mean Targeting the polyamine pathway—“a means” to overcome chemoresistance in triple-negative breast cancer [Cell Biology] By www.jbc.org Published On :: 2020-05-08T03:41:14-07:00 Triple-negative breast cancer (TNBC) is characterized by its aggressive biology, early metastatic spread, and poor survival outcomes. TNBC lacks expression of the targetable receptors found in other breast cancer subtypes, mandating use of cytotoxic chemotherapy. However, resistance to chemotherapy is a significant problem, encountered in about two-thirds of TNBC patients, and new strategies are needed to mitigate resistance. In this issue of the Journal of Biological Chemistry, Geck et al. report that TNBC cells are highly sensitive to inhibition of the de novo polyamine synthesis pathway and that inhibition of this pathway sensitizes cells to TNBC-relevant chemotherapy, uncovering new opportunities for addressing chemoresistance. mean Age at Diagnosis and Patient Preferences for Treatment Outcomes in AML: A Discrete Choice Experiment to Explore Meaningful Benefits By cebp.aacrjournals.org Published On :: 2020-05-01T00:05:36-07:00 Background: The recent expansion of treatment options in acute myeloid leukemia (AML) has necessitated a greater understanding of patient preferences for treatment benefits, about which little is known. Methods: We sought to quantify and assess heterogeneity of the preferences of AML patients for treatment outcomes. An AML-specific discrete choice experiment (DCE) was developed involving multiple stakeholders. Attributes included in the DCE were event-free survival (EFS), complete remission (CR), time in the hospital, short-term side effects, and long-term side effects. Continuously coded conditional, stratified, and latent-class logistic regressions were used to model preferences of 294 patients with AML. Results: Most patients were white (89.4%) and in remission (95.0%). A 10% improvement in the chance of CR was the most meaningful offered benefit (P < 0.001). Patients were willing to trade up to 22 months of EFS or endure 8.7 months in the hospital or a two-step increase in long-term side effects to gain a 10% increase in chance of CR. Patients diagnosed at 60 years or older (21.6%) more strongly preferred to avoid short-term side effects (P = 0.03). Latent class analysis showed significant differences of preferences across gender and insurance status. Conclusions: In this national sample of mostly AML survivors, patients preferred treatments that maximized chance at remission; however, significant preference heterogeneity for outcomes was identified. Age and gender may affect patients' preferences. Impact: Survivor preferences for outcomes can inform patient-focused drug development and shared decision-making. Further studies are necessary to investigate the use of DCEs to guide treatment for individual patients. Full Article

mean What It Truly Means to ‘Believe Survivors’ By feedproxy.google.com Published On :: Sat, 09 May 2020 09:17:34 GMT Photo Illustration by Lyne Lucien/The Daily Beast/GettyTwo years ago, the voices of survivors of sexual harassment and assault launched an unprecedented movement the world over, using Tarana Burke’s MeToo framework. That one moment was a spark into the unknown, but the movement itself was no happy accident. It was built to fuel a reclamation of power for those who had been silenced for too long, a laborious undertaking by activists, advocates, and organizations including mine—the National Women’s Law Center, which also houses and administers the TIME’S UP Legal Defense Fund.Since then, the #MeToo movement has enabled considerable progress, including making space for more and more people who have claims of sexual misconduct to come forward. People like Tara Reade. But despite this dramatic cultural awakening, our institutions and systems are just beginning to stir. The lack of necessary legal and policy changes both in our government and in our workplaces, schools, houses of worship, and otherwise have created a world that very imperfectly serves the needs of survivors. Every domestic worker who is entirely unprotected by our federal and most state civil rights statutes, and every person who is classified as an independent contractor and left out of civil rights protections, have proven that our work must take on the reform and re-envisioning of the very systems that excluded them in the first place.
