Excitotoxicity and Overnutrition Additively Impair Metabolic Function and Identity of Pancreatic {beta}-cells

A sustained increase in intracellular Ca2+ concentration (referred to herein as excitotoxicity), brought on by chronic metabolic stress, may contribute to pancreatic β-cell failure. To determine the additive effects of excitotoxicity and overnutrition on β-cell function and gene expression, we analyzed the impact of a high fat diet (HFD) on Abcc8 knock-out mice. Excitotoxicity caused β-cells to be more susceptible to HFD-induced impairment of glucose homeostasis, and these effects were mitigated by verapamil, a Ca2+ channel blocker. Excitotoxicity, overnutrition and the combination of both stresses caused similar but distinct alterations in the β-cell transcriptome, including additive increases in genes associated with mitochondrial energy metabolism, fatty acid β-oxidation and mitochondrial biogenesis, and their key regulator Ppargc1a. Overnutrition worsened excitotoxicity-induced mitochondrial dysfunction, increasing metabolic inflexibility and mitochondrial damage. In addition, excitotoxicity and overnutrition, individually and together, impaired both β-cell function and identity by reducing expression of genes important for insulin secretion, cell polarity, cell junction, cilia, cytoskeleton, vesicular trafficking, and regulation of β-cell epigenetic and transcriptional program. Sex had an impact on all β-cell responses, with male animals exhibiting greater metabolic stress-induced impairments than females. Together, these findings indicate that a sustained increase in intracellular Ca2+, by altering mitochondrial function and impairing β-cell identity, augments overnutrition-induced β-cell failure.

S-Palmitoylation of the sodium channel Nav1.6 regulates its activity and neuronal excitability [Cell Biology]

S-Palmitoylation is a reversible post-translational lipid modification that dynamically regulates protein functions. Voltage-gated sodium channels are subjected to S-palmitoylation and exhibit altered functions in different S-palmitoylation states. Our aim was to investigate whether and how S-palmitoylation regulates Nav1.6 channel function and to identify S-palmitoylation sites that can potentially be pharmacologically targeted. Acyl-biotin exchange assay showed that Nav1.6 is modified by S-palmitoylation in the mouse brain and in a Nav1.6 stable HEK 293 cell line. Using whole-cell voltage clamp, we discovered that enhancing S-palmitoylation with palmitic acid increases Nav1.6 current, whereas blocking S-palmitoylation with 2-bromopalmitate reduces Nav1.6 current and shifts the steady-state inactivation in the hyperpolarizing direction. Three S-palmitoylation sites (Cys1169, Cys1170, and Cys1978) were identified. These sites differentially modulate distinct Nav1.6 properties. Interestingly, Cys1978 is exclusive to Nav1.6 among all Nav isoforms and is evolutionally conserved in Nav1.6 among most species. Cys1978 S-palmitoylation regulates current amplitude uniquely in Nav1.6. Furthermore, we showed that eliminating S-palmitoylation at specific sites alters Nav1.6-mediated excitability in dorsal root ganglion neurons. Therefore, our study reveals S-palmitoylation as a potential isoform-specific mechanism to modulate Nav activity and neuronal excitability in physiological and diseased conditions.

Excess BMI Accelerates Islet Autoimmunity in Older Children and Adolescents

OBJECTIVE

Sustained excess BMI increases the risk of type 1 diabetes (T1D) in autoantibody-positive relatives without diabetes of patients. We tested whether elevated BMI also accelerates the progression of islet autoimmunity before T1D diagnosis.

RESEARCH DESIGN AND METHODS

We studied 706 single autoantibody–positive pediatric TrialNet participants (ages 1.6–18.6 years at baseline). Cumulative excess BMI (ceBMI) was calculated for each participant based on longitudinally accumulated BMI ≥85th age- and sex-adjusted percentile. Recursive partitioning analysis and multivariable modeling defined the age cut point differentiating the risk for progression to multiple positive autoantibodies.

RESULTS

At baseline, 175 children (25%) had a BMI ≥85th percentile. ceBMI range was –9.2 to 15.6 kg/m2 (median –1.91), with ceBMI ≥0 kg/m2 corresponding to persistently elevated BMI ≥85th percentile. Younger age increased the progression to multiple autoantibodies, with age cutoff of 9 years defined by recursive partitioning analysis. Although ceBMI was not significantly associated with progression from single to multiple autoantibodies overall, there was an interaction with ceBMI ≥0 kg/m2, age, and HLA (P = 0.009). Among children ≥9 years old without HLA DR3-DQ2 and DR4-DQ8, ceBMI ≥0 kg/m2 increased the rate of progression from single to multiple positive autoantibodies (hazard ratio 7.32, P = 0.004) and conferred a risk similar to that in those with T1D-associated HLA haplotypes. In participants <9 years old, the effect of ceBMI on progression to multiple autoantibodies was not significant regardless of HLA type.

CONCLUSIONS

These data support that elevated BMI may exacerbate islet autoimmunity prior to clinical T1D, particularly in children with lower risk based on age and HLA. Interventions to maintain normal BMI may prevent or delay the progression of islet autoimmunity.

The Contemporary Prevalence of Diabetic Neuropathy in Type 1 Diabetes: Findings From the T1D Exchange

OBJECTIVE

To evaluate the contemporary prevalence of diabetic peripheral neuropathy (DPN) in participants with type 1 diabetes in the T1D Exchange Clinic Registry throughout the U.S.

RESEARCH DESIGN AND METHODS

DPN was assessed with the Michigan Neuropathy Screening Instrument Questionnaire (MNSIQ) in adults with ≥5 years of type 1 diabetes duration. A score of ≥4 defined DPN. Associations of demographic, clinical, and laboratory factors with DPN were assessed.

RESULTS

Among 5,936 T1D Exchange participants (mean ± SD age 39 ± 18 years, median type 1 diabetes duration 18 years [interquartile range 11, 31], 55% female, 88% non-Hispanic white, mean glycated hemoglobin [HbA1c] 8.1 ± 1.6% [65.3 ± 17.5 mmol/mol]), DPN prevalence was 11%. Compared with those without DPN, DPN participants were older, had higher HbA1c, had longer duration of diabetes, were more likely to be female, and were less likely to have a college education and private insurance (all P < 0.001). DPN participants also were more likely to have cardiovascular disease (CVD) (P < 0.001), worse CVD risk factors of smoking (P = 0.008), hypertriglyceridemia (P = 0.002), higher BMI (P = 0.009), retinopathy (P = 0.004), reduced estimated glomerular filtration rate (P = 0.02), and Charcot neuroarthropathy (P = 0.002). There were no differences in insulin pump or continuous glucose monitor use, although DPN participants were more likely to have had severe hypoglycemia (P = 0.04) and/or diabetic ketoacidosis (P < 0.001) in the past 3 months.

CONCLUSIONS

The prevalence of DPN in this national cohort with type 1 diabetes is lower than in prior published reports but is reflective of current clinical care practices. These data also highlight that nonglycemic risk factors, such as CVD risk factors, severe hypoglycemia, diabetic ketoacidosis, and lower socioeconomic status, may also play a role in DPN development.

Renal and Cardiovascular Outcomes After Weight Loss From Gastric Bypass Surgery in Type 2 Diabetes: Cardiorenal Risk Reductions Exceed Atherosclerotic Benefits

OBJECTIVE

We examined detailed renal and cardiovascular (CV) outcomes after gastric bypass (GBP) surgery in people with obesity and type 2 diabetes mellitus (T2DM), across several renal function categories, in a nationwide cohort study.

RESEARCH DESIGN AND METHODS

We linked data from the National Diabetes Register and the Scandinavian Obesity Surgery Register with four national databases holding information on socioeconomic variables, medications, hospitalizations, and causes of death and matched 5,321 individuals with T2DM who had undergone GBP with 5,321 who had not (age 18–65 years, mean BMI >40 kg/m2, mean follow-up >4.5 years). The risks of postoperative outcomes were assessed with Cox regression models.

RESULTS

During the first years postsurgery, there were small reductions in creatinine and albuminuria and stable estimated glomerular filtration rate (eGFR) in the GBP group. The incidence rates of most outcomes relating to renal function, CV disease, and mortality were lower after GBP, being particularly marked for heart failure (hazard ratio [HR] 0.33 [95% CI 0.24, 0.46]) and CV mortality (HR 0.36 [(95% CI 0.22, 0.58]). The risk of a composite of severe renal disease or halved eGFR was 0.56 (95% CI 0.44, 0.71), whereas nonfatal CV risk was lowered less (HR 0.82 [95% CI 0.70, 0.97]) after GBP. Risks for key outcomes were generally lower after GBP in all eGFR strata, including in individuals with eGFR <30 mL/min/1.73 m2.

CONCLUSIONS

Our data suggest robust benefits for renal outcomes, heart failure, and CV mortality after GBP in individuals with obesity and T2DM. These results suggest that marked weight loss yields important benefits, particularly on the cardiorenal axis (including slowing progression to end-stage renal disease), whatever the baseline renal function status.

While derivations of the characterization of the $d$-variate exchangeable Marshall–Olkin copula via $d$-monotone sequences relying on basic knowledge in probability theory exist in the literature, they contain a myriad of unnecessary relatively complicated computations. We revisit this issue and provide proofs where all undesired artefacts are removed, thereby exposing the simplicity of the characterization. In particular, we give an insightful analytical derivation of the monotonicity conditions based on the monotonicity properties of the survival probabilities.

Excess registered deaths in England and Wales during the COVID-19 pandemic, March 2020 and April 2020. (arXiv:2004.11355v4 [stat.AP] UPDATED)

Official counts of COVID-19 deaths have been criticized for potentially including people who did not die of COVID-19 but merely died with COVID-19. I address that critique by fitting a generalized additive model to weekly counts of all registered deaths in England and Wales during the 2010s. The model produces baseline rates of death registrations expected in the absence of the COVID-19 pandemic, and comparing those baselines to recent counts of registered deaths exposes the emergence of excess deaths late in March 2020. Among adults aged 45+, about 38,700 excess deaths were registered in the 5 weeks comprising 21 March through 24 April (612 $pm$ 416 from 21$-$27 March, 5675 $pm$ 439 from 28 March through 3 April, then 9183 $pm$ 468, 12,712 $pm$ 589, and 10,511 $pm$ 567 in April's next 3 weeks). Both the Office for National Statistics's respective count of 26,891 death certificates which mention COVID-19, and the Department of Health and Social Care's hospital-focused count of 21,222 deaths, are appreciably less, implying that their counting methods have underestimated rather than overestimated the pandemic's true death toll. If underreporting rates have held steady, about 45,900 direct and indirect COVID-19 deaths might have been registered by April's end but not yet publicly reported in full.

SmartExchange: Trading Higher-cost Memory Storage/Access for Lower-cost Computation. (arXiv:2005.03403v1 [cs.LG])

We present SmartExchange, an algorithm-hardware co-design framework to trade higher-cost memory storage/access for lower-cost computation, for energy-efficient inference of deep neural networks (DNNs). We develop a novel algorithm to enforce a specially favorable DNN weight structure, where each layerwise weight matrix can be stored as the product of a small basis matrix and a large sparse coefficient matrix whose non-zero elements are all power-of-2. To our best knowledge, this algorithm is the first formulation that integrates three mainstream model compression ideas: sparsification or pruning, decomposition, and quantization, into one unified framework. The resulting sparse and readily-quantized DNN thus enjoys greatly reduced energy consumption in data movement as well as weight storage. On top of that, we further design a dedicated accelerator to fully utilize the SmartExchange-enforced weights to improve both energy efficiency and latency performance. Extensive experiments show that 1) on the algorithm level, SmartExchange outperforms state-of-the-art compression techniques, including merely sparsification or pruning, decomposition, and quantization, in various ablation studies based on nine DNN models and four datasets; and 2) on the hardware level, the proposed SmartExchange based accelerator can improve the energy efficiency by up to 6.7$ imes$ and the speedup by up to 19.2$ imes$ over four state-of-the-art DNN accelerators, when benchmarked on seven DNN models (including four standard DNNs, two compact DNN models, and one segmentation model) and three datasets.

Abstract:
This paper considers the problem of testing if a sequence of means $(mu_{t})_{t=1,ldots ,n}$ of a nonstationary time series $(X_{t})_{t=1,ldots ,n}$ is stable in the sense that the difference of the means $mu_{1}$ and $mu_{t}$ between the initial time $t=1$ and any other time is smaller than a given threshold, that is $|mu_{1}-mu_{t}|leq c$ for all $t=1,ldots ,n$. A test for hypotheses of this type is developed using a bias corrected monotone rearranged local linear estimator and asymptotic normality of the corresponding test statistic is established. As the asymptotic variance depends on the location of the roots of the equation $|mu_{1}-mu_{t}|=c$ a new bootstrap procedure is proposed to obtain critical values and its consistency is established. As a consequence we are able to quantitatively describe relevant deviations of a nonstationary sequence from its initial value. The results are illustrated by means of a simulation study and by analyzing data examples.

Abstract:
Sparse exchangeable graphs on $mathbb{R}_{+}$, and the associated graphex framework for sparse graphs, generalize exchangeable graphs on $mathbb{N}$, and the associated graphon framework for dense graphs. We develop the graphex framework as a tool for statistical network analysis by identifying the sampling scheme that is naturally associated with the models of the framework, formalizing two natural notions of consistent estimation of the parameter (the graphex) underlying these models, and identifying general consistent estimators in each case. The sampling scheme is a modification of independent vertex sampling that throws away vertices that are isolated in the sampled subgraph. The estimators are variants of the empirical graphon estimator, which is known to be a consistent estimator for the distribution of dense exchangeable graphs; both can be understood as graph analogues to the empirical distribution in the i.i.d. sequence setting. Our results may be viewed as a generalization of consistent estimation via the empirical graphon from the dense graph regime to also include sparse graphs.

Integration of Swimming-Related Synaptic Excitation and Inhibition by olig2+ Eurydendroid Neurons in Larval Zebrafish Cerebellum

The cerebellum influences motor control through Purkinje target neurons, which transmit cerebellar output. Such output is required, for instance, for larval zebrafish to learn conditioned fictive swimming. The output cells, called eurydendroid neurons (ENs) in teleost fish, are inhibited by Purkinje cells and excited by parallel fibers. Here, we investigated the electrophysiological properties of glutamatergic ENs labeled by the transcription factor olig2. Action potential firing and synaptic responses were recorded in current clamp and voltage clamp from olig2+ neurons in immobilized larval zebrafish (before sexual differentiation) and were correlated with motor behavior by simultaneous recording of fictive swimming. In the absence of swimming, olig2+ ENs had basal firing rates near 8 spikes/s, and EPSCs and IPSCs were evident. Comparing Purkinje firing rates and eurydendroid IPSC rates indicated that 1-3 Purkinje cells converge onto each EN. Optogenetically suppressing Purkinje simple spikes, while preserving complex spikes, suggested that eurydendroid IPSC size depended on presynaptic spike duration rather than amplitude. During swimming, EPSC and IPSC rates increased. Total excitatory and inhibitory currents during sensory-evoked swimming were both more than double those during spontaneous swimming. During both spontaneous and sensory-evoked swimming, the total inhibitory current was more than threefold larger than the excitatory current. Firing rates of ENs nevertheless increased, suggesting that the relative timing of IPSCs and EPSCs may permit excitation to drive additional eurydendroid spikes. The data indicate that olig2+ cells are ENs whose activity is modulated with locomotion, suiting them to participate in sensorimotor integration associated with cerebellum-dependent learning.

SIGNIFICANCE STATEMENT The cerebellum contributes to movements through signals generated by cerebellar output neurons, called eurydendroid neurons (ENs) in fish (cerebellar nuclei in mammals). ENs receive sensory and motor signals from excitatory parallel fibers and inhibitory Purkinje cells. Here, we report electrophysiological recordings from ENs of larval zebrafish that directly illustrate how synaptic inhibition and excitation are integrated by cerebellar output neurons in association with motor behavior. The results demonstrate that inhibitory and excitatory drive both increase during fictive swimming, but inhibition greatly exceeds excitation. Firing rates nevertheless increase, providing evidence that synaptic integration promotes cerebellar output during locomotion. The data offer a basis for comparing aspects of cerebellar coding that are conserved and that diverge across vertebrates.

Rapid Release of Ca2+ from Endoplasmic Reticulum Mediated by Na+/Ca2+ Exchange

Phototransduction in Drosophila is mediated by phospholipase C (PLC) and Ca2+-permeable TRP channels, but the function of endoplasmic reticulum (ER) Ca2+ stores in this important model for Ca2+ signaling remains obscure. We therefore expressed a low affinity Ca2+ indicator (ER-GCaMP6-150) in the ER, and measured its fluorescence both in dissociated ommatidia and in vivo from intact flies of both sexes. Blue excitation light induced a rapid (tau ~0.8 s), PLC-dependent decrease in fluorescence, representing depletion of ER Ca2+ stores, followed by a slower decay, typically reaching ~50% of initial dark-adapted levels, with significant depletion occurring under natural levels of illumination. The ER stores refilled in the dark within 100–200 s. Both rapid and slow store depletion were largely unaffected in InsP3 receptor mutants, but were much reduced in trp mutants. Strikingly, rapid (but not slow) depletion of ER stores was blocked by removing external Na+ and in mutants of the Na+/Ca2+ exchanger, CalX, which we immuno-localized to ER membranes in addition to its established localization in the plasma membrane. Conversely, overexpression of calx greatly enhanced rapid depletion. These results indicate that rapid store depletion is mediated by Na+/Ca2+ exchange across the ER membrane induced by Na+ influx via the light-sensitive channels. Although too slow to be involved in channel activation, this Na+/Ca2+ exchange-dependent release explains the decades-old observation of a light-induced rise in cytosolic Ca2+ in photoreceptors exposed to Ca2+-free solutions.

SIGNIFICANCE STATEMENT Phototransduction in Drosophila is mediated by phospholipase C, which activates TRP cation channels by an unknown mechanism. Despite much speculation, it is unknown whether endoplasmic reticulum (ER) Ca2+ stores play any role. We therefore engineered flies expressing a genetically encoded Ca2+ indicator in the photoreceptor ER. Although NCX Na+/Ca2+ exchangers are classically believed to operate only at the plasma membrane, we demonstrate a rapid light-induced depletion of ER Ca2+ stores mediated by Na+/Ca2+ exchange across the ER membrane. Selective Disruption of Inhibitory Synapses Leading to Neuronal Hyperexcitability at an Early Stage of Tau Pathogenesis in a Mouse Model

Synaptic dysfunction provoking dysregulated cortical neural circuits is currently hypothesized as a key pathophysiological process underlying clinical manifestations in Alzheimer's disease and related neurodegenerative tauopathies. Here, we conducted PET along with postmortem assays to investigate time course changes of excitatory and inhibitory synaptic constituents in an rTg4510 mouse model of tauopathy, which develops tau pathologies leading to noticeable brain atrophy at 5-6 months of age. Both male and female mice were analyzed in this study. We observed that radiosignals derived from [11C]flumazenil, a tracer for benzodiazepine receptor, in rTg4510 mice were significantly lower than the levels in nontransgenic littermates at 2-3 months of age. In contrast, retentions of (E)-[11C]ABP688, a tracer for mGluR5, were unaltered relative to controls at 2 months of age but then gradually declined with aging in parallel with progressive brain atrophy. Biochemical and immunohistochemical assessment of postmortem brain tissues demonstrated that inhibitory, but not excitatory, synaptic constituents selectively diminished without overt loss of somas of GABAergic interneurons in the neocortex and hippocampus of rTg4510 mice at 2 months of age, which was concurrent with enhanced immunoreactivity of cFos, a well-characterized immediate early gene, suggesting that impaired inhibitory neurotransmission may cause hyperexcitability of cortical circuits. Our findings indicate that tau-induced disruption of the inhibitory synapse may be a critical trigger of progressive neurodegeneration, resulting in massive neuronal loss, and PET assessments of inhibitory versus excitatory synapses potentially offer in vivo indices for hyperexcitability and excitotoxicity early in the etiologic pathway of neurodegenerative tauopathies.

SIGNIFICANCE STATEMENT In this study, we examined the in vivo status of excitatory and inhibitory synapses in the brain of the rTg4510 tauopathy mouse model by PET imaging with (E)-[11C]ABP688 and [11C]flumazenil, respectively. We identified inhibitory synapse as being significantly dysregulated before brain atrophy at 2 months of age, while excitatory synapse stayed relatively intact at this stage. In line with this observation, postmortem assessment of brain tissues demonstrated selective attenuation of inhibitory synaptic constituents accompanied by the upregulation of cFos before the formation of tau pathology in the forebrain at young ages. Our findings indicate that selective degeneration of inhibitory synapse with hyperexcitability in the cortical circuit constitutes the critical early pathophysiology of tauopathy.

M-Current Inhibition in Hippocampal Excitatory Neurons Triggers Intrinsic and Synaptic Homeostatic Responses at Different Temporal Scales

Persistent alterations in neuronal activity elicit homeostatic plastic changes in synaptic transmission and/or intrinsic excitability. However, it is unknown whether these homeostatic processes operate in concert or at different temporal scales to maintain network activity around a set-point value. Here we show that chronic neuronal hyperactivity, induced by M-channel inhibition, triggered intrinsic and synaptic homeostatic plasticity at different timescales in cultured hippocampal pyramidal neurons from mice of either sex. Homeostatic changes of intrinsic excitability occurred at a fast timescale (1–4 h) and depended on ongoing spiking activity. This fast intrinsic adaptation included plastic changes in the threshold current and a distal relocation of FGF14, a protein physically bridging Nav1.6 and Kv7.2 channels along the axon initial segment. In contrast, synaptic adaptations occurred at a slower timescale (~2 d) and involved decreases in miniature EPSC amplitude. To examine how these temporally distinct homeostatic responses influenced hippocampal network activity, we quantified the rate of spontaneous spiking measured by multielectrode arrays at extended timescales. M-Channel blockade triggered slow homeostatic renormalization of the mean firing rate (MFR), concomitantly accompanied by a slow synaptic adaptation. Thus, the fast intrinsic adaptation of excitatory neurons is not sufficient to account for the homeostatic normalization of the MFR. In striking contrast, homeostatic adaptations of intrinsic excitability and spontaneous MFR failed in hippocampal GABAergic inhibitory neurons, which remained hyperexcitable following chronic M-channel blockage. Our results indicate that a single perturbation such as M-channel inhibition triggers multiple homeostatic mechanisms that operate at different timescales to maintain network mean firing rate.

SIGNIFICANCE STATEMENT Persistent alterations in synaptic input elicit homeostatic plastic changes in neuronal activity. Here we show that chronic neuronal hyperexcitability, induced by M-type potassium channel inhibition, triggered intrinsic and synaptic homeostatic plasticity at different timescales in hippocampal excitatory neurons. The data indicate that the fast adaptation of intrinsic excitability depends on ongoing spiking activity but is not sufficient to provide homeostasis of the mean firing rate. Our results show that a single perturbation such as M-channel inhibition can trigger multiple homeostatic processes that operate at different timescales to maintain network mean firing rate.

Pakistan minorities commission excludes Ahmadi religious group

CNA Staff, May 9, 2020 / 06:01 am (CNA).- Pakistan’s government has declined to include the Ahmadi religious group in its National Commission for Minorities, drawing attention to the group whose Muslim self-identification is rejected by many Muslims.

In a note seen by Reuters, Pakistan’s Ministry of Religious Affairs said Ahmadis should not be included in the commission “given the religious and historical sensitivity of the issue.” Pakistan’s constitution does not recognize the Ahmadis as Muslim.

However, Ahmadis consider themselves part of Islam. The movement was founded in 1889 in British-ruled India. They consider their founder Mirza Ghulam Ahmad a “subordinate prophet.” Other Muslims see this as a violation of the tenet that Muhammad was the last prophet.

There are about 500,000 Ahmadis in Pakistan and up to 20 million adherents worldwide. Some observers estimate the Ahmadi population in Pakistan is higher, but persecution encourages Ahmadis to hide their identity.

Pakistan’s religious freedom record has been a matter of international concern.

The 2020 report of the U.S. Commission on International Religious Freedom has said Ahmadis continue to face “severe persecution from authorities as well as societal harassment due to their beliefs.”

Both government authorities and mobs target their places of worship. In October 2019, the report said, police in Punjab partially demolished a 70-year-old Ahmadiyya mosque.

Pakistan’s National Commission for Minorities gives some status, voice, and protections to minorities in a country where over 90% of people identify as Muslim.

A Hindu has been nominated to chair the minorities commission, whose members include representatives of Christian, Kalash, Sikh, and Zoroastrian communities. Government officials and the head of Pakistan’s Council of Islamic Ideology also have commission seats.

State Minister for Parliamentary Affairs Ali Muhammad Khan, a vocal opponent of including the Ahmadis on the commission, has referred to them as agents of chaos.

“If they want to avail constitutional rights they must accept the constitution first,” he told Reuters. “The Pakistani constitution considers them non-Muslims.”

Usman Ahmad, an Ahmadi representative, told Reuters it is a “complete myth” that they did not accept the constitution. He added that many people disagree with parts of the constitution but still have rights under it.   

He said his community is used to exclusion and has never accepted classification as non-Muslim.

“We’ve never joined such commissions that require us to accept our non-Muslim status,” he said.

Minister of Information Shibli Faraz has said the rights of all people were fully respected in the handling of the commission.

“Every country has the sovereign right to make judgments according to its ground realities,” he told Reuters.

Khan, the Minister for Parliamentary Affairs, had posted to Twitter, then deleted, a comment “There is only one punishment for insulting the Prophet - chopping off the head.” He said he believed in “legal procedures and court proceedings” for those accused of blasphemy. Twitter told him to delete the post, Reuters reports.

Pakistan’s blasphemy laws impose strict punishment on those who desecrate the Quran or who defame or insult Muhammad. Although the government has never executed a person under the blasphemy laws, accusations alone have inspired mob and vigilante violence.

The laws, introduced in the 1980s, are reportedly used to settle scores or to persecute religious minorities. While non-Muslims constitute only 3 percent of the Pakistani population, 14 percent of blasphemy cases have been levied against them.

Many of those accused of blasphemy are murdered, and advocates of changing the law are also targeted by violence.

The Governor of Punjab Salman Taseer was one such critic of the law who was assassinated in January 2011.

Just months later, in March 2011, Shahbaz Bhatti, the first Federal Minister For Minorities Affairs and the only Christian in Pakistan’s cabinet, was assassinated by extremists who characterized him as a blasphemer. Bhatti had criticized the law and defended Asia Bibi, a Catholic woman sentenced to death by hanging in 2010 for blasphemy.

Bibi spent nine years on death row, but left Pakistan for Canada in 2019 at the age of 53 after her death sentence was overturned in October 2018.

The verdict and her subsequent release from prison sparked protests from Islamic hardliners who support strong blasphemy laws.

In Punjab last year, a mob attacked a Christian community after a mosque broadcast over loudspeaker a claim that the Christians had insulted Islam. In another incident in Karachi, false blasphemy accusations against four Christian women prompted mob violence that forced nearly 200 Christian families to flee their homes, the U.S. Commission on International Religious Freedom said.

The situation in Pakistan has attention from some prominent Catholics.

In a Jan. 21, 2020 letter written on behalf of Philadelphia’s Pakistani Catholic community, then-Archbishop of Philadelphia Charles J. Chaput encouraged Pakistan’s Prime Minister Imran Khan to shape a culture of religious freedom

The U.S. Commission on International Religious Freedom’s latest annual report said religious freedom conditions in Pakistan continued to deteriorate last year, citing “The systematic enforcement of blasphemy and anti-Ahmadiyya laws, and authorities’ failure to address forced conversions of religious minorities—including Hindus, Christians, and Sikhs—to Islam.”

The bipartisan federal commission advises the U.S. government on policy. Its report recommended that the U.S. government name Pakistan a country of particular concern for “systematic, ongoing, and egregious violations of religious freedom.”

In December 2018, for the first time, the U.S. State Department designated Pakistan a “Country of Particular Concern.” The designation, which can trigger sanctions under U.S. law, had been recommended by the U.S. Commission for International Religious Freedom in 2017 and 2018.

The latest commission report recommended that Pakistan be re-designated a “Country of Particular Concern,” given “systematic, ongoing, and egregious violations of religious freedom.”

Evidence-Based Updates on the First Week of Exclusive Breastfeeding Among Infants >=35 Weeks

The nutritional and immunologic properties of human milk, along with clear evidence of dose-dependent optimal health outcomes for both mothers and infants, provide a compelling rationale to support exclusive breastfeeding. US women increasingly intend to breastfeed exclusively for 6 months. Because establishing lactation can be challenging, exclusivity is often compromised in hopes of preventing feeding-related neonatal complications, potentially affecting the continuation and duration of breastfeeding. Risk factors for impaired lactogenesis are identifiable and common. Clinicians must be able to recognize normative patterns of exclusive breastfeeding in the first week while proactively identifying potential challenges. In this review, we provide new evidence from the past 10 years on the following topics relevant to exclusive breastfeeding: milk production and transfer, neonatal weight and output assessment, management of glucose and bilirubin, immune development and the microbiome, supplementation, and health system factors. We focus on the early days of exclusive breastfeeding in healthy newborns ≥35 weeks’ gestation managed in the routine postpartum unit. With this evidence-based clinical review, we provide detailed guidance in identifying medical indications for early supplementation and can inform best practices for both birthing facilities and providers.
