A 47-year-old truck driver died after delivering 1,500-pound bales of recycled cardboard to a paper production mill.

On Feb. 28, 2020, a forklift operator died after being crushed by a loaded pallet of soft drink cans.

A 64-year-old truck driver and equipment operator working for a gravel-hauling and equipment-moving company drove his employer’s truck with an attached flatbed trailer to a jobsite.

A 54-year-old forklift operator and his co-worker were installing large metal storage racks in a warehouse.

A 57-year-old delivery driver was killed after being struck by a slab of granite in the parking lot of a countertop wholesaler. The wholesaler secured granite slabs for transport on a flatbed trailer using a metal A-frame rack that held multiple slabs in a near-vertical orientation.

A 54-year-old worker died when the raised boom of a backhoe loader swung and crushed him against one of the machine’s retracted stabilizers.

On the day of the incident, a semitruck with a trailer backed up to a supermarket chain’s loading dock. A freight clerk opened the back of the trailer, checked the freight and determined that the truck was at the wrong dock.

A semitruck driver arrived at a construction site to deliver roofing materials on a flatbed trailer. Because the load left no room for the driver to bring a forklift, he asked workers at the site to use their company’s telehandler forklift to unload the trailer.

A 29-year-old steelworker was using a telehandler to move and stage insulated metal panels at a municipal garage building site. He had received training on the equipment and had an operator’s permit.

A 61-year-old loader operator was killed while attempting to repair a hydraulic line on a front-end loader used to move logs. The company had a written lockout/tagout program, but the program did not contain specific procedures for the front-end loader maintenance.

A 62-year-old mechanic died after being crushed when the 994-pound electrical cabinet he was working on tipped over.

Nearly half of all work-related injuries involving mechanical power presses result in amputation, statistics compiled by OSHA show. Around 60% of amputations involve a worker’s fingers or arm getting caught or compressed by a press or other machinery such as a conveyer, according to data from the Bureau of Labor Statistics.

The heavy nuclear-powered missile cruiser Admiral Nakhimov, which is undergoing repairs and extensive modernization at Sevmash shipyard, will begin trials in the upcoming autumn of 2024, the Commander-in-Chief of the Russian Navy, Admiral Alexander Moiseyev told TASS. "The Admiral Nakhimov is to go to sea for factory sea trials in the autumn of 2024. She is to be transferred to the Navy in 2025," he said during the Army-2024 Forum. Admiral Nakhimov of Project 1144.2M has been under repair since 1999, although the works started 2013. During the modernization, the cruiser will receive a universal shipborne firing system 3S14 to carry Kalibr, Onyx, or Zircon cruise missiles.

The growth quality of AlN single crystals was improved by optimizing the crucible structure for Al vapor transport with the help of finite element simulation.

In the conventional crucible structure for AlN crystal growth by physical vapor transport, owing to the long molecular transport path of Al vapor and the disruption of the gas flow by the presence of a deflector, the Al vapor easily forms polycrystals in the growth domain. The result is increased internal stress in the crystals and increased difficulty in growing large-sized crystals. On this basis, with the help of finite element simulations, a novel crucible structure is designed. This crucible not only optimizes the gas transport but also increases the radial gradient of the AlN crystal surface, making the enhanced growth rate in the central region more obvious. The thermal stresses between the deflector and the crystal are also reduced. High-quality AlN crystals with an FWHM of 79 arcsec were successfully grown with this structure, verifying the accuracy of finite element simulation of the growth of AlN crystals. Our work has important guiding significance for the growth of high-quality AlN crystals.
