Fibroblast activation protein (FAP) is one of key molecules in tissue fibrosis, and 68Ga-labeled FAP inhibitor-46 (FAPI46) PET is a recently developed method for evaluating FAP. The aim of this study was to evaluate FAP expression and fibrosis in a hypertension model and to test the feasibility of 68Ga-FAPI46 PET in hypertension. Methods: Hypertension was induced in mice by angiotensin II infusion for 4 wk. 68Ga-FAPI46 biodistribution studies and PET scanning were conducted at 1, 2, and 4 wk after hypertension modeling, and uptake in the major organs was measured. The FAP expression and fibrosis formation of the heart and kidney tissues were analyzed and compared with 68Ga-FAPI46 uptake. Subgroups of the hypertension model underwent angiotensin receptor blocker administration and high-dose FAPI46 blocking, for comparison. As a preliminary human study, 68Ga-FAPI46 PET images of lung cancer patients were analyzed and compared between hypertension and control groups. Results: Uptake of 68Ga-FAPI46 in the heart and kidneys was significantly higher in the hypertension group than in the sham group as early as week 1 and decreased after week 2. The uptake was specifically blocked in the high-dose blocking study. Immunohistochemistry also revealed FAP expression in both heart and kidney tissues. However, overt fibrosis was observed in the heart, whereas it was absent from the kidneys. The angiotensin receptor blocker–treated group showed lower uptake in the heart and kidneys than did the hypertension group. In the pilot human study, renal uptake of 68Ga-FAPI46 significantly differed between the hypertension and control groups. Conclusion: In hypertension, FAP expression is increased in the heart and kidneys from the early phases and decreases over time. FAP expression appears to represent fibrosis activity preceding or underlying fibrotic tissue formation. 68Ga-FAPI46 PET has potential as an effective imaging method for evaluating FAP expression in progressive fibrosis by hypertension. ess Efficacy and Toxicity of [177Lu]Lu-PSMA-617 for Metastatic Castration-Resistant Prostate Cancer: Results from the U.S. Expanded-Access Program and Comparisons with Phase 3 VISION Data By jnm.snmjournals.org Published On :: 2024-11-01T04:25:31-07:00 The phase 3 VISION trial demonstrated that [177Lu]Lu-PSMA-617 prolonged progression-free survival and overall survival (OS) in prostate-specific membrane antigen [PSMA]–positive metastatic castration-resistant prostate cancer (mCRPC) patients who progressed on taxane-based chemotherapy and androgen receptor–signaling inhibitors (ARSIs). The U.S. expanded-access program (EAP; NCT04825652) was opened to provide access to [177Lu]Lu-PSMA-617 for eligible patients until regulatory approval was obtained. This study aimed to evaluate the efficacy and safety profile of [177Lu]Lu-PSMA-617 within the EAP and compare the results with those from the VISION trial. Methods: Patients enrolled in the EAP at 4 institutions in the United States with available toxicity and outcome data were included. Outcome measures included OS, a prostate-specific antigen (PSA) response rate (RR) of at least 50%, and incidences of toxicity according to Common Terminology Criteria for Adverse Events version 5.0. Differences in baseline characteristics, outcome data, and toxicity between the EAP and VISION were evaluated using t testing of proportions and survival analyses. Results: In total, 117 patients with mCRPC who received [177Lu]Lu-PSMA-617 within the EAP between May 2021 and March 2022 were eligible and included in this analysis. Patients enrolled in the EAP were more heavily pretreated with ARSI (≥2 ARSI regimens: 70% vs. 46%; P < 0.001) and had worse performance status at baseline (Eastern Cooperative Oncology Group score ≥ 2: 19% vs. 7%; P < 0.001) than VISION patients. EAP and VISION patients had similar levels of grade 3 or higher anemia (18% vs. 13%; P = 0.15), thrombocytopenia (13% vs. 8%; P = 0.13), and neutropenia (3% vs. 3%; P = 0.85) and similar PSA RRs (42% vs. 46%; P = 0.50) and OS (median: 15.1 vs. 15.3 mo; P > 0.05). Conclusion: Patients with PSMA-positive mCRPC who received [177Lu]Lu-PSMA-617 within the EAP were later in their disease trajectory than VISION patients. Patients enrolled in the EAP achieved similar PSA RRs and OS and had a safety profile similar to that of the VISION trial patients. Full Article
ess Utility of Early Postoperative DWI to Assess the Extent of Resection of Adult-Type World Health Organization Grade 2 and 3 Diffuse Gliomas [CLINICAL PRACTICE] By www.ajnr.org Published On :: 2024-11-07T15:14:12-08:00 BACKGROUND AND PURPOSE: World Health Organization (WHO) grade 2 and 3 diffuse gliomas account for approximately 5% of primary brain tumors. They are invasive and infiltrative tumors and have considerable morbidity, causing progressive neurologic deterioration. The mean survival time is <10 years from diagnosis. Surgical debulking represents first-line management. The extent of resection is associated with progression-free and overall survival. Radiologic assessment of the extent of resection is challenging. This can be underestimated on early postoperative MRI, meaning that accurate assessment may be achieved only on delayed follow-up imaging. We hypothesized that DWI may help facilitate more reliable estimates of the extent of resection on early postoperative MRI. This study aimed to assess the utility of DWI in early postoperative MRI to evaluate the extent of resection. MATERIALS AND METHODS: A single-center observational cohort study was performed. All patients with histologically confirmed WHO grade 2 and 3 gliomas managed with surgical debulking between January 2015 and December 2020 were identified. Preoperative, early postoperative, and follow-up imaging were reviewed independently by 2 consultant neuroradiologists. The extent of resection was estimated with and without DWI sequences for each case. RESULTS: Two hundred twenty-four patients with WHO grade 2 and 3 gliomas were managed with surgical debulking between 2015 and 2020. DWI was not performed on early postoperative MRI in 2 patients. With the use of DWI, the extent of resection was upgraded in 30% of cases (n = 66/222) and classified as "complete" or "supramaximal" in 58% of these patients (n = 38/66). In cases in which the extent of resection was upgraded with the use of DWI, signal abnormality was stable or reduced at follow-up in 78% (n = 49/63). In cases with worsening signal abnormality, 64% were deemed to be secondary to adjuvant radiation therapy (n = 9/14). Eight percent (n = 5/63) of patients with an increased estimated extent of resection using DWI demonstrated signal progression attributed to true disease progression at follow-up. CONCLUSIONS: DWI is a helpful and reliable adjunct in differentiating residual tumor from marginal ischemia in early postoperative MRI in WHO grade 2 and 3 diffuse gliomas and increases the accuracy in assessing the extent of resection. ess Preoperative Assessment of Meningioma Consistency Using a Combination of MR Elastography and DTI [RESEARCH] By www.ajnr.org Published On :: 2024-11-07T15:14:12-08:00 BACKGROUND AND PURPOSE: Preoperative assessment of meningioma consistency is beneficial for optimizing surgical strategy and prognosis of patients. We aim to develop a noninvasive prediction model for meningioma consistency utilizing MR elastography and DTI. MATERIALS AND METHODS: Ninety-four patients (52 ± 22 years old, 69 women, 25 men) diagnosed with meningioma were recruited in the study. Each patient underwent preoperative T1WI, T2WI, DTI, and MR elastography. Combined MR elastography–DTI model was developed based on multiple logistic regression. Intraoperative tumor descriptions served as clinical criteria for evaluating meningioma consistency. The diagnostic efficacy in determining meningioma consistency was evaluated by using a receiver operating characteristic curve. Further validation was conducted in 27 stereotactic biopsies by using indentation tests and underlying mechanism was investigated by histologic analysis. RESULTS: Among all the imaging modalities, MR elastography demonstrated the highest efficacy with the shear modulus magnitude (|G*|) achieving an area under the curve (AUC) of 0.81 (95% CI: 0.699–0.929). When combined with DTI, the diagnostic accuracy further increased (AUC: 0.88, 95% CI: 0.784–0.971), surpassing any technique alone. Indentation measurement based on stereotactic biopsies further demonstrated that the MR elastography–DTI model was suitable for predicting intratumor consistency. Histologic analysis suggested that meningioma consistency may be correlated with tumor cell density and fibrous content. CONCLUSIONS: The MR elastography–DTI combined model is effective in noninvasive prediction of meningioma consistency. Full Article
ess Optimal Endovascular Therapy Technique for Isolated Intracranial Atherothrombotic Stroke-Related Large-Vessel Occlusion in the Acute-to-Subacute Stage [CLINICAL PRACTICE] By www.ajnr.org Published On :: 2024-11-07T15:14:12-08:00 BACKGROUND AND PURPOSE: Reocclusion after treatment is a concern in endovascular therapy for isolated intracranial atherothrombotic stroke-related large-vessel occlusion (AT-LVO). However, the optimal endovascular therapy technique for AT-LVO has not yet been investigated. This study evaluated the optimal endovascular therapy technique for AT-LVO in a real-world setting. MATERIALS AND METHODS: We conducted a historical, multicenter registry study at 51 centers that enrolled patients with AT-LVO. We divided the patients into 3 groups based on the endovascular therapy technique: mechanical thrombectomy alone, percutaneous transluminal angioplasty (PTA), and stent deployment. Mechanical thrombectomy alone was classified into the mechanical thrombectomy-only group; PTA and mechanical thrombectomy–PTA, into the PTA group; and mechanical thrombectomy–stent deployment, mechanical thrombectomy–PTA–stent deployment, PTA–stent deployment, and stent deployment–only into the stent group. The primary outcome was incidence of reocclusion of the treated vessels within 90 days of endovascular therapy completion. RESULTS: We enrolled 770 patients and analyzed 509 patients. The rates in the mechanical thrombectomy-only, PTA, and stent deployment groups were 40.7%, 44.4%, and 14.9%, respectively. Incidence rate of residual stenosis >70% of final angiography was significantly higher in the mechanical thrombectomy-only group than in the PTA and stent deployment groups (mechanical thrombectomy-only versus PTA versus stent deployment: 34.5% versus 26.3% versus 13.2%, P = .002). Reocclusion rate was significantly lower in the PTA group than in the mechanical thrombectomy-only group (adjusted hazard ratio, 0.48; 95% CI, 0.29–0.80). Of the patients, 83.5% experienced reocclusion within 10 days after endovascular therapy. Alarmingly, a substantial subset (approximately 62.0%) of patients experienced reocclusion within 2 days of endovascular therapy. Incidence of mRS scores of 0–2 ninety days after endovascular therapy was not significantly different among the 3 groups. Incidences of symptomatic intracranial hemorrhage, any other intracranial hemorrhage, and death were not significantly different. CONCLUSIONS: Incidence rate of reocclusion was significantly lower in the PTA group than in the mechanical thrombectomy-only group. We found no meaningful difference in reocclusion rates between the stent deployment and mechanical thrombectomy-only groups. In Japan, glycoprotein IIb/IIIa inhibitors are not reimbursed. Therefore, PTA might be the preferred choice for AT-LVOs due to the higher reocclusion risk with mechanical thrombectomy-only. Reocclusion was likely to occur within 10 days, particularly within 2 days post-endovascular therapy. Full Article
ess Deep Learning-Based Reconstruction of 3D T1 SPACE Vessel Wall Imaging Provides Improved Image Quality with Reduced Scan Times: A Preliminary Study [ARTIFICIAL INTELLIGENCE] By www.ajnr.org Published On :: 2024-11-07T15:14:12-08:00 BACKGROUND AND PURPOSE: Intracranial vessel wall imaging is technically challenging to implement, given the simultaneous requirements of high spatial resolution, excellent blood and CSF signal suppression, and clinically acceptable gradient times. Herein, we present our preliminary findings on the evaluation of a deep learning–optimized sequence using T1-weighted imaging. MATERIALS AND METHODS: Clinical and optimized deep learning–based image reconstruction T1 3D Sampling Perfection with Application optimized Contrast using different flip angle Evolution (SPACE) were evaluated, comparing noncontrast sequences in 10 healthy controls and postcontrast sequences in 5 consecutive patients. Images were reviewed on a Likert-like scale by 4 fellowship-trained neuroradiologists. Scores (range, 1–4) were separately assigned for 11 vessel segments in terms of vessel wall and lumen delineation. Additionally, images were evaluated in terms of overall background noise, image sharpness, and homogeneous CSF signal. Segment-wise scores were compared using paired samples t tests. RESULTS: The scan time for the clinical and deep learning–based image reconstruction sequences were 7:26 minutes and 5:23 minutes respectively. Deep learning–based image reconstruction images showed consistently higher wall signal and lumen visualization scores, with the differences being statistically significant in most vessel segments on both pre- and postcontrast images. Deep learning–based image reconstruction had lower background noise, higher image sharpness, and uniform CSF signal. Depiction of intracranial pathologies was better or similar on the deep learning–based image reconstruction. CONCLUSIONS: Our preliminary findings suggest that deep learning–based image reconstruction–optimized intracranial vessel wall imaging sequences may be helpful in achieving shorter gradient times with improved vessel wall visualization and overall image quality. These improvements may help with wider adoption of intracranial vessel wall imaging in clinical practice and should be further validated on a larger cohort. Full Article
ess Artificial Intelligence Efficacy as a Function of Trainee Interpreter Proficiency: Lessons from a Randomized Controlled Trial [RESEARCH] By www.ajnr.org Published On :: 2024-11-07T15:14:12-08:00 BACKGROUND AND PURPOSE: Recently, artificial intelligence tools have been deployed with increasing speed in educational and clinical settings. However, the use of artificial intelligence by trainees across different levels of experience has not been well-studied. This study investigates the impact of artificial intelligence assistance on the diagnostic accuracy for intracranial hemorrhage and large-vessel occlusion by medical students and resident trainees. MATERIALS AND METHODS: This prospective study was conducted between March 2023 and October 2023. Medical students and resident trainees were asked to identify intracranial hemorrhage and large-vessel occlusion in 100 noncontrast head CTs and 100 head CTAs, respectively. One group received diagnostic aid simulating artificial intelligence for intracranial hemorrhage only (n = 26); the other, for large-vessel occlusion only (n = 28). Primary outcomes included accuracy, sensitivity, and specificity for intracranial hemorrhage/large-vessel occlusion detection without and with aid. Study interpretation time was a secondary outcome. Individual responses were pooled and analyzed with the t test; differences in continuous variables were assessed with ANOVA. RESULTS: Forty-eight participants completed the study, generating 10,779 intracranial hemorrhage or large-vessel occlusion interpretations. With diagnostic aid, medical student accuracy improved 11.0 points (P < .001) and resident trainee accuracy showed no significant change. Intracranial hemorrhage interpretation time increased with diagnostic aid for both groups (P < .001), while large-vessel occlusion interpretation time decreased for medical students (P < .001). Despite worse performance in the detection of the smallest-versus-largest hemorrhages at baseline, medical students were not more likely to accept a true-positive artificial intelligence result for these more difficult tasks. Both groups were considerably less accurate when disagreeing with the artificial intelligence or when supplied with an incorrect artificial intelligence result. CONCLUSIONS: This study demonstrated greater improvement in diagnostic accuracy with artificial intelligence for medical students compared with resident trainees. However, medical students were less likely than resident trainees to overrule incorrect artificial intelligence interpretations and were less accurate, even with diagnostic aid, than the artificial intelligence was by itself. Based on gene expression profiling, it has been subdivided into germinal center (GC)-derived and activated B-cell (ABC) types. Advances in molecular methodologies have further refined the subclassification of DLBCL, based on recurrent genetic abnormalities. Here, we describe a distinct case of DLBCL that presented in leukemic form. DNA sequencing targeting 275 genes revealed pathogenically relevant mutations of CD79B, MyD88, TP53, TBL1XR1, and PIM1 genes, indicating that this lymphoma would be best classified as MCD/C5 DLBCL, an ABC subtype. Despite an initial good clinical response to BTK inhibitor ibrutinib, anti-CD20 antibody rituxan, alkylating agent bendamustine, and hematopoietic stem-cell transplant, the lymphoma relapsed, accompanied by morphologic and molecular evidence of disease progression. Specifically, the recurrent tumor developed loss of TP53 heterozygosity (LOH) and additional chromosomal changes central to ABC DLBCL pathogenesis, such as PRDM1 loss. Acquired resistance to ibrutinib and rituxan was indicated by the emergence of BTK and FOXO1 mutations, respectively, as well as apparent activation of alternative cell-activation pathways, through copy-number alterations (CNAs), detected by high-resolution chromosomal microarrays. In vitro, studies of relapsed lymphoma cells confirmed resistance to standard BTK inhibitors but sensitivity to vecabrutinib, a noncovalent inhibitor active against both wild-type as well as mutated BTK. In summary, we provide in-depth molecular characterization of a de novo leukemic DLBCL and discuss mechanisms that may have contributed to the lymphoma establishment, progression, and development of drug resistance. ess The Odyssey of HOMER: Comparative Effectiveness Research on Medication for Opioid Use Disorder During the COVID-19 Pandemic [Special Report] By www.annfammed.org Published On :: 2024-09-23T14:00:14-07:00 The usual challenges of conducting primary care research, including randomized trials, have been exacerbated, and new ones identified, during the COVID-19 pandemic. HOMER (Home versus Office for Medication Enhanced Recovery; subsequently, Comparing Home, Office, and Telehealth Induction for Medication Enhanced Recovery) is a pragmatic, comparative-effectiveness research trial that aims to answer a key question from patients and clinicians: What is the best setting in which to start treatment with buprenorphine for opioid use disorder for this patient at this time? In this article, we describe the difficult journey to find the answer. The HOMER study began as a randomized trial comparing treatment outcomes in patients starting treatment with buprenorphine via induction at home (unobserved) vs in the office (observed, synchronous). The study aimed to enroll 1,000 participants from 100 diverse primary care practices associated with the State Networks of Colorado Ambulatory Practices and Partners and the American Academy of Family Physicians National Research Network. The research team faced unexpected challenges related to the COVID-19 pandemic and dramatic changes in the opioid epidemic. These challenges required changes to the study design, protocol, recruitment intensity, and funding conversations, as well as patience. As this is a participatory research study, we sought, documented, and responded to practice and patient requests for adaptations. Changes included adding a third study arm using telehealth induction (observed via telephone or video, synchronous) and switching to a comprehensive cohort design to answer meaningful patient-centered research questions. Using a narrative approach based on the Greek myth of Homer, we describe here the challenges and adaptations that have provided the opportunity for HOMER to thrive and find the way home. These clinical trial strategies may apply to other studies faced with similar cultural and extreme circumstances. Full Article

This study aimed to determine the early impact of a digital innovation to grow quality care through an interprofessional care team (DIG IT) on the blood pressure (BP) and 10-year atherosclerotic cardiovascular disease (ASCVD) risk score of medically underserved patients. METHODS This was a 3-month, prospective intervention study that included patients aged 40 years or more with BP of 140/90 mmHg or higher who received care from DIG IT from August through December 2021. Sociodemographic and clinical outcomes of DIG IT were compared with historical controls (controls) whose data were randomly extracted by the University of California Data Warehouse and matched 1:1 based on age, ethnicity, and baseline BP of the DIG IT arm. Multiple linear regression was performed to adjust for potential confounding factors. RESULTS A total of 140 patients (70 DIG IT, 70 controls) were included. Both arms were similar with an average age (SD) of 62.8 (9.7) years. The population was dominated by Latinx (79.3%) persons, with baseline mean BP of 163/81 mmHg, and mean ASCVD risk score of 23.9%. The mean (SD) reduction in systolic BP at 3 months in the DIG IT arm was twice that of the controls (30.8 [17.3] mmHg vs 15.2 [21.2] mmHg; P <.001). The mean (SD) ASCVD risk score reduction in the DIG IT arm was also twice that of the controls (6.4% [7.4%] vs 3.1% [5.1%]; P = .003). CONCLUSIONS The DIG IT was more effective than controls (receiving usual care). Twofold improvement in the BP readings and ASCVD scores in medically underserved patients were achieved with DIG IT. ess [Evolutionary Biology] How Important Is Variation in Extrinsic Reproductive Isolation to the Process of Speciation? However, most speciation research has been limited to studying how ecologically dependent RI varies among a handful of broadly categorized environments. Very few studies consider the variability of RI and its effects on speciation at finer scales—that is, within each environment due to spatial or temporal environmental heterogeneity. Such variation in RI across time and/or space may inhibit speciation through leaky reproductive barriers or promote speciation by facilitating reinforcement. To investigate this overlooked aspect of speciation research, we conducted a literature review of existing studies of variation in RI in the field and then conducted individual-based simulations to examine how variation in hybrid fitness across time and space affects the degree of gene flow. Our simulations indicate that the presence of variation in hybrid fitness across space and time often leads to an increase in gene flow compared to scenarios where hybrid fitness remains static. This observation can be attributed to the convex relationship between the degree of gene flow and the strength of selection on hybrids. Our simulations also show that the effect of variation in RI on facilitating gene flow is most pronounced when RI, on average, is relatively low. This finding suggests that it could serve as an important mechanism to explain why the completion of speciation is often challenging. While direct empirical evidence documenting variation in extrinsic RI is limited, we contend that it is a prevalent yet underexplored phenomenon. We support this argument by proposing common scenarios in which RI is likely to exhibit variability and thus influence the process of speciation. Full Article

Here we review recent theoretical work and compile empirical data from crossing experiments and genomic and phylogenetic studies to assess the effect of mating systems on the speciation process. In accordance with theoretical predictions, we find that accumulation of hybrid incompatibilities seems to be accelerated in selfers, but there is so far limited empirical support for a predicted bias toward underdominant loci. Phylogenetic evidence is scarce and contradictory, including studies suggesting that selfing either promotes or hampers speciation rate. Further studies are therefore required, which in addition to measures of reproductive barrier strength and selfing rate should routinely include estimates of demographic history and genetic divergence as a proxy for divergence time. Because of its lipid and protein composition, the outer segments are prone to photo-oxidation, requiring photoreceptors to have robust antioxidant defenses and high metabolic synthesis rates to regenerate the outer segments every 10 days. Both processes required high levels of glucose uptake and utilization. Retinitis pigmentosa is a prevalent form of inherited retinal degeneration characterized by initial loss of low-light vision caused by the death of rod photoreceptors. In this disease, rods die as a direct effect of an inherited mutation. Following the loss of rods, cones eventually degenerate, resulting in complete blindness. The progression of vision loss in retinitis pigmentosa suggested that rod photoreceptors were necessary to maintain healthy cones. We identified a protein secreted by rods that functions to promote cone survival, and we named it rod-derived cone viability factor (RdCVF). RdCVF is encoded by an alternative splice product of the nucleoredoxin-like 1 (NXNL1) gene, and RdCVF was found to accelerate the uptake of glucose by cones. Without RdCVF, cones eventually die because of compromised glucose uptake and utilization. The NXNL1 gene also encodes for the thioredoxin RdCVFL, which reduces cysteines in photoreceptor proteins that are oxidized, providing a defense against radical oxygen species. We will review here the main steps of discovering this novel intercellular signaling currently under translation as a broad-spectrum treatment for retinitis pigmentosa. Full Article

"So for us, for our reflection, we probably need to have a lot of gates, including user testing or internal evaluation, and the timing of such gates. And then we need to bring them forward, and we should have done those gates much earlier than we did. "Also, we have a siloed organisation, so going beyond the boundaries of those organisations in terms of development, and also sales, I think that could have been much smoother. "And then going forward, in our own titles and in third-party titles, we do have many different windows. And we want to be able to select the right and optimal window so that we can deploy them on our own platform without cannibalization, so that we can maximize our performance in terms of title launches. "We launched two live-service games this year," he said. "Helldivers 2 was a huge hit, while Concord ended up being shut down. We gained a lot of experience and learned a lot from both. "We intend to share the lessons learned from our successes and failures across our studios, including in the areas of title development management as well as the process of continually adding expanded content and scaling the service after its release so as to strengthen our development management system. And within that first hour, Slitterhead's body-possessing and Hong Kong-inspired streets had me thinking, "Is this it, the sleeper hit of 2024?!" No, sadly not. It's no doubt built a compelling universe filled with brain-sucking aliens that masquerade as humans, and it attempts plenty else besides: bouncing between bodies as you stealth around dingy apartment blocks, fighting with blood katanas, and gorging on pools of red plasma to refuel skills, many of which require more body-flitting. In all, the fires have consumed more than 220,000 acres, an area larger than New York City. More than 5,700 structures have been destroyed. And at least 41 people have died, making it the deadliest wildfire in the state’s history. The wine industry and the tourism business connected with it are trying to take stock. More than $50 billion in California’s economy comes from the wine business. And nearly 24 million people visit the region for that reason every year. Special correspondent Joanne Jennings reports from Napa County. JOANNE JENNINGS, Special Correspondent: The Mayacamas mountain range creates a natural barrier between Sonoma and Napa Counties. And it is here where the massive Nuns fire is posing a tough challenge for some 11,000 firefighters who are taming the blaze with aircraft and units on the ground. CAPT. MARK BRENNERMAN, Viejas fire Department: We’re going around and making sure none of these fires that are still smoldering and smoking, we’re not going to get another big fire out of them. JOANNE JENNINGS: Even as firefighters are battling shifting winds, owners and workers in Wine Country are trying to determine just how much damage has been done. The tony Highlands gated community was among the first to be consumed by flames when the Atlas fire raced through this canyon, leaving several mansions in rubble. Down the hill, at the Silverado resort, charred remnants of the Safeway PGA Tour remain. The major golf event had just wrapped up last Sunday afternoon, a few hours before flames engulfed tents and grandstands, forcing spectators and athletes to evacuate. MAN: Do you see how it burned right up to the retaining wall here? JOANNE JENNINGS: Silverado resident Steve Messina stayed behind and shot video of fire crews containing the flames, which consumed some condos. Within minutes, flames raced three miles down Silverado Trail, home to several storied hillside vineyards. Most wineries in the region have been spared the worst. But hundreds suffered some damage. And at least eight vineyards have been significantly damaged or destroyed. Pierre Birebent, who has been making wines for the family-owned Signorello estate for 20 years, rushed to his winery as quickly as he could. PIERRE BIREBENT, Signorello Estate Vineyards: I jumped right in my truck, came down, and then when I was riding down, I saw the hill all flaming. JOANNE JENNINGS: Two vineyard workers joined him to help save the estate’s tasting room. PIERRE BIREBENT: But the smoke was getting very thick, and the wind was very strong. And after an hour, we couldn’t breathe anymore. At the moment, I was so upset. It was rage to see that I couldn’t do anything. But it was like fighting a giant. JOANNE JENNINGS: The tasting room, which also housed the winery’s office and a dining room, burned to the ground. But Birebent says he wants to focused on what survived. Fortunately, he said, the fire stopped short of reaching the vineyard, the crush pad, or any of the barrels of wine stored on site; 95 percent of this year’s grapes were already picked. But, to be on the safe side, Birebent is taking these samples to a lab to make sure the juice is not too acidic for winemaking. If the crops are OK, a staff of 25 employees will have jobs to return to. As the fires begin to recede and the smoke clears, people here are beginning to wonder when the tourists, who fuel much of the economy, will return. It’s a serious concern for Andrew and Jeni (ph) Schluter, who are self-employed and are raising a young family. ANDREW SCHLUTER, Andrew’s Tours and Transportation: I do wine tours and transportation for people. And my business started to do really, really well. I was on track to have the best month ever. JOANNE JENNINGS: Andrew just bought this new SUV, which has been idle in his driveway collecting ash. Jeni is a personal trainer and has family who lost their homes in the fires. She’s just not sure how they’re going to make ends meet. WOMAN: I think we’re just overwhelmed, you know? And uncertainty is kind of scary. ANDREW SCHLUTER: We will hopefully get by for awhile, but we might make — have to make some hard decisions shortly. JOANNE JENNINGS: While fires burn nearby, some vineyards are already open to tourists. At the Raymond Vineyard, workers are crushing grapes at a feverish pitch. The tasting room is open for the first time since the fires started. Jeremy and Erika Moore arrived from Tennessee yesterday. They considered canceling their trip, but decided the best way they could help people here is to give them their business. JEREMY MOORE, Tourist: On the one hand, a few hundred yards from here, you can see them shuttling up with the helicopters fighting fires, but then here it’s beautiful. They are doing some great tastings, and they are working outside on the crops. So, it’s a weird combination of tragedy, but then at the same time business must go on, too. JOANNE JENNINGS: Proprietor Jean-Charles Boisset owns several wineries in California, France and Canada, but like many other people here, he and his family had to evacuate their home when the flames came dangerously close. Still, he is bullish about the future of the wine industry in this region. JEAN-CHARLES BOISSET, Boisset Collection: Napa has been one of the most amazing agricultural places in California for a long time, so it will survive those fires. What I love, as a Frenchman here in California, is that amazing American positive attitude. We will recover. President Donald Trump’s suggestion Monday that his predecessors fell short in that duty brought a visceral reaction from those who witnessed those grieving encounters. “He’s a deranged animal,” Alyssa Mastromonaco, a former deputy chief of staff to President Barack Obama, tweeted about Trump. With an expletive, she called Trump’s statement in the Rose Garden a lie. Trump said in a news conference he had written letters to the families of four soldiers killed in an Oct. 4 ambush in Niger and planned to call them, crediting himself with taking extra steps in honoring the dead properly. “Most of them didn’t make calls,” he said of his predecessors. He said it’s possible that Obama “did sometimes” but “other presidents did not call.” The record is plain that presidents reached out to families of the dead and to the wounded, often with their presence as well as by letter and phone. The path to Walter Reed and other military hospitals, as well as to the Dover, Delaware, Air Force Base where the remains of fallen soldiers are often brought, is a familiar one to Obama, George W. Bush and others. Bush, even at the height of two wars, “wrote all the families of the fallen,” said Freddy Ford, spokesman for the ex-president. Ford said Bush also called or met “hundreds, if not thousands” of family members of the war dead. READ MORE: What Trump said about his drug czar pick, health care fixes Obama’s official photographer, Pete Souza, tweeted that he photographed Obama “meeting with hundreds of wounded soldiers, and family members of those killed in action.” Others recalled his frequent visits with Gold Star families, and travels to Walter Reed, Dover and other venues with families of the dead and with the wounded. Retired Gen. Martin E. Dempsey, the former chairman of the Joint Chiefs of Staff, confirmed these contacts, tweeting: “POTUS 43 & 44 and first ladies cared deeply, worked tirelessly for the serving, the fallen, and their families. Not politics. Sacred Trust.” Trump addressed the matter when asked why he had not spoken about the four soldiers killed in Niger. They died when militants thought to be affiliated with the Islamic State group ambushed them while they were patrolling in unarmored trucks with Nigerien troops. “I actually wrote letters individually to the soldiers we’re talking about, and they’re going to be going out either today or tomorrow,” he said, meaning he wrote to the families of the fallen soldiers. He did not explain why letters had not been sent yet, more than a week after the attack. “If you look at President Obama and other presidents, most of them didn’t make calls,” Trump said. Pressed on that statement later, he said of Obama: “I was told that he didn’t often, and a lot of presidents don’t. They write letters.” He went on: “President Obama, I think, probably did sometimes, and maybe sometimes he didn’t. I don’t know. That’s what I was told. … Some presidents didn’t do anything.” Press Secretary Sarah Huckabee Sanders said later that Trump “wasn’t criticizing predecessors, but stating a fact.” She argued that presidents didn’t always call families of those killed in battle: “Sometimes they call, sometimes they send a letter, other times they have the opportunity to meet family members in person.” She said anyone claiming a former president had called every family was “mistaken.” Bush’s commitment to writing to all military families of the dead and to reaching out by phone or meeting with many others came despite the enormity of the task. In the Iraq war alone, U.S. combat deaths were highest during his presidency, exceeding 800 each year from 2004 through 2007. The number fell to 313 in Bush’s last year in office as the insurgency faded. Bush once said he felt the appropriate way to show his respect was to meet family members in private. As Obama wound down that war, he sent tens of thousands more troops into Afghanistan in 2009 and 2010, and the death count mounted. From a total of 155 Americans killed in Afghanistan in 2008, which was Bush’s last full year in office, the number jumped to 311 in 2009 and peaked the next year at 498. In all, more than 1,700 died in Afghanistan on Obama’s watch. Among other rituals honoring military families, the Obamas had a “Gold Star” Christmas tree in the White House decorated with hundreds of photos and notes from people who had lost loved ones in war. Gold Star families visited during the holidays, bringing ornaments. Trump visited Dover early in his presidency, going in February with his daughter Ivanka for the return of the remains of a U.S. Navy SEAL killed during a raid in Yemen, William “Ryan” Owens. Trump’s relations with Gold Star families have not always been smooth, dating from his belittlement of the parents of slain U.S. soldier Humayun Khan, who was Muslim. Trump was angered when the soldier’s father, Khizr Khan, was given a platform to criticize him at the Democratic National Convention. Owens’ grieving father said he didn’t want to talk with Trump at Dover. At a hearing today, he bridled at Democratic Senator Al Franken’s accusation that he’d — quote — “moved the goalposts” on the nature of his discussions. SEN. AL FRANKEN, D-Minn.: First it was, I didn’t have communications with Russians, which wasn’t true. Then it was, I never met with any Russians to discuss any political campaign, which may or may not be true. Now it’s, I didn’t discuss interference in the campaign. JEFF SESSIONS, Attorney General: Well, let me just say without hesitation, that I conducted no improper discussions with the Russians at any time regarding the campaign or any other item facing this country. HARI SREENIVASAN: Sessions has recused himself from the Justice Department’s investigation into Russia’s election meddling. President Trump had new criticism today for former FBI Director James Comey over the Hillary Clinton e-mail probe. He complained again that Comey decided to clear Clinton before she was even interviewed. That’s based on newly released draft statements by Comey from May of 2016. FBI officials say it was already clear that no charges were warranted. On another issue, the president faced fallout over the death of Army Sergeant La David Johnson in Niger this month. Congresswoman Frederica Wilson says she was with Mrs. Johnson when the president called. The Florida Democrat told The Washington Post that Mr. Trump said — quote — “He knew what he was signing up for, but I guess it hurts anyway.” The sergeant’s mother confirmed it, but the president denied it, and White House Press Secretary Sarah Huckabee Sanders went after Wilson. SARAH SANDERS, White House Press Secretary: This is a president who loves our country very much, who has the greatest level of respect for men and women in uniform and wanted to call and offer condolences to the family, and I think to try to create something from that, that the congresswoman is doing, is, frankly, appalling and disgusting. It quoted the father of a soldier killed in Afghanistan as saying the president offered $25,000 from his personal account, but never followed through. We will get more detail on all of this after the news summary. The death toll in Northern California’s wildfires rose to 42 today. Officials in Sonoma County found the remains of the latest victim, as they searched hundreds of burned homes. Meanwhile, fire crews made new gains overnight with the help of cooler weather and low winds. A two-time Olympic medalist says the former team doctor for U.S. women’s gymnastics sexually abused her for years. McKayla Maroney is the highest profile athlete to come forward in the scandal. In a statement today, she said Dr. Larry Nassar began molesting her when she was just 13. He’s awaiting sentencing on a child pornography charge, but has denied any sexual abuse. More questions tonight about drug pricing. A new study finds the costs of injectable cancer drugs, approved since 1996, rose an average of 25 percent over eight years. That’s far higher than the rate of inflation. The study was based at Emory University and published in “The Journal of Clinical Oncology.” And on Wall Street, health insurers and IBM fueled a surge in stocks today. The Dow Jones industrial average gained 160 points, more than half-a-percent, to close above 23000 for the first time. The Nasdaq rose just a fraction, and the S&P 500 was up two points.
