Identifying this variation is typically achieved by aligning sequence reads to a reference genome, but this approach is susceptible to reference biases and requires careful filtering of called genotypes. There is a need for tools that can process this growing volume of bacterial genome data, providing rapid results, but that remain simple so they can be used without highly trained bioinformaticians, expensive data analysis, and long-term storage and processing of large files. Here we describe split k-mer analysis (SKA2), a method that supports both reference-free and reference-based mapping to quickly and accurately genotype populations of bacteria using sequencing reads or genome assemblies. SKA2 is highly accurate for closely related samples, and in outbreak simulations, we show superior variant recall compared with reference-based methods, with no false positives. SKA2 can also accurately map variants to a reference and be used with recombination detection methods to rapidly reconstruct vertical evolutionary history. SKA2 is many times faster than comparable methods and can be used to add new genomes to an existing call set, allowing sequential use without the need to reanalyze entire collections. With an inherent absence of reference bias, high accuracy, and a robust implementation, SKA2 has the potential to become the tool of choice for genotyping bacteria. Timeliness is key, but sequence alignment and phylogeny slow most surveillance techniques. Millions of SARS-CoV-2 genomes have been assembled. Phylogenetic methods are ill equipped to handle this sheer scale. We introduce a pangenomic measure that examines the information diversity of a k-mer library drawn from a country's complete set of clinical, pooled, or wastewater sequence. Quantifying diversity is central to ecology. Hill numbers, or the effective number of species in a sample, provide a simple metric for comparing species diversity across environments. The more diverse the sample, the higher the Hill number. We adopt this ecological approach and consider each k-mer an individual and each genome a transect in the pangenome of the species. Structured in this way, Hill numbers summarize the temporal trajectory of pandemic variants, collapsing each day's assemblies into genome equivalents. For pooled or wastewater sequence, we instead compare days using survey sequence divorced from individual infections. Across data from the UK, USA, and South Africa, we trace the ascendance of new variants of concern as they emerge in local populations well before these variants are named and added to phylogenetic databases. Using data from San Diego wastewater, we monitor these same population changes from raw, unassembled sequence. This history of emerging variants senses all available data as it is sequenced, intimating variant sweeps to dominance or declines to extinction at the leading edge of the COVID-19 pandemic. As a first step toward successful translation, our primary aim was to validate the FAPI PET uptake through immunohistochemistry in patients with advanced ILD who underwent lung transplantation after a FAPI PET scan. Methods: This is a preliminary analysis of a single-center, open-label, single-arm, prospective exploratory biodistribution study of 68Ga-FAPI-46 PET imaging in patients with ILD (NCT05365802). Patients with ILD confirmed by high-resolution CT and scheduled for lung transplant were included. Tissue samples of explanted lungs were obtained from both the central and peripheral lung parenchyma of each lobe. Additional samples were obtained from areas of the lung corresponding to regions of FAPI PET activity. Immunohistochemical staining was performed with an anti-FAP antibody. Percentages of FAP immunohistochemistry-positive area were measured semiautomatically using QuPath software. SUVs in the areas of pathologic samples were measured on FAPI PET/CT by referencing the gross photomap of the explanted lung. A Spearman correlation coefficient test was used to assess the relationship between FAPI PET uptake and FAP immunohistochemical expression in each specimen. Results: Four patients with advanced ILD who underwent FAPI PET/CT before lung transplantation were included. The types of ILD were idiopathic pulmonary fibrosis (n = 2), rheumatoid arthritis–associated ILD (n = 1), and nonspecific interstitial pneumonia (n = 1). FAPI uptake was visualized mainly in the fibrotic area on CT. Twenty-nine surgical pathology samples from 3 patients were analyzed. FAP staining was predominantly positive in fibroblastic foci. FAPI PET SUVmax and SUVmean showed a positive correlation with the immunohistochemical FAP expression score (SUVmax: r = 0.57, P = 0.001; SUVmean: r = 0.54, P = 0.002). Conclusion: In this analysis conducted in patients who underwent lung transplantation after a FAPI PET scan, FAPI PET uptake was positively correlated with FAP immunohistochemistry. These findings provide a rationale for further investigation of FAPI PET as a potential imaging biomarker for ILD. Full Article

Fibroblast activation protein (FAP) is one of key molecules in tissue fibrosis, and 68Ga-labeled FAP inhibitor-46 (FAPI46) PET is a recently developed method for evaluating FAP. The aim of this study was to evaluate FAP expression and fibrosis in a hypertension model and to test the feasibility of 68Ga-FAPI46 PET in hypertension. Methods: Hypertension was induced in mice by angiotensin II infusion for 4 wk. 68Ga-FAPI46 biodistribution studies and PET scanning were conducted at 1, 2, and 4 wk after hypertension modeling, and uptake in the major organs was measured. The FAP expression and fibrosis formation of the heart and kidney tissues were analyzed and compared with 68Ga-FAPI46 uptake. Subgroups of the hypertension model underwent angiotensin receptor blocker administration and high-dose FAPI46 blocking, for comparison. As a preliminary human study, 68Ga-FAPI46 PET images of lung cancer patients were analyzed and compared between hypertension and control groups. Results: Uptake of 68Ga-FAPI46 in the heart and kidneys was significantly higher in the hypertension group than in the sham group as early as week 1 and decreased after week 2. The uptake was specifically blocked in the high-dose blocking study. Immunohistochemistry also revealed FAP expression in both heart and kidney tissues. However, overt fibrosis was observed in the heart, whereas it was absent from the kidneys. The angiotensin receptor blocker–treated group showed lower uptake in the heart and kidneys than did the hypertension group. In the pilot human study, renal uptake of 68Ga-FAPI46 significantly differed between the hypertension and control groups. Conclusion: In hypertension, FAP expression is increased in the heart and kidneys from the early phases and decreases over time. FAP expression appears to represent fibrosis activity preceding or underlying fibrotic tissue formation. 68Ga-FAPI46 PET has potential as an effective imaging method for evaluating FAP expression in progressive fibrosis by hypertension. Methods: This study included 32 newly diagnosed LA-ESCC participants who underwent 18F-FAPI PET/CT at baseline, of whom 23 also underwent scanning after 2 cycles of nCC. The participants underwent surgery after 2 cycles of nCC. Recorded PET parameters included maximum, peak, and mean SUVs and tumor-to-background ratios (TBRs), metabolic tumor volume, and total lesion FAP expression. PET parameters were compared between patient groups with good and poor pathologic responses, and the predictive performance for treatment response was analyzed. Results: The good and poor response groups each included 16 participants (16/32, 50.0%). On 18F-FAPI PET/CT, the posttreatment SUVs were significantly lower in good responders than in poor responders, whereas the changes in SUVs with treatment were significantly higher (all P < 0.05). SUVmax (area under the curve [AUC], 0.87; P = 0.0026), SUVpeak (AUC, 0.89; P = 0.0017), SUVmean (AUC, 0.88; P = 0.0021), TBRmax (AUC, 0.86; P = 0.0031), and TBRmean (AUC, 0.88; P = 0.0021) after nCC were significant predictors of pathologic response to nCC, with sensitivities of 63.64%–81.82% and specificities of 83.33%–100%. Changes in SUVmax (AUC, 0.81; P = 0.0116), SUVpeak (AUC, 0.82; P = 0.0097), SUVmean (AUC, 0.81; P = 0.0116), and TBRmean (AUC, 0.74; P = 0.0489) also were significant predictors of the pathologic response to nCC, with sensitivities and specificities in similar ranges. Conclusion: 18F-FAPI PET/CT parameters after treatment and their changes from baseline can predict the pathologic response to nCC in LA-ESCC participants. Full Article

This study aimed to evaluate the utility of portable MRI (pMRI) in assessing dissemination in space (DIS) in patients presenting with optic neuritis and determine whether deploying pMRI in the MS clinic can shorten the time from symptom onset to MRI. MATERIALS AND METHODS: Newly diagnosed patients with optic neuritis referred to a tertiary academic MS center from July 2022 to January 2024 underwent both point-of-care pMRI and subsequent 3T conventional MRI (cMRI). Images were evaluated for periventricular (PV), juxtacortical (JC), and infratentorial (IT) lesions. DIS was determined on brain MRI per 2017 McDonald criteria. Test characteristics were computed by using cMRI as the reference. Interrater and intermodality agreement between pMRI and cMRI were evaluated by using the Cohen . Time from symptom onset to pMRI and cMRI during the study period was compared with the preceding 1.5 years before pMRI implementation by using Kruskal-Wallis with post hoc Dunn tests. RESULTS: Twenty patients (median age: 32.5 years [interquartile range {IQR}, 28–40]; 80% women) were included, of whom 9 (45%) and 5 (25%) had DIS on cMRI and pMRI, respectively. Median time interval between pMRI and cMRI was 7 days (IQR, 3.5–12.5). Interrater agreement was very good for PV (95%, = 0.89), and good for JC and IT lesions (90%, = 0.69 for both). Intermodality agreement was good for PV (90%, = 0.80) and JC (85%, = 0.63), and moderate for IT lesions (75%, = 0.42) and DIS (80%, = 0.58). pMRI had a sensitivity of 56% and specificity of 100% for DIS. The median time from symptom onset to pMRI was significantly shorter (8.5 days [IQR 7–12]) compared with the interval to cMRI before pMRI deployment (21 days [IQR 8–49], n = 50) and after pMRI deployment (15 days [IQR 12–29], n = 30) (both P < .01). Time from symptom onset to cMRI in those periods was not significantly different (P = .29). CONCLUSIONS: In patients with optic neuritis, pMRI exhibited moderate concordance, moderate sensitivity, and high specificity for DIS compared with cMRI. Its integration into the MS clinic reduced the time from symptom onset to MRI. Further studies are warranted to evaluate the role of pMRI in expediting early MS diagnosis and as an imaging tool in resource-limited settings. Full Article

They affect males and females, with an estimated worldwide prevalence of 1 in 5000 live births. These malformations are clinically heterogeneous and can be part of a syndromic presentation (syndromic ARM) or as a nonsyndromic entity (nonsyndromic ARM). Despite the well-recognized heritability of nonsyndromic ARM, the genetic etiology in most patients is unknown. In this study, we describe three siblings with diverse congenital anomalies of the genitourinary system, anemia, delayed milestones, and skeletal anomalies. Genome sequencing identified a novel, paternally inherited heterozygous Caudal type Homeobox 2 (CDX2) variant (c.722A > G (p.Glu241Gly)), that was present in all three affected siblings. The variant identified in this family is absent from population databases and predicted to be damaging by most in silico pathogenicity tools. So far, only two other reports implicate variants in CDX2 with ARMs. Remarkably, the individuals described in these studies had similar clinical phenotypes and genetic alterations in CDX2. CDX2 encodes a transcription factor and is considered the master regulator of gastrointestinal development. This variant maps to the homeobox domain of the encoded protein, which is critical for interaction with DNA targets. Our finding provides a potential molecular diagnosis for this family's condition and supports the role of CDX2 in anorectal anomalies. It also highlights the clinical heterogeneity and variable penetrance of ARM predisposition variants, another well-documented phenomenon. Finally, it underscores the diagnostic utility of genomic profiling of ARMs to identify the genetic etiology of these defects. Full Article

Lung biopsy is traditionally required to establish the diagnosis. We report a 22-mo-old male who presented with anemia, severe pulmonary hypertension, and right heart failure. He had a complicated hospital course resulting in cardiac arrest and requirement for extracorporeal membrane oxygenation. Computed tomography of the chest showed a heterogenous pattern of interlobular septal thickening and pulmonary edema. The etiology of his condition was unknown, lung biopsy was contraindicated because of his medical fragility, and discussions were held to move to palliative care. Rapid whole-genome sequencing (rWGS) was performed. In 2 d it resulted, revealing a novel FOXF1 gene pathogenic variant that led to the presumptive diagnosis of atypical ACD. Cases of atypical ACD have been reported with survival in patients using medical therapy or lung transplantation. Based on the rWGS diagnosis and more favorable potential of atypical ACD, aggressive medical treatment was pursued. The patient was discharged home after 67 d in the hospital; he is currently doing well more than 30 mo after his initial presentation with only one subsequent hospitalization and no requirement for lung transplantation. Our case reveals the potential for use of rWGS in a critically ill child in which the diagnosis is unknown. rWGS and other advanced genetic tests can guide clinical management and expand our understanding of atypical ACD and other conditions. Lymphomatous suprarenal masses are possible but rare. Here, we discuss the case of a previously healthy 3-yr-old male who presented with mediastinal T-LLy with bilateral suprarenal masses. Following initial treatment, surgical biopsy of persisting adrenal masses revealed bilateral neuroblastoma (NBL). A clinical genetics panel for germline cancer predisposition did not identify any pathogenic variants. Combination large panel (864 genes) profiling analysis in the context of a precision oncology study revealed two novel likely pathogenic heterozygous variants: SMARCA4 c.1420-1G > T p.? and EZH2 c.1943G > C p.(Ile631Phefs*44). Somatic analysis revealed potential second hits/somatic variants in EZH2 (in the T-LLy) and a segmental loss in Chromosome 19p encompassing SMARCA4 (in the NBL). Synchronous cancers, especially at a young age, warrant genetic evaluation for cancer predisposition; enrollment in a precision oncology program assessing germline and tumor DNA can fulfill that purpose, particularly when standard first-line genetic testing is negative and in the setting of tumors that are not classic for common cancer predisposition syndromes. The Fairfax County Circuit Court indicted Darwin Martinez-Torres of Sterling, Virginia, on Monday on four counts of capital murder for killing Nabra, who was with friends while they had a meal before Ramadan services. Dozens of people had gathered outside the courthouse today, chanting “Justice for Nabra.” Virginia law has specific conditions for pursuing the death penalty, but the Associated Press reported that the grand jury’s indictment described in graphic detail how Nabra’s killing was grounds for a death penalty against Martinez-Torres. The indictment appears to acknowledge for the first time that the 17-year-old Muslim teen was raped. Under state law, the combination of a rape charge with a premeditated murder charge means the death penalty can be pursued. Police have said that Martinez-Torres, who is an undocumented immigrant, got into a confrontation on June 18 with a group of teens walking back to the All Dulles Area Muslim Society after grabbing a late meal. He is accused of returning later and beating Nabra with a baseball bat. Police said Nabra’s body was later discovered in a pond. A search warrant affidavit revealed that Martinez-Torres admitted to killing Nabra and had led authorities to where he dumped her body, AP reported. Nabra’s parents and Muslim advocates have said that Nabra’s death was motivated by hate, but police has said that they will not treat the killing as a hate crime. Instead, police have said it was a road rage incident. “The reason this guy he hit my daughter is because she’s Muslim,” Nabra’s father Mahmoud Hassanen told WAMU. “Why [didn’t he] hit the boy who bothered him?” Nabra’s father added that he hoped for the death penalty, while her mother said she wanted Martinez-Torres to serve life in prison. “I just want people to remember her, and don’t forget her,” Mahmoud told WAMU. “I think nobody can forget her too, for what she did in her life.” A preliminary hearing for Martinez-Torres reportedly turned emotional on Friday, with Nabra’s parents both shouting at the suspect in court. Yesterday, Weinstein resigned from the board of his production company following numerous revelations of sexual harassment and several allegations of assault. More than three dozen women have said Weinstein harassed them. While Weinstein has admitted to behaving inappropriately, he has said he didn’t physically assault anyone. One of those women is Katherine Kendall. She was a 23-year-old actress who met Weinstein in 1993. She alleges that he invited her to his apartment in New York, where, she says, he took off his clothes and asked for a massage. As other actresses began coming forward about their painful experiences, she also went public with her own story. She joins me now from Los Angeles. First, thanks for joining us. And I don’t want to relive something that’s painful for you, but you are taking a public stance on it. For people who don’t know your story, what happened? KATHERINE KENDALL, Actress/ Photographer: Well, I was you know, a young actress, and I had had a formal meeting at the Miramax office earlier that day. And then, at the end of the meeting, which I thought went really well, he invited me to come to screenings. He said: “Welcome to the Miramax family. You know, come to premieres, screenings, et cetera. In fact, there’s one this afternoon. Would you like to come?” And I said, “Sure.” And I ended up going to see a movie with him. It ended up just being a movie, not a screening, but the film “Red Rock West.” And, you know, that’s right when I had this sort of sinking feeling that something wasn’t going right. And then, after the movie, we walked for a few blocks. And he said he needed to go up to his apartment to get something, and would I just come with him real quick? And I sort of said no, and we went back and forth on that for a minute. It was sort of a negotiation with him always, trying to sort of stand my ground, but then be convinced it was OK. I did go into his apartment. Once there, we talked for a long time about art and movies. And I felt like he was treating me like an intellect. And I felt like the meeting was going really well, and sort of continued. I didn’t feel unsafe once I was in there. And, at one point, then, he got up to go to the bathroom. And he came back in a robe and asked me to give him a massage. And I was extremely uncomfortable. And I was like, oh, God, no, I’m not comfortable with that. And we went back and forth on that. And then he went back to the bathroom again, and came back this time completely naked. And, you know, that changed it entirely for me, too. It just took it to the next place. It was completely disorienting. And I was scared, you know? I was really scared. And then it became sort of a cat-and-mouse game of, like, how am I going to get out of there? And I’m — it’s hard to make sense of what someone is trying to do to you when they’re fully naked, and they’re… HARI SREENIVASAN: Yes. KATHERINE KENDALL: You know, I’m 105 pounds. He’s a large man standing between me and the door. And, I mean, I felt very resolute, like, I will definitely get out of here somehow. But I’m not — I’m not sure — I’m not sure what’s going to happen here. You know, a lot was going through my head. And he said, well, if you won’t give me a massage, will you at least show me your breasts? And it was just — you know, it was, all in all, an extremely humiliating experience for me. And even though I got away, I felt like something had still — like something horrible had just happened to me. HARI SREENIVASAN: You know, in the immediate aftermath, did you tell someone about it? Because you have said before that you felt ashamed… KATHERINE KENDALL: I did. HARI SREENIVASAN: … even though you were the victim. KATHERINE KENDALL: I did. It’s really interesting how that happens. And I think — you know, I’m older now, and I have done some work on myself. And I have learned that a lot of people feel that way. It’s — it’s not — it wasn’t just me. But the just me feeling that this is my fault, this must have only happened to me, there’s something wrong with me, is so common when someone perpetrates against you. HARI SREENIVASAN: What were the… KATHERINE KENDALL: And I did. I told my mom. And I told some good friends. But, you know, one of the things that happened was, I didn’t want them to tell anybody. You know, people wanted to help me, but they didn’t know how, and I didn’t want them to try too hard, because I didn’t want it to backlash. I was scared. And I think that it’s important to remember that we don’t really come from a culture that supports women in talking about sexual harassment, in my — in my experience, that is. And, you know, I just haven’t felt like it was something I was going to get support on… HARI SREENIVASAN: You know, how long… KATHERINE KENDALL: … in the bigger picture. HARI SREENIVASAN: Yes. How long did this feeling last? Or, I guess, what are the longer-term ripple effects here? Did it shake your confidence in your abilities? KATHERINE KENDALL: I think it did. I think it did. I think it did. I think it made me feel like, wow, you know, that was a wash. He wasn’t interested at all in what I had to say, or, you know, he didn’t see any talent there or intellect there. He was assessing the situation the whole time for something else. And I think that — that did hurt. You know, I wish it didn’t. HARI SREENIVASAN: Yes. KATHERINE KENDALL: But he had produced so many movies that I thought were wonderful. And it was — it’s hard when someone has made art that you love, and how do you stay attached to liking their art, but feeling conflicted about them? And, yes, I think it does have long-term effects. I think you tuck it away. And then, for me, also, I realized that it came back when I would see his name or see him in person. I would start to sort of tremble all over again. I mean, I wouldn’t think about him on a daily basis or anything for years, and then I would see him, and I would think, oh, I don’t feel well. I got to get out of here. HARI SREENIVASAN: Right. KATHERINE KENDALL: You know, it would bring up so much emotion. And the most recent one was the woman in New York, the Italian model. I felt so, so enraged when I saw what happened there, and that they sort of — the police had him, and that then he got away. And then she was being dragged through the press as somebody who just, you know, wanted a payout, et cetera. HARI SREENIVASAN: You know, in the wake of that, there was — a friend of yours had tweeted, “At some point, all the women who have been afraid to speak out about Harvey Weinstein are going to have to hold hands and jump.” This was back in 2015. And from your Twitter account, you said, “Agreed.” It seemed like you almost had the opportunity to come forward. What made you want to come forward now? Has this become a turning point in the industry? KATHERINE KENDALL: This is a turning point. It’s a turning point. There are so many times when I thought about it, and then felt like — there were times when I thought about it and said, well, I have nothing to lose, I will just do it. And then I thought, I — I just didn’t have the strength or the courage yet. And I think somebody like Jodi Kantor doing the story for The New York Times, the fact that she thought it was a story at all was startling to me and made me feel like, wow, something is going to be done. And I knew she had told me — I mean, they were looking for women that this had happened to, because they’d been hearing rumors for so long that it happened to so many people. And she had told me other people were coming out. And I thought, I can’t — when I watched Rose McGowan or any of the other actresses come forward, I just — or Ashley Judd — I just thought, they look strong to me, and I don’t want to be the one that stays silent. HARI SREENIVASAN: Well, Katherine Kendall… KATHERINE KENDALL: I want to stand beside them. HARI SREENIVASAN: Katherine Kendall, thank you very much for speaking with us.
