Prognostic Models for Stillbirth and Neonatal Death in Very Preterm Birth: A Validation Study

Two UK models predict the risk of mortality in very preterm Western infants (1) alive at onset of labor and (2) admitted for neonatal intensive care. Prognostic models need temporal and geographic validation to evaluate their performance.

Validation of a Clinical Prediction Rule to Distinguish Lyme Meningitis From Aseptic Meningitis

Available clinical prediction rules to identify children with cerebrospinal fluid pleocytosis at low risk for Lyme meningitis include headache duration, cranial nerve palsy, and percent cerebrospinal fluid mononuclear cells. These rules require independent validation.

These clinical prediction rules accurately identify patients at low risk for Lyme meningitis in our large multicenter cohort. Children at low risk may be considered for outpatient management while awaiting Lyme serology. (Read the full article)

Concurrent Validity of Ages and Stages Questionnaires in Preterm Infants

Preterm children born between 29 and 36 gestational weeks are at higher risk of developmental delay. The Ages and Stages Questionnaires (ASQ) have been recommended as a developmental screening tool.

At 12 months’ corrected age (CA), the ASQ was insufficient in identifying delays on both mental and psychomotor scales of the Bayley Scales of Infant Development but was accurate in detecting mental delay at CA of 24 months. (Read the full article)

The Rapid Neurodevelopmental Assessment (RNDA) tool has been shown to be reliable and valid for assessment of a range of neurodevelopmental impairments (NDIs) and disabilities in children younger than 2 years. There is currently a lack of professional expertise for assessing NDIs in 2- to 5-year-old children in low- and middle-income countries.

We developed a set of instruments as part of the RNDA for administration by a single professional with experience in child development to assess >2- to 5-year-old children for a wide range of NDIs. The tool was acceptable to mothers, interrater reliability was high, and proportions of children with NDIs were elevated among the lowest income groups and in stunted children, demonstrating discriminant validity. The RNDA was valid for identifying >2- to 5-year-old children with a range of NDIs, especially in cognitive, behavior, and motor functions. Adherence to infant feeding recommendations in the United States is low. The prevalence of early introduction of solid foods (<4 months of age) in the United States has been estimated to range from 19% to 29%.

Timing of Solid Food Introduction and Obesity: Hong Kong's "Children of 1997" Birth Cohort

Some Western studies show early introduction of solid food is associated with subsequent obesity. However, introduction of solid food and obesity share social patterning, making these observations vulnerable to residual confounding.

In a non-Western developed setting, there was no clear association of the early introduction of solid food with childhood obesity. Validity of the Ages and Stages Questionnaires in Term and Preterm Infants

The Ages and Stages Questionnaires (ASQ) has been validated in many countries and translated into numerous languages. In most publications, it has been reported that the ASQ is accurate in detecting true problems in apparently healthy children and even in children with biological risk factors.

This report compares the third version of the ASQ and the Bayley Scales of Infant and Toddler Development, Third Edition, assessments. Psychometric properties showed a tendency to improve with testing age and when comparing term versus extremely preterm children. (Read the full article)

The Faces Pain Scale–Revised and Color Analog Scale are self-report pain scales that are commonly used for children in the clinical and research settings.

The Faces Pain Scale–Revised and Color Analog Scale overall demonstrate strong psychometric properties in children 4 to 17 years of age, including within subgroups of age, sex, and ethnicity. Convergent validity, however, is questionable in children <7 years old. (Read the full article)

Validity of Different Pediatric Early Warning Scores in the Emergency Department

Pediatric early warning scores (PEWS) for hospital inpatients have been developed to identify patients at risk for deterioration. Beyond triage, similar systems that identify ill patients and predict requirements for a higher level of care are needed in the emergency department.

End-Stage Kidney Disease After Pediatric Nonrenal Solid Organ Transplantation

End-stage kidney disease (ESKD) causes significant morbidity and mortality after solid organ transplantation. Adults commonly develop advanced kidney disease, particularly after liver and intestinal transplantation. Previous pediatric studies have not compared the relative incidence of ESKD by organ type.

This national cohort study shows the highest risk of ESKD among pediatric lung and intestinal transplant recipients, reflecting unique organ-specific causes of kidney injury. Longitudinal Validation of a Tool for Asthma Self-Monitoring

To prevent asthma exacerbations, asthma guidelines recommend ongoing monitoring of patients’ asthma symptoms to promote timely adjustments of therapy to achieve and maintain optimal control. Existing tools, validated for ongoing monitoring, have significant limitations in children.

Screening for autism spectrum disorders (ASDs) using the Modified Checklist for Autism in Toddlers (M-CHAT) improves early detection and long-term prognosis of ASD. Reducing the false-positive rate may increase implementation of screening for ASDs.

The Modified Checklist for Autism in Toddlers, Revised with Follow-up (M-CHAT-R/F), simplifies wording of the original M-CHAT. Validity of Brief Screening Instrument for Adolescent Tobacco, Alcohol, and Drug Use

The widely disseminated National Institute on Alcohol Abuse and Alcoholism screening tool for adolescent alcohol use was developed based on epidemiologic data. It has not been validated in a clinical sample and does not screen for tobacco or drug use.
