tons Cerebral Palsy and Neonatal Death in Term Singletons Born Small for Gestational Age By pediatrics.aappublications.org Published On :: 2012-11-19T00:08:02-08:00 Children born small for gestational age (SGA) have increased risk of perinatal morbidity and mortality, neonatal death, and cerebral palsy (CP). Causes of SGA, such as congenital malformations, intrauterine infections, and preeclampsia, are also risk factors for the same outcomes. In 90% of singletons born SGA, CP is apparently of prenatal origin. Low proportions of intrapartum events leading to CP could not be fully explained by a higher neonatal mortality rate in SGA than in non-SGA children. (Read the full article) Full Article
tons Variation in Quality of Tonsillectomy Perioperative Care and Revisit Rates in Children's Hospitals By pediatrics.aappublications.org Published On :: 2014-01-20T00:06:48-08:00 Tonsillectomy is one of the most commonly performed surgeries in children and is one of the most cumulatively expensive conditions in pediatric hospital care. Little is known about how the quality of tonsillectomy care varies across hospitals. In a large cohort of low-risk children undergoing same-day tonsillectomy, there was substantial variation in quality measures of process, dexamethasone and antibiotic use, and outcome, revisits to the hospital within the first 30 days after surgery. tons Growth After Adenotonsillectomy for Obstructive Sleep Apnea: An RCT By pediatrics.aappublications.org Published On :: 2014-07-28T00:07:13-07:00 Growth failure has been frequently reported in children who have obstructive sleep apnea syndrome (OSAS) owing to adenotonsillar hypertrophy. Adenotonsillectomy (AT) has been reported to accelerate weight gain in children who have OSAS in nonrandomized uncontrolled studies. This randomized controlled trial of AT for pediatric OSAS demonstrated significantly greater weight increases 7 months after AT in all weight categories. AT normalizes weight in children who have failure to thrive, but increases risk for obesity in overweight children. (Read the full article) Full Article
tons Quality of Life and Obstructive Sleep Apnea Symptoms After Pediatric Adenotonsillectomy By pediatrics.aappublications.org Published On :: 2015-01-19T00:05:31-08:00 Pediatric obstructive sleep apnea syndrome (OSAS) has been associated with decreased health-related quality of life (QoL). Observational studies suggest that adenotonsillectomy for pediatric OSAS improves QoL, but these studies did not use a randomized study design or a control group of children with OSAS managed nonsurgically.A prospective, randomized controlled study of adenotonsillectomy for pediatric OSAS showed significantly greater QoL and symptom improvements in children undergoing adenotonsillectomy than in the nonsurgical control arm. The extent of improvement was not appreciably influenced by baseline OSAS severity or obesity. (Read the full article) Full Article
tons Morphine or Ibuprofen for Post-Tonsillectomy Analgesia: A Randomized Trial By pediatrics.aappublications.org Published On :: 2015-01-26T00:05:28-08:00 Sleep apnea is a common condition in childhood, mainly managed by tonsillectomy. Codeine was recently contraindicated for pain management after surgery. Controversy exists regarding the safety and effectiveness of alternative medications, morphine, and ibuprofen. Our findings suggest that ibuprofen does not increase tonsillar bleeding and in combination with acetaminophen is effective for pain management after tonsillectomy. Furthermore, standard morphine doses increased postoperative respiratory events and were not safe in all children. (Read the full article) Full Article
tons Association of National Guidelines With Tonsillectomy Perioperative Care and Outcomes By pediatrics.aappublications.org Published On :: 2015-06-22T00:06:54-07:00 Tonsillectomy guidelines make evidence-based recommendations for the perioperative use of dexamethasone, no routine use of antibiotics, and discharge education of families and for surgeons to monitor bleeding complication rates. The impact of the guidelines on processes and outcomes is unknown. The guidelines were associated with improvement in perioperative care processes but no improvement in outcomes. Perioperative dexamethasone use increased slightly, and antibiotic use decreased substantially. Bleeding rates were stable, but revisit rates for complications increased because of revisits for pain. tons Do Farm Characteristics or Social Dynamics Explain the Conversion to Organic Farming by Dairy Farmers? An Agent-Based Model of Dairy Farming in 27 French Cantons By jasss.soc.surrey.ac.uk Published On :: Wed, 01 Apr 2020 12:52:00 +0000 Qing Xu, Sylvie Huet, Eric Perret and Guillaume Deffuant: The drivers of conversion to organic farming, which is still a residual choice in agriculture, are poorly understood. Many scholars argue that farm characteristics can determine this choice but do not exclude the role of social dynamics. To study this issue, we developed an agent-based model in which agents' decisions to shift to organic farming are based on a comparison between satisfaction with the current situation and potential satisfaction with an alternative farming strategy. A farmer agent’s satisfaction is modelled using the Theory of Reasoned Action. This makes it necessary to compare an agent's productions over time with those of other agents to whom the former attributes considerable credibility (“important others”). Moreover, farmers make technical changes that affect their productions by imitating other credible farmers. While we first used this model to examine simple and abstract farm populations, here we also adapted it for use with data from an Agricultural Census concerning the farm characteristics of dairy farming in 27 French “cantons”. Based on domain expertise, data and previous research, we propose certain laws for modelling the impact of conversion on the farm production of milk and the environment. The simulations with “real” populations of farms confirm the important impact of farm characteristics. However, our results also suggest a complex impact of social dynamics that can favour or impede the diffusion of organic farming through dynamic implicit networks of similarity and credibility. We confirm the great importance of demographic changes in farm characteristics. Full Article Article
