The judge overseeing Donald Trump’s criminal hush money case has put off ruling on whether the president-elect’s conviction should be thrown out on immunity grounds, enabling prosecutors to weigh next steps following his November 5 election victory.

Justice Juan Merchan had been due to rule on Tuesday on Trump’s argument that the US Supreme Court’s decision in July that presidents are immune from prosecution involving their official acts meant the New York state case should be dismissed.

Instead, Merchan granted a request by Manhattan District Attorney Alvin Bragg’s office to have until Nov 19 to consider how to approach the case in light of Trump’s looming inauguration in January 2025, email correspondence made public on Tuesday showed.

Trump’s scheduled Nov 26 sentencing is now widely expected to be postponed.

Trump in May became the first US president — former or sitting — convicted of a crime when a jury in Manhattan found him guilty on 34 felony counts of falsifying business records to cover up a potential sex scandal shortly before his first election win in 2016. Trump, who pleaded not guilty, has vowed to appeal the verdict after sentencing.

Prosecutor Matthew Colangelo wrote there were “competing interests” between ensuring a criminal case proceeds as usual and protecting the office of the president.

“The People agree that these are unprecedented circumstances,” Colangelo wrote.

Trump is set to be the first felon inaugurated as president after his victory over Vice President Kamala Harris.

At issue in the six-week Manhattan trial was a $130,000 payment made by Trump’s then-lawyer Michael Cohen to adult film actress Stormy Daniels to keep quiet about a sexual encounter she said she had with him in 2006 but which he has denied.

Trump’s defense lawyer Emil Bove wrote that the case ultimately needed to be dismissed to avoid interfering with Trump’s presidential duties.

“The stay, and dismissal, are necessary to avoid unconstitutional impediments to President Trump’s ability to govern,” Bove wrote.

Trump faced four criminal cases

Trump, 78, is hoping to enter office unencumbered by any of four criminal cases he has faced and which once were thought to have threatened to derail his 2024 candidacy to return to the White House after having served from 2017 to 2021.

The Republican Trump has portrayed the hush money case brought by Bragg, a Democrat, and the three other state and federal criminal indictments brought in 2023 as politically motivated attempts to harm his presidential campaign. He pleaded not guilty in all four cases.

“It is now abundantly clear that Americans want an immediate end to the weaponization of our justice system,” Trump campaign spokesperson Steven Cheung said in a statement on Tuesday.

Special Counsel Jack Smith brought two of the cases against Trump, one involving classified documents he kept after leaving office and the other involving his efforts to overturn his 2020 election loss. A Florida-based federal judge in July dismissed the documents case. The Justice Department is now evaluating how to wind down Smith’s election-related case.

Trump also faces state criminal charges in Georgia over his bid to reverse his 2020 loss in that state, but the case remains in limbo.

The Supreme Court, in a decision arising from one of Smith’s two cases against Trump, decided that presidents are immune from prosecution involving their official acts and that juries cannot be presented evidence of official acts in trials over personal conduct. It marked the first time that the court recognized any degree of presidential immunity from prosecution.

In making the case for immunity, Trump’s lawyers said the jury that convicted Trump in the hush money case was shown evidence by prosecutors of his social media posts as president and heard testimony from his former aides about conversations that occurred in the White House during his 2017-2021 term.

Bragg’s office countered that the Supreme Court’s ruling has no bearing on the case, which they said concerned “wholly unofficial conduct.” The Supreme Court in its ruling found no immunity for a president’s unofficial acts.

BERLIN: Germany is headed for snap elections on February 23, the main parties agreed on Tuesday, aiming to form a stable government after Chan­cellor Olaf Scholz’s three-party coalition collapsed last week.

The country’s two biggest parties agreed on the early timetable, which will see centre-left leader Scholz seek a vote of confidence on December 16, said the parliamentary leader of Scholz’s Social Democrats (SPD), Rolf Muetzenich.

This would pave the way for the February vote in a compromise hammered out with the conservative opposition Christian Democrats (CDU) and their Bavarian allies CSU.

The agreement seeks to quickly restore political stability at a time when Europe’s biggest economy is set to shrink for a second year in a row and amid heightened geopolitical volatility, with wars raging in Ukraine and the Middle East.

The election date would mean Germany will be ruled by a lame-duck chancellor and stuck in the middle of its campaign period when Donald Trump is inaugurated as US president on January 20.

Embattled Scholz, who wants to run again despite poor poll ratings, initially suggested an election in late March but came under heavy pressure from all other parties to speed up the process.

The CDU is riding high in the polls and its leader Friedrich Merz had pushed strongly for an election as early as possible — a demand backed by two thirds of voters, according to a recent survey.

“We don’t have unlimited time to elect a new government in Germany, regardless of who leads it… because the world around us is not waiting,” Merz said on Tuesday.

“It’s not as if everyone is holding their breath and watching Germany in fascination, as decisions are taken in Europe, Asia and the United States.

“The world expects a Germany that is capable of taking action.”

Winter election campaign

The February 23 date would force politicians to run their campaigns in the dark and cold of winter, when voters will be less enthusiastic to flock to outdoor events than during the usual summer campaign seasons.

Scholz is expected to lose the confidence vote in the lower house of parliament, after which President Frank-Walter Steinmeier will have 21 days to dissolve the Bundestag.

Elections have to be held within 60 days of the dissolution.

Scholz took office in late 2021, taking over from the CDU’s Angela Merkel, in a three-way coalition with the left-leaning Greens and the liberal and pro-business Free Democrats (FDP).

But mounting differences over economic and fiscal policy came to a head last week when Scholz fired the rebellious FDP finance minister Christian Linder, prompting the small party to leave the government.

Since then, Scholz’s SPD and the Greens have carried on in a minority government that would need opposition support to pass any laws — something the CDU/CSU had rejected in the absence of clarity on an election date.

Future scenarios

Scholz’s coalition marked the first time a tripartite alliance has ruled at the federal level, and it may not be the last, given Germany’s increasingly fragmented political party landscape.

Fears about immigration have driven the rise of the far-right Alternative for Germany (AfD) party. It is now polling at close to 20 per cent, but other parties have pledged to shun it as a coalition partner.

The latest polls put the centre-right CDU/CSU alliance firmly in the lead at 32pc.

To win a majority, however, the conservatives would likely need the future backing of the SPD, which is currently polling at 15.5pc, in a so-called grand coalition, plus a third party.

Contenders for that spot, according to current polling, would be either the FDP, with five percent support, or possibly the Greens, who are polling at 11pc.

Lindner, who has said he wants to be finance minister again, on Tuesday welcomed the date for new elections, saying:“Happily we now have clarity on this question.” He earlier said that he thought Merz “will almost certainly be the next chancellor of Ger­many. The only question is: Who will chancellor Merz govern with?”

Only One Quarter of Family Physicians Are Very Satisfied with Their Electronic Health Records Platform

Two decades into the era of Electronic Health Records (EHRs), the promise of streamlining clinical care, reducing burden, and improving patient outcomes has yet to be realized. A cross-sectional family physician census conducted by the American Board of Family Medicine in 2022 and 2023 included self-reported physician EHR satisfaction. Of the nearly 10,000 responding family physicians, only one-in-four (26.2%) report being very satisfied and one-in-three (33.8%) were not satisfied. These low levels of satisfaction point to the need for greater transparency in the marketplace and pressure to increase user-centric EHR design.

Using Primary Health Care Electronic Medical Records to Predict Hospitalizations, Emergency Department Visits, and Mortality: A Systematic Review

Introduction:

High-quality primary care can reduce avoidable emergency department visits and emergency hospitalizations. The availability of electronic medical record (EMR) data and capacities for data storage and processing have created opportunities for predictive analytics. This systematic review examines studies which predict emergency department visits, hospitalizations, and mortality using EMR data from primary care.

Methods:

Six databases (Ovid MEDLINE, PubMed, Embase, EBM Reviews (Cochrane Database of Systematic Reviews, Database of Abstracts of Reviews of Effects, Cochrane Central Register of Controlled Trials, Cochrane Methodology Register, Health Technology Assessment, NHS Economic Evaluation Database), Scopus, CINAHL) were searched to identify primary peer-reviewed studies in English from inception to February 5, 2020. The search was initially conducted on January 18, 2019, and updated on February 5, 2020.

Results:

A total of 9456 citations were double-reviewed, and 31 studies met the inclusion criteria. The predictive ability measured by C-statistics (ROC) of the best performing models from each study ranged from 0.57 to 0.95. Less than half of the included studies used artificial intelligence methods and only 7 (23%) were externally validated. Age, medical diagnoses, sex, medication use, and prior health service use were the most common predictor variables. Few studies discussed or examined the clinical utility of models.

Conclusions:

This review helps address critical gaps in the literature regarding the potential of primary care EMR data. Despite further work required to address bias and improve the quality and reporting of prediction models, the use of primary care EMR data for predictive analytics holds promise.

Molecular Mechanisms for the Selective Transport of Dichlorofluorescein by Human Organic Anion Transporting Polypeptide 1B1 [Articles]

Human organic anion transporting polypeptide (OATP) 1B1 and 1B3 are two highly homologous liver-specific uptake transporters. However, 2’,7’-dichlorofluorescein (DCF) is preferably transported by OATP1B1. In the present study, the molecular mechanisms for the selective transport of DCF by OATP1B1 were investigated by constructing and characterizing an array of OATP1B1/1B3 chimeras and site-directed mutagenesis. Our results show that transmembrane domain (TM) 10 is crucial for the surface expression and function of OATP1B1, in which Q541 and L545 play the most important roles in DCF transport. Replacement of TM10 in OATP1B1 with its OATP1B3 counterpart led to OATP1B1’s complete intracellular retention. Q541 and L545 may interact with DCF directly via hydrogen bonding and hydrophobic interactions. The decrease of DCF uptake by Q541A and L545S was due to their reduced binding affinity for DCF as compared with OATP1B1. In addition, Q541 and L545 are also crucial for the transport of estradiol-17β-glucuronide (E17βG) but not for the transport of estrone-3-sulfate (E3S), indicating different interaction modes between DCF/E17βG and E3S in OATP1B1. Taken together, Q541 and L545 in TM10 are critical for OATP1B1-mediated DCF uptake, but their effect is substrate-dependent.

SIGNIFICANCE STATEMENT

The key TMs and amino acid residues for the selective transport of DCF by OATP1B1 were identified. TM10 is crucial for the surface expression and function of OATP1B1. Within TM10, Q541 and L545 played the most significant roles and affected the function of OATP1B1 in a substrate-dependent manner. This information is crucial for a better understanding of the mechanism of the multispecificity of OATP1B1 and as a consequence the mechanism of OATP1B1-mediated drug–drug interactions.

Comparison of the CYP3A Selective Inhibitors CYP3cide, Clobetasol, and Azamulin for Their Potential to Distinguish CYP3A7 Activity in the Presence of CYP3A4/5 [Articles]

The CYP3A7 enzyme accounts for ~50% of the total cytochrome P450 (P450) content in fetal and neonatal livers and is the predominant P450 involved in neonatal xenobiotic metabolism. Additionally, it is a key player in healthy birth outcomes through the oxidation of dehydroepiandrosterone (DHEA) and DHEA-sulfate. The amount of the other hepatic CYP3A isoforms, CYP3A4 and CYP3A5, expressed in neonates is low but highly variable, and therefore the activity of individual CYP3A isoforms is difficult to differentiate due to their functional similarities. Consequently, a better understanding of the contribution of CYP3A7 to drug metabolism is essential to identify the risk that drugs may pose to neonates and developing infants. To distinguish CYP3A7 activity from CYP3A4/5, we sought to further characterize the selectivity of the specific CYP3A inhibitors CYP3cide, clobetasol, and azamulin. We used three substrate probes, dibenzylfluorescein, luciferin-PPXE, and midazolam, to determine the IC50 and metabolism-dependent inhibition (MDI) properties of the CYP3A inhibitors. Probe selection had a significant effect on the IC50 values and P450 inactivation across all inhibitory compounds and enzymes. CYP3cide and azamulin were both identified as MDIs and were most specific for CYP3A4. Contrary to previous reports, we found that clobetasol propionate (CP) was not an MDI of CYP3A5 but was more selective for CYP3A5 over CYP3A4/7. We further investigated CYP3cide and CP’s ability to differentiate CYP3A7 activity in an equal mixture of recombinant CYP3A4, CYP3A5, and CYP3A7, and our results provide confidence of CYP3cide’s and CP’s ability to distinguish CYP3A7 activity in the presence of the other CYP3A isoforms.

SIGNIFICANCE STATEMENT

These findings provide valuable insight regarding in vitro testing conditions to investigate the metabolism of new drug candidates and help determine drug safety in neonates. The results presented here also clearly demonstrate the effect that probe selection may have on CYP3A cytochrome P450 inhibition studies.

Early Prediction and Impact Assessment of CYP3A4-Related Drug-Drug Interactions for Small-Molecule Anticancer Drugs Using Human-CYP3A4-Transgenic Mouse Models [Articles]

Early detection of drug-drug interactions (DDIs) can facilitate timely drug development decisions, prevent unnecessary restrictions on patient enrollment, resulting in clinical study populations that are not representative of the indicated study population, and allow for appropriate dose adjustments to ensure safety in clinical trials. All of these factors contribute to a streamlined drug approval process and enhanced patient safety. Here we describe a new approach for early prediction of the magnitude of change in exposure for cytochrome P450 (P450) CYP3A4-related DDIs of small-molecule anticancer drugs based on the model-based extrapolation of human-CYP3A4-transgenic mice pharmacokinetics to humans. Victim drugs brigatinib and lorlatinib were evaluated with the new approach in combination with the perpetrator drugs itraconazole and rifampicin. Predictions of the magnitude of change in exposure deviated at most 0.99- to 1.31-fold from clinical trial results for inhibition with itraconazole, whereas exposure predictions for the induction with rifampicin were less accurate, with deviations of 0.22- to 0.48-fold. Results for the early prediction of DDIs and their clinical impact appear promising for CYP3A4 inhibition, but validation with more victim and perpetrator drugs is essential to evaluate the performance of the new method.

SIGNIFICANCE STATEMENT

The described method offers an alternative for the early detection and assessment of potential clinical impact of CYP3A4-related drug-drug interactions. The model was able to adequately describe the inhibition of CYP3A4 metabolism and the subsequent magnitude of change in exposure. However, it was unable to accurately predict the magnitude of change in exposure of victim drugs in combination with an inducer.

Evaluating the Abuse Potential of Lenabasum, a Selective Cannabinoid Receptor 2 Agonist [Special Section: Cannabinoid Signaling in Human Health and Disease]

Endocannabinoids, which are present throughout the central nervous system (CNS), can activate cannabinoid receptors 1 and 2 (CB1 and CB2). CB1 and CB2 agonists exhibit broad anti-inflammatory properties, suggesting their potential to treat inflammatory diseases. However, careful evaluation of abuse potential is necessary. This study evaluated the abuse potential of lenabasum, a selective CB2 receptor agonist in participants (n = 56) endorsing recreational cannabis use. Three doses of lenabasum (20, 60, and 120 mg) were compared with placebo and nabilone (3 and 6 mg). The primary endpoint was the peak effect (Emax) on a bipolar Drug Liking visual analog scale (VAS). Secondary VAS and pharmacokinetic (PK) endpoints and adverse events were assessed. Lenabasum was safe and well tolerated. Compared with placebo, a 20-mg dose of lenabasum did not increase ratings of Drug Liking and had no distinguishable effect on other VAS endpoints. Dose-dependent increases in ratings of Drug Liking were observed with 60 and 120 mg lenabasum. Drug Liking and all other VAS outcomes were greatest for nabilone 3 mg and 6 mg, a medication currently approved by the US Food and Drug Administration (FDA). At a target therapeutic dose (20 mg), lenabasum did not elicit subjective ratings of Drug Liking. However, supratherapeutic doses of lenabasum (60 and 120 mg) did elicit subjective ratings of Drug Liking compared with placebo. Although both doses of lenabasum were associated with lower ratings of Drug Liking compared with 3 mg and 6 mg nabilone, lenabasum does have abuse potential and should be used cautiously in clinical settings.

SIGNIFICANCE STATEMENT

This work provides evidence that in people with a history of recreational cannabis use, lenabasum was safe and well tolerated, although it did demonstrate abuse potential. This work supports further development of lenabasum for potential therapeutic indications.

Current medical interventions, such as opioid drugs, are effective at relieving pain but are accompanied by many undesirable side effects. This is one reason increased numbers of chronic pain patients have been turning to Cannabis for pain management. Cannabis contains many bioactive chemical compounds; however, current research looking into lesser-studied minor cannabinoids in Cannabis lacks uniformity between experimental groups and/or excludes female mice from investigation. This makes it challenging to draw conclusions between experiments done with different minor cannabinoid compounds between laboratories or parse out potential sex differences that could be present. We chose five minor cannabinoids found in lower quantities within Cannabis: cannabinol (CBN), cannabidivarin (CBDV), cannabigerol (CBG), 8-tetrahydrocannabinol (8-THC), and 9-tetrahydrocannabivarin (THCV). These compounds were then tested for their cannabimimetic and pain-relieving behaviors in a cannabinoid tetrad assay and a chemotherapy-induced peripheral neuropathy (CIPN) pain model in male and female CD-1 mice. We found that the minor cannabinoids we tested differed in the cannabimimetic behaviors evoked, as well as the extent. We found that CBN, CBG, and high-dose 8-THC evoked some tetrad behaviors in both sexes, while THCV and low-dose 8-THC exhibited cannabimimetic tetrad behaviors only in females. Only CBN efficaciously relieved CIPN pain, which contrasts with reports from other researchers. Together these findings provide further clarity to the pharmacology of minor cannabinoids and suggest further investigation into their mechanism and therapeutic potential.

SIGNIFICANCE STATEMENT

Minor cannabinoids are poorly studied ligands present in lower levels in Cannabis than cannabinoids like THC. In this study, we evaluated five minor cannabinoids (CBN, CBDV, CBG, THCV, and 8-THC) for their cannabimimetic and analgesic effects in mice. We found that four of the five minor cannabinoids showed cannabimimetic activity, while one was efficacious in relieving chronic neuropathic pain. This work is important in further evaluating the activity of these drugs, which are seeing wider public use with marijuana legalization.
