Con: indwelling pleural catheters cause harm to patients

Indwelling pleural catheters (IPCs) have rapidly grown in popularity since their introduction for the management of recurrent pleural effusions. In malignant pleural effusions especially, there has been a shift away from measuring pleurodesis success and towards more patient-centred outcomes. Multiple randomised controlled trials have shown that despite lower rates of pleurodesis, symptom control and quality of life outcomes are comparable when compared to alternatives such as talc pleurodesis. IPCs have the added benefit of minimising inpatient hospital stays and reducing the need for recurrent pleural interventions, key priorities for patients with palliative disease. As a result, IPC treatment is associated with excellent patient satisfaction coupled with acceptably low complication rates. Furthermore, in patients with a short life expectancy they confer a cost benefit for the healthcare system.

Far from causing harm, IPCs are now recommended as first-line treatment by current clinical guidelines. In malignant pleural disease, guidance advocates IPCs should be offered as a first-line option with the focus on patient priorities and preferences. Ultimately IPCs provide a safe, effective, ambulatory option for managing recurrent pleural effusions.

Steroids in severe community-acquired pneumonia

There is conflicting evidence regarding the use of steroids in severe community-acquired pneumonia (CAP), with previous randomised controlled trials limited by small sample sizes. ESCAPe and CAPE COD are two recently published large trials on steroids in severe CAP. ESCAPe assessed the initiation of methylprednisolone within 72–96 h of hospital admission, while CAPE COD studied the use of hydrocortisone within 24 h of the development of severe CAP. ESCAPe did not show any differences in all-cause 60-day mortality or any of its secondary outcomes. CAPE COD showed that hydrocortisone improved all-cause 28-day mortality and reduced the risk of intubation or vasopressor-dependent shock. Important differences between the trials included the steroid regimens used, timing of steroid administration and baseline characteristics, with more diabetic patients included in ESCAPe. The results of CAPE COD support the initiation of hydrocortisone within 24 h of developing severe CAP, but more research is needed to evaluate long-term outcomes and optimum dosing regimens for steroids in severe CAP.

Pro: indwelling pleural catheters cause harm to patients

Malignant pleural effusions (MPE) tend to recur and require definitive treatment with either chest drain and talc pleurodesis or indwelling pleural catheters (IPCs), which offer similar symptomatic benefits. In recent years, IPCs have become popular due to the presumed convenience of an outpatient procedure followed by home drainage leading to a misconception of IPCs being an ideal treatment for MPE. However, IPCs predispose the patient to multiple complications and have significant physical and psychological implications that are under-recognised. Patients require additional clinical reviews, hospital admissions and treatment for these complications related to IPCs. Additionally, there is a huge psychological impact of living with a home catheter that is a constant reminder of their cancer and this has been shown to affect quality of life negatively. Hence, IPCs should not be considered the "ideal" treatment for MPE management and clinicians should reflect the equipoise of the evidence for the benefits and accurately reflect the adverse effects of IPCs in their discussions with patients to facilitate informed decision making.

Hundreds dead after massive truck bomb strikes Mogadishu

At least 231 people were killed and hundreds more wounded after a massive truck bomb on Saturday struck Somalia’s capital city of Mogadishu.

The Somali government has blamed the al-Qaida-linked militant group al-Shabab for the attack, and called it the deadliest ever to hit the nation.

The blast took place outside the Safari Hotel, where rescue workers dug through the rubble of collapsed buildings overnight in search of survivors. Witnesses described a devastating scene with large-scale carnage, as doctors worked feverishly to attend to the dead and injured, many badly burned.

“The hospital is overwhelmed by both dead and wounded,” Dr. Mohamed Yusuf, the director of Medina hospital located near the blast, told the Associated Press. “We also received people whose limbs were cut away by the bomb. This is really horrendous, unlike any other time in the past.”

Photos and videos of the bombing, which took place on a busy street near a section of the city housing foreign embassies, showed collapsed walls, twisted metal, and sporadic fires spewing smoke. The Qatari government said its embassy was “severely damaged” in the strike.

Family members searched through the wreckage and waited at local hospitals with the hopes of finding relatives who survived the bombing.

Somali President Mohamed Abdullahi Mohamed announced three days of mourning. The attacks received international condemnation, including from the United States.

Iraqi, Kurdish forces in standoff, weeks after Kurdish vote for independence

HARI SREENIVASAN: The desire of the Kurds along Iraq’s northern border to govern themselves is receiving more resistance from Iraq’s central government. Iraqi army forces are demanding Kurdish troops withdraw from oil fields and military bases around Kirkuk, a city in the Kurdistan region that voted for independence last month. Kirkuk also has 10% of Iraq’s known oil reserves. Washington Post’s Loveday Morris is in Baghdad covering this standoff joins me now via Skype. First of all the significance of this. Why is it so important?

LOVEDAY MORRIS: There’s been a longtime conflict between Baghdad and Kurdistan over these disputed territories. Most significant of which is Kirkuk because of the oil reserves. But the referendum last month has really sharpened these disputes because you have Baghdad opposing independence and so it feels like they have to restate its territorial claims these areas. So that’s why we’re seeing a lot of tension right now.

HARI SREENIVASAN: And just to give people a little bit of a brief timeline – Iraqi forces control this area for a while and then in June ISIS took over the area and now it’s kind of back in Kurdish hands?

LOVEDAY MORRIS: Right. So in June 2014 Iraq lost control of a lot of the areas and we have this huge collapse in the face of an ISIS offensive. Over 100,000 soldiers fled and Kurdish forces moved in some of these areas – some of them maybe took from ISIS and others just moved into into the vacuum. And so Iraqi forces have been in these areas since June 2014. And that’s their main demand that they return to the areas.

HARI SREENIVASAN: What’s the likelihood that this standoff right now turns violent? Into some sort of a civil war?

LOVEDAY MORRIS:: I think at this point both sides don’t want violence. Al-Abadi, the prime minister, is really trying to defuse the situation by saying there’s going to be no military attack. But at the same time there is this buildup of forces so that I think they are trying to, in a way, intimidate the Kurds to withdraw from some areas but they don’t want to see a fight per say. But in this really tense situation there can be a small spark and things can turn violent quite easily.

JUDY WOODRUFF: And in the day’s other news: More than 300 people are now confirmed dead after Saturday’s massive truck bombing in Somalia, one of the world’s worst attacks in years.

Nearly 400 more were wounded. The government blamed the al-Qaida-linked Al-Shabaab group. Rescue crews today searched for survivors at the scene of the bombing, a crowded street in the capital, Mogadishu. With dozens still missing, officials say they expect the death toll to rise.

OSMAN LIBAH IBRAHIM, Deputy Minister for Natural Resources, Somalia (through interpreter): More bodies are gradually being found and removed from the rubble. There are other people who are under the rubble. We have heard them as they scream for help. My biggest worry is that even the wounded are succumbing to their injuries.

JUDY WOODRUFF: The attack happened two days after Somalia’s defense minister and army chief resigned for undisclosed reasons.

There’s been yet another shift to the right in European politics; 31-year-old conservative Sebastian Kurz, Austria’s foreign minister, is set to become that country’s next leader. But he’s short of a majority in Parliament and will likely form a coalition with the far-right Freedom Party. It was founded by ex-Nazis in the 1950s.

Kurz has called for the European Union to focus more on internal trade and securing borders. He celebrated in Vienna.

SEBASTIAN KURZ, Austrian People’s Party (through interpreter): I have a big request for you. Use today to celebrate. You all have earned it through hard work and dedication. At the same time, I need to tell you that tomorrow the work starts. We didn’t just run to win the elections. We did so to bring Austria back to the top. We ran in this election to achieve real change.

JUDY WOODRUFF: A final result in the election is likely to be decided on Thursday.

Wildfires that broke out over the weekend in Portugal have killed at least 35 people, including a one-month-old infant. Today, more than 5,300 firefighters with some 1,600 vehicles were battling the fires, some of which officials say were started by arsonists. Wildfires have also left at least four people dead in neighboring Spain.

Army Sergeant Bowe Bergdahl pleaded guilty today to desertion and misbehavior before the enemy. He was captured by the Taliban in 2009, after leaving his post in Afghanistan. It prompted an intense search and a prisoner swap. Bergdahl appeared before a military judge in Fort Bragg, North Carolina, today. The 31-year-old could be sentenced to life in prison. He said his actions were very inexcusable, adding he didn’t — quote — “think there’d be any reason to pull off a crucial mission to look for one guy.”

The truck driver in deadly immigrant smuggling run has pleaded guilty in court. San Antonio police found at least 39 immigrants, 10 of whom died, packed into a sweltering semi-trailer last year and died. The driver, James Matthew Bradley Jr., pleaded to conspiracy and transporting immigrants, resulting in death. He faces now up to life in prison.

A New Jersey man has been convicted of planting two pressure-cooker bombs on New York City streets last year. Ahmed Khan Rahimi faces a maximum sentence of life in prison for charges including using a weapon of mass destruction. One of the bombs exploded in Manhattan’s Chelsea neighborhood, wounding 30. The second didn’t detonate. Officials said Rahimi was inspired by ISIS and al-Qaida.

JOHN MILLER, Deputy Commissioner, NYPD Intelligence & Counterterrorism: Ahmed Khan Rahimi learned a lesson which we keep reminding people of. This is the wrong place to try and carry out an act of terrorism. Witnesses will come forward, evidence will be developed, arrests will be made, prosecutions will be brought forth, and they will be successful.

JUDY WOODRUFF: Prosecutors said Rahimi also planted a pipe bomb in Seaside Heights, New Jersey, but no one was injured.

Colin Kaepernick has filed a grievance against the national football league. The former San Francisco 49ers quarterback says that he remains unsigned due to collusion by team owners over his national anthem protests. Kaepernick sparked a debate when he kneeled during the anthem last year, protesting police mistreatment of African-Americans.

On Wall Street today, the Dow Jones industrial average gained 85 points to close at 22957. The Nasdaq rose 18. And the S&P 500 added four.

It was a milestone day in the world of astronomy. For the first time, researchers say they have detected gravitational waves with a flash of light from the same cosmic event. The dual observation supports Albert Einstein’s general theory of relativity. The ripples in space and the light burst were caused by the collision of two neutron stars. They were first detected in August.

Tsipras says his country has made economic strides and is “leaving behind the economic model that led to the crisis.” He says Greece’s relationship with the U.S. is “more important than ever.”

Capcom Updates Its Best-Sellers List - Monster Hunter World at 20.9M, RE4 Remake at 8M, More

Capcom has updated its list of Platinum Titles, games that have sold over one million units as of September 30, 2024. The list includes 122 games with sales over one million units, 64 over two million units, 14 over five million units, and six over 10 million units sold.

Resident Evil 4 remake sold 400,000 units to bring lifetime sales to eight million units, Resident Evil 2 remake sold 300,000 units to bring sales to 14.5 million units, and Resident Evil 3 remake sold 200,000 units to bring lifetime sales to 9.2 million units. 

Resident Evil 7: biohazard sold 300,000 units to bring lifetime sales to 14 million units and Resident Evil Village sold 400,000 units to bring sales to 10.5 million units.

Monster Hunter Rise sold 600,000 units in the quarter to bring lifetime sales to 16 million units. Monster Hunter Rise: Sunbreak sold 400,000 units to bring lifetime sales to 8.8 million units.

Monster Hunter: World sold 400,000 units in the quarter to bring lifetime sales to 20.9 million units, while the Iceborne expansion sold 600,000 units for a total of 14 million units.

Dragon's Dogma 2 sold 300,000 units in the quarter to bring lifetime sales to 3.3 million units. Street Fighter 6 sold 400,000 units in the quarter to bring lifetime sales to 4.1 million units. Devil May Cry 5 sold 300,000 units to bring sales to 8.7 million units.

Neowiz to Publish Western RPG by Poland-Based Developer Zakazane

Neowiz announced it has signed a global publishing rights partnership with Warsaw, Poland-based developer Zakazane to publish the first title from the studio. The game from Zakazane is a single-player noir western RPG.

Zakazane is led by Jan Bartkowicz, Piotr Chomiak, Radosław Gwarek, and Marta Sobek, and features developers who previously worked at central European game companies, including  CD Projekt RED and 11 bit studios.

"Neowiz is committed to delivering remarkable, story-driven games that resonate with gamers worldwide, and we’re actively searching for talented developers with a passion for well-crafted narratives,” said Neowiz co-CEO Seung-Cheol Kim.

"When we first met Zakazane, we felt an immediate kinship around their passion for developing projects that profoundly captivate audiences. Planet Coaster 2 and Farming Simulator 25 Debut on the Steam Charts

Call of Duty: Black Ops 6 in its third week remained in first place on the Steam Weekly Top Sellers chart (excluding revenue generated by free games) for Week 46, 2024, which ended November 12, 2024.

There were two new releases in the top 10 this week. Planet Coaster 2 debuted in fourth place, while Farming Simulator 25 came in fifth place.

Steam Deck is up two spots to second place, while Baldur's Gate 3 re-entered the top 10 in third place. EA Sports FC 25 is up two spots to sixth place, Cyberpunk 2077 re-entered the top 10 in seventh place, and Dragon Age: The Veilguard fell from second to eighth place.

Ahead of its release next week pre-orders for S.T.A.L.K.E.R. 2: Heart of Chornobyl came in ninth place.

One Direction Star Liam Payne Dead After Fall in Argentina

Liam Payne, a former member of the boy band One Direction, was found dead in a hotel courtyard in Buenos Aires, Argentina on Wednesday evening, according to CNN and La Nacion, both of which cited local police.

The singer died after an apparent fall from the balcony of his third-floor hotel room. Argentinian authorities told Good Morning America that Payne had been staying at the hotel CasaSur in the upscale neighborhood of Palermo.

He was 31.

Liam Payne Was Embroiled in Bitter Legal Fight With Ex When He Died

Prior to his untimely death in Argentina on Wednesday, One Direction’s Liam Payne, 31, was in the middle of a legal spat with his ex-fiancée Maya Henry.

On Monday, The Sun reported that Henry, 23, had lawyered up in an attempt to stop Payne from constantly messaging her family and friends.

“Maya Henry issued a cease and desist last week to Liam Payne following the emergence of new and concerning information…She has retained attorneys Marco Crawford and Daniel Cerna to represent her. At this time, that is her only comment on the matter,” a spokesperson for Henry told the tabloid.

Fox News’ Bret Baier Whines About Harris After Bad-Tempered Interview

Fox News host Bret Baier recapped his Wednesday night interview with Vice President Kamala Harris, telling his colleagues that he got a sense early on that Harris “was going to be tough to redirect without me trying to interrupt.”

The interview, Harris’ first on the right-wing network since becoming the Democratic nominee, was broadcast on Special Report after being filmed in the previous hour. According to Baier, the interview had been scheduled for 5 p.m., but Harris showed up 15 minutes late. This, he complained, was like “icing the kicker” in football.

“We were supposed to start at 5 p.m. This was the time they gave us. Originally, we were going to do 25 or 30 minutes. They came in and said, ‘Well, maybe 20.’ So, it’s already getting whittled down. And then the vice president showed up at about 5:15 p.m. We were pushing the envelope to be able to turn it around for the top of the 6 p.m.. So that’s how it started,” Baier said.

Chris Hayes Furious With Fox Over ‘Edited’ Soundbite in Harris Interview

MSNBC’s Chris Hayes called out Fox News for not showing the full context of Donald Trump’s “enemy within” comments during Vice President Kamala Harris’ interview on the network earlier Wednesday.

The anchor of All In opened his broadcast by calling attention to something that Harris herself did in the sit-down with Fox’s Bret Baier. The clip that the right-wing network showed to Harris as part of Baier’s question was of Trump during his all-female town hall event in Georgia—the one where Trump bizarrely argued he was the “father of IVF.”

In the clip that Fox aired directly to Harris during her interview, Trump wasn’t heard talking about how former House Speaker Nancy Pelosi or Rep. Adam Schiff are the “enemy within.” Instead, Fox made it so Harris had to react to him talking more mildly about “phony investigations.” The network omitted Trump’s line about “the enemy within.”

Anderson Cooper Literally Calls Bulls*** on Surrogate’s Trump Defense

CNN anchor Anderson Cooper was flabbergasted Wednesday by a surrogate’s defense of Donald Trump, calling his explanation for the former president’s bombastic statements literal “bulls---.”

On AC360, former California Lieutenant Gov. Abel Maldonado, a Republican, said that Trump’s recent comments calling for the military to “handle” Democrats were simply his way of expressing his inner New Yorker. “He’s a fighter,” Maldonado added.

The conversation began when Cooper brought up Trump’s former chairman of the Joint Chiefs of Staff, Mark Milley, who, alongside other military leaders, have begun to warn of the dangers of re-electing the former president. Milley has called Trump a “fascist to his core.”

Trump Gushes Over ‘Fair’ Bret Baier After Fox News Kamala Harris Interview

Donald Trump praised Fox News chief political anchor Bret Baier for a “great job” in his interview with Democratic 2024 presidential hopeful Kamala Harris on Wednesday night, claiming Baier had “showed how totally incompetent Kamala is” while himself demanding she undergo a cognitive ability test.

Trump joined his campaign in calling the interview a total disaster for Harris. During the interview, Harris and Baier engaged in multiple back-and-forths; at one point, on the topic of immigration, Harris clapped back at Baier as he interrupted her: “May I finish responding to you?”

Immediately after the interview aired, the campaign—notably advisers Stephen Miller and Steven Cheung—took to X to decry Harris’ responses.

In his monologue Wednesday, Jimmy Kimmel played a montage of Fox News anchors praising Bret Baier for his interview with Kamala Harris, where they described Baier’s performance as “incredible” and “a masterful job.”

Kimmel responded, “Alright, save it for the post-show orgy, everybody! Come on now.”

The late night host was skeptical that the Fox News pundits’ praise came from a genuine place of journalistic integrity. “They all sit there in fear, imagining Donald Trump on the toilet watching them,” he said.

Former President Donald Trump said Wednesday that Jan. 6, 2021— the day his supporters occupied Congress in a failed insurrection to try to stop lawmakers from certifying Joe Biden’s election victory—was a “day of love.”

Trump made the baffling claim during a televised election town hall hosted by Univision.

Ramiro González, a construction worker from Tampa, told the meeting he deregistered as a Republican because he found Trump’s “inaction” during both Jan. 6 and the COVID-19 pandemic “disturbing.” He asked Trump to square his controversial behavior during the attack on the U.S. Capitol—and the fact that many of his own former administration officials don’t support him any longer—with why he should be re-elected.

Donald Trump Cancels Second Mainstream Interview in Days

Donald Trump pulled out of another mainstream interview Thursday–this time nixing a sit-down with NBC News.

The interview, CNN reported, would be in Philadelphia with NBC News' senior business correspondent, Christine Romans. CNN’s Brian Stelter said one source suggested that it had only been “postponed.”

It was the second time in a week that he had canceled a scheduled appearance outside the conservative news sphere, CNN’s Reliable Sources reported Thursday. He had canceled an in-studio appearance on the CNBC flagship show, Squawk Box, which was due on Friday.

Part of the fun of Dragon Age's fantasy is that it's inconsistent - or at least, inconsistent by the standards of fantasy RPGs, which often break down into a million neatly organised and interlocking codex entries. It all rides on who you speak to. The humans believe one thing about the origins and workings of Thedas, the elves another, the qunari something else entirely. These differences are the basis for many factional disagreements and thus, many core series plot developments. According to former lead writer David Gaider, however, there's an "uber-plot" behind it all that may one day be resolved and bring the series to a close, assuming BioWare continue to refer to his original (and closely guarded) narrative documents.

Thysiastery is an anime Legend Of Grimrock, and you can attack the dinosaur merchants if you’re a complete monster

One of my lesser quality tests for an RPG is whether the shopkeepers complain at you for not buying anything. Grumpy shopkeepers, good RPG. This most specific of litmus tests has served me well, although I must admit that I’d happily upgrade it to ‘shopkeepers you can attack’, would that not disqualify 99% of games. But not turn based dungeon crawler Thysiastery, it turns out. This “dungeon crawler RPG featuring traditional roguelike and turn-based gameplay” apparently trusts you enough to let you recklessly batter its friendly wandering lizard merchants. You’d be a monster for it, of course, but it’s nice to have options.

No, you're not imagining Monster Hunter Wilds' beta combat feeling off - there's a good reason for it

I didn’t get much further in the extremely popular beta for the haute-couture-asaurus action of Monster Hunter Wilds than perfecting the exact orange-to-white ratio of my cat. Not because I wasn’t having fun, but because I immediately started looking up GPU prices after playing for ten minutes. As such, I didn’t spend enough time with the combat to get a proper feel for it. Cultural osmosis has once again allowed me to form an uneducated take, however, and I’m getting the sense there’s been some mixed reactions re: bonk quality. According to a clip shared on X by user Blue Stigma, there's a good reason for those misgivings. It's all about frames, you see.
