male Temporal Dynamics of the Adult Female Lower Urinary Tract Microbiota By mbio.asm.org Published On :: 2020-04-21T01:31:26-07:00 ABSTRACT Temporal dynamics of certain human microbiotas have been described in longitudinal studies; variability often relates to modifiable factors or behaviors. Early studies of the urinary microbiota preferentially used samples obtained by transurethral catheterization to minimize vulvovaginal microbial contributions. Whereas voided specimens are preferred for longitudinal studies, the few studies that reported longitudinal data were limited to women with lower urinary tract (LUT) symptoms, due to ease of accessing a clinical population for sampling and the impracticality and risk of collecting repeated catheterized urine specimens in a nonclinical population. Here, we studied the microbiota of the LUT of nonsymptomatic, premenopausal women using midstream voided urine (MSU) specimens to investigate relationships between microbial dynamics and personal factors. Using 16S rRNA gene sequencing and a metaculturomics method called expanded quantitative urine culture (EQUC), we characterized the microbiotas of MSU and periurethral swab specimens collected daily for approximately 3 months from a small cohort of adult women. Participants were screened for eligibility, including the ability to self-collect paired urogenital specimens prior to enrollment. In this population, we found that measures of microbial dynamics related to specific participant-reported factors, particularly menstruation and vaginal intercourse. Further investigation of the trends revealed differences in the composition and diversity of LUT microbiotas within and across participants. These data, in combination with previous studies showing relationships between the LUT microbiota and LUT symptoms, suggest that personal factors relating to the genitourinary system may be an important consideration in the etiology, prevention, and/or treatment of LUT disorders. IMPORTANCE Following the discovery of the collective human urinary microbiota, important knowledge gaps remain, including the stability and variability of this microbial niche over time. Initial urinary studies preferentially utilized samples obtained by transurethral catheterization to minimize contributions from vulvovaginal microbes. However, catheterization has the potential to alter the urinary microbiota; therefore, voided specimens are preferred for longitudinal studies. In this report, we describe microbial findings obtained by daily assessment over 3 months in a small cohort of adult women. We found that, similarly to vaginal microbiotas, lower urinary tract (LUT) microbiotas are dynamic, with changes relating to several factors, particularly menstruation and vaginal intercourse. Our study results show that LUT microbiotas are both dynamic and resilient. They also offer novel opportunities to target LUT microbiotas by preventative or therapeutic means, through risk and/or protective factor modification. Full Article

Bats have particularly high energy demands compared to other mammals and regularly use torpor to save energy. However, while torpor limits energy expenditure, it can also downregulate important processes, such as sperm production. This constraint could result in a trade-off between energy saving and future reproductive capacity. We mimicked harsh conditions by restricting food and tested the effect on changes in body mass, torpor use and seasonal sexual maturation in male parti-coloured bats (Vespertilio murinus). Food-restricted individuals managed to maintain their initial body mass, while in well-fed males, mass increased. Interestingly, despite large differences in food availability, there were only small differences in torpor patterns. However, well-fed males reached sexual maturity up to half a month earlier. Our results thus reveal a complex trade-off in resource allocation; independent of resource availability, males maintain a similar thermoregulation strategy and favour fast sexual maturation, but limited resources and low body mass moderate this latter process. male Unmet Need for Family Planning and Experience of Unintended Pregnancy Among Female Sex Workers in Urban Cameroon: Results From a National Cross-Sectional Study By ghspjournal.org Published On :: 2020-03-31T15:28:55-07:00 ABSTRACTBackground:Female sex workers (FSWs) in Cameroon commonly have unmet need for contraception posing a high risk of unintended pregnancy. Unintended pregnancy leads to a range of outcomes, and due to legal restrictions, FSWs often seek unsafe abortions. Aside from the high burden of HIV, little is known about the broader sexual and reproductive health of FSWs in Cameroon.Methods:From December 2015 to October 2016, we recruited FSWs aged ≥18 years through respondent-driven sampling across 5 Cameroonian cities. Cross-sectional data were collected through a behavioral questionnaire. Modified-robust Poisson regression was used to approximate adjusted prevalence ratios (aPR) for TOP and current use of effective nonbarrier contraception.Results:Among 2,255 FSWs (median age 28 years), 57.6% reported history of unintended pregnancy and 40.0% reported prior TOP. In multivariable analysis, TOP history was associated with current nonbarrier contraceptive use (aPR=1.23, 95% confidence interval [CI]=1.07, 1.42); ever using emergency contraception (aPR=1.34, 95% CI=1.17, 1.55); >60 clients in the past month (aPR=1.29, 95% CI= 1.07, 1.54) compared to ≤30; inconsistent condom use with clients (aPR=1.17, 95% CI=1.00, 1.37); ever experiencing physical violence (aPR=1.24, 95% CI=1.09, 1.42); and older age. Most (76.5%) women used male condoms for contraception, but only 33.2% reported consistent condom use with all partners. Overall, 26.4% of women reported currently using a nonbarrier contraceptive method, and 6.2% reported using a long-acting method. Previous TOP (aPR=1.41, 95%CI=1.16, 1.72) and ever using emergency contraception (aPR=2.70, 95% CI=2.23, 3.26) were associated with higher nonbarrier contraceptive use. Recent receipt of HIV information (aPR=0.72, 95% CI=0.59, 0.89) and membership in an FSW community-based organization (aPR=0.73, 95% CI=0.57, 0.92) were associated with lower use nonbarrier contraceptive use.Conclusions:Experience of unintended pregnancies and TOP is common among FSWs in Cameroon. Given the low use of nonbarrier contraceptive methods and inconsistent condom use, FSWs are at risk of repeat unintended pregnancies. Improved integration of client-centered, voluntary family planning within community-led HIV services may better support the sexual and reproductive health and human rights of FSWs consistent with the United Nations Declaration of Human Rights. We show here that the phenotypic sex of medaka is affected by changes in lipid metabolism. Medaka larvae subjected to 5 days of starvation underwent female-to-male sex reversal. Metabolomic and RT-qPCR analyses indicated that pantothenate metabolism was suppressed by starvation. Consistently, inhibiting the pantothenate metabolic pathway caused sex reversal. The final metabolite in this pathway is coenzyme A, an essential factor for lipogenesis. Inhibiting fatty acid synthesis, the first step of lipogenesis, also caused sex reversal. The expression of dmrt1, a critical gene for male development, was suppressed by starvation, and a dmrt1 (13) mutant did not show sex reversal under starvation. Collectively, these results indicate that fatty acid synthesis is involved in female-to-male sex reversal through ectopic expression of male gene dmrt1 under starvation. Full Article

We examined the sex distribution of recipients of Canadian residency association awards. Methods: We conducted a retrospective observational study of the sex of staff and resident physician recipients of resident-selected awards from provincial and national residency associations using data from 2000–2018. We classified awards into professionalism, advocacy and wellness awards, and education and teaching awards based on award names and descriptions, and compared the proportion of male and female recipients in these categories. Results: We identified 314 recipients of staff physician awards and 129 recipients of resident physician awards. Male staff and resident physicians had higher odds of receiving awards than their female counterparts (odds ratio [OR] 1.45, 95% confidence interval [CI] 1.13–1.89 and OR 1.70, 95% CI 1.18–2.46, respectively). There was a reduction in the odds of male residents’ receiving an award over the study period (OR 0.94, 95% CI 0.90–0.98). Male physicians had higher odds of receiving education and teaching awards than female physicians as staff but not as residents (OR 3.21, 95% CI 1.72–5.95 and OR 1.96, 95% CI 0.84–4.60, respectively). Interpretation: Male staff and resident physicians in Canada had higher odds of receiving awards from provincial and national residency associations between 2000 and 2018 than their female counterparts. Given this disparity, it would be prudent for organizations that distribute awards to physicians, residents and medical students to examine their nomination criteria and processes for potential bias. Chances are, no matter what you’re wearing, what time of the day it is, or where you are, you’re going to get stared at, commented on, sung at, touched, photographed, or a beautiful combination of all of the above. Being a woman who likes to backpack across different places in the country, I’ve seen that this has been a universal experience – though perhaps in varying degrees of intensity – no matter where I’ve travelled, whether it’s been in Maharashtra, Delhi, Punjab, Kerala, Karnataka, Himachal Pradesh, Goa, or Assam. But, a couple of months ago, on a trip to Meghalaya, I was filled with a sense of wonder. Sure, the views were jaw-dropping levels of gorgeous, and I was filling my tummy with delicious pork and beef specialties that I’d never get in my home city of Mumbai, but the best part was truly the feeling of walking around and feeling “like a man”. Nah, I didn’t ingest an exotic substance that made me gain a couple of inches (I meant of height, what were you thinking?) or grow a moustache. My voice didn’t get a whole lot deeper and I didn’t develop a sudden disdain for my flowery night shorts. It’s just that I was given a teensy, temporary insight into male privilege. Image courtesy: Aamna Khan I was travelling with a woman friend, Aamna Khan, and given that the two of us were on a budget, we often had to walk or wait for public transport from place to place. It didn’t help that it got pitch dark by six every evening and that public transport was usually restricted to a share taxi of some sort. But no matter how early we set out or how dark it was by the time we got back to our base, we had this overwhelming feeling of safety. It wasn’t just that we weren’t getting felt up (or worse); we felt it in the smallest things. Though we looked very much like tourists with our backpacks and cameras, nobody forced conversation with us. When they did speak to us, everyone was exactly the right level of friendly – curious about where we came from and where we’d be travelling without being overly specific, without questions of how much money we were spending or whether our families were okay with us travelling alone (which are real questions we’d both been asked multiple times while travelling in other places). Image courtesy: Batista The kids who stared at us were only excited about waving at us and yelling out “hellooooo!” from their school buses. Women weren’t judgmental about what we wore or how loudly we spoke. Men smiled at us and wished us luck on our travels. They didn’t stare at us hoping to develop X-ray vision to see through our clothes the way a lot of men in the rest of our country do. When squeezed next to us in share cabs – despite the fact that there were usually five other people in the back seat of an Alto – they took care to ensure that we were comfortable. They avoided ‘innocuous’ brushes of their hands against our bodies, and some of them even asked us if we minded that they were playing Khasi music in the car. We stood out like sore thumbs (for one, we were nowhere close to as well-dressed or attractive as the local women), but nobody took photos of us without our consent. Image courtesy: Antara Telang We even spent one night at a campsite run by five men in the middle of nowhere, where we were the only two women, without the slightest discomfort (excluding the bugs that we’d inevitably find in the folds of our clothes). Because I am a disabled woman, I am used to even greater scrutiny and questions than nondisabled women are, but even that didn’t really make much of a difference to people in Meghalaya. If you’re a man reading this, you’re probably thinking, “What’s the big deal?” But like I mentioned earlier, living each day like this for nine whole days at a stretch without exception was a shocker for my friend and me. Every evening when we’d come back, we’d look at each other with incredulity that yet another day had passed without lecherous vultures swooping down on us in one way or another. While one cannot deny the privilege my friend and I carry – of being well-dressed, English speaking, upper caste, urban women – the fact remains that we have never felt so safe from daily gender violence as we did on that trip to Meghalaya. I think a large part of this can be attributed to Meghalaya’s largely matrilineal culture. Though not a matriarchal society, women enjoy a far better position there than in most other parts of India, and indeed the world. It is common to see businesses completely run by women, and for women to be roaming freely on the streets. Whatever the root cause may be, I’ve never felt that way in my adult life, no matter which part of India or the world I’ve travelled in. And it’s for that reason (okay, yes, maybe the pork curry is another major reason) that I’m sure I’ll visit again. (Views expressed are author's own) Full Article

Unleashing the creative potential of half of Japan’s population will require an equally sustained effort to promote female entrepreneurship. This is a tall order for Japan where female entrepreneurs face a two-fold challenge: the modest development of venture capital and a host of legal and cultural hurdles to individual entrepreneurship; plus the additional hurdles for women in gaining access to the assets widely perceived as essential to success such as business networks, financing, technology, and access to markets at home and abroad. However, entrepreneurship offers Japanese women significant benefits through the opportunity to bypass rigid corporate hierarchies, custom tailor their workloads to better achieve work-life balance, and offer new and innovative products and services to the Japanese consumer. On February 8, the Center for East Asia Policy Studies at Brookings hosted a distinguished group of policy experts and entrepreneurs for a discussion on the current state of female entrepreneurship in Japan and concrete strategies to promote female-run businesses in the country. They compared Japan and the United States, both in terms in differing results but also on-going common challenges, and discussed their own personal experiences.
