Direct-to-Consumer Prescription Drug Advertising and Patient-Provider Interactions

Background:

Direct-to-consumer prescription drug advertising is prevalent and affects patient care. Previous research that examined its effect on the patient-provider relationship predates many changes in the advertising and medical landscape that have occurred in the last decade, such as the rise in online promotion and the push for value-based medicine.

Methods:

We conducted a nationally representative mail-push-to-web survey of 1744 US adults in 2017 to explore how patients view the effects of direct-to-consumer prescription drug advertising on patient-provider interactions.

Results:

Most respondents (76%) said they were likely to ask a health care provider about advertised drugs; 26% said they had already done so. Among the 26% of respondents who talked to a health care provider about a specific prescription drug they saw advertised, 16% said they received a prescription for the advertised drug. Few respondents (5%) reported that advertising had caused conflict with a health care provider, 16% said it had caused them to question their provider’s advice, and 23% said they were likely to look for a different provider if their provider refused to prescribe a requested brand name drug.

Discussion:

These results suggest that direct-to-consumer advertising is driving some patients to discuss specific products with their health care providers but that most patients do not believe advertising has a negative influence on the patient-provider interaction itself.

We continuously invest in our defenses against ad fraud

By using a combination of people, policies, and technology, our global team of subject matter experts, PhDs, and engineers have fine-tuned our ad systems policies to provide clear guidance on what is and is not acceptable. To date the team has launched over 200 automated filters that help defend our ad systems from invalid activity in a lasting way. 

One of the ways we did this in 2019 was by investing in new technology to better identify policy-violating behavior at the account level, as opposed to the ad level. Our efforts resulted in 2.7 billion bad ads being taken down in 2019—more than 5,000 bad ads per minute—and the termination of 1.5 million advertiser accounts for violations, 3x more than in 2018.

  • Ads.txt and app-ads.txt: These projects are aimed at preventing counterfeit inventory, which diverts revenue from publishers. They allow Ad tech companies to identify unauthorized and domain-spoofed inventory being sold across the industry by letting website owners publicly declare who is allowed to sell their ad space. We scan more than 30 million domains a day and are proud to say that nearly 90 percent of our publisher partners have adopted ads.txt.
  • The Better Ads Standards: These standards are based on extensive user research conducted by the Coalition for Better Ads about which ad formats and ad experiences consumers think are the most annoying and disruptive. They’ve identified 4 desktop and 8 mobile web display ad experiences that companies should avoid in order to maintain a good user experience, and help create a better online environment for everyone.
  • Open Measurement: This software development kit (SDK) is an industry-wide solution to the challenge of measuring viewability of ads in apps. We offer our partners access to the Open Measurement Initiative by integrating the SDK into our mobile ads products. This preserves your revenue stream by ensuring your inventory is considered for purchase.

Treat ad costs as investment, give tax relief: Advertising Agencies Association of India to government

The business of advertising agencies has shrunk by more than 70 per cent following the spread of Covid-19 and the lockdown. In a recommendation to the government, the Advertising Agencies Association of India (AAAI), which is the national association of advertising agencies, has proposed that advertising expense be treated as an investment and the cost be amortised over the next three years.

The objective is to encourage a higher level of expenditure by profitable companies, which will in turn accelerate the revival of economic activity. In a letter to Prakash Javadekar, the Union minister of information & broadcasting, AAAI said on a Rs 100 expense, an advertiser be allowed to claim an expenditure of, say, twice that amount (Rs 200), while computing income tax.

The bulk of the agency's costs are fixed in nature, be it salaries, rent, electricity and communication, among others. "The income and cash flow has been in serious stress in the last month and unless there is some intervention, many businesses will either file for bankruptcy or will have to under-size considerably," the note said.

AAAI president Ashish Bhasin, who is also CEO, Dentsu Aegis Network Greater South and chairman & CEO of India, told TOI, "Advertising industry is like the lubricant to the economy. And the lubricant for the advertising industry is liquidity. If we can infuse liquidity in advertising by encouraging profitable companies to continue to advertise, it will spur consumer demand, which in turn will kick-start the economy. The positive benefit from an improved consumer demand will far outweigh the offsets we are asking for."

With hotels, airlines and other industries impacted by the lockdown, Bhasin said overall average advertising industry revenues have come down by more than 70%. "We had earlier estimated a growth projection of 11-12% for this calendar year. However, with the current situation, we might just end the year with a degrowth," said Bhasin.

AAAI has requested the government to pay its dues to advertising agencies at the earliest possible. AAAI has also asked for an expedited refund of GST applications within 2-4 weeks.

A Colombian advertising company is pitching a novel if morbid solution to shortages of hospital beds and coffins during the coronavirus pandemic: combine them. ABC Displays has created a cardboard bed with metal railings that designers say can double as a casket if a patient dies. Company manager Rodolfo Gmez said he was inspired to find a way to help after watching events unfold recently in nearby Ecuador. Families in the coastal city of Guayaquil waited with dead loved ones in their homes for days last month as COVID-19 cases surged. Many could not find or were unable to afford a wood coffin, using donated cardboard ones instead. Gmez said he plans to donate 10 of his new beds to Colombia's Amazonas department, where resources are in short supply. So far there is no indication whether the beds will be put to use and no orders have been placed.
