elderly Commentary on Persistent Hyponatremia in an Elderly Patient By academic.oup.com Published On :: Thu, 30 Apr 2020 00:00:00 GMT Hyponatremia, the most common electrolyte disorder, is present in 2%–4% of ambulatory patients and up to 22% of geriatric inpatients (1). Clinicians classify hyponatremia into three main categories: hypovolemic (decreased intravascular volume from salt and water loss with inadequate replacement), euvolemic (normal intravascular volume, but increased water dilutes sodium), and hypervolemic (total body water is increased, usually causing edema, while intravascular volume is often decreased). Less frequent causes are pseudohyponatremia (due to decreased water in a fixed volume of plasma, usually due to increased lipids or protein) and water shifting due to increased osmolality (most commonly due to glucose). While the most extreme examples of hypovolemic and hypervolemic states can be determined by physical examination, in many cases the patient is euvolemic clinically. Full Article

Most (27/29) plasmids were of the FIA, FIB, and FII multireplicon type with the same plasmid multilocus sequence typing (pMLST). Several plasmids (7/23) from LTCF residents harbored only blaCTX-M-27 as the resistance gene; however, their fundamental structures were very similar to those of previously isolated blaCTX-M-27/F1:A2:B20 plasmids, suggesting their prevalence as a newly arising public health concern. elderly Mumbai: Ghatkopar elderly couple cries foul over Rs 27,000 power bill By www.mid-day.com Published On :: 20 Jul 2018 09:36:05 GMT The Lodhs of Ghatkopar loathe Tata Power, because they received a Rs 27,000 power bill from the company in May. This was shocking for the family because the only people who reside in the 1BHK flat in Amrut Nagar are Kalidas Lodh, 76, and his wife Gopa, both of whom make use of only basic electrical appliances. Kalidas, the owner of the flat, is a Navy veteran and has been following up the issue with the service provider since May with the help of his son Kaushik, who is visiting him from the USA. The family alleges their complaint of electricity theft has been ignored. They've switched to another service provider after the incident and were stunned to see their electricity bill for the month of June, which was only Rs 2,000. Meter is fineThe Lodhs had been receiving odd electricity bills since June 2016. However, the May bill this year shocked the family, after which they finally complained to the service provider. "Their technical team visited our house to check the problem. But they kept saying that the meter is working fine and if there is any tampering done beyond the main meter, it is out of their control to check and verify it. They kept insisting that we should check it ourselves with help from another electrician. Isn't it the responsibility of the electricity service providers to verify the theft?" asked Kaushik, who said their former electricity service provider completely shrugged off their responsibility. Kaushik added, "In one of the visits, even their technician saw how the meter showed units being consumed even when the main electricity supply switch of the house was off. It only made our doubts of theft stronger. But shockingly, in their written report, there was no mention of this observation as they kept insisting the service provider has no control over tampering beyond the main supply point of the building." Kaushik also approached the police to complain about the theft, "But officers at the Parksite police station told me that the right platform for me to complain is the consumer forum and not the police." Vilas Jadhav, senior police inspector of Parksite police station remained unavailable for comment. Tata Power saysRanjit Ganguly, group head of customer relationship management, Tata Power said, "For the excess billing, we'd checked the functionality of the meter, which was intact. He'd installed a separate meter in his house; we checked that one as well and found it to be in sync with the main meter. The complainant suspected tampering through the wires going from the main meter to his house. Regulatory guidelines for service providers clearly state that any wiring beyond the point of supply, which is the main meter box, is in the purview of the consumer... So...Tata Power as a utility service provider is unable to help." elderly Physically challenged, elderly, get little help at polling station By www.mid-day.com Published On :: 30 Apr 2019 02:00:06 GMT Despite the Election Commission (EC) announcing several initiatives to help the physically challenged and senior citizens during the voting process, no such arrangements were visible at many of the polling centres on Monday. Contrary to claims made by EC, no volunteers were present to guide the physically challenged or senior citizens to their polling booths, leaving them to either find a way on their own or rely on their family members. No one came to help Bagubai Shirwale, 70, who has a hunched back, had to inch forward using her walking stick as she tried to find her polling booth. She only had the help of her brother, Trimbak Shirwale, 72, who, like her relies on a walking stick. "We come to vote every time without fail. But I have diabetes, due to which I am not able to walk properly. My sister has chronic backache and can't walk much either. No one came to help us; we had to find the way around ourselves," he said. They had to walk nearly 1 km to reach the polling station. Even after they successfully reached their venue, SK Pantwalkar Madhyamik Vidyalaya in Kurla west, finding the booth took some time. After asking around, they were pointed in the right direction. In the absence of a wheelchair at the St Judes High School in Malad West, local Antony Roasario, who is physically challenged, is forced to crawl over a gutter under the burning sun. Pics/Suresh Karkera Despite the sweltering heat, Sayaji Mokashi, 50, a visually challenged man, walked to his polling station, the Mahatma Gandhi Vidya Mandir in Bandra east, along with his wife Prajakta who also has problems with her eyesight, and their seven-year-old son. Mokashis, residents of the government colony in Bandra east, recall walking to their booth back in 2014 as well. "It is our responsibility to vote and we take it seriously. We don't mind the walk but it would help if EC could arrange for some transportation," said Sayaji. Sudha Shantaram Nadkar, 75, who requires a walking aid, is thankful that her polling station is just across the road from her building in Kurla. However, she too has to rely on her neighbours to help her reach her booth. "I have been voting since I turned eligible and I don't miss any election. But I fell three years ago, which made it difficult for me to walk. There is no one to help us find the booth and no one came forward to help me walk in either," she said. Where are the ramps? EC had also promised to provide ramps at all polling stations, as well as dolis (chairs with poles). Many of the polling stations across the city neither had any ramps for wheelchairs nor dolis. The police personnel stationed at the Central Railway Social Welfare Centre in Kurla east said no volunteers had showed up at the venue and no dolis were sent either. "We are allowing senior citizens to go in directly; they don't have to wait in line. But their family members are helping them," said a policeman. Similarly, at L S Raheja College, an elderly man with an amputated leg walked to his booth with the help of crutches and his relative. Another aged man at the same venue had been brought inside by his family on a wheelchair, but had to walk the rest of the way with the help of his walking stick, while leaning on his son for support.
