Netanyahu Looks to Vote in New Israeli Government on Thursday

JERUSALEM — Israeli Prime Minister-designate Benjamin Netanyahu called a vote in parliament on his new government for Thursday Dec. 29, the speaker of the Knesset said on Monday, after almost two months of coalition wrangling.  Netanyahu's bloc of right-wing and religious parties won a clear victory in parliamentary elections last month, but the veteran leader has had a harder time than expected in finalizing deals with his partners.  Despite campaigning together, Netanyahu has struggled to meet the demands of his allies, who have demanded a significant slice of power in exchange for their support.   Ahead of the vote in parliament and a formal swearing in of the new government, Netanyahu will have to officially present the members of his cabinet.  Israel's longest serving prime minister has vowed to govern for all Israelis but he will head one of the most right-wing governments in the country's history with key ministries in the hands of hardliners.  Itamar Ben-Gvir, head of the Jewish Power party will have authority for police as security minister while Bezalel Smotrich's Religious Zionism party will have broad authority to allow the expansion of Jewish settlements in the West Bank.  Both oppose Palestinian statehood and support extending Israeli sovereignty into the West Bank, adding another obstacle to a two-state solution, the resolution backed by Palestinian leaders, the United States and European governments.  The finance ministry is expected to be shared by Smotrich and Aryeh Deri, from the religious Shas party, with each man serving for two years. Deri's appointment will depend on parliamentary support for a legal amendment allowing him to serve despite a conviction for tax fraud.  Liberal Israelis have also been alarmed by statements from a number of other members of coalition parties against gay rights and in favor of allowing some businesses to refuse services to people based on religious grounds.  President Isaac Herzog, the head of state who stands outside day-to-day politics, said on Sunday that any threat to the rights of Israeli citizens based on their identity or values would be counter to Israel's democratic and ethical traditions.

Feasibility of Delivering 5-Day Normobaric Hypoxia Breathing in a Hospital Setting

BACKGROUND:Beneficial effects of breathing at FIO2 < 0.21 on disease outcomes have been reported in previous preclinical and clinical studies. However, the safety and intra-hospital feasibility of breathing hypoxic gas for 5 d have not been established. In this study, we examined the physiologic effects of breathing a gas mixture with FIO2 as low as 0.11 in 5 healthy volunteers.METHODS:All 5 subjects completed the study, spending 5 consecutive days in a hypoxic tent, where the ambient oxygen level was lowered in a stepwise manner over 5 d, from FIO2 of 0.16 on the first day to FIO2 of 0.11 on the fifth day of the study. All the subjects returned to an environment at room air on the sixth day. The subjects' SpO2, heart rate, and breathing frequency were continuously recorded, along with daily blood sampling, neurologic evaluations, transthoracic echocardiography, and mental status assessments.RESULTS:Breathing hypoxia concentration dependently caused profound physiologic changes, including decreased SpO2 and increased heart rate. At FIO2 of 0.14, the mean SpO2 was 92%; at FIO2 of 0.13, the mean SpO2 was 93%; at FIO2 of 0.12, the mean SpO2 was 88%; at FIO2 of 0.11, the mean SpO2 was 85%; and, finally, at an FIO2 of 0.21, the mean SpO2 was 98%. These changes were accompanied by increased erythropoietin levels and reticulocyte counts in blood. All 5 subjects concluded the study with no adverse events. No subjects exhibited signs of mental status changes or pulmonary hypertension.CONCLUSIONS:Results of the current physiologic study suggests that, within a hospital setting, delivering FIO2 as low as 0.11 is feasible and safe in healthy subjects, and provides the foundation for future studies in which therapeutic effects of hypoxia breathing are tested.

The Day I Almost Walked Away: Trust, Gratitude, and the Power of Teamwork [Reflection]

Practicing family medicine is really hard; the emotional toll of sharing patients’ distress, vulnerability, and trauma can build up and become overwhelming. A family physician experienced such a moment during one particularly complex morning. Feeling nearly ready to walk out of patient care, she reached out to the team nurse, who helped her get through the moment and re-engage with the waiting patients. Sharing vulnerability in the moment, and later reflecting and deciding to write about it shows the power of prioritizing teamwork in practice.

Trump Calls Jan. 6, the Day His Supporters Led a Failed Insurrection, ‘A Day of Love’

Marco Bello/Reuters

Former President Donald Trump said Wednesday that Jan. 6, 2021— the day his supporters occupied Congress in a failed insurrection to try to stop lawmakers from certifying Joe Biden’s election victory—was a “day of love.”

Trump made the baffling claim during a televised election town hall hosted by Univision.

Ramiro González, a construction worker from Tampa, told the meeting he deregistered as a Republican because he found Trump’s “inaction” during both Jan. 6 and the COVID-19 pandemic “disturbing.” He asked Trump to square his controversial behavior during the attack on the U.S. Capitol—and the fact that many of his own former administration officials don’t support him any longer—with why he should be re-elected.

Donald Trump Cancels Second Mainstream Interview in Days

Marco Bello/Reuters

Donald Trump pulled out of another mainstream interview Thursday–this time nixing a sit-down with NBC News.

The interview, CNN reported, would be in Philadelphia with NBC News' senior business correspondent, Christine Romans. CNN’s Brian Stelter said one source suggested that it had only been “postponed.”

It was the second time in a week that he had canceled a scheduled appearance outside the conservative news sphere, CNN’s Reliable Sources reported Thursday. He had canceled an in-studio appearance on the CNBC flagship show, Squawk Box, which was due on Friday.
