In a bid to do my part to save the world from this crippling pandemic without actually moving my backside for fear of stepping over the Lakshman Rekha, I decided to draw up a survival guide of Dos and Don’ts to weather the tsunami of trouble stirred up by this nasty critter. Trust me, preliminary studies conducted by the reputed folks at WhatsApp University have confirmed their efficacy beyond a shadow of doubt. The first step is to make soap and water your most intimate acquaintances. They have proven themselves good friends to humanity long before diamonds and dogs usurped their rightful spot and will continue to prove their worth long after corona has shrunk back into the bowels of hell from which it has erupted like a particularly foul effluent. It would also help if one were to make personal hygiene a priority and public hygiene an even bigger one if only as a preventive measure against the spread of communicable diseases and related hazards that could prove to have epic stakes in the survival game. Roughly translated, that means not ever dumping your trash outside the sparkling interiors of your home into the streets, using the great outdoors as a spittoon or latrine, defacing public property by scratching your lover’s name, throwing up pornographic sketches on every available surface or blowing your nose and allowing the discharge to fly every which way. Steer clear of social media if you value your sanity or at least use it in moderation especially if you lack the balancing power of a monkey on stilts to take the better while leaving out the bitter, barf – inducing garbage out there. Ever since some wiseacre got word out that democracy works best when your voice is heard out loud, people have been screaming themselves hoarse to make sure that their endless torrent of nonsense is amplified to the furthest reaches of the internet, bolstered by twitchy fingers compulsively hitting like and share in the desperate hope that they will be rewarded in kind. Therefore we have internet idiots and savants with interchangeable attributes raving and ranting fit to burst. The insufferably self-righteous and superior types tend to drip scorn for everyone and everything from the admitted showmanship of the supreme leader and inadequacy of the ruling party to the extreme foolishness of the folks who tentatively stepped out of their homes to find a bite to eat or medicine for an ailing grandmother only to be sneakily photographed or recorded so that they may be viciously trolled, endlessly abused and publicly shamed for putting millions at risk. Then there are the fanatical religious nuts who swear that India need not be subjected to a lockdown because Indians practise the downward facing dog regularly on their balconies with their rear ends strategically raised towards the benign sun which unleashes the Kundalini Shakti with all its serpentine strength which is more than sufficient to slay corona, sharona and whatever else have you or something equally ludicrous about the virus killing power of pots and pans banged in unison. Relatively innocuous Instagram must also be avoided at all costs too. According to the illustrious professors of WhatsApp University, those pretty pictures have great envy-inducing powers which can corrupt the best and brightest till they are reduced to gibbering, glass-eyed cyber stalkers who can spend years staring at pics and videos of Janhvi Kapoor sexily pretending to eat a slice of pizza, giving her puppy a bath or showing you the correct technique for applying lipstick to the exclusion of all things useful or worthy. Avoid fake news like the plague. Fear and panic cause a lot more damage than pandemics. Be sure to verify the credentials and reliability of all purveyors of information and check the facts. Even if you can’t be bothered, be sure to think thrice before you join hands with the mob to tear apart the hapless medical personnel getting off a gruelling nightshift because they are suspected to be carriers of COVID-16 (or is it COVID-17?). And believing self-proclaimed experts who have declared that the only country in the world that is currently corona free is Kailasa and you are considering applying for permanent citizenship under the benevolent rule of HDH Nithyananda even if you run a far higher risk of contracting HIV, do yourself a favour and check yourself into the nearest hospital for a lobotomy. While many endorse vegetating in front of Netflix, it is advisable to take time out to work out, read a book, cook a meal, take the dog for a walk, play with the kids or teach them something useful that does not involve staring at a screen, converse with the spouse about something other than whose turn it is to do the dishes or swab the floors and do whatever it takes to tough things out as long as it is legal and non-fattening. Do all the things you always wished you had the time to do, even if it is something impracticable like working on a script that will be picked up by Leonardo DiCaprio at an exorbitant price or perfecting the pincha mayurasana in the hopes that a video of you executing the same or at least the blooper version will go viral on the internet. If you are inclined to feel that this survival guide is lacking in scientific detail, didn’t once mention social distancing, testing, gaumutra, masks, sanitiser and is somewhat on the rambling side not unlike the supreme leader’s impassioned addresses to the nation, allow me to stress that if the buzz on WhatsApp is to be believed I am a shoo-in for the Noble Prize thanks to this priceless contribution towards the greater good, entirely free of cost. If that doesn’t convince you to take every word put down here as the gospel truth which deserves to be liked and shared till it has spread to the furthest corners of civilized society like a contagion, I don’t know what will. In a recent Brookings India paper on the Indian government’s health care policy, we argue that it should prioritize expanding and effectively delivering those aspects of health that fall under the definition of “public goods’” for example, vaccination, health education, sanitation, public health, primary care and screening, family planning through empowering women, and reproductive and child health. Large gains in the nation’s health, and particularly the health of the poorest and most marginalized, can be made with this limited focus. As just one estimate, a 2010 World Bank study showed that India lost 53.8 billion USD annually in premature mortality, lost productivity, health care provision and other losses due to inadequate sanitation. Not about the money: Reforming India’s management systems Importantly, these gains can come very cost effectively, as demonstrated by India’s neighbors Bangladesh and Sri Lanka, which spend less as a percentage of GDP on health than India, but have better outcomes. It is not an expansion in spending that is critical for improving health outcomes. Instead, India needs to set appropriate goals and reform the public health care sector’s governance and management systems so that it is able to deliver on those goals. Evidence gathered globally and within India suggests that without good governance, additional spending would be worth little. One potential model to adopt is to set up publicly owned corporations at the state level that can take over the existing state health infrastructure and health delivery operations, thus permitting greater flexibility in management than the government’s notoriously inefficient and hidebound administrative systems. India needs to set appropriate goals and reform the public health care sector’s governance and management systems so that they are able to deliver against those goals. Where secondary and tertiary care are concerned, we believe that the government’s role should be to provide a different public good—sensible and responsive regulation that allows a health care market to develop. The government’s regulatory mechanism will need to address issues of information asymmetry between doctors and patients, for which we recommend government action to supplement market solutions for doctor discovery and quality appraisal that are already springing up. Hospital accreditation, increased importance for patient safety standards and guidelines, standardized, and, in time, mandated, Electronic Medical Records are all measures that will go toward ameliorating market failures that arise from information asymmetry in health care. Increased focus on patient safety in medical curriculums will help, but providing regulation that balances the twin objectives of improving monitoring, reporting and prevention of adverse events while disincentivizing the events themselves will be a key challenge for regulators. Addressing the shortage of qualified medical professionals Human resource expansion in health care is an area where transparent and responsive government regulation on the supply side is a public good of fundamental importance. The paucity of qualified health workers in India is well documented. The distribution, too, is skewed – the public health system, particularly in rural areas, is very short of qualified personnel. As many as 18 percent of government Primary Health Centers (PHCs) were entirely without doctors, and many others faced shortages. One promising way forward is offered by Indian state Chhattisgarh’s experience with a 3 year long medical training course. While the course was shut down in a few years after opposition from doctors, its graduates were hired as Rural Medical Assistants (RMAs) in PHCs. A Public Health Foundation of India (PHFI) study in 2010 evaluated PHCs across the state, focusing on diseases and conditions that PHCs most need to treat. They found that PHCs run by RMAs were just as good as those run by regular MBBS doctors in terms of provider competence, prescription practices and patient and community satisfaction. Practitioners with training in traditional medicine can also be potentially mainstreamed into such roles. Such avenues toward overcoming the shortage of medical personnel in rural areas must be explored. As many as 18 percent of government Primary Health Centers (PHCs) were entirely without doctors, and many others faced shortages. Health care financing is another area where government can play a large role. Medical insurance has proved to be a poor model for financing health care. It faces several theoretical pitfalls and has been one of the major factors behind the expensive and unsustainable healthcare system in the U.S. One approach that circumvents the adverse selection and moral hazard issues of medical insurance is that of introducing Medical Savings Accounts (MSAs). MSAs can be encouraged by tax deductions that would apply if the accounts were used to pay for medical expenses, and equity concerns can be alleviated by direct payments for those that cannot pay for themselves. rio U.S. priorities at the Seventh Summit of the Americas By webfeeds.brookings.edu Published On :: Wed, 08 Apr 2015 16:50:00 -0400 On Friday, April 3, the Brookings Latin America Initiative hosted Assistant Secretary of State for Western Hemisphere Affairs Roberta Jacobson to discuss the state of inter-American relations and expectations for the Seventh Summit of the Americas to be held on April 10 to 11 in Panama City, Panama. With Cuba in attendance for the first time, this summit will be a chance for the entire region to have a robust conversation on hemispheric challenges and opportunities. The event began with a keynote address by Assistant Secretary Jacobson, and was followed with a discussion moderated by Richard Feinberg—dubbed the “godfather” of the Summit process for his role in the first Miami Summit of the Americas in 1994—and Harold Trinkunas. This event also launched a new Brookings policy brief by Richard Feinberg, Emily Miller, and Harold Trinkunas, entitled "Better Than You Think: Reframing Inter-American Relations." Assistant Secretary Jacobson began her remarks by highlighting the areas where her own thinking coincides with the arguments in this new policy brief. Principally, she argued that developments in the hemisphere over the past few decades have largely been positive for U.S. interests. Although this does not mean Latin America and the United States will agree on everything, she noted that there are many areas of mutual interests on which the United States can work together with Latin America countries as equal partners. Jacobson explained that this desire to forge equal partnerships based on common values and interests was precisely the notion expressed by President Obama at the 2009 Summit in Trinidad. The upcoming Summit is a chance to showcase this updated architecture for cooperation and partnership, which includes the CEO Summit of the Americas (initiated in 2012) and the Civil Society and Social Actors Forum (new this year). Key issues for the U.S. at the Summit of the Americas Assistant Secretary Jacobson outlined the four priorities for the United States going into the Summit: Democracy and human rights: Jacobson stated that the United States “applauds governments around the hemisphere that have supported a more robust civil society role.” The civil society side event provides a critical feedback loop that is one way for leaders to be held accountable by their citizens. Jacobson noted, however, that there remain very real challenges to democracy in Venezuela. While this is something that should concern the entire hemisphere, it is ultimately up to the Venezuelans to resolve. Global competitiveness: The focus of the United States will be on small businesses, which are important job creators but do not always receive the support they need in terms of access to credit or support in job training. The Small Business Network of the Americas has fostered over 4,000 small business development centers, and in Colombia alone has created nearly 6,000 jobs. Social development: Latin America remains the most unequal region of the world. There have been important reductions in poverty and growth of the middle class, but sustained improvements will require economic diversification and targeted efforts to reach vulnerable populations. To address the education deficit in the region, Jacobson highlighted the 100,000 Strong in the Americas program which connects institutions to institutions and seeks to provide students with actionable and employable skills. Energy and climate change: The high cost of energy prevents some countries from realizing their full potential and feeds migration, poverty, and violence. Sharing in the enormous energy wealth of other nations must be done responsibly and sustainably, noted Jacobson. The Energy and Climate Partnership of the Americas and Connecting the Americas 2022 aim to “promote renewable energy efficiency, cleaner fossil fuels, resilient infrastructure, and interconnection.” U.S. rationale behind targeted sanctions on Venezuela When asked about flashpoints or problems areas for the United States in the upcoming summit, Jacobson pointed to the sanctions on seven Venezuelan officials and the concern they have generated. However, she was careful to clarify that the executive order used standard language and was in no way a prelude to invasion or a forced regime change. Moreover, she noted that the legislation had been pending in Congress for two years, during which a dialogue between the opposition and government facilitated by the Union of South American Nations (UNASUR) was attempted but stalled. Jacobson explained that it is important to remember that these sanctions are very targeted and do not intend to harm the Venezuelan people or even the Venezuelan government as a whole. Engagement with Cuba and Brazil In Jacobson’s view, there are no large systemic issues that stand to block progress at the Summit. She explained that the Obama administration’s greater flexibility on counter-narcotics policies, reestablishment of diplomatic ties with Cuba, and focus on the Trans-Pacific Partnership have removed many historic obstacles. There remains work to be done, however. Jacobson stated that while interaction at the Summit between President Obama and Raúl Castro will serve to further the relationship and continue momentum for the normalization process, the engagement with Cuba will not deter the United States from speaking out on human rights violations. The administration’s view is that the human rights situation in Cuba is inadequate. Jacobson reiterated the need to respect international norms of human rights and that the United States will continue to support those who peacefully fight for that space to be open. Finally, she recognized the importance of U.S. engagement with Brazil. According to Jacobson, the United States sees Brazil as a leader on social inclusion, and even on economic competitiveness as it openly debates how to restart economic growth. Though the United States and Brazil do not see eye-to-eye on issues of climate change, she recognized that working with Brazil will be crucial in this area as well. A desire for cooperation With a desire to focus on pragmatic approaches rather than ideology, Jacobson expressed an openness to cooperation: “We’re willing to engage with every country in the hemisphere, every country in the hemisphere, any country that wants to partner with us. Because they’re in all of our interests. rio Nine Priority Commitments to be made at the United Nations July 2015 Financing for Development Conference in Addis Ababa, Ethiopia By webfeeds.brookings.edu Published On :: Tue, 03 Feb 2015 10:31:00 -0500 The United Nations will convene a major international conference on Financing for Development (FfD) in Addis Ababa, Ethiopia from July 13 to 16, 2015, to discuss financing for the post-2015 agenda on sustainable development. This conference, the third of its kind, will hope to replicate the success of the Monterrey conference in 2002 that has been credited with providing the glue to bind countries to the pursuit of the Millennium Development Goals (MDGs). The analogy is pertinent but should not be taken too far. The most visible part of the Monterrey Consensus was the commitment by rich countries to “make concrete efforts towards the target of 0.7 percent of gross national product” as official development assistance (ODA). This was anchored in a clear premise that “each country has primary responsibility for its own economic and social development,” which includes support for market-oriented policies that encourage the private sector. While not all of the Monterrey targets have been met, there has been a considerable increase in resources flowing to developing countries, as a central plank of efforts to achieve the MDGs. Today, aid issues remain pivotal for a significant number of countries, but they are less relevant for an even larger number of countries. The core principles of Monterrey need to be reaffirmed again in 2015, but if the world is to follow-through on a universal sustainable development agenda, it must address the multi-layered financing priorities spanning all countries. A simple “30-30-130” mnemonic helps to illustrate the point. There are 193 U.N. member states. Of these, only around 30 are still low-income countries (33 at the latest count). These are the economies that are, and will continue to be, the most heavily dependent on aid as the world looks to how it should implement the sustainable development goals (SDGs). Conversely, there are only around 30 “donor” countries (including 28 members of the OECD Development Assistance Committee, or DAC) that have made international commitments to provide more aid. For the remaining 130 or so emerging middle-income economies that have achieved higher levels of average prosperity, aid discussions risk forming a sideshow to the real issues that constrain their pursuit of sustainable development. The bottom line is that for most countries, the Financing for Development conference should unlock finance from many different sources, including but not exclusively aid, to implement the SDGs. Addis will take place in the context of sluggish global growth, an upsurge in conflict, considerable strains in multilateral 2 political cooperation, and challenging ODA prospects in many countries. There are other differences between Addis and Monterrey. Monterrey took place after agreement had been reached on the MDGs, while Addis will precede formal agreement on the SDGs by a few months. Monterrey was focused on a government-to-government agreement, while Addis should be relevant to a far larger number of stakeholders—including businesses, academics, civil society, scientists, and local authorities. Monterrey was held against a backdrop of general optimism about the global economy and widespread desire for intensified international collaboration following the terrorist events of September 11, 2001. Meanwhile, Addis will take place in the context of sluggish global growth, an upsurge in conflict, considerable strains in multilateral political cooperation, and challenging ODA prospects in many countries. In addition, regulators are working to reduce risk-taking by large financial institutions, increasing the costs of providing long-term capital to developing countries. Against this backdrop, an Intergovernmental Committee of Experts on Sustainable Development Finance (ICESDF) crafted a report for the United Nations on financing options for sustainable development. The report provides an excellent overview of issues and the current state of global financing, and presents over 100 recommendations. But it falls short on prescribing the most important priorities and action steps on which leaders should focus at Addis. This paper seeks to identify such a priority list of actions, with emphasis on the near-term deliverables that could instigate critical changes in trajectories towards 2030. At the same time, the paper does not aim to describe the full range of outcomes that need to be in place by roughly 2025 in order to achieve the SDGs by their likely deadline of 2030. Addis will be a critical forum to provide political momentum to a few of the many useful efforts already underway on improving global development finance. Time is short, so there is limited ability to introduce new topics or ideas or to build consensus where none already exists. We identify three criteria for identifying top priorities for agreement in Addis: Priorities should draw from, and build on, on-going work—including the ICESDF report and the outputs of several other international workstreams on finance that are underway. Agreements should have significant consequences for successful implementation of the SDGs at the country, regional or global level. Recommendations should be clearly actionable, with next steps in implementation that are easy to understand and easy to confirm when completed. It is not necessary (or desirable) that every important topic be resolved in Addis. In practical terms, negotiators face two groups of issues. First are those on which solutions can be negotiated in time for the July conference. Second are those for which the problems are too complex to be solved by July, but which are still crucial to be resolved over the coming year or two if the SDGs are to be achieved. For this second group of issues, the intergovernmental agreement can set specific timetables for resolving each problem at hand. There is some precedent for this, including in the 2005 U.N. World Summit, which included timetables for some commitments. What is most critical is that the moment be used to anchor and advance processes that will shift toward creating a global financing system for achieving sustainable development across all countries. Committing to timetables for action and building on reforms already undertaken could be important ways of enhancing the credibility of new agreements. In this paper, we lay out nine areas where we believe important progress can be made. In each area, we start from identifying a gap or issue that could present an obstacle to the successful implementation of the SDGs if left unattended. In some cases the gaps will affect all countries, in other cases only a subset of countries. But we believe that the package of actions, taken as a whole, reflects a balance of opportunities, responsibilities and benefits for all countries. We also believe that by making the discussion issue-focused, the needs for financing can be balanced with policy actions that will be required to make sure financing is effectively and efficiently deployed. In addition to the nine areas listed below, there are other commitments already made which have not yet been met. We urge renewed efforts to meet these commitments, but also recognize that political and financial realities must be managed to make progress. Such commitments include meeting the Monterrey Consensus target to provide 0.7 percent of GNI in official development assistance (ODA), the May 2005 agreement of all EC-15 countries to reach that target by 2015, and bringing the Doha Development Round of trade talks to a successful conclusion. These remain important and relevant, but in this paper we choose to focus on new areas and fresh ideas so as to avoid treading over well-worn territory again. rio Periodic payment of the Earned Income Tax Credit revisited By webfeeds.brookings.edu Published On :: Thu, 17 Dec 2015 00:00:00 -0500 Each year, one in five households filing a federal income tax return claims the Earned Income Tax Credit (EITC). Targeted primarily to lower-income workers with children, it is one of many credits and deductions filers take each year on their federal income tax forms. However, unlike typical credits and deductions, the EITC is a refundable credit, meaning that after offsetting what is owed to the government filers receive the remainder of the benefit as a refund. By supplementing earnings for low- and moderate-income households, the EITC helps bridge the gap between what the labor market provides and what it takes to support a family. It encourages and rewards work and has become one of the nation’s largest and most effective anti-poverty programs. In contrast to other work support and poverty alleviation programs, it achieves this with very little bureaucracy beyond what otherwise exists to administer the tax code. Although the EITC began in 1975 as a small credit (no more than $400), a number of targeted expansions in subsequent years mean that today the EITC’s assistance can be considerable. In 2015, a single parent with three children working full-time all year at the federal minimum wage ($7.25 an hour) is eligible for a credit of $6,242, a boost of more than 40 percent above her earnings of $15,080 (though combined it still leaves her 12 percent below the federal poverty level). However, the only way to obtain these substantial benefits is to claim the EITC on the annual federal income tax return. While lump-sum payments have perceived benefits (such as being able to pay off debts, make larger purchases, or force savings), the EITC’s single annual disbursement can present a challenge for the working parent trying to make ends meet throughout the year. It can also be problematic for households wanting to stretch out their refund as an emergency savings reserve. My 2008 paper, “Periodic Payment of the Earned Income Tax Credit,” proposed an option that would allow a family to receive a portion of the EITC outside of tax time, striking a balance between lump-sum delivery and the need for resources throughout the year. Specifically, half of the credit could be claimed in four payments spread out during the year, while the remaining credit would continue to be paid as part of the tax refund. Since then, several significant developments have occurred. A little-used option for receiving some of the EITC in each paycheck ended in 2010. In 2014, the federal government initiated a new tax credit advance payment process to subsidize health insurance premiums through monthly disbursement of the Affordable Care Act’s Premium Tax Credit. Other countries providing assistance similar to the EITC have continued to innovate and offer access to benefits during the year. Finally, members of Congress and think tanks have proposed alternatives to a single lump-sum disbursement of the EITC, and others have begun to explore and experiment with alternatives, most notably in Chicago, where a 2014 pilot program made quarterly payments to 343 households. In light of these developments, this paper reviews the author’s original EITC periodic payment proposal, examines emerging alternatives, and addresses the following key questions: What is the demand for periodic payment alternatives? What benefits will accrue from the availability of periodic payment? What risks are associated with periodic payment and how can they be managed? What is the administrative feasibility of periodic payment? The emerging answers point a way forward for identifying different distribution options that would enhance the EITC’s value to low- and moderate-income working families. To date, however, progress in the development of precision medicines has been limited due to a lack of reliable biomarkers for many diseases. Biomarkers include any defined characteristic—ranging from blood pressure to gene mutations—that can be used to measure normal biological processes, disease processes, or responses to an exposure or intervention. They can be extremely powerful tools for guiding decision-making in both drug development and clinical practice, but developing enough scientific evidence to support their use requires substantial time and resources, and there are many scientific, regulatory, and logistical challenges that impede progress in this area. On October 27th, 2015, the Center for Health Policy at The Brookings Institution convened an expert workshop that included leaders from government, industry, academia, and patient advocacy groups to identify and discuss strategies for addressing these challenges. Discussion focused on several key areas: the development of a universal language for biomarker development, strategies for increasing clarity on the various pathways for biomarker development and regulatory acceptance, and approaches to improving collaboration and alignment among the various groups involved in biomarker development, including strategies for increasing data standardization and sharing. The workshop generated numerous policy recommendations for a more cohesive national plan of action to advance precision medicine.
