Jones) is a small, inconspicuous parasite that has significant effects on tree growth and stand structure in coastal forest ecosystems of western North America. Most previous research focused on the effects of hemlock dwarf mistletoe on timber production. Previous clearcut harvesting of large areas that removed virtually all infected trees and forestry practices that established even-aged stands of trees effectively prevented or minimized future hemlock dwarf mistletoe impacts. Under this regime, further research on hemlock dwarf mistletoe was considered unnecessary. However, current forestry practices that restrict clearcut harvesting to small openings and retain live trees to preserve attributes of old-growth forests create conditions that appear highly favorable for enhanced seed production by hemlock dwarf mistletoe, early spread of the mistletoe to infect young trees, and, consequently, increased growth impacts to residual trees over time. More information is needed on the biology and impacts of hemlock dwarf mistletoe in coastal western hemlock retention harvested forests in the United States of America and Canada. Further work is recommended to develop sampling and monitoring procedures to determine hemlock dwarf mistletoe spread and impacts. We also need to investigate several unusual aspects of hemlock dwarf mistletoe biology and development such as long-distance seed dispersal and persistence in old-growth forests. Detailed tree, stand, and forest-level models are needed to monitor and project hemlock dwarf mistletoe effects over a wide range of ecological conditions and management regimes in coastal forests. The restoration and maintenance of whitebark pine populations require an understanding of all aspects of seed fate. In this paper, we review the literature on seed dispersal in whitebark pine. Clark's nutcracker, pine squirrels, and scatter-hoarding rodents are all known to influence whitebark pine seed fate and ultimately affect the ability of whitebark pine populations to regenerate. We also provide a general overview of the natural histories of the most influential species involved with whitebark pine seed fate: Clark's nutcracker and the pine squirrel. Full Article

This synthesis of literature illustrates information about the socioeconomic, political, health, and cultural effects of climate change on socially vulnerable populations in the United States, with some additional examples in Canada. Through this synthesis, social vulnerability, equity, and climate justice are defined and described, and key issues, themes, and considerations that pertain to the effects of climate change on socially vulnerable populations are identified. The synthesis reviews what available science says about social vulnerability and climate change, and documents the emergence of issues not currently addressed in academic literature. While the US's population is becoming more diverse, it is apparently not reflected in children's books. Lucy Maria Boston was a head of the curve since four of the Green Knowe books could be regarded as multicultural since they contain not only Asian and African main characters but also a physically disabled character and deal with the issues of slavery and exile due to war.Ping, a young refugee from Burma, is the main human character in A Stranger at Greene Knowe and a supporting character in The River at Green Knowe and An Enemy at Green Knowe. Ping has spent most of his life in a hostel for displaced children and goes to stay at Green Knowe during his summer holiday. He is eventually asked by Mrs. Oldknow to live with her and Tolly at Green Knowe. His experiences as a homeless child trapped in the grey world of the London home cause him to appreciate not only the natural world around the house but also to empathize with the escaped gorilla, Hanno. Boston wanted to dedicate Stranger to a gorilla keeper that she knew but was forbidden to do so by the zoo since it portrayed captivity for animals as cruel and harmful to the animal. When Green Knowe is under siege from evil in Enemy, Ping calls back Hanno with a traditional prayer to help save the house.Jacob, in Treasure at Green Knowe, is bought as a child in a slave auction by Captain Oldknowe as a companion for the Captain's blind daughter, Susan. Susan's mother is uninterested in Susan since she views her as an unmarriageable burden. Susan's blindness puts her outside of the normal constraints for an upper-class girl so she can spend her time climbing trees with Jacob and learning how to write with him and their tutor Jonathan. Susan's brother Sefton views Jacob as less than human, buying him clothes patterned on those of an organ-grinder's monkey. Both Jacob and Susan rely on each other to navigate the rules of a society that views them as worthless because of their respective race and disability. They work together to educate themselves and lead successful adult lives despite their differences in race, sex, and station.Despite the fifty or so years since they were written, the books still hold up due to the quality of the writing, the strong characterizations, and the universal themes. They are well-worth being placed on any reading list, multicultural or not. Good children's books should be read whether or not they are written by US authors. We aimed to review and synthesize the research literature to determine how operational failures in primary care affect the work of primary care physicians. METHODS We conducted a critical interpretive synthesis. We searched 7 databases for papers published in English from database inception until October 2017 for primary research of any design that addressed problems interfering with primary care physicians’ work. All potentially eligible titles/abstracts were screened by 1 reviewer; 30% were subject to second screening. We conducted an iterative critique, analysis, and synthesis of included studies. RESULTS Our search retrieved 8,544 unique citations. Though no paper explicitly referred to "operational failures," we identified 95 papers that conformed to our general definition. The included studies show a gap between what physicians perceived they should be doing and what they were doing, which was strongly linked to operational failures—including those relating to technology, information, and coordination—over which physicians often had limited control. Operational failures actively configured physicians’ work by requiring significant compensatory labor to deliver the goals of care. This labor was typically unaccounted for in scheduling or reward systems and had adverse consequences for physician and patient experience. CONCLUSIONS Primary care physicians’ efforts to compensate for suboptimal work systems are often concealed, risking an incomplete picture of the work they do and problems they routinely face. Future research must identify which operational failures are highest impact and tractable to improvement. Full Article

literatur Reliability of CT Angiography in Cerebral Vasospasm: A Systematic Review of the Literature and an Inter- and Intraobserver Study [INTERVENTIONAL] By www.ajnr.org Published On :: 2020-04-09T06:30:48-07:00 BACKGROUND AND PURPOSE: Computed tomography angiography offers a non-invasive alternative to DSA for the assessment of cerebral vasospasm following subarachnoid hemorrhage but there is limited evidence regarding its reliability. Our aim was to perform a systematic review (Part I) and to assess (Part II) the inter- and intraobserver reliability of CTA in the diagnosis of cerebral vasospasm. MATERIALS AND METHODS: In Part I, articles reporting the reliability of CTA up to May 2018 were systematically searched and evaluated. In Part II, 11 raters independently graded 17 arterial segments in each of 50 patients with SAH for the presence of vasospasm using a 4-category scale. Raters were additionally asked to judge the presence of any moderate/severe vasospasm (≥ 50% narrowing) and whether findings would justify augmentation of medical treatment or conventional angiography ± balloon angioplasty. Four raters took part in the intraobserver reliability study. RESULTS: In Part I, the systematic review revealed few studies with heterogeneous vasospasm definitions. In Part II, we found interrater reliability to be moderate at best ( ≤ 0.6), even when results were stratified according to specialty and experience. Intrarater reliability was substantial ( > 0.6) in 3/4 readers. In the per arterial segment analysis, substantial agreement was reached only for the middle cerebral arteries, and only when senior raters’ judgments were dichotomized (presence or absence of ≥50% narrowing). Agreement on the medical or angiographic management of vasospasm based on CTA alone was less than substantial ( ≤ 0.6). CONCLUSIONS: The diagnosis of vasospasm using CTA alone was not sufficiently repeatable among observers to support its general use to guide decisions in the clinical management of patients with SAH.
