Efficacy of fasting in reducing gastric volume has not been well established. The primary objective of this study was to assess the effect of 4 h of fasting on prevalence of empty stomach using gastric ultrasonography in critically ill subjects who are fasted for extubation. The secondary objectives were to evaluate the change in gastric volumes during 4 h of fasting and to determine factors associated with empty stomach after fasting.METHODS:This was a single-center, prospective, observational study on adult ICU subjects who were enterally fed for at least 6 h continuously and mechanically ventilated. Gastric ultrasound was performed immediately prior to commencement of fasting, after 4 h of fasting, and after nasogastric (NG) aspiration after 4 h of fasting. An empty stomach was defined as a gastric volume ≤ 1.5 mL/kg.RESULTS:Forty subjects were recruited, and 38 (95%) had images suitable for analysis. The prevalence of empty stomach increased after 4 h of fasting (25 [65.8%] vs 31 [81.6%], P = .041) and after 4 h of fasting with NG aspiration (25 [65.8%] vs 34 [89.5%], P = .008). There was a significant difference in median (interquartile range) gastric volume per body weight between before fasting and 4 h after fasting (1.0 [0.5–1.8] mL/kg vs 0.4 [0.2–1.0] mL/kg, P < .001). No patient factors were associated with higher prevalence of empty stomach after 4 h of fasting.CONCLUSIONS:Most mechanically ventilated subjects had empty stomachs prior to fasting for extubation. Fasting for 4 h further increased the prevalence of empty stomach at extubation to > 80%. Full Article

That temporarily blocks enforcement of the order nationwide, but the Justice Department says it will appeal. The ban extended to six mostly Muslim nations, plus North Korea and Venezuela. Pennsylvania Congressman Tom Marino withdrew today from consideration to be President Trump’s drug czar. That followed an investigation by The Washington Post and CBS News. They found Marino was key in passing a 2016 law that limits the Drug Enforcement Administration’s ability to rein in opioid distribution. A new verbal battle has broken out between the president and Republican Senator John McCain. It began last night in Philadelphia, when the Arizona senator and former Vietnam POW appeared to criticize Mr. Trump and his followers. He cited a list of failings. SEN. JOHN MCCAIN, R-Ariz.: To fear the world we have organized and led for three-quarters-of-a-century, to abandon the ideals we have advanced around the globe for the sake of some half-baked, spurious nationalism cooked up by people who would rather find scapegoats than solve problems. (CHEERING AND APPLAUSE) JUDY WOODRUFF: The president answered by saying, “At some point, I fight back, and it won’t be pretty.” In turn, McCain said, “I have faced tougher adversaries.” In Afghanistan, Taliban bombings and shootings left at least 74 people dead today. The worst was Paktika province in the east, where two car bombs killed dozens, including the provincial police chief, and wounded more than 100 others. Taliban militants also staged attacks in the south and west of the country. In Syria, militia forces backed by the U.S. say they have retaken the Islamic State group’s de facto capital. The referendum is finished and has become a thing from the past. We hoped that they would cancel it, but we have finished it on the ground. JUDY WOODRUFF: Meanwhile, the president of Iraqi Kurdistan, Massoud Barzani, insisted that the referendum will not be in vain. Another 10,000 to 15,000 Rohingya Muslims fled Buddhist Myanmar for Bangladesh over the weekend. Drone video showed snaking lines of refugees making the trek to already crowded camps. Many told of villages torched by mobs and soldiers. Others said they were starved out of their homes. Back in this country, a new fire broke out in the San Francisco Bay Area, just as crews had made major progress against other fires in Northern California. Thick smoke billowed from the new site early today, as it burned through forests in the Santa Cruz Mountains. Weary fire crews said they’re calling in more help. Speaking from my own experience, moving to a big city and needing to find an apartment ASAP causes you to compromise on some things, but roommates shouldn't be one of them. Unfortunately, that's often the way the cookie crumbles. From messy roommates to roommates who use the kitchen to cook the weirdest and most unappealing foods, to roommates who never take their clothes out of the dryer, there often is much to be desired. For the original poster of the thread we are featuring today, a frustrating feline situation left them feeling confused and fed up as to how to approach their roommate. The roommate in question completely crossed the line when it came to OP's cat, often referring to the cat as their shared cat, rather than OP's cat. The kicker is that this cat is OP's childhood cat and of course, OP covers all of the cat's expenses. What would you do in a situation like this? We would have to move out at all costs. Much like us humans, each one has their own completely developed personality and quirks. Some cats are perpetual balls of energy, pausing from their mischief only to engage in a well deserved cat nap. Some cats are totally indifferent to your existence, you could provide them with treats, pets, the whole lot and still not receive much attention in return. And some cats need their favorite human to accompany them to the litter box any time they need to take care of business. The cat featured in the story we are covering today is the latter, and might we add, a total goofball. To be honest, this is a new one for us. We have yet to come across a cat who demands that their human be present during all bathroom activities. We've heard of cats who like to go to the bathroom while their humans are in the bathroom, to protect them from whatever evil lies dormant in the toilet, but yeah, this is a new one. What advice would you give to the original poster? All we can think of is to laugh and cry at the same time.
