A Qualitative Analysis of a Primary Care Medical-Legal Partnership: Impact, Barriers, and Facilitators

Background:

Certain health-related risk factors require legal interventions. Medical-legal partnerships (MLPs) are collaborations between clinics and lawyers that address these health-harming legal needs (HHLNs) and have been shown to improve health and reduce utilization.

Objective:

The objective of this study is to explore the impact, barriers, and facilitators of MLP implementation in primary care clinics.

Methods:

A qualitative design using a semistructured interview assessed the perceived impact, barriers, and facilitators of an MLP, among clinicians, clinic and MLP staff, and clinic patients. Open AI software (otter.ai) was used to transcribe interviews, and NVivo was used to code the data. Braun & Clarke’s framework was used to identify themes and subthemes.

Results:

Sixteen (n = 16) participants were included in this study. Most respondents were women (81%) and white (56%). Four respondents were clinic staff, and 4 were MLP staff while 8 were clinic patients. Several primary themes emerged including: Patients experienced legal issues that were pernicious, pervasive, and complex; through trusting relationships, the MLP was able to improve health and resolve legal issues, for some; mistrust, communication gaps, and inconsistent staffing limited the impact of the MLP; and, the MLP identified coordination and communication strategies to enhance trust and amplify its impact.

Conclusion:

HHLNs can have a significant, negative impact on the physical and mental health of patients. Respondents perceived that MLPs improved health and resolved these needs, for some. Despite perceived successes, integration between the clinical and legal organizations was elusive.

Establishment of the First Institution-Based Poison Information Center in Nepal Through a Multilateral International Partnership

ABSTRACTToxicological emergencies present a significant health challenge in Nepal. Despite the high burden, the country has inadequate formal toxicology training, medical toxicology expertise, and adequate poison control infrastructure. In recognition of this need, the Nepal Poison Information Center (PIC) was established as a collaborative effort involving local and international partners. Through a comprehensive partnership framework, the Nepal PIC provides 24 hours a day, 7 days a week expert guidance to health care workers, conducts educational webinars, and engages in research. Initial data from the pilot phase indicate successful consultation delivery. Challenges include bureaucratic hurdles and the need for sustainable funding. Despite these challenges, the Nepal PIC demonstrates early feasibility and potential for expansion into a comprehensive toxicology center, contributing to the advancement of clinical toxicology in Nepal. Long-term sustainability relies on governmental support and continued advocacy efforts.

Twinning Partnership Network: A Learning and Experience-Sharing Network Among Health Professionals in Rwanda to Improve Health Services

ABSTRACTWe describe the development, implementation, and evaluation of a novel twinning approach: the Twinning Partnership Network (TPN). Twinning is a well-known approach to peer learning that has been used in a variety of settings to build organizational capacity. Although twinning takes many forms, the heart of the approach is that institutions with shared characteristics collaborate via sharing information and experiences to achieve a specific goal. We adapted a twinning partnership strategy developed by the World Health Organization to create a network of like-minded health institutions. The key innovation of the TPN is the network, which ensures that an institution always has a high-performing peer with whom to partner on a specific topic area of interest. We identified 10 hospitals and 30 districts in Rwanda to participate in the TPN. These districts and hospitals participated in a kickoff workshop in which they identified capacity gaps, clarified goals, and selected twinning partners. After the workshop, districts and hospitals participated in exchange visits, coaching visits, and virtual and in-person learning events. We found that districts and hospitals that selected specific areas and worked on them throughout the duration of the TPN with their peers improved their performance significantly when compared with those that selected and worked on other areas. Accreditation scores improved by 5.6% more in hospitals selecting accreditation than those that did not. Districts that selected improving community-based health insurance coverage improved by 4.8% more than districts that did not select this topic area. We hypothesize that these results are due to senior management’s interest and motivation to improve in these specific areas, the motivation gained by learning from high-performing peers with similar resources, and context-specific knowledge sharing from peer hospitals and districts.

We primary care clinicians, scholars, and leaders ascribe value to Barbara Starfield’s core tenets of primary care—the 4 Cs: first contact, comprehensiveness, coordination, and continuity. In today’s era of rapid technological advancements and dwindling resources, what are the implications for face-to-face interactions of patient-clinician relationships? We propose adding a 5th C: "Contiguity." Contiguity—or physical proximity and presence—is a key dimension that not only enables the necessary technical aspects of a physical exam but also authenticates the most human aspects of a relationship and occurs specifically when we are physically vulnerable and responsible for the other before us. This, in turn, may best enable us to bridge difference and nurture trust with our patients. We measure what we value and, thus, naming Contiguity as a core tenet assures that we will not lose sight of this keystone in a patient’s relationship with their personal physician.

PS5 Ships 65.5 Million Units as of September 2024

Sony Interactive Entertainment in its financial results announced it has shipped 65.5 million PlayStation 5 consoles as of September 30, 2024.

With 65.5 million PlayStation 5 consoles shipped through the end of June that means 3.8 million units were shipped from July to September. This is down 1.1 million (-22.45%) from the same quarter in 2023 when 4.9 million units were shipped.

For reference, the PlayStation 4 had shipped 4.2 million units in the same quarter for a lifetime total of 67.7 million units shipped as of September 30, 2017. This puts the PS5 behind shipped PS4 units by 2.2 million units.

There were a total of 77.7 million games sold on the PlayStation 5 and PlayStation 4 for the quarter. This is up 10.1 million from 67.6 million during the same period a year earlier. Digital sales accounted for 70 percent of software sales.

There were 5.3 million first-party games sold across the PlayStation 5 and PlayStation 4. This is up 0.6 million from 4.7 million a year ago.

There were 116 million monthly active users on the PlayStation Network, an increase of nine million from 107 million a year ago. Sony did not disclose the number of PlayStation Plus subscribers. It was at 47.4 million at the end of March 2023.

Sony's Game & Network Services Segment for the quarter ending September 30, 2024, reported revenue increased 244.5 billion yen ($1.60 billion) year-over-year to 2,973.4 billion yen ($19.46 billion), while operating income increased 142.0 billion yen ($0.93 billion) to 389.3 billion yen ($2.55 billion).

Twitter chat: How the gun control debate mirrors larger issues of partisanship in America

What would it take to turn Texas, a Republican stronghold, into a blue state? According to data from SurveyMonkey, just remove all the gun owners from the Lone Star State and it would have gone to Hillary Clinton in 2016. You can do the same thing in liberal California. Remove all the non-gun owners and the state would have voted for Donald Trump.

That’s how divisive the issue of gun control is in American politics.

SurveyMonkey found that no other demographic — not race, religion or gender — so perfectly divided voters. In the 2016 election, 47 percent of Trump supporters said gun control was an issue important enough to influence their vote. That’s compared to just 27 percent of voters who supported Hillary Clinton.

But what does this divide mean? How is it impacting gun control policy, and how might this issue change in light of recent mass shootings like Las Vegas, Orlando and Newtown?
