The U.S. Senate's Democratic majority began a crusade on Tuesday to confirm as many new federal judges nominated by President Joe Biden as possible to avoid leaving vacancies that Republican Donald Trump could fill after taking office on Jan. 20. With Republicans set to take control of the chamber on Jan. 3, the Senate on Tuesday held a confirmation vote on one of Biden's judicial nominees - former prosecutor April Perry - for the first time since Trump won the Nov. 5 presidential election. The Senate voted 51-44 in favor of her becoming a U.S. district court judge in Illinois. All told, Biden has announced another 30 judicial nominees who are awaiting Senate confirmation votes. Sixteen have already have been reviewed by the Senate Judiciary Committee and are awaiting a final confirmation vote by the full Senate. Another 14 nominees are awaiting committee review. The U.S. Constitution assigns to the Senate the power to confirm a president's nominees for life-tenured seats on the federal judiciary. "We are going to get as many done as we can," Democratic Senate Majority Leader Chuck Schumer said in a statement. Trump made 234 judicial appointments during his first four years in office, the second most of any president in a single term, and succeeded in moving the judiciary rightward - including building a 6-3 conservative majority on the U.S. Supreme Court with three appointees. Biden has appointed a host of liberal judges. Since the beginning of his presidency in 2021, the Senate has confirmed 214 Biden judicial nominees, including liberal Supreme Court Justice Ketanji Brown Jackson. About two-thirds were women, and the same share were racial minorities. Senate Democrats are under pressure to swiftly confirm the remaining nominees, along with any new picks Biden may name in the waning weeks of his presidency. How many nominees Senate Democrats will be able to confirm remains to be seen. Trump in a social media post on Sunday called on the Senate to halt approving Biden's nominees, saying, "Democrats are looking to ram through their Judges." Billionaire Trump backer Elon Musk on Tuesday wrote on social media that "activist" judicial nominees are "bad for the country." Mike Davis, a Trump ally at the conservative judicial advocacy group Article III Project, in another post urged Senate Republicans to vote down all judicial appointments until January. "The American people voted for monumental change," Davis wrote on social media last week. "Grind the Senate to a halt." Current Senate Republican leader Mitch McConnell's office declined comment. McConnell has consistently opposed Biden's nominees and, as majority leader, was instrumental in getting Trump's previous nominees confirmed. Trump's judicial appointees have been involved in major decisions welcomed by conservatives including Supreme Court rulings rolling back abortion rights, widening gun rights, rejecting race-conscious collegiate admissions and limiting the power of federal regulatory agencies. Judicial nominees require a simple majority for confirmation. Democrats currently hold a slim 51-49 majority, meaning that they can ill afford any defections or absences if Republicans show up in force to oppose Biden's nominees during the chamber's post-election "lame duck" session. West Virginia Senator Joe Manchin, an independent who caucuses with the Democrats, has said he would not vote for any nominee who does not garner at least one Republican vote. Must-pass legislation like a spending bill to avert a government shutdown also may consume precious time during the session. 'Every possible nominee' Biden's allies have said a concerted push to confirm his remaining nominees would allow him to build on his legacy of helping to diversify a federal bench long dominated by white men. He is not done nominating judges. On Friday, Biden announced his first post-election nominee, Tali Farhadian Weinstein, who after unsuccessfully running in the 2021 Democratic primary to be Manhattan district attorney was picked for a job as a federal district judge in New York. A spokesperson for Senator Dick Durbin, the No. 2 Senate Democrat and chair of the Judiciary Committee, said in a statement that he "aims to confirm every possible nominee before the end of this Congress." White House spokesperson Andrew Bates on Monday noted that during Trump's first term, the Republican-controlled Senate confirmed 18 judges after Biden had won the 2020 election but before he took office. Pending nominees include five to the influential federal appeals courts. Republicans said before the election that they had the votes to block two of them: Adeel Mangi, who would become the first Muslim federal appellate judge, and North Carolina Solicitor General Ryan Park, who unsuccessfully defended the race-conscious admissions policies before the Supreme Court. There are several others nominated to serve as trial court judges like Perry, a former prosecutor now working at Chicago-headquartered GE HealthCare who would join the bench in Illinois. Biden nominated her to a judgeship in April after her prior nomination to become Chicago's top federal prosecutor was blocked by Republican Senator JD Vance. Vance began placing a hold on Biden's nominees to the U.S. Justice Department in 2023 after Special Counsel Jack Smith secured the first of two federal indictments against Trump, who subsequently picked the senator as his vice presidential running mate.

Compound probiotics have been widely used and commonly coadministered with other drugs for treating various chronic illnesses, yet their effects on drug pharmacokinetics remain underexplored. This study elucidated the impact of VSL#3 on the metabolism of probe drugs for cytochrome P450 enzymes (P450s), specifically omeprazole, tolbutamide, midazolam, metoprolol, phenacetin, and chlorzoxazone. Male Wistar rats were administered drinking water containing VSL#3 or not for 14 days and then intragastrically administered a P450 probe cocktail; this was done to investigate the host P450’s metabolic phenotype. Stool, liver/jejunum, and serum samples were collected for 16S ribosomal RNA sequencing, RNA sequencing, and bile acid profiling. The results indicated significant differences in both α and β diversity of intestinal microbial composition between the probiotic and vehicle groups in rats. In the probiotic group, the bioavailability of omeprazole increased by 269.9%, whereas those of tolbutamide and chlorpropamide decreased by 28.1% and 27.4%, respectively. The liver and jejunum exhibited 1417 and 4004 differentially expressed genes, respectively, between the two groups. In the probiotic group, most of P450 genes were upregulated in the liver but downregulated in the jejunum. The expression of genes encoding metabolic enzymes and drug transporters also changed. The serum-conjugated bile acids in the probiotic group were significantly reduced. Shorter duodenal villi and longer ileal villi were found in the probiotic group. In summary, VSL#3 administration altered the gut microbiota, host drug–processing gene expression, and intestinal structure in rats, which could be reasons for pharmacokinetic changes.

Despite a significant decrease in the number of prescriptions for opioids, the opioid crisis continues, fueled in large part by the availability of the phenylpiperidine mu opioid receptor (MOR) agonist fentanyl. In contrast, the number of prescriptions for and the off-label use of gabapentinoids (gabapentin and pregabalin) has increased dramatically, with gabapentinoids commonly detected in opioid overdose victims. Although gabapentinoids can decrease the potency of the opioid receptor antagonist naloxone to reverse heroin-induced hypoventilation in male rats, the specificity and nature of interaction between gabapentinoids and MOR agonists and any potential sex difference in those interactions are not well characterized. Gabapentinoids were studied in female and male rats discriminating fentanyl (0.0032 mg/kg, i.p.) or cocaine (3.2 mg/kg, i.p.). Alone, neither gabapentin nor pregabalin significantly increased fentanyl- or cocaine-appropriate responding. In rats discriminating fentanyl, each gabapentinoid dose-dependently shifted the fentanyl and heroin discrimination dose-effect functions to the left, whereas naloxone dose-dependently shifted the fentanyl and heroin discrimination dose-effect functions to the right. Each gabapentinoid (100 mg/kg) significantly decreased the potency of naloxone to antagonize the discriminative stimulus effect of fentanyl or heroin. In contrast, each gabapentinoid dose-dependently shifted the cocaine and d-methamphetamine discrimination dose-effect functions to the right. There were no significant sex differences in this study. These results suggest that gabapentinoids impact the misuse of opioids, the co-use of opioids and stimulant drugs, and the increasing number of overdose deaths in individuals using opioids, stimulant drugs, and gabapentinoids in mixtures.

Previous studies demonstrated that sigma receptor (R) antagonists alone fail to alter cocaine self-administration despite blocking various other effects of cocaine. However, R antagonists when combined with dopamine transporter (DAT) inhibitors substantially decrease cocaine self-administration. To better understand the effects of this combination, the present study examined the effects of R antagonist and DAT inhibitor combinations in male rats discriminating cocaine (10 mg/kg, i.p.) from saline injections. The DAT inhibitors alone [(–)-2-β-carbomethoxy-3-β-(4-fluorophenyl)tropane 1,5-naphthalenedisulfonate monohydrate (WIN 35,428) and methylphenidate] at low (0.1-mg/kg) doses that were minimally active failed to shift the dose-effect function for discriminative-stimulus effects of cocaine to the left more than 2-fold. At 0.32 mg/kg the DAT inhibitors alone shifted the cocaine dose-effect function leftward 24- or 6.6-fold, respectively. The R antagonists (BD1008, BD1047, and BD1063) failed to fully substitute for cocaine, although BD1008 and BD1047 substituted partially. At 10 mg/kg, BD1008, BD1047, or BD1063 alone shifted the cocaine dose-effect function leftward less than 6.0-fold. In combination with 0.1 mg/kg WIN 35,428, the 10 mg/kg doses of R antagonists shifted the cocaine dose-effect function from 12.3- to 36.7-fold leftward, and with 0.32 mg/kg WIN 35,428 from 14.3- to 440-fold leftward. In combination with 0.1 mg/kg methylphenidate, those R antagonist doses shifted the cocaine dose-effect function from 5.5- to 55.0-fold leftward, and with 0.32 mg/kg methylphenidate from 10.5- to 48.1-fold leftward. The present results suggest that dual DAT/R inhibition produces agonist-like subjective effects that may promote decreases in self-administration obtained in previous studies.

Neuropathic pain is a form of chronic pain that develops because of damage to the nervous system. Treatment of neuropathic pain is often incompletely effective, and most available therapeutics have only moderate efficacy and present side effects that limit their use. Opioids are commonly prescribed for the management of neuropathic pain despite equivocal results in clinical studies and significant abuse potential. Thus, neuropathic pain represents an area of critical unmet medical need, and novel classes of therapeutics with improved efficacy and safety profiles are urgently needed. The cannabidiol structural analog and novel antagonist of GPR55, KLS-13019, was screened in rat models of neuropathic pain. Tactile sensitivity associated with chemotherapy exposure was induced in rats with once-daily 1-mg/kg paclitaxel injections for 4 days or 5 mg/kg oxaliplatin every third day for 1 week. Rats were then administered KLS-13019 or comparator drugs on day 7 in an acute dosing paradigm or days 7–10 in a chronic dosing paradigm, and mechanical or cold allodynia was assessed. Allodynia was reversed in a dose-dependent manner in the rats treated with KLS-13019, with the highest dose reverting the response to prepaclitaxel injection baseline levels with both intraperitoneal and oral administration after acute dosing. In the chronic dosing paradigm, four consecutive doses of KLS-13019 completely reversed allodynia for the duration of the phenotype in control animals. Additionally, coadministration of KLS-13019 with paclitaxel prevented the allodynic phenotype from developing. Together, these data suggest that KLS-13019 represents a potential new drug for the treatment of neuropathic pain.

The consumption of ⁹-tetrahydrocannabinol (THC)- or cannabis-containing edibles has increased in recent years; however, the behavioral and neural circuit effects of such consumption remain unknown, especially in the context of ingestion of higher doses resulting in cannabis intoxication. We examined the neural and behavioral effects of acute high-dose edible cannabis consumption (AHDECC). Sprague-Dawley rats (six males, seven females) were implanted with electrodes in the prefrontal cortex (PFC), dorsal hippocampus (dHipp), cingulate cortex (Cg), and nucleus accumbens (NAc). Rats were provided access to a mixture of Nutella (6 g/kg) and THC-containing cannabis oil (20 mg/kg) for 10 minutes, during which they voluntarily consumed all of the provided Nutella and THC mixture. Cannabis tetrad and neural oscillations were examined 2, 4, 8, and 24 hours after exposure. In another cohort (16 males, 15 females), we examined the effects of AHDECC on learning and prepulse inhibition and serum and brain THC and 11-hydroxy-THC concentrations. AHDECC resulted in higher brain and serum THC and 11-hydroxy-THC levels in female rats over 24 hours. AHDECC also produced: 1) Cg, dHipp, and NAc gamma power suppression, with the suppression being greater in female rats, in a time-dependent manner; 2) hypolocomotion, hypothermia, and antinociception in a time-dependent manner; and 3) learning and prepulse inhibition impairments. Additionally, most neural activity and behavior changes appear 2 hours after ingestion, suggesting that interventions around this time might be effective in reversing/reducing the effects of AHDECC.

Even though that perception is partly the creation of right-wing media, the Democrats surely need to hone their identity.
