Tropical Storm Rafael strengthens into Category 1 hurricane, barrels toward Cuba

SAN JUAN, Puerto Rico — Tropical Storm Rafael strengthened into a Category 1 hurricane on Tuesday as it swirled past the Cayman Islands and chugged toward western Cuba.  It was another stroke of bad news for Cuba, which has been struggling with blackouts while recovering from another hurricane two weeks ago that killed at least six people in the eastern part of the island.  The storm was located 20 miles (35 kilometers) southeast of Little Cayman in the Cayman Islands on Tuesday morning. It had maximum sustained winds of 75 miles per hour (120 kilometers per hour) and was moving northwest at 15 mph (24 kph), according to the National Hurricane Center in Miami.    Forecasters warned that Rafael was expected to slam into Cuba on Wednesday after dumping rain on Jamaica and the Cayman Islands on Tuesday. The center warned of floods, storm surges and mudslides.  The U.S. State Department issued an advisory for Cuba on Tuesday afternoon, offering departure flights to non-essential staff and American citizens, and advising others to “reconsider travel to Cuba due to the potential impact of Tropical Storm Rafael.”  On Tuesday morning, the Cuban Civil Defense called on Cubans to prepare as soon as possible, because when the storm makes landfall “it’s important to stay where you are.” The day before, authorities said they had issued an evacuation order for 37,000 people in far eastern Cuba, in the province of Guantanamo, due to bad weather.  A hurricane warning was in effect Tuesday for the Cayman Islands and the Cuban provinces of Pinar del Rio, Artemisa, La Habana, Mayabeque, Matanzas and the Isle of Youth.    A tropical storm warning was in effect for the Cuban provinces of Villa Clara, Cienfuegos, Sancti Spiritus and Ciego de Avila, as well as the lower and middle Florida Keys from Key West to west of the Channel 5 Bridge, and Dry Tortugas. The warning was lifted in Jamaica after the storm passed by the western coast. A tropical storm watch was in effect for the Cuban provinces of Camaguey and Las Tunas.  Officials in the Cayman Islands closed schools and government offices as they urged residents to prepare. Long lines were reported at grocery stores as the storm approached.    Forecasters warned Rafael would unleash heavy rains across the western Caribbean that could lead to flooding and mudslides in parts of Cuba and the Cayman Islands.  Heavy rainfall also was expected to spread north into Florida and nearby areas of the southeast U.S. during the middle to late part of the week. The Hurricane Center predicted storm surges in Florida could reach 1 to 3 feet in Dry Tortugas and 1 to 2 feet in the Lower Florida Keys. A few tornadoes also were expected Wednesday over the Keys and southwestern Florida.    Rafael is the 17th named storm of the season.  The National Oceanic and Atmospheric Administration predicted the 2024 hurricane season was likely to be well above average, with between 17 and 25 named storms. The forecast called for as many as 13 hurricanes and four major hurricanes.    An average Atlantic hurricane season produces 14 named storms, seven of them hurricanes and three major hurricanes.

HAVANA — Rafael strengthened Wednesday into a powerful Category 3 hurricane ahead of its expected landfall in western Cuba, where it was forecast to bring "life-threatening" storm surges, winds and flash floods.  The storm, which knocked out power and dumped rain on the Cayman Islands and Jamaica, is expected to hit the Isle of Youth in the coming hours and make landfall later on Wednesday.  Classes and public transport were suspended on parts of the island as authorities issued an alarm for the incoming weather for the west of the country. Workers secured buildings and cleaned up garbage along Havana's coastline in preparation for floods. Authorities also canceled flights in certain areas like Havana and Varadero. Thousands of people in the west of the island were evacuated as a prevention measure.  "Preparations to protect life and property should be rushed to completion," warned the National Hurricane Center in Miami.  The storm was located about 65 kilometers (40 miles) east-southeast of the Isle of Youth and around 135 kilometers (84 miles) south-southeast of Havana. It had maximum sustained winds of 185 kph (115 mph) and was moving northwest at 22 kph (14 mph), according to the National Hurricane Center in Miami.  Forecasters expected the storm to later weaken over Cuba, but emerge in the southeastern Gulf of Mexico as a hurricane.  Cubans have already been struggling with blackouts while recovering from another hurricane two weeks ago that killed at least six people in the eastern part of the island.  The U.S. State Department issued an advisory for Cuba on Tuesday afternoon, offering departure flights to non-essential staff and American citizens, and advising others to "reconsider travel to Cuba due to the potential impact of Tropical Storm Rafael."  On Tuesday morning, the Cuban Civil Defense called on Cubans to prepare as soon as possible, because when the storm makes landfall "it's important to stay where you are."  Silvia Perez, a 72-year-old retiree living in a coastal area of Havana, was among those scrambling to prepare. As other neighbors moved appliances and other furniture from ground floor homes, worried about floods, Perez stocked up on water and food.  "This is a night I don't want to sleep through, between the battering air and the trees," Perez said. "I'm scared for my friends and family."  A hurricane warning was in effect on Wednesday for a portion of the Cayman Islands and the Cuban provinces of Pinar del Rio, Artemisa, La Habana, Mayabeque, Matanzas and the Isle of Youth.  A tropical storm warning was in effect for the Cuban provinces of Villa Clara, Cienfuegos, Sancti Spiritus and Ciego de Avila, as well as the lower and middle Florida Keys from Key West to west of the Channel 5 Bridge, and Dry Tortugas.  The storm on Tuesday knocked out power in parts of Jamaica and unleashed flooding and landslides. How Early Career Family Medicine Women Physicians Negotiate Their First Job After Residency

Background:

Nested within a growing body of evidence of a gender pay gap in medicine are more alarming recent findings from family medicine: a gender pay gap of 16% can be detected at a very early career stage. This article explores qualitative evidence of women’s experiences negotiating for their first job out of residency to ascertain women’s engagement with and approach to the negotiation process.

Methods:

We recruited family physicians who graduated residency in 2019 and responded to the American Board of Family Medicine 2022 graduate survey. We developed a semistructured interview guide following a modified life history approach to uncover women’s experiences through the transitory stages from residency to workforce. A qualitative researcher used Zoom to interview 19 geographically and racially diverse early career women physicians. Interviews were transcribed verbatim and analyzed using NVivo software following an Inductive Content Analysis approach.

Results:

Three main themes emerged from the data. First, salary was found to be nonnegotiable, exemplified by participants’ inability to change initial salary offers. Second, the role of peer support throughout residency and early career was crucial to uncovering and rectifying salary inequity. Third, a pay expectation gap was identified among women from minority and low-income households.

Conclusion:

To rectify the gender pay gap in medicine, a systems-level approach is required. This can be achieved through various levels of interventions: societally expanding the use of and removing the stigma around parental leave, recognizing the importance of contributions not currently valued by productivity-based payment models, examining assumptions about leadership; and institutionally moving away from fee-for-service systems, encouraging flexible schedules, increasing salary transparency, and improving advancement transparency.

The influence of categorical stimuli on relational memory binding [RESEARCH PAPERS]

Binding of arbitrary information into distinct memory representations that can be used to guide behavior is a hallmark of relational memory. What is and is not bound into a memory representation and how those things influence the organization of that representation remain topics of interest. While some information is intentionally and effortfully bound—often the information that is consistent with task goals or expectations about what information may be required later—other information appears to be bound automatically. The present set of experiments sought to investigate whether spatial memory would be systematically influenced by the presence and absence of distinct categories of stimuli on a spatial reconstruction task. In this task, participants must learn multiple item-location bindings and place each item back in its studied location after a short delay. Across three experiments, participants made significantly more within-category errors (i.e., misassigning one item to the location of a different item from the same category) than between-category errors (i.e., misassigning one item to the location of an item from a different category) when categories were perceptually or semantically distinct. These data reveal that category information contributed to the organization of the memory representation and influenced spatial reconstruction performance. Together, these results suggest that categorical information can influence memory organization, and not always to the benefit of overall task performance.

Negotiating with North Korea: Key Lessons Learned from Negotiators' Genesis Period

Only a small handful of people in the world have sat at the negotiating table with the North Koreans and extensively interacted with them. Yet, this knowledge is fragmented and has not been collected or analyzed in a systematic manner. This report captures the findings from in-depth, one-on-one interviews with former senior negotiators from the United States and South Korea, who gained unique knowledge about North Korean negotiating behavior by dealing directly with their high-level North Korean counterparts. 

These negotiators collectively represent a body of negotiation experience and expertise starting from the early 1990s to late 2019, when North Korea ceased all negotiations with the United States. During that time, the conditions for productive negotiation changed dramatically – indeed, the conditions for the 1994 U.S.-North Korea Agreed Framework negotiations were much more favorable than during the Six-Party Talks of the mid-2000s or the Season of Summits during 2018-2019. For the “Negotiating with North Korea: Key Lessons Learned from Negotiators’ Genesis Period” project, a spotlight was placed on former senior negotiators’ early-stage experience preparing for and engaging in negotiations with the North Koreans. In doing so, tacit knowledge was captured to serve as a resource for future negotiators to inform and accelerate their own genesis period.
