OK, first, let me drop my biases: I can be a cynical person and I'm pretty high energy — like hyper hyper — which has made meditation difficult for me. I can get both mentally and physically uncomfortable while trying anything resembling meditation or therapy or sincere reflection.

Respective Contributions of Glycemic Variability and Mean Daily Glucose as Predictors of Hypoglycemia in Type 1 Diabetes: Are They Equivalent?

OBJECTIVE

To evaluate the respective contributions of short-term glycemic variability and mean daily glucose (MDG) concentration to the risk of hypoglycemia in type 1 diabetes.

RESEARCH DESIGN AND METHODS

People with type 1 diabetes (n = 100) investigated at the University Hospital of Montpellier (France) underwent continuous glucose monitoring (CGM) on two consecutive days, providing a total of 200 24-h glycemic profiles. The following parameters were computed: MDG concentration, within-day glycemic variability (coefficient of variation for glucose [%CV]), and risk of hypoglycemia (presented as the percentage of time spent below three glycemic thresholds: 3.9, 3.45, and 3.0 mmol/L).

RESULTS

MDG was significantly higher, and %CV significantly lower (both P < 0.001), when comparing the 24-h glycemic profiles according to whether no time or a certain duration of time was spent below the thresholds. Univariate regression analyses showed that MDG and %CV were the two explanatory variables that entered the model with the outcome variable (time spent below the thresholds). The classification and regression tree procedure indicated that the predominant predictor for hypoglycemia was %CV when the threshold was 3.0 mmol/L. In people with mean glucose ≤7.8 mmol/L, the time spent below 3.0 mmol/L was shortest (P < 0.001) when %CV was below 34%.

CONCLUSIONS

In type 1 diabetes, short-term glycemic variability relative to mean glucose (i.e., %CV) explains more hypoglycemia than does mean glucose alone when the glucose threshold is 3.0 mmol/L. Minimizing the risk of hypoglycemia requires a %CV below 34%.

Marian basilica offers daily Eucharistic blessing of Rome under lockdown

Rome, Italy, May 7, 2020 / 09:30 am (CNA).- A Eucharistic blessing of the city of Rome has been offered each day from the doorstep of Rome’s largest Marian basilica as coronavirus measures restricted the public from attending Masses.

The daily Eucharistic procession and benediction takes place in the Papal Basilica of St. Mary Major immediately following a livestreamed Mass at 11 a.m. local time from the chapel containing the Marian icon Salus Populi Romani, Mary Protection of the Roman People.

“The cardinal archpriest, all the canons, priests and religious of this basilica wish to give testimony to the Real Presence of Our Lord Jesus Christ, Doctor and Medicine for humanity tried by this pandemic,” states a caption for the video of a benediction from the loggia on March 21. 

As Rome entered phase two of coronavirus restrictions this week, more people have been able to witness the Eucharistic procession and blessing in person. In the second phase of Italy’s lockdown, which began May 4, residents are allowed to exercise and go for walks in the city with facemasks.

On the first day of the eased restrictions, a tour guide in Rome stumbled upon the Eucharistic procession in the basilica.

“I saw that confession was available and adoration was taking place. Following adoration, the priests processed the Blessed Sacrament through the church and into the piazza for a benediction over the city. They then walked to the back door and out into that piazza for another benediction,” Mountain Butorac told CNA. 

“This being one of my first experiences in church in nearly two months brought tears to my eyes and hope to my heart,” he said.

Public Masses will be able to resume in the Diocese of Rome and throughout Italy beginning on May 18, 70 days after the restrictions on Mass went into effect.

During Rome’s lockdown, Pope Francis made a brief walking pilgrimage to the Basilica of St. Mary Major to pray for the Virgin Mary’s protection from the coronavirus pandemic affecting Italy and the world.

The pope later had the basilica’s Byzantine icon of Salus Populi Romani brought to St. Peter’s Square for the extraordinary Urbi et Orbi benediction on March 27. The Marian icon remained inside the basilica throughout the Easter Triduum liturgies.

The Salus Populi Romani icon was also processed through Rome by Pope Gregory I for an end to a plague in 593.

Among the four major papal basilicas in Rome, St. Mary Major is the only one that maintained its original structure. Mosaics dating back to the 5th century can be seen in the central nave of the basilica, which also houses the relic of the Holy Crib from the birth of Christ.

According to tradition, the Virgin Mary appeared to both a nobleman named John and to Pope Liberius (352-366) in a dream foretelling the August snow and asking for a church to be built in her honor on the site of the snowfall on Aug. 5 in the year 358. The church was rebuilt by Pope Sixtus III (432-440), after the Council of Ephesus in 431 declared Mary to be the Mother of God.

Vespers and prayers of the holy rosary are offered via livestream each day from the Marian basilica just before 5 p.m. in Rome.

A Comparison of Clinical Outcomes among Intensive Care Unit Patients Receiving Ceftriaxone 1 gram daily or 2 grams daily [Clinical Therapeutics]

Background: Intensive care unit (ICU) patients may experience ceftriaxone underexposure but clinical outcomes data are lacking. The objective of this study was to determine the impact of ceftriaxone dosing on clinical outcomes amongst ICU patients without central nervous system (CNS) infection.

Methods: A retrospective study of ICU patients receiving intravenous, empiric ceftriaxone for non-CNS infections was conducted. Patients ≥18 years of age who received ≤2 grams of ceftriaxone daily for ≥72 hours were included and categorized as receiving ceftriaxone 1 gram or 2 grams daily. The primary, composite outcome was treatment failure: inpatient mortality and/or antibiotic escalation due to clinical worsening. Propensity score matching was performed based on the probability of receiving ceftriaxone 2 grams daily. Multivariable logistic regression determined the association between ceftriaxone dose and treatment failure in a propensity-matched cohort.

Results: A total of 212 patients were included in the propensity-matched cohort. The most common diagnoses (83.0%) were pneumonia and urinary tract infection. Treatment failure occurred in 17.0% and 5.7% of patients receiving 1 gram and 2 grams daily, respectively (p=0.0156). Overall inpatient mortality was 8.5%. Ceftriaxone 2 gram dosing was associated with a reduced likelihood of treatment failure (adjusted odds ratio=0.190; 95% confidence interval: 0.059 – 0.607). Other independent predictors of treatment failure included sequential organ failure assessment score (aOR 1.440, 95% CI 1.254 – 1.653) and creatinine clearance at 72 hours from ceftriaxone initiation (aOR 0.980, 95% CI (0.971 – 0.999).

Conclusions: Ceftriaxone 2 grams daily when used as appropriate antimicrobial coverage may be appropriate for ICU patients with lower mortality risk.
