A Mechanistic and Translational Pharmacokinetic-Pharmacodynamic Model of Abicipar Pegol and Vascular Endothelial Growth Factor Inhibition [Drug Discovery and Translational Medicine]

Abicipar pegol (abicipar) is a novel DARPin therapeutic and highly potent vascular endothelial growth factor (VEGF) inhibitor intended for the treatment of neovascular age-related macular degeneration (nAMD). Here we develop a translational pharmacokinetic/pharmacodynamic (PK/PD) model for abicipar to guide dosing regimens in the clinic. The model incorporated abicipar-VEGF binding kinetics, VEGF expression levels, and VEGF turnover rates to describe the ocular and systemic PK data collected from the vitreous, aqueous humor (AH), choroid, retina, and serum of rabbits after a 1-mg abicipar intravitreal (IVT) dose. The model was translated to humans using human-specific mechanistic parameters and refitted to human serum and AH concentrations from patients with diabetic macular edema and nAMD. The model was then used to simulate 8-, 12- (quarterly), and 16-week dosing intervals in the clinic. Simulations of 2 mg abicipar IVT at 8-week or quarterly dosing in humans indicates minimum steady-state vitreal concentrations are maintained above both in vitro IC50 and in vivo human IC50 values. The model predicted virtually complete VEGF inhibition for the 8-week and quarterly dosing schedule during the 52-week treatment period. In the 16-week schedule, clinically significant VEGF inhibition was maintained during the 52-week period. The model quantitatively described abicipar-VEGF target engagement leading to rapid reduction of VEGF and a long duration of VEGF inhibition demonstrating the clinical feasibility of up to a 16-week dosing interval. Abicipar is predicted to reduce IVT dosing compared with other anti-VEGF therapies with the potential to lessen patient treatment burden.

SIGNIFICANCE STATEMENT

Current anti-VEGF treatments for neovascular age-related macular degeneration require frequent (monthly) intravitreal injections and monitoring, which increases patient burden. We developed a mechanistic pharmakinetic/pharmadynamic model to describe the interaction between abicipar (a novel VEGF inhibitor) and VEGF to evaluate the duration of action. The model demonstrates extended abicipar-VEGF target engagement leading to clinical feasibility of up to a 16-week dosing interval. Our model predicted that abicipar 8-week and quarterly dosing schedules maintain virtually complete VEGF inhibition during the 52-week period.

Pluripotent stem cells hold great potential for regenerative medicine. Increased replication and division, such is the case during regeneration, concomitantly increases the risk of adverse outcomes through the acquisition of mutations. Seeking for driving mechanisms of such outcomes, we challenged a pluripotent stem cell system during the tightly controlled regeneration process in the planarian Schmidtea mediterranea. Exposure to the genotoxic compound methyl methanesulfonate (MMS) revealed that despite a similar DNA-damaging effect along the anteroposterior axis of intact animals, responses differed between anterior and posterior fragments after amputation. Stem cell proliferation and differentiation proceeded successfully in the amputated heads, leading to regeneration of missing tissues. Stem cells in the amputated tails showed decreased proliferation and differentiation capacity. As a result, tails could not regenerate. Interference with the body-axis-associated component β-catenin-1 increased regenerative success in tail fragments by stimulating proliferation at an early time point. Our results suggest that differences in the Wnt signalling gradient along the body axis modulate stem cell responses to MMS.

Previously, we have shown that bulk microtubule (MT) movement correlates with neurite elongation, and blocking either dynein activity or MT assembly inhibits both processes. However, whether the contributions of MT dynamics and dynein activity to neurite elongation are separate or interdependent is unclear. Here, we investigated the underlying mechanism by testing the roles of dynein and MT assembly in neurite elongation of Aplysia and chick neurites using time-lapse imaging, fluorescent speckle microscopy, super-resolution imaging and biophysical analysis. Pharmacologically inhibiting either dynein activity or MT assembly reduced neurite elongation rates as well as bulk and individual MT anterograde translocation. Simultaneously suppressing both processes did not have additive effects, suggesting a shared mechanism of action. Single-molecule switching nanoscopy revealed that inhibition of MT assembly decreased the association of dynein with MTs. Finally, inhibiting MT assembly prevented the rise in tension induced by dynein inhibition. Taken together, our results suggest that MT assembly is required for dynein-driven MT translocation and neurite outgrowth.

Cofilin regulates axon growth and branching of Drosophila {gamma}-neurons [SHORT REPORT]

The mechanisms that control intrinsic axon growth potential, and thus axon regeneration following injury, are not well understood. Developmental axon regrowth of Drosophila mushroom body -neurons during neuronal remodeling offers a unique opportunity to study the molecular mechanisms controlling intrinsic growth potential. Motivated by the recently uncovered developmental expression atlas of -neurons, we here focus on the role of the actin-severing protein cofilin during axon regrowth. We show that Twinstar (Tsr), the fly cofilin, is a crucial regulator of both axon growth and branching during developmental remodeling of -neurons. tsr mutant axons demonstrate growth defects both in vivo and in vitro, and also exhibit actin-rich filopodial-like structures at failed branch points in vivo. Our data is inconsistent with Tsr being important for increasing G-actin availability. Furthermore, analysis of microtubule localization suggests that Tsr is required for microtubule infiltration into the axon tips and branch points. Taken together, we show that Tsr promotes axon growth and branching, likely by clearing F-actin to facilitate protrusion of microtubules.

Early growth response 1 regulates hematopoietic support and proliferation in human primary bone marrow stromal cells

Human bone marrow stromal cells (BMSC) are key elements of the hematopoietic environment and they play a central role in bone and bone marrow physiology. However, how key stromal cell functions are regulated is largely unknown. We analyzed the role of the immediate early response transcription factor EGR1 as key stromal cell regulator and found that EGR1 was highly expressed in prospectively-isolated primary BMSC, down-regulated upon culture, and low in non-colony-forming CD45neg stromal cells. Furthermore, EGR1 expression was lower in proliferative regenerating adult and fetal primary cells compared to adult steady-state BMSC. Overexpression of EGR1 in stromal cells induced potent hematopoietic stroma support as indicated by an increased production of transplantable CD34+CD90+ hematopoietic stem cells in expansion co-cultures. The improvement in bone marrow stroma support function was mediated by increased expression of hematopoietic supporting genes, such as VCAM1 and CCL28. Furthermore, EGR1 overexpression markedly decreased stromal cell proliferation whereas EGR1 knockdown caused the opposite effects. These findings thus show that EGR1 is a key stromal transcription factor with a dual role in regulating proliferation and hematopoietic stroma support function that is controlling a genetic program to co-ordinate the specific functions of BMSC in their different biological contexts.

Human Epidermal Growth Factor Receptor 2-Positive Breast Cancer Is Associated with Indigenous American Ancestry in Latin American Women

Women of Latin American origin in the United States are more likely to be diagnosed with advanced breast cancer and have a higher risk of mortality than non-Hispanic White women. Studies in U.S. Latinas and Latin American women have reported a high incidence of HER2 positive (+) tumors; however, the factors contributing to this observation are unknown. Genome-wide genotype data for 1,312 patients from the Peruvian Genetics and Genomics of Breast Cancer Study (PEGEN-BC) were used to estimate genetic ancestry. We tested the association between HER2 status and genetic ancestry using logistic and multinomial logistic regression models. Findings were replicated in 616 samples from Mexico and Colombia. Average Indigenous American (IA) ancestry differed by subtype. In multivariate models, the odds of having an HER2+ tumor increased by a factor of 1.20 with every 10% increase in IA ancestry proportion (95% CI, 1.07–1.35; P = 0.001). The association between HER2 status and IA ancestry was independently replicated in samples from Mexico and Colombia. Results suggest that the high prevalence of HER2+ tumors in Latinas could be due in part to the presence of population-specific genetic variant(s) affecting HER2 expression in breast cancer.Significance:The positive association between Indigenous American genetic ancestry and HER2+ breast cancer suggests that the high incidence of HER2+ subtypes in Latinas might be due to population and subtype-specific genetic risk variants.

Policy Ideas to Share the Fruits of Economic Growth


In a new essay, “The New Challenge to Market Democracies,” Senior Fellow William Galston argues that “the centrality of economic well-being in our politics reflects long-held assumptions about the purposes of our politics. If economic growth and well-being are in jeopardy, so are our political arrangements.”

Galston, the Ezra K. Zilkha Chair in Governance Studies, makes the case that economic growth and well-being are indeed in jeopardy for a variety of reasons, including: wage growth that has just kept up with inflation; family and household incomes that remain below their pre-Great Recession peak; the share of national income going to wages and salaries is as low as it’s been in nearly 50 years; and a difficult jobs situation in which workers are getting paid less, the number of people working part-time who want full-time work remains high, and few new jobs offer middle range incomes. “These trends,” Galston writes, “bode ill for the future of the middle class; many parents now doubt that their children will enjoy the same opportunities that they did.”

Galston offers three broad policy prescriptions related to employment and tax reform:

  1. “We should adopt full employment as a high-priority goal of economic policy and welcome the wage increases that it would generate.”
  2. “We should use the tax code to restore the relationship between wage increases and productivity gains.”
  3. “We should adopt a strong presumption against provisions of the tax code that treat some sources of income more favorably than wages and salaries,” which includes scrapping tax expenditures that “disproportionately benefit upper-income investors.”

Calling economic growth a “moral enterprise” as well as a material goal, Galston—acknowledging economist Benjamin Friedman—concludes that:

the central question the United States now faces is whether the next generation will again achieve broadly shared prosperity or rather experience the stagnation of living standards. Broad prosperity is both the oil that lubricates the machinery of government and the glue that binds our society together. Economic stagnation means a continuation of gridlocked, zero-sum politics and a turn away from the spirit of generosity that only a people confident of its future can sustain.

Mexico City and Chicago explore new paths for economic growth


Last month, a team from the Metropolitan Policy Program, along with a delegation from the city of Chicago, traveled to Mexico City as part of the Global Cities Economic Partnership (GCEP). Launched at a 2013 event sponsored by the Global Cities Initiative (GCI), this novel partnership aims to expand growth and job creation in both cities  by building on complementary economic assets and opportunities.

Together with representatives from World Business Chicago, the Illinois governor’s office, and members of Chicago’s tech startup scene (organized by TechBridge), the Brookings team arrived in Mexico City just as, after a 20 year debate, reforms to devolve greater autonomy and powers to the largest metropolitan area in the Western Hemisphere were finalized.  Central to that reform is Mexico City’s enhanced ability to plan and implement its own economic development policy, underscoring the growing importance of city-regions assuming roles once solely the province of state and national governments: fostering trade, investment, and economic growth.

Chicago and Mexico City illustrate this trend through the GCEP. Emerging from a GCI analysis that identified unique economic, demographic and and social connections between the cities, Chicago Mayor Rahm Emanuel and Mexico City Mayor Miguel Angel Mancera established a novel city-to-city collaboration. Since signing the agreement, government, business, and civic leaders in both cities have been experimenting with new approaches to jointly grow their economies.  They have tried to foster more trade and investment within shared industry clusters; link economic development support services; and leverage similar strengths in research, innovation, and human capital.

This trip to Mexico City focused on one of GCEP’s early outcomes, a formal partnership between Chicago tech business incubator 1871 and Mexico City incubator Startup Mexico (SUM) that facilitates the early internationalization of firms in both cities. Both organizations advanced the creation of a residency program that will enable entrepreneurs from both incubators to have a presence in each other’s markets.

The GCEP approach of city-to-city global engagement has inspired other GCI participants to try their own models, forming economic alliances to ease global navigation and engagement. San Antonio, Phoenix, and Los Angeles also crafted agreements with Mexico City, each focused on different opportunities built off their distinctive economic assets and relationships. Portland and Bristol have investigated how to leverage their comparable “green city” reputations in the U.S. and U.K., connecting mid-size firms in their unique sustainability clusters for collaboration on research and joint ventures. Similarly, San Diego and London are testing how to promote synergies among companies, academic centers, investors, and workers in their shared life sciences subsectors such as cell and gene therapy.

Home to half of the world’s population, cities generate about three quarters of the world’s GDP, and now serve as the hubs for the growth in global flows of trade, capital, visitors, and information. The future prosperity and vitality of city-regions demands finding new approaches that take full advantage of these global connections.

Growth and Income of the Poor


Buried in the middle of Table 1 in our new paper, Growth still is good for the poor, is a remarkable statistic: in a sample of 118 countries the average change in the income share of the bottom quintile of the population during the 2000s was 0.004. This is a small change, but what is striking is that it is positive. A common concern these days is that the people in the bottom part of the income distribution are being left behind. But these data show that there is no global trend in that direction. Similarly for the income share of the bottom 40%, there is no trend across countries, either in the 2000s or in earlier decades.

The other striking finding in this study, written together with Tatjana Kleineberg of Yale and Aart Kraay of the World Bank, is that changes in income share of the poor are uncorrelated with growth. In general, the relationship between the growth of mean income and the growth of income of the bottom 20% (or bottom 40%) is one-to-one; hence the title. Furthermore, about three-fourths of the variation in income of the poor across countries and over time can be accounted for by growth of average income. There are some interesting exceptions to the one-to-one relationship: Latin America in the 2000s had pro-poor growth with income of the poor rising significantly faster than mean income, while Asia had the opposite, pro-rich growth. We try to explain the changes in income share of the poor with a large number of variables covering dimensions of globalization, macroeconomic policy, and social policy (for example, government expenditure on health and education, primary school enrollment, or Gini coefficient on educational attainment). This part of the paper leads to a non-result: there are no robust correlates with changes in income shares.

What are the policy implications? I see both good news and bad news here. The fact that there is no worldwide trend towards lower income shares for the poor is good news. If there were such a trend it would suggest that globalization or some other general force was biased against the poor, and it would be hard to resist such a trend. But that is not the case. The rising inequality that we see in the U.S., for example, is not a general trend in rich countries. Other countries have found ways to maintain or increase the income share of the poor. It is also good news that growth will tend to raise the income of the poor proportionately, as it should always be possible to get most of the population to support a growth agenda. On the other hand, to the extent that we care about poverty reduction, it is bad news that we cannot explain what leads to changes in income shares of the poor and in particular what might bring about pro-poor growth.

Our findings do not imply that interventions aimed directly at the poor are pointless. But given the key role of growth in poverty reduction I favor interventions that build up the assets of the poor and enable them to participate in the market economy. A good example would be programs to ensure that the poor have access to maternal and child health services and early childhood education. Intuitively, you may think that such programs should shift income distribution in favor of the poor, and perhaps in some cases they do. But it also possible that the programs have powerful spillover benefits for the whole economy (more skilled labor, less crime – not to mention that the next potential Einstein will probably be born to a poor family in the developing world). If programs aimed at the poor have the side effect of stimulating the whole economy we should be happy about the higher growth, not disappointed that it is not pro-poor.
