Haiti replaces PM, marking more turmoil in its democratic transition process

Port-au-Prince, Haiti — A transitional council created to reestablish democratic order in Haiti signed a degree Sunday firing the country's interim Prime Minister Garry Conille and replacing him with Alix Didier Fils-Aime, a businessman who was previously considered for the job. The decree, set to be published on Monday, was provided to The Associated Press by a government source. It marks even more turmoil in an already rocky democratic transition process for Haiti, which hasn't held democratic elections in years in large part due to the soaring levels of gang violence plaguing the Caribbean nation. Fils-Aime, is the former president of Haiti's Chamber of Commerce and Industry and in 2015 ran an unsuccessful campaign for Senate. The businessman studied at Boston University and was previously considered for the position as a private sector candidate for the post before Conille took the seat. Conille, a longtime civil servant who has worked with the United Nations, served as prime minister for only six months. The transitional council was established in April, tasked with choosing Haiti's next prime minister and Cabinet with the hope that it would help quell turmoil Haiti. But the council has been plagued with politics and infighting and has long been at odds with Conille. Organizations like the Organization of American States tried and failed last week to mediate disagreements in an attempt to save the fragile transition, according to reporting from The Miami Herald. The process suffered another blow in October when three members on the council faced corruption accusations, from anti-corruption investigators alleging that they demanded $750,000 in bribes from a government bank director to secure his job. The report was a significant blow to the nine-member council and is expected to further erode people's trust in it. Those same members accused of bribery, Smith Augustin, Emmanuel Vertilaire and Louis Gerald Gilles, were among those to sign the decree. Only one member, Edgard Leblanc Fils, did not sign the order.

Impact of COVID-19 on Chronic Ambulatory-Care-Sensitive Condition Emergency Department Use Among Older Adults

Background:

The COVID-19 pandemic social distancing requirements encouraged patients to avoid public spaces including in-office health care visits. Ambulatory-care-sensitive conditions (ACSCs) represent conditions that can be managed with quality primary care and when access is limited, these conditions can lead to avoidable emergency department (ED) visits.

Methods:

Using national data on ED visits from 2019 to 2021 in the National Hospital Ambulatory Care Survey, we examined the impact of COVID-19 pandemic on ACSC ED visits among older adults (aged ≥65).

Results:

The proportion of ED visits among older adults that were for ACSCs increased between 2019 (17.4%) and 2021 (18.5%). The trend in both rural (26.4%–28.6%) and urban areas (15.4%–16.8%) shows a significant jump from 2019 to 2021 (P < .001).

Conclusions:

This rise in ACSC ED use is consistent with a delay in normal primary care during the pandemic.

Assessing Patient Readiness for Hospital Discharge, Discharge Communication, and Transitional Care Management

Background:

Discharge communication between hospitalists and primary care clinicians is essential to improve care coordination, minimize adverse events, and decrease unplanned health services use. Health-related social needs are key drivers of health, and hospitalists and primary care clinicians value communicating social needs at discharge.

Objective:

To 1) characterize the current state of discharge communications between an academic medical center hospital and primary care clinicians at associated clinics; 2) seek feedback about the potential usefulness of discharge readiness information to primary care clinicians.

Design:

Exploratory, convergent mixed methods.

Participants:

Primary care clinicians from Family Medicine and General Internal Medicine of an academic medical center in the US Intermountain West.

Approach:

Literature-informed REDCap survey. Semistructured interview guide developed with key informants, grounded in current literature. Survey data were descriptively summarized; interview data were deductively and inductively coded, organized by topics.

Results:

Two key topics emerged: 1) discharge communication, with interrelated topics of transitional care management and follow-up appointment challenges, and recommendations for improving discharge communication; and 2) usefulness of the discharge readiness information, included interrelated topics related to lack of shared understanding about roles and responsibilities across settings and ethical concerns related to identifying problems that may not have solutions.

Conclusions:

While reiterating perennial discharge communication and transitional care management challenges, this study reveals new evidence about how these issues are interrelated with assessing and responding to patients’ lack of readiness for discharge and unmet social needs during care transitions. Primary care clinicians had mixed views on the usefulness of discharge readiness information. We offer recommendations for improving discharge communication and transitional care management (TCM) processes, which may be applicable in other care settings.

Development and Validation of a Customized Amplex UltraRed Assay for Sensitive Hydrogen Peroxide Detection in Pharmaceutical Water

For clean-room technologies such as isolators and restricted access barrier systems (RABS), decontamination using hydrogen peroxide (H2O2) is increasingly attractive to fulfill regulatory requirements. Several approaches are currently used, ranging from manual wipe disinfection to vapor phase hydrogen peroxide (VPHP) or automated nebulization sanitization. Although the residual airborne H2O2 concentration can be easily monitored, detection of trace H2O2 residues in filled products is rather challenging. To simulate the filling process in a specific clean room, technical runs with water for injection (WfI) are popular. Thus, the ability to detect traces of H2O2 in water is an important prerequisite to ensure a safe and reliable use of H2O2 for isolator or clean room decontamination. The objective of this study was to provide a validated quantitative, fluorometric Amplex UltraRed assay, which satisfies the analytical target profile of quantifying H2O2 in WfI at low nanomolar to low micromolar concentrations (ppb range) with high accuracy and high precision. The Amplex UltraRed technology provides a solid basis for this purpose; however, no commercial assay kit that fulfills these requirements is available. Therefore, a customized Amplex UltraRed assay was developed, optimized, and validated. This approach resulted in an assay that is capable of quantifying H2O2 in WfI selectively, sensitively, accurately, precisely, and robustly. This assay is used in process development and qualification approaches using WfI in H2O2-decontaminated clean rooms and isolators.

Could cough hypersensitivity symptom profile differentiate phenotypes of chronic cough?

Background

Recently, cough reflex hypersensitivity has been proposed as a common underlying feature of chronic cough in adults. However, symptoms and clinical characteristics of cough hypersensitivity have not been studied amongst phenotypes of chronic cough. This study aimed to compare symptom features, such as cough triggers and associated throat sensations, of cough hypersensitivity in patients with asthmatic chronic cough and those with refractory chronic cough (RCC).

Methods

Patients with chronic cough from the Korean Chronic Cough Registry were prospectively evaluated over 6 months. Physicians determined the aetiological diagnosis based on clinical evaluations and responses to treatment at the 6-month follow-up visit. Symptoms of cough hypersensitivity and cough-specific quality of life were assessed using the Cough Hypersensitivity Questionnaire (CHQ) and the Leicester Cough Questionnaire (LCQ), respectively.

Results

The analysis included 280 patients who completed the follow-up: 79 with asthmatic cough (cough variant asthma or eosinophilic bronchitis) and 201 with RCC. Baseline CHQ scores were similar between the groups (8.3±3.7 in asthmatic cough versus 8.9±3.9 in RCC; p=0.215, adjusted for age, sex and LCQ score). There were no significant between-group differences in the LCQ and cough severity Visual Analog Scale scores. Both groups showed a similar negative correlation with LCQ scores (asthmatic cough: r=–0.427, p<0.001; RCC: r=–0.306, p<0.001).

Conclusions

The symptoms of cough hypersensitivity may not distinguish between asthmatic cough and RCC. This suggests that chronic cough is the primary diagnosis in both phenotypes. It indicates a shared mechanism in their cough pathogenesis, despite having potentially different treatable traits.

Validation of a Simplified Tissue-to-Reference Ratio Measurement Using SUVR to Assess Synaptic Density Alterations in Alzheimer Disease with [11C]UCB-J PET

Simplified methods of acquisition and quantification would facilitate the use of synaptic density imaging in multicenter and longitudinal studies of Alzheimer disease (AD). We validated a simplified tissue-to-reference ratio method using SUV ratios (SUVRs) for estimating synaptic density with [11C]UCB-J PET. Methods: Participants included 31 older adults with AD and 16 with normal cognition. The distribution volume ratio (DVR) using simplified reference tissue model 2 was compared with SUVR at short scan windows using a whole-cerebellum reference region. Results: Synaptic density was reduced in AD participants using DVR or SUVR. SUVR using later scan windows (60–90 or 70–90 min) was minimally biased, with the strongest correlation with DVR. Effect sizes using SUVR at these late time windows were minimally reduced compared with effect sizes with DVR. Conclusion: A simplified tissue-to-reference method may be useful for multicenter and longitudinal studies seeking to measure synaptic density in AD.

As with the streaming giant's existing labels for M-rated material, sexual themes or depictions of gambling, the idea is that viewers can filter out such streams in advance by altering their settings.

Advertisers, similarly, can "make better choices about the content they want to advertise next to" - in other words, pull their ads from a whole swathe of material if they don't want to be associated with anything controversial. Twitch's hope is that "the labels will allow advertisers to have more context to inform which types of streams they show their ads alongside, which we expect to increase brands’ confidence in running ads on Twitch, and could bring new advertisers to our service."

Who Supports Gender Quotas in Transitioning and Authoritarian States in the Middle East and North Africa?

What are the drivers of citizens’ support for electoral gender quotas in transitioning and authoritarian states? Despite extensive research examining public support for women in politics in democracies, we know little about how the public perceives them in less democratic settings. To address this shortcoming, we use original survey data from authoritarian Morocco and transitioning Tunisia – two Arab countries hailed for their progressive gender policies. We argue that in these countries where citizens lack political information, they instead rely on their assessment of the government’s performance to form attitudes toward gender quotas. Furthermore, electoral legitimacy plays an important role in shaping citizens’ support for quotas, which are closely linked to how elections and legislatures operate. The findings offer strong support for our theoretical expectations and uncover important gender differences.
