Incidental Findings Suggestive of COVID-19 in Asymptomatic Patients Undergoing Nuclear Medicine Procedures in a High-Prevalence Region

Infection with the novel severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) may remain asymptomatic, leading to under-recognition of the related disease, coronavirus disease, 2019 (COVID-19), and to incidental findings in nuclear imaging procedures performed for standard clinical indications. Here, we report about our local experience in a region with high COVID-19 prevalence and dynamically increasing infection rates. Methods: Within the 8-d period of March 16–24, 2020, hybrid imaging studies of asymptomatic patients who underwent 18F-FDG PET/CT or 131I SPECT/CT for standard oncologic indications at our institution in Brescia, Italy, were analyzed for findings suggestive of COVID-19. The presence, radiologic features, and metabolic activity of interstitial pneumonia were identified, correlated with the subsequent short-term clinical course, and described in a case series. Results: Six of 65 patients (9%) who underwent PET/CT for various malignancies showed unexpected signs of interstitial pneumonia on CT and elevated regional 18F-FDG avidity. Additionally, 1 of 12 patients who received radioiodine for differentiated thyroid carcinoma also showed interstitial pneumonia on SPECT/CT. Five of 7 patients had subsequent proof of COVID-19 by reverse-transcriptase polymerase chain reaction. The remaining 2 patients were not tested immediately but underwent quarantine and careful monitoring. Conclusion: Incidental findings suggestive of COVID-19 may not be infrequent in hybrid imaging of asymptomatic patients in regions with an expansive spread of SARS-CoV-2. Nuclear medicine services should prepare accordingly.

Risk of Ipsilateral Reamputation Following an Incident Toe Amputation Among U.S. Military Veterans With Diabetes, 2005-2016

OBJECTIVE

To assess whether the risk of subsequent lower-limb amputations and death following an initial toe amputation among individuals with diabetes has changed over time and varies by demographic characteristics and geographic region.

RESEARCH DESIGN AND METHODS

Using Veterans Health Administration (VHA) electronic medical records from 1 October 2004 to 30 September 2016, we determined risk of subsequent ipsilateral minor and major amputation within 1 year after an initial toe/ray amputation among veterans with diabetes. To assess changes in the annual rate of subsequent amputation over time, we estimated age-adjusted incidence of minor and major subsequent ipsilateral amputation for each year, separately for African Americans (AAs) and whites. Geographic variation was assessed across VHA markets (n = 89) using log-linear Poisson regression models adjusting for age and ethnoracial category.

RESULTS

Among 17,786 individuals who had an initial toe amputation, 34% had another amputation on the same limb within 1 year, including 10% who had a major ipsilateral amputation. Median time to subsequent ipsilateral amputation (minor or major) was 36 days. One-year risk of subsequent major amputation decreased over time, but risk of subsequent minor amputation did not. Risk of subsequent major ipsilateral amputation was higher in AAs than whites. After adjusting for age and ethnoracial category, 1-year risk of major subsequent amputation varied fivefold across VHA markets.

CONCLUSIONS

Nearly one-third of individuals require reamputation following an initial toe amputation, although risks of subsequent major ipsilateral amputation have decreased over time. Nevertheless, risks remain particularly high for AAs and vary substantially geographically.

Circulating Biomarker Score for Visceral Fat and Risks of Incident Colorectal and Postmenopausal Breast Cancer: The Multiethnic Cohort Adiposity Phenotype Study

Background:

Visceral adipose tissue (VAT) may play a greater role than subcutaneous fat in increasing cancer risk but is poorly estimated in epidemiologic studies.

Methods:

We developed a VAT prediction score by regression equations averaged across 100 least absolute shrinkage and selection operator models in a cross-sectional study of 1,801 older adults in the Multiethnic Cohort (MEC). The score was then used as proxy for VAT in case–control studies of postmenopausal breast (950 case–control pairs) and colorectal (831 case–control pairs) cancer in an independent sample in MEC. Abdominal MRI–derived VAT; circulating biomarkers of metabolic, hormonal, and inflammation dysfunctions; and ORs for incident cancer adjusted for BMI and other risk factors were assessed.

Results:

The final score, composed of nine biomarkers, BMI, and height, explained 11% and 15% more of the variance in VAT than BMI alone in men and women, respectively. The area under the receiver operator curve for VAT >150 cm2 was 0.90 in men and 0.86 in women. The VAT score was associated with risk of breast cancer [OR (95% confidence interval [CI]) by increasing tertiles: 1.00, 1.09 (0.86–1.39), 1.48 (1.16–1.89); Ptrend = 0.002] but not with colorectal cancer (P = 0.84), although an association [1.00, 0.98 (0.68–1.39), 1.24 (0.88–1.76); Ptrend = 0.08] was suggested for this cancer after excluding cases that occurred within 7 years of blood draw (Pheterogeneity = 0.06).

Conclusions:

The VAT score predicted risks of postmenopausal breast cancer and can be used for risk assessment in diverse populations.

Impact:

These findings provide specific evidence for a role of VAT in breast cancer.

Ferguson Incident and America’s Image in Africa: Social Media Weighs in on Race and Human Rights


The full story of the killing of Michael Brown, a young, black, unarmed man shot by a policeman in Ferguson, Missouri, is still unfolding—and the truth will not be known for some time. It is only after full investigations are completed that an objective evaluation of the incident can be made. Nonetheless, there is no doubt that the killing of the young man was unfortunate and has generated a serious debate about race relations in America, and on the relations between police and the communities that they are supposed to protect. The riots and massive looting portrayed not only the extent of criminality in America’s inner cities, but also the economic marginalization of the minority communities.

Coming not long after the successful U.S.-Africa Leaders Summit held in Washington, the Ferguson incident and the follow-up demonstrations have been rather unfortunate in as far as how Africans view America—in a way questioning America’s standing as a protector of human rights. The hostility towards the United States in regard to its treatment of African-Americans has dominated social media with claims that the incident shows that America should not claim leadership when it comes to human rights. Such criticisms by many other countries, including Russia and China, are widespread.

I was particularly surprised by the comments in the Kenyan media coverage of this topic. Here are some statements on the topic by readers of the most popular paper there—the Daily Nation:

The US is a community fueled by hate. They claim not to be racist yet most of them are racist to the core including the black Americans. Yet they want to dictate and lecture us about human rights."

Still waiting for GOK [Government of Kenya] to issue travel advisory to the U.S."

(This is an apparent reference to the fact that the United States government issues travel advisories to countries like Kenya when such incidents occur and there are riots.)

Extra judicial killing. Let UN order an independent investigation & file handed to ICC (international Criminal Court) for prosecution of the culprits. US justice system is biased against its own black community."

(The U.S. and human rights organizations have been critical of many countries for extra-judicial killings and have called for the prosecution of government officials in Africa at the International Criminal Court for such actions.)

U.S. preaches democracy and good governance all over the world but lo and behold, Ferguson has exposed the preacher who cannot take care of business in his backyard."

Has the Kenyan ambassador issued a statement yet? The US must have a coalition government so as to end the violence. It will no longer be business as usual. We will have only necessary contact. Choices have consequences."

(This statement is in reference to the U.S. government’s actions following the 2007-2008 post-election violence in Kenya.)

In the USA, they give absolute rights to women, children and pets, the men are left on their own, owe [sic] un to you if you happen to be a young black man. You are as good as dead."

Scanning media in other African countries, the same kind of reactions are evident. While some opinions differ, the general sentiment expressed in social media is that the United States remains a divided country and thus lacks moral authority to “lecture” Africans on human rights and tribalism.

To an extent, these sentiments expressed by Africans are misguided and are largely a gross exaggeration of the character of American society. The views expressed in the media portray an American society that is totally divided across racial lines, which Africans often equate to tribalism on their own continent. They see the economic desperation of many African-Americans as a reflection of a society that has continued to deny a large section of its people opportunities for advancement. All these views, right or wrong, weaken America’s standing among Africans and undermine the country’s ability to influence policy on human rights and governance in the continent. Such incidents give solace to dictators that undertake gross violations of human rights through extrajudicial killings. Many Africans consider the U.S. judicial system to be discriminatory against black men. They also cite biases in many previous cases of police killings of black men. The Zimmerman case in Florida is commonly used in the African media as an example of such incidents where they feel justice did not prevail.

But American global leadership in the advancement of human rights and ensuring equal protection under the law—and also in opening up opportunities for all groups—remains critically important. Through fair and transparent adjudication of the Ferguson case, the U.S. will be in a position to demonstrate to the Africans and others who have been critical of the state of affairs in this country that the U.S. remains a country governed by the rule of law. Still, the issue of poverty among some communities gives the U.S. a bad name as a country where a large segment of the population is economically marginalized. As the U.S. encourages Africans to build united and inclusive societies, it should be cognizant of the fact that its voice will carry more weight and be respected if Africans see the same happening in United States.

Bombay HC sends notice to Maharashtra govt on plea seeking CBI probe, speedy trial in Palghar incident

Bombay High Court on Thursday issued notice's to Maharashtra government and Central Bureau of Investigation (CBI) on a petition seeking a CBI probe, a fast track court trial and compensation in the Palghar mob lynching incident. A single-judge bench of Justice Ujjal Bhuyan issued the notices and sought a reply from the respondents on the petition within a period of two weeks.

The petition, filed by Supreme Court lawyer Alakh Alok Srivastava, sought directions to transfer the investigation in the incident from CID-Crime to the Central Bureau of Investigation or alternatively constitute a special investigation team (SIT) monitored by the court to prove the incident.

The petition also sought directions to conduct the trial in the case by a fast track court in an expeditious and time-bound manner, and a compensation of Rs 1 crore to the family of the driver who was killed in the incident. The plea said that there are serious allegations of commission and omission on the part of the local police personnel themselves in the brutal crime and added that it is in the interest of justice to get the investigation of the case done by any independent agency.

"Directions in the nature of Mandamus to the State of Maharashtra to initiate stringent disciplinary action as well as penal action under Indian Penal Code (IPC) by way of registration of FIR, against the concerned policemen, whose acts and omissions led to the aforesaid brutal lynching," the plea said. "The brutal lynching of the old aged spiritual leaders has sent a wrong message to the society at large and thus it is in the interest of justice to direct for speedy and expeditious trial of the instant case," it said.

Two sadhus and their driver, who were travelling from Kandivali in Mumbai to Gujarat, were beaten to death on April 16, allegedly by villagers in Gadchinchle of Palghar, as they suspected them of being thieves.

In the complaint, Markam said that in his TV programme, the editor has instigated a crime and tried to disrupt communal harmony.

"I condemn the attempts made by Arnab Goswami to communalise the unfortunate incident at Palghar & also the derogatory language used by him against @INCIndia President Sonia ji Gandhi. I have spoken to CM @OfficeofUT & HM @AnilDeshmukhNCP Strict action will be initiated against him," he said, in a tweet.

The Congress has also initiated an online campaign against Goswami on various social media platforms.

Vizag Gas Leak: Political leaders condemn 'disturbing' incident on Twitter

A gas leak in Andhra Pradesh’s Vizag district in the wee hours of Thursday has left eight people, including a child and over 120 people hospitalized. According to ANI, Styrene gas leakage had occurred at the LG Polymers industry in RR Venkatapuram village in Vizag early this morning.

Residents in the area complained of irritation in eyes, breathlessness, nausea and rashes on their bodies following the gas leak. Teams of the National Disaster Response Force (NDRF) have rushed to the spot and reports say that the gas leak has been contained.

Many politicians have taken to Twitter to condemn the incident and are urging their local party members to provide help to people who have been affected by the gas leak. Prime Minister Narendra Modi tweeted saying that he monitoring the situation in Vizag closely and is praying for the wellbeing of the people.

President Ram Nath Kovind offered his condolence to the victims of the gas leak and said that he is saddened by the incident.

Congress leader Rahul Gandhi expressed shock over the gas leak incident in Visakhapatnam and prayed for the speedy recovery of those hospitalised. He also urged Congress workers in the area to provide all necessary support to those affected by the gas leak.

Union Home Minister Amit Shah termed the incident as disturbing and  is praying for the well-being of the people of Visakhapatnam

Many other political leaders have taken to the micro-blogging site and expressed their shock over the incident.

MP Suresh Prabhu reacted to the incident, calling it discomforting. "I am contacting Administration and our local unit to address the problems of those who are affected and to provide medical help and other necessary relief," he tweeted.

Several police personnel, who came for the rescue operation, also suffered symptoms like breathlessness, irritation in eyes and fell unconscious. Sources said the vapour leak occurred in the early hours of Thursday when some workers of the plant were making preparations for the re-opening of the unit following easing of the ongoing lockdown restrictions.

The country woke to the news of migrant workers being mowed to death by a goods train in Aurangabad on the wee hours of Friday . The workers, who were walking to Bhusawal from Jalna, were returning to Madhya Pradesh. They had been walking along the rail tracks and slept on the rail tracks due to exhaustion.

President Kovind said that he was saddened ‘beyond words’ after learning about the incident and wished speedy recovery to the injured.

Prime Minister Modi tweeted expressing his anguish over the incident and said that all possible assistance is being provided.

Union Railway Minister Piyush Goyal expressed his condolences to the deceased and said that he has called for an inquiry in the incident.

Gloucestershire batsman George Hankins has been arrested for a drunk-driving incident in Surrey following a car crash.

Hankins, who has featured in 28 first-class games, was arrested after the collision in Portsmouth Road, Cobham, on April 19 and will now appear in Guildford Magistrates' Court in July, reports BBC Sport.

"Gloucestershire Cricket has been made aware of an incident - being investigated by Surrey Police - regarding one of its players, who has been based in Surrey during the COVID-19 lockdown period.

The club is in touch with the player concerned and has initiated a full internal investigation," the club said in a statement.

The 23-year-old has scored 961 first-class runs since making his debut in 2016 against Durham. He has also featured in 15 List A games and seven T20 games - scoring 535 and 17 runs respectively.

Gloucestershire is the same club where ace India Test batsman Cheteshwar Pujara was supposed to play the first six matches of this year's County Championship. The deal was, however, called off earlier this month due to the COVID-19 pandemic which is spreading rapidly in the United Kingdom and has claimed thousands of lives in the country.

Parthiv Patel recalls incident when Matthew Hayden wanted to punch him

India wicket-keeper Parthiv Patel recalled an incident when former Australia opener Matthew Hayden wanted to punch him in the face. Parthiv said he made fun of the Australian after he was dismissed during a match and that made Hayden lose his cool.

Parthiv was just 18-year-old when he travelled Down Under and got into the bad books of Hayden. Chasing a stiff target of 304 against India in Brisbane in 2004, the southpaw slammed a century but was dismissed for 109 by Irfan Pathan.

Hayden's wicket proved to costly for Australia as India ended up winning the match by 19 runs. After getting dismissed, Hayden was in no mood for fun and games and Pathiv got to know that in the most hostile manner.

"I was carrying drinks in Brisbane, it was the match in which Irfan Pathan got him out. He had already scored a hundred and it was a crucial stage when Irfan got him out. I was just passing him by and I did a 'hoo hoo' to him," Parthiv said during an interaction on 100 hours 100 stars.

"He got so angry at me. He was standing at the Brisbane dressing room which is like a tunnel. He was standing there and said 'if you do this again, I will punch your face'. I said 'sorry', I stood around and he went off," he added.

Parthiv also revealed the duo went on to become good friends after they opened the innings for Chennai Super Kings in the Indian Premier League.

"Yeah, Hayden wanted to beat me up in Brisbane. But we became good friends after that. We played a lot of cricket together at CSK," Parthiv said.

"We enjoyed each other's company. Opening the batting was fun with him. We've had good times off the field as well. So we patched up after that Brisbane incident.

"Even after the IPL finished, I went to Australia with an emerging side. Hayden called me home, prepared chicken biryani and dal for me," he added.
