractions an hour. I paid no attention to these because in my previous pregnancies, I had weeks and weeks of contractions prior to actual labor. This pregnancy, I had NO "practice" contractions. I just assumed it was because she was breech and so there was nothing to get ready for. My cervix was also closed at every appointment in the last weeks which is also not normal for me. I normally am between 3-4 cm before I go into labor. So back to the few contractions, I just assumed these where the ones I had been without for the last 6 weeks. I started cooking dinner on Thursday night and got about halfway through cooking when I felt the first painful and long contraction. It was at 5:15. From that point on, they were coming every 2-3 mins without stopping and lasting over a minute. They went from 0-60 immediately. I couldn't finish making dinner so I really have no idea what the kids ate that night. Rick walked in the door at 6 and I asked him to take Kate to dance and take all of the other kids with him. He asked if it was "time" and I said, "No, I just need to try and make them stop." I had 2 times with Luke where I had similar contractions prior to labor that lasted 2 hours and then stopped. I was assuming that's what was going on here. So from 5:15-9:15, I walked around my house, laid down in my bed, took 2 baths, but nothing was stopping them. It was the same pattern, 2-3 mins apart and lasting from 1-2 mins. I wasn't getting any relief or downtime in between contractions. At some point during this 4 hour period, Rick picked Kate up from dance, printed and folded the bulletins for Sunday, and took them to the church. I told myself if they were still going at 9:15 that I would call my doctor. My doctor told me to come on in and get checked out and that they would probably keep me overnight to monitor me and decide what to do early Friday morning. I still wasn't sure if I was in labor or not and really didn't want to go and then get sent home so I was relieved that they were going to keep me and that Charlotte would probably be born the next day when my doctor was on call. (He delivered all 7 of our other kids.) I texted my sister to come stay with the kids and started packing my bags. I always wondered why people packed their hospital bags early. I thought you would have plenty of time in between contractions to get what you needed. Haha! I ended up at the hospital with 6 shirts and 1 pair of shorts. :) I guess I really was in pain. The ride to the hospital was horrible. Every bump magnified the pain. The contractions didn't slow down and that was when I finally realized that I was actually in labor. It took us about 45 minutes to get to the hospital. I think we arrived around 11:00pm and I was taken to triage to get things checked out. I had never actually been to triage so that was something new. They monitored me for an hour and I had to lay on my back that entire time. That was horrible. I'm pretty sure you are not meant to lay on your back without pain relief during labor. :) They also did an ultrasound to check her position and she was in fact, still breech. Around 12:20am, Friday morning, the doctor on call came in said it was time for Charlotte to be born and that we couldn't wait any longer. My contractions were not stopping and my cervix was opening. From that point on, everything moved quickly and slowly at the same time. The anesthesiologist came in and said I would be getting a spinal because they couldn't wait for an epidural to work. He told me all of the side effects but I'm not really sure what he said. I had to sign about 20 pages of release forms and they I was wheeled off to the operating room while Rick waited in triage to be called back. I remember the operating room being very, very bright and there were a lot of people in there - I think 10. I'm used to the quiet dark delivery rooms with just one nurse and the doctor so this was a change. I moved from the triage bed to the operating table and was given my spinal. I do remember the anesthesiologist saying that if the spinal didn't take for some reason that they would immediately knock me out and that when I would wake up I'd have a baby. I was hoping that didn't happen. He poked my arm with some kind of sharp object and then would poke on my stomach to make sure I didn't feel anything. The first 2 times he did it I felt it but by the third time, I couldn't feel anything. At that point Rick was let in the room and they began. They had been prepping and draping me during this time as well. Charlotte was born at 12:50 am. She was born toward the beginning of the surgery and it seemed that the bulk of the time was spent getting stitched back together. She was immediately shown to me over my drape and then taken to get cleaned up a bit. About 2 minutes later she was brought over to me and placed on my chest and I held her for about 5 mins. She was then taken away (in the same room again) to be weighed, etc and Rick went with her. She was then brought back to me all bundled me for a few kisses and then Rick was able to carry her to the recovery room to wait for me. I was in the recovery room for 2 hours and during that time my parents came up to see her. I was able to nurse her which was great. I was also given a pump for my pain medication. The nurse sacred me because she told me to push the button every 8 mins and to stay on top of the pain. She warned me not to fall asleep because if I did and then woke up an hour later, I would not be able to handle the amount of pain I would be in. It was 2:00 in the morning and I was drugged up on morphine and she was telling me not to fall asleep! I guess it worked because I didn't sleep until later that morning, after the pump was turned off. :) I was in love with her from the moment I saw her. My main concern with having a c-section, prior to having it, was that I would somehow feel like I missed out on the birthing process. I never felt that way. I carried her for 9 months and she arrived in the safest way possible for both of us. It was perfect. The only thing I was sad about was that my doctor missed delivering her by 5 hours. However, that was the Lord's will too. He was on call all weekend so he checked on me everyday while I was in the hospital. C-section recovery is painful. That's all I want to say about that. I just want to remember the hours and minutes before, during, and after her birth. I am thankful for every moment of it. Full Article

Thursday, July 26, 2012 The High Court has today granted permission to two disabled people to bring a claim forjudicial review against the Secretary of State for Work and Pensions to challenge theoperation of the Work Capability Assessment (WCA).WCAs are face to face interviews carried out by healthcare professionals (HCPs)employed by Atos Healthcare (a private contractor), to assess disabled people’sentitlement to Employment and Support Allowance (a sickness benefit that has replacedthe old Incapacity Benefit). Each existing recipient of Incapacity Benefit is now beingassessed for eligibility for ESA, at the rate of some 11,000 people per week. WCAs havebeen the subject of serious criticism by all relevant stakeholders in civil society includingdoctors and NGOs working on behalf of disabled people.The present case concerns some of the problems with the system as experienced bypeople with mental health problems. Although medically trained, Atos HCPs typicallyhave very limited knowledge of mental health. The interviews are often hurried, and relyon applicants to explain the limitations on their ability to work.This is a serious problem for people with mental health conditions who lack insight intotheir conditions, whose conditions fluctuate in seriousness, or who cannot easily talkabout their disability. Such people are placed at a substantial disadvantage in navigatingthe system. Even if they appreciate the need to get expert medical evidence forthemselves, they are often less able to navigate the system successfully and to obtainthe medical report that they need. The Equality Act 2010 requires the DWP to makereasonable adjustments to avoid such disadvantage.The reasonable adjustment to the process that the claimants seek is for medicalevidence to be sought by the Atos HCP and the DWP at the very outset of the claim.This would ensure that very sick people for whom having to go through a WCA would beextremely distressing are exempted from the process, and for those that do attend aWCA, the assessment of fitness to work takes place in the correct medical context, sothat dangers associated with forcing people back to work are correctly identified.At present, the DWP do not routinely ask for expert medical report from an applicant’scommunity-based doctor. The judge has held that it is arguable that this failure is abreach of the duty to make reasonable adjustments, and is therefore unlawful.In granting permission to apply for judicial review, the judge stated:“I consider that it is reasonably arguable that the reasonable adjustmentsrequired by the [Equality Act 2010] include the early obtaining of independentmedical evidence where the documents submitted with the claim show that theclaimant suffers from mental health problems and that this has not been done, orat least not done on a sufficiently widespread basis”.The claimants, known as MM and DM, were granted anonymity by the court. Theirsolicitor, Ravi Low-Beer of the Public Law Project said:“The present system results in many thousands of unnecessary appeals at greatpublic expense, with a high success rate. What is not counted is the cost inhuman misery for those people who should never have had to go through theappeals process in the first place. This could be avoided if doctors were involvedin the assessments at the outset. The Government’s policy of by-passing doctorsis inefficient, unfair, and inhumane. Why these limitations? I suspect that they are there to prevent us from doing something idiotic. This is the table used for all examples that follow: CREATE TABLE EMP (EMPNO NUMBER(4) CONSTRAINT PK_EMP PRIMARY KEY, ENAME VARCHAR2(10), JOB VARCHAR2(9), MGR NUMBER(4), HIREDATE DATE, SAL NUMBER(7,2), COMM NUMBER(7,2), DEPTNO NUMBER(2) ) PARTITION BY HASH (EMPNO) PARTITIONS 4; the usual EMP table, with a partitioning clause appended. It is of course a contrived example. Perhaps I am recruiting so many employees concurrently that a non-partitioned table has problems with buffer contention that can be solved only with hash partitioning. Why can't I have a local non-prefixed unique index? A local non-unique index is no problem, but unique is not possible: orclz> create index enamei on emp(ename) local; Index created. orclz> drop index enamei; Index dropped. orclz> create unique index enamei on emp(ename) local; create unique index enamei on emp(ename) local * ERROR at line 1: ORA-14039: partitioning columns must form a subset of key columns of a UNIQUE index You cannot get a around the problem by separating the index from the constraint (which is always good practice): orclz> create index enamei on emp(ename) local; Index created. orclz> alter table emp add constraint euk unique (ename); alter table emp add constraint euk unique (ename) * ERROR at line 1: ORA-01408: such column list already indexed orclz> So what is the issue? Clearly it is not a technical limitation. But if it were possible, consder the implications for performance. When inserting a row, a unique index (or a non-unique index enforcing a unique constraint) must be searched to see if the key value already exists. For my little four partition table, that would mean four index searches: one of each local index partition. Well, OK. But what if the table were range partitioned into a thousand partitions? Then every insert would have to make a thousand index lookups. This would be unbelievably slow. By restricting unique indexes to global or local prefixed, Uncle Oracle is ensuring that we cannot create such an awful situation. Why can't I have a global non-prefixed index? Well, why would you want one? In my example, perhaps you want a global index on deptno, partitioned by mgr. But you can't do it: orclz> create index deptnoi on emp(deptno) global partition by hash(mgr) partitions 4; create index deptnoi on emp(deptno) global partition by hash(mgr) partitions 4 * ERROR at line 1: ORA-14038: GLOBAL partitioned index must be prefixed orclz>This index, if it were possible, might assist a query with an equality predicate on mgr and a range predicate on deptno: prune off all the non-relevant mgr partitions, then a range scan. But exactly the same effect would be achieved by using global nonpartitioned concatenated index on mgr and deptno. If the query had only deptno in the predicate, it woud have to search each partition of the putative global partitioned index, a process which would be just about identical to a skip scan of the nonpartitioned index. And of course the concatenated index could be globally partitioned - on mgr. So there you have it: a global non-prefixed index would give you nothing that is not available in other ways. Why can't I have a global partitioned bitmap index? This came up on the Oracle forums recently, https://forums.oracle.com/thread/2575623 Global indexes must be prefixed. Bearing that in mind, the question needs to be re-phrased: why would anyone ever want a prefixed partitioned bitmap index? Something like this: orclz> orclz> create bitmap index bmi on emp(deptno) global partition by hash(deptno) partitions 4; create bitmap index bmi on emp(deptno) global partition by hash(deptno) partitions 4 * ERROR at line 1: ORA-25113: GLOBAL may not be used with a bitmap index orclz> If this were possible, what would it give you? Nothing. You would not get the usual benefit of reducing contention for concurrent inserts, because of the need to lock entire blocks of a bitmap index (and therefore ranges of rows) when doing DML. Range partitioning a bitmap index would be ludicrous, because of the need to use equality predicates to get real value from bitmaps. Even with hash partitions, you would not get any benefit from partition pruning, because using equality predicates on a bitmap index in effect prunes the index already: that is what a bitmap index is for. So it seems to me that a globally partitioned bitmap index would deliver no benefit, while adding complexity and problems of index maintenance. So I suspect that, once again, Uncle Oracle is protecting us from ourselves. Is there a technology limitation? I am of course open to correction, but I cannot see a technology limitation that enforces any of these three impossibilities. I'm sure they are all technically possible. When I turned 16, I was ecstatic because I was able to get my drivers license. When I became 18, I was finally considered an adult... for the most part. And on my 21st birthday, I was able to " legally " get into clubs and bars. Alas, those glorious days of positive milestones are gone... long gone!Now birthdays bring with them more negative than positive connotation's.This years birthday is a good example. I'm about to become a member of the "Forty Something Crowd"... You know, that group of people who aren't really that old but their not really that young either.Last year I turned forty. It really wasn't that big of deal. After all, just a year before I had been in my thirties. Forty Something, on the other hand, has a totally different feel. I'm entering into that obscure age zone where the really crazy shit starts to happen. MILFs start transforming into GILFs. The word cougar takes on a whole new meaning. Hair color goes from fashion choice to necessity. Visions of boob jobs dance in your head. And the term " Her Age" starts being inserted into the most positive statements about you, i.e. "She's fucking hot... for her age".
