Inequality and the Safety Net Throughout the Income Distribution, 1929-1940 -- by James J. Feigenbaum, Price V. Fishback, Keoka Grayson

We explored two measures of inequality that described the full income distribution in cities. One measure is an income gini based on family incomes in 1929 for 33 cities and in 1933 for up to 48 cities in 1933 were spread throughout the country. We also estimated gini coefficients that made use of contract rents for renters and implicit rents for home owners for up to 955 cities throughout the country. We were able to expand to all counties when looking at a top-end inequality measure, the number of taxpayers per family. All three measures varied substantially across the country. We show the correlations between the various measures and also estimate the relationship between the measures and various relief programs developed by governments at all levels during the period.

Michael Williams (1949–2017)

Over a long and distinguished career in international affairs, Michael Williams stepped forward to tackle some of the most difficult conflict situations of the late 20th and early 21st centuries.

A message from the director

It is with great sadness that we mark the death of Michael Williams, Baron Williams of Baglan, who passed away peacefully on Sunday 23 April at his home in Oxfordshire, following a brief battle with cancer.

Michael became a distinguished fellow at Chatham House, one of our first, in October 2011, when he returned to London after completing his time at the United Nations and becoming a life peer and the international trustee at the BBC. Michael brought to the institute his extensive experience both at the UN, where he reached the level of under secretary general (having served as the UN special coordinator for Lebanon and, earlier, as the special adviser to the secretary general on the Middle East), and in government, where he served as special diplomatic adviser to foreign secretaries Robin Cook and Jack Straw and UK special envoy for the Middle East. 

Earlier in his career, Michael worked as part of the UN Transitional Administration for Cambodia (UNTAC) and the UN Protection Force for the Former Yugoslavia (UNPROFOR), after stints as a senior editor at the BBC World Service and the head of the Asia research department at Amnesty International.

Michael's principal passions at Chatham House were the Middle East and Southeast Asia. He was executive chair of our Middle East and North Africa Programme’s Syria and its Neighbours project and chaired or contributed to numerous events and debates, while offering a steady stream of incisive insights into the difficult situation there through his writing, his regular commentary to the media and addresses to high-level seminars and conferences. 

Michael also sustained his keen interest in Southeast Asia, having gained his MA and PhD on Indonesia from the School of Oriental and African Studies. Michael generously stepped in to serve as the acting head of the Asia Programme from mid-2012 to early 2014. In 2012, he helped to welcome Aung San Suu Kyi to London as a Chatham House Prize winner, following her release from house arrest, and went on to develop and lead a major Chatham House roundtable in Myanmar in September 2014.

Michael was a wonderful colleague; humorous, approachable and engaged with senior and junior staff in equal measure. He treasured his time at Chatham House and believed deeply in the institute's mission. He served on the editorial board on International Affairs from 1998–2006 and as a member of Council from 2001–05, and chaired the steering committee of our annual London Conference since its inception in 2014. He was also invaluable to me as an informal adviser throughout his time with the institute. We will miss him greatly.

Effects of MK-0941, a Novel Glucokinase Activator, on Glycemic Control in Insulin-Treated Patients With Type 2 Diabetes

OBJECTIVE

To assess the efficacy and safety of MK-0941, a glucokinase activator (GKA), when added to stable-dose insulin glargine in patients with type 2 diabetes.

RESEARCH DESIGN AND METHODS

In this double-blind study, 587 patients taking stable-dose insulin glargine (±metformin ≥1,500 mg/day) were randomized (1:1:1:1:1) to MK-0941 10, 20, 30, or 40 mg or matching placebo t.i.d. before meals (a.c.). This study included an initial 14-week, dose-ranging phase followed by a 40-week treatment phase during which patients were to be uptitrated as tolerated to 40 mg (or placebo) t.i.d. a.c. The primary efficacy end point was change from baseline in A1C at Week 14.

RESULTS

At Week 14, A1C and 2-h postmeal glucose (PMG) improved significantly versus placebo with all MK-0941 doses. Maximal placebo-adjusted least squares mean changes from baseline in A1C (baseline A1C 9.0%) and 2-h PMG were –0.8% and –37 mg/dL (–2 mmol/L), respectively. No significant effects on fasting plasma glucose were observed at any dose versus placebo. By 30 weeks, the initial glycemic responses noted at 14 weeks were not sustained. MK-0941 at one or more doses was associated with significant increases in the incidence of hypoglycemia, triglycerides, systolic blood pressure, and proportion of patients meeting criteria for predefined limits of change for increased diastolic blood pressure.

CONCLUSIONS

In patients receiving stable-dose insulin glargine, the GKA MK-0941 led to improvements in glycemic control that were not sustained. MK-0941 was associated with an increased incidence of hypoglycemia and elevations in triglycerides and blood pressure.

Sampling random graph homomorphisms and applications to network data analysis. (arXiv:1910.09483v2 [math.PR] UPDATED)

A graph homomorphism is a map between two graphs that preserves adjacency relations. We consider the problem of sampling a random graph homomorphism from a graph $F$ into a large network $mathcal{G}$. We propose two complementary MCMC algorithms for sampling a random graph homomorphisms and establish bounds on their mixing times and concentration of their time averages. Based on our sampling algorithms, we propose a novel framework for network data analysis that circumvents some of the drawbacks in methods based on independent and neigborhood sampling. Various time averages of the MCMC trajectory give us various computable observables, including well-known ones such as homomorphism density and average clustering coefficient and their generalizations. Furthermore, we show that these network observables are stable with respect to a suitably renormalized cut distance between networks. We provide various examples and simulations demonstrating our framework through synthetic networks. We also apply our framework for network clustering and classification problems using the Facebook100 dataset and Word Adjacency Networks of a set of classic novels.

The National Perinatal Hepatitis B Prevention Program, 1994-2008

Infants born to women who are hepatitis B surface antigen–positive have a 90% risk of chronic hepatitis B virus infection, which may cause premature death from liver failure or cancer. Postexposure prophylaxis in infancy prevents 85% to 95% of perinatal infections.

The Perinatal Hepatitis B Prevention Program was created to identify and manage infants born to women who are hepatitis B surface antigen–positive. National Trends in Psychotropic Medication Use in Young Children: 1994-2009

Studies of psychotropic use in very young US children in the last decade have been limited by the regions, insurance types, or medication classes examined. There is a paucity of recent, nationally representative investigations of US preschool psychotropic use.

In a national sample of 2 to 5 year olds, the likelihood of psychotropic prescription peaked in the mid-2000s, then stabilized in the late 2000s. Increased psychotropic use in boys, white children, and those lacking private health insurance was documented. (Read the full article)

Phase 2a Pharmacokinetic, Safety, and Exploratory Efficacy Evaluation of Oral Gepotidacin (GSK2140944) in Female Participants With Uncomplicated Urinary Tract Infection (Acute Uncomplicated Cystitis) [Clinical Therapeutics]

Gepotidacin, a triazaacenaphthylene bacterial type II topoisomerase inhibitor, is in development for treatment of uncomplicated urinary tract infection (uUTI). This Phase 2a study in female participants with uUTI evaluated the pharmacokinetics (primary objective), safety, and exploratory efficacy of gepotidacin. Eligible participants (N = 22) were confined to the clinic at baseline, received oral gepotidacin 1,500 mg twice daily for 5 days (on-therapy; Days 1 to 5), and returned to the clinic for test-of-cure (Days 10 to 13) and follow-up (Day 28±3). Pharmacokinetic, safety, clinical, and microbiological assessments were performed. Maximum plasma concentrations were observed approximately 1.5 to 2 hours postdose. Steady state was attained by Day 3. Urinary exposure over the dosing interval increased from 3,742 μg.h/ml (Day 1) to 5,973 μg.h/ml (Day 4), with trough concentrations of 322 to 352 μg/ml from Day 3 onward. Gepotidacin had an acceptable safety-risk profile with no treatment-limiting adverse events and no clinically relevant safety trends. Clinical success was achieved in 19 (86%) and 18 (82%) of 22 participants at test-of-cure and follow-up, respectively. Eight participants had a qualifying baseline uropathogen (growth; ≥105 CFU/ml). A therapeutic (combined clinical and microbiological [no growth; <103 CFU/ml]) successful response was achieved in 6 (75%) and 5 (63%) of 8 participants at test-of-cure and follow-up, respectively. Plasma area under the free-drug concentration-time curve over 24 hours at steady state divided by the MIC (fAUC0-24/MIC) and urine AUC0-24/MIC ranged from 6.99 to 90.5 and 1,292 to 121,698, respectively. Further evaluation of gepotidacin in uUTI is warranted. (NCT03568942)

AN ACT TO AMEND TITLE 13 OF THE DELAWARE CODE RELATING TO CHILD SUPPORT LIENS ON INSURANCE CLAIM PAYMENTS.

This Act will increase payments of child support arrears or retroactive support from payments for settlements and awards from negligence, personal injury, and workers compensation cases by requiring that all insurers share information with an insurance claim data collection organization. The Division of Child Support Services ("Division") uses an insurance claim data collection organization to match individuals who are in arrears on child support or who owe retroactive child support with individuals with insurance claims based upon negligence, personal injury, or workers' compensation cases. The data collection organization performs a data match of the claimants provided by insurers and child support obligors provided by the Division. The data collection organization provides reports of matches to the Division so that the Division can issue a lien against the insurance claim to collect the child support arrears or retroactive support. Currently, some insurers voluntarily submit claim information to the insurance claim data collection organization and in 2018, $424,977 in unpaid child support was collected through this process. This Act requires that all insurers submit claim information to the insurance claim data collection organization and is expected to collect an additional $150,000 - $200,000 in unpaid child support. This Act also allows written notice of a lien or action to perfect the lien to be sent by electronically or by first class mail to conform to industry practices. This Act also makes technical corrections to conform existing law to the standards of the Delaware Legislative Drafting Manual.

AN ACT TO AMEND TITLE 18 OF THE DELAWARE CODE RELATING TO PHARMACY BENEFITS MANAGERS.

Over 80% of pharmaceuticals in the United States are purchased through pharmacy benefits manager (“PBM”) networks. PBMs serve as intermediaries between health plans, pharmaceutical manufacturers and pharmacies, and PBMs establish networks for consumers to receive reimbursement for drugs. Given the scope of PBMs in the healthcare delivery system, this Act is designed to provide enhanced oversight and transparency as it relates PBMs. Specifically, this Act does the following: (1) Requires PBMs to register with the Insurance Commissioner. (2) Permits the Insurance Commissioner to issue cease and desist orders based on fraudulent acts or violations of Chapter 33A of Title 18 committed by PBMs. (3) Requires PBMs to maintain certain records. (4) Permits the Insurance Commissioner to examine the affairs of PBMs. (5) Grants the Insurance Commissioner the authority to enforce Chapter 33A of Title 18 by imposing fines, requiring PBMs to take affirmative actions, and suspending, denying, or revoking a PBM’s registration. In addition, this Act updates existing law regarding maximum allowable cost lists and establishes a more transparent appeals process for a pharmacy to rely on if a PBM does not reimburse the pharmacy the amount owed under their contract or the maximum allowable cost list. Finally, this Act makes technical corrections to conform existing law to the standards of the Delaware Legislative Drafting Manual.

AN ACT TO AMEND TITLE 13 OF THE DELAWARE CODE RELATED TO ADOPTION.

Section 1 of this Act amends Section 903 of Title 13 of the Delaware Code. Substantively, this Act allows persons who are not married but who are cohabitating to jointly adopt a child that is not theirs. The Act also rewrites the statute so as to be more clearly understandable. Section 2 of this Act amends Section 951 of Title 13 of the Delaware Code. It allows persons who are not married but who are cohabitating to jointly adopt a child that is not theirs. The Act also changes the phrase "husband and wife" to "married couple."
