Malta Philharmonic Orchestra

Malta Philharmonic Orchestra has given concerts in series of concerts called Community Outreach Concert including one concert in St Gregory Church in Sliema, which The Observer attended. Conductor was Michael Laus. It was a fantastic experience to listen to this orchestra, that Malta has all reasons in the world to be proud of. The concert included such well known works as Sarabande by G. F. Handel and Adagio in G minor by Albinoni. The entrance was free of charge. It is a pity that so few people came to listen to this wonderful concert. One can only admire the people who took the initiative to these concerts and hope that more people will attend future concerts. Well done Malta Philharmonic Orchestra!

Towards harmonization of voriconazole CLSI and EUCAST breakpoints for Candida albicans using a validated in vitro pharmacokinetic/pharmacodynamic model [Susceptibility]

Background. CLSI and EUCAST susceptibility breakpoints for voriconazole and C. albicans differ by one dilution (≤0.125 and ≤0.06 mg/l, respectively) whereas the epidemiological cutoff values (ECOFF/ECV) with both methodologies are the same (0.03 mg/L). We therefore determined the pharmacokinetic-pharmacodynamic (PK/PD) breakpoints of voriconazole against C. albicans for both methodologies with an in vitro PK/PD model, which was validated using existing animal PK/PD data.

Methods. Four clinical wild-type and non-wild-type C. albicans isolates (voriconazole MICs 0.008-0.125 mg/l) were tested in an in vitro PK/PD model. For validation purposes, mouse PK were simulated and in vitro PD were compared with in vivo outcome. Human PK were simulated and the exposure-effect relationship fAUC0-24/MIC was described for EUCAST and CLSI24/48h methods. PK/PD breakpoints were determined using the fAUC0-24/MIC associated with half-maximal activity (EI50) and Monte Carlo simulation analysis.

Results. The in vitro 24h-PD EI50 of voriconazole against C. albicans were 2.5-5 (1.5-17) fAUC/MIC. However, the 72h-PD were higher, 133 (51-347) fAUC/MIC for EUCAST and 94 (35-252) fAUC/MIC for CLSI. The mean (95% confidence interval) probability of target attainment (PTA) was 100(95-100)%, 97(72-100)%, 83(35-99)%, and 49(8-91)% and 100(97-100)%, 99(85-100)%, 91(52-100)% and 68(17-96)% for EUCAST and CLSI MICs 0.03, 0.06, 0.125, and 0.25 mg/L, respectively. Significantly, >95% PTAs were found for EUCAST/CLSI MICs ≤0.03 mg/ll. For MICs 0.06-0.125 mg/l trough levels 1-4 mg/ll would be required.

Conclusion. A PK/PD breakpoint of C. albicans voriconazole at the ECOFF/ECV of 0.03 mg/L was determined for both EUCAST/CLSI methods, indicating the need for breakpoint harmonization for the reference methodologies.

Watkins Family Hour: Brother Sister review – a model of sibling harmony

(Family Hour/Thirty Tigers)
Sean and Sara Watkins are back and in reflective mood

California’s Sean and Sara Watkins are akin to royalty in American folk circles, firstly as founding members of the hugely successful Nickel Creek, and secondly as hosts of an 18-year residency at LA’s Largo club, where they perform alongside invited guests. Brother Sister draws on both strands of their history. Like its self-titled 2015 predecessor, the album sets aside the pizzazz of Nickel Creek for a down-home approach, but instead of boisterous, star-studded cover versions come five original songs and a minimal musical palette.

Alternating on lead, the pair’s vocals remain a model of sibling harmony, while the interplay between Sean’s intricate guitar picking and Sara’s elegant fiddle is similarly impressive – the breakneck bluegrass instrumental Bella and Ivan is a case in point. Mostly, however, the mood is reflective. Lafayette and Miles of Desert Sand chronicle the search for a better life, and Fake Badge, Real Gun is an artful snipe at Trump – “Throw your tantrums but the truth will be waiting”. Warren Zevon’s forlorn Accidentally Like a Martyr fits in neatly, while Charley Jordan’s ribald Keep It Clean is a gleeful example of a Largo session.

New play pays tribute to Mumbai's harmony despite cultural diversity

Even before the term globalisation was coined, the city of Mumbai had neighbourhoods that were multicultural and yet lived in complete harmony. Director-actor Akshay Shimpi's upcoming play Mumbai – The City United, cashes on that beautiful diversity that has now become the nerve of the city. Shimpi says, "As an artist and Mumbaikar, we love this city. It inspires us. Many artists have expressed how the city has served as their muse. Their love has found shape in their respective art form. We have compiled their literature, poems, articles, paintings, plays and songs in this play as a tribute to our beloved city." The play has stitched together songs, skits and readings of stories from the glorious past of the city's literature.

Shimpi and his team acquired works of renowned artists in various languages. When asked why he chose to go with a multilingual play, he says, "Thanks to its cosmopolitan nature, Mumbai, over the years, has developed its own language. In fact, it has its own dialect, and there's also Hindi, which is spoken across the country. Using multiple languages was one way to reflect the true spirit of the city."

Shimpi and Dhanashree Khandkar who have written and performed the play

The cast will be performing songs by yesteryear folk artists such as Vasant Bapat, Anna Bhau Sathe, Patthe Bapu Rao to name a few. Their songs talk about various historical movements in the city such as the shutting down of mills, for instance. These verses have been woven into the script that touches upon the culinary and architectural heritage, festivals and even the city's underbelly.

Shimpi believes that Mumbai is so vast and varied, everyone has a different interpretation of what the city means to them. As for him, he says, "Whenever I'm asked to describe the city, I take reference from the folktale where seven blind men were asked to describe an elephant. For someone, it was a long tube (trunk) while for someone else it was a curtain like a flap (ear). For me, Mumbai is that elephant's stomach. It is so big that it can always accommodate everyone."
