The propensity of using non-medicinal pain relief methods is increasing. The present study aimed to compare the effect of Acupressure at two GB-21 and SP06 points on the severity of labor pain. In this quasi-experimental single blind study started on December 2010 and ended on June 2011 in which 150 primiparous women were divided into three groups of Acupressure at GB-21 point, Acupressure at SP-6 point and control group. The intervention was carried out for 20 min at 3-4 and 20 min at 7-8 cm dilatation of Cervix. The pain severity was measured by Visual Analog Scale before and immediately, 30 and 60 min after the intervention. Then, the data were statistically analyzed. No significant difference was found among the 3 groups regarding the pain severity before the intervention. However, the pain severity it was reduced at 3-4 and 7-8 cm dilatation immediately, 30 and 60 min after the intervention in the two intervention groups compared to the control group (p<0.001). Nonetheless, no statistically significant difference was observed between the two intervention groups (p = 0.93). The results of the study showed that application of Acupressure at two GB-21 and SP-6 points was effective in the reduction of the severity of labor pain. Therefore, further studies are recommended to be performed on the application of Acupressure together with non-medicinal methods. Full Article

The location and scale parameters together with the proportion are allowed to depend nonparametrically on covariates. After settling identifiability, we provide local M-estimators for these parameters which converge in the sup-norm at the optimal rates over Hölder-smoothness classes. We also introduce an adaptive version of the estimators based on the Lepski-method. Sup-norm bounds show that the local M-estimator properly estimates the functions globally, and are the first step in the construction of useful inferential tools such as confidence bands. In our analysis we develop general results about rates of convergence in the sup-norm as well as adaptive estimation of local M-estimators which might be of some independent interest, and which can also be applied in various other settings. We investigate the finite-sample behaviour of our method in a simulation study, and give an illustration to a real data set from bioinformatics. Full Article

An effective parallel computing methodology, developed by us harnesses the power of parallel processing technology to increase the efficiencies of our conditionally independent Gibbs sampling and Transformation based MCMC (TMCMC) methods. Applications of our model and methods to simulation studies with biologically realistic genotype datasets and a real, case-control based genotype dataset on early onset of myocardial infarction (MI) have yielded quite interesting results beside providing some insights into the differential effect of gender on MI. Furthermore, we develop optimal semiparametric estimation for parameters such as the population mean. Specifically, we propose two semiparametric optimal estimators that do not require any model assumptions other than the response mechanism. Asymptotic properties of the proposed estimators are discussed. An extensive simulation study is presented to compare with some existing methods. For many semiparametric models, however, the commonly used two-step (plug-in) empirical likelihood ratio is not asymptotically distribution-free, that is, its asymptotic distribution contains unknown quantities, and hence Wilks’ theorem breaks down. This article suggests a general approach to restore Wilks’ phenomenon in two-step semiparametric empirical likelihood inferences. The main insight consists in using as the moment function in the estimating equation the influence function of the plug-in sample moment. The proposed method is general; it leads to a chi-squared limiting distribution with known degrees of freedom; it is efficient; it does not require undersmoothing; and it is less sensitive to the first-step than alternative methods, which is particularly appealing for high-dimensional settings. The invariance structures naturally suggest tests that are based on the ranks of the increments of the observations, their average and an assumed reference density for the innovations. The tests are semiparametric in the sense that they are valid, that is, have the correct (asymptotic) size, irrespective of the true innovation density. For a correctly specified reference density, our test is point-optimal and nearly efficient. For arbitrary reference densities, we establish a Chernoff–Savage-type result, that is, our test performs as well as commonly used tests under Gaussian innovations but has improved power under other, for example, fat-tailed or skewed, innovation distributions. To avoid nonparametric estimation, we propose a simplified version of our test that exhibits the same asymptotic properties, except for the Chernoff–Savage result that we are only able to demonstrate by means of simulations. The quantification of the effects of exposure to an environmental mixture poses several statistical challenges. It is often the case that exposure to multiple pollutants interact with each other to affect an outcome. Further, the exposure-response relationship between an outcome and some exposures, such as some metals, can exhibit complex, nonlinear forms, since some exposures can be beneficial and detrimental at different ranges of exposure. To estimate the health effects of complex mixtures, we propose a flexible Bayesian approach that allows exposures to interact with each other and have nonlinear relationships with the outcome. We induce sparsity using multivariate spike and slab priors to determine which exposures are associated with the outcome and which exposures interact with each other. The proposed approach is interpretable, as we can use the posterior probabilities of inclusion into the model to identify pollutants that interact with each other. We utilize our approach to study the impact of exposure to metals on child neurodevelopment in Bangladesh and find a nonlinear, interactive relationship between arsenic and manganese. Full Article

We extend BART to a semiparametric regression framework in which the conditional expectation of an outcome is a function of treatment, its effect modifiers, and confounders. The confounders are allowed to have unspecified functional form, while treatment and effect modifiers that are directly related to the research question are given a linear form. The result is a Bayesian semiparametric linear regression model where the posterior distribution of the parameters of the linear part can be interpreted as in parametric Bayesian regression. This is useful in situations where a subset of the variables are of substantive interest and the others are nuisance variables that we would like to control for. An example of this occurs in causal modeling with the structural mean model (SMM). Under certain causal assumptions, our method can be used as a Bayesian SMM. Our methods are demonstrated with simulation studies and an application to dataset involving adults with HIV/Hepatitis C coinfection who newly initiate antiretroviral therapy. Among the strengths of these models is their ability to account for treatment (or intervention) effects after an event occurrence. Despite their flexibility for modeling recurrent events, the number of applications is limited. This seems to be a result of the fact that in the semiparametric setting all the existing results assume the virtual age function that describes the treatment (or intervention) effects to be known. This shortcoming can be overcome by considering semiparametric virtual age models with parametrically specified virtual age functions. Yet, fitting such a model is a difficult task. Indeed, it has recently been shown that for these models the standard profile likelihood method fails to lead to consistent estimators. Here we show that consistent estimators can be constructed by smoothing the profile log-likelihood function appropriately. We show that our general result can be applied to most of the relevant virtual age models of the literature. Our approach shows that empirical process techniques may be a worthwhile alternative to martingale methods for studying asymptotic properties of these inference methods. A simulation study is provided to illustrate our consistency results together with an application to real data. Full Article

As it is known that Gaussian processes are a popular tool in nonparametric regression because of their flexibility and the fact that much of the ensuing computation is parametric Gaussian computation. However, in the absence of covariates, the most frequently used covariance functions of a Gaussian process will not be well defined. We propose an imputation method to solve this issue and perform our analysis using Bayesian inference, where we specify the objective priors on the parameters of Gaussian process models. Several simulations are conducted to illustrate effectiveness of our proposed method and further, our method is exemplified via two real datasets, one through Langmuir equation, commonly used in pharmacokinetic models, and another through Auto-mpg data taken from the StatLib library. The properties of the proposed model are studied theoretically through the analysis of inter-arrival times and of the number of change-points in a given time interval. The prior-posterior analysis by Markov chain Monte Carlo techniques is developed on a forward-backward algorithm for sampling the various regime indicators. mip Imipenem population pharmacokinetics: therapeutic drug monitoring data collected in critically ill patients with or without extracorporeal membrane oxygenation [Pharmacology] By aac.asm.org Published On :: 2020-04-06T08:49:26-07:00 Carbapenem pharmacokinetic profiles are significantly changed in critically ill patients because of the drastic variability of the patients' physiological parameters. Published population PK studies have mainly focused on specific diseases and the majority of these studies had small sample sizes. The aim of this study was to develop a population PK model of imipenem in critically ill patients that estimated the influence of various clinical and biological covariates and the use of Extracorporeal Membrane Oxygenation (ECMO) and Continuous Renal Replacement Therapy (CRRT). A two-compartment population PK model with Creatinine clearance (CrCL), body weight (WT), and ECMO as fixed effects was developed using the non-linear mixed effect model (NONMEM). A Monte Carlo simulation was performed to evaluate various dosing schemes and different levels of covariates based on the pharmacokinetic/pharmacodynamic index (f%T>MIC) for the range of clinically relevant minimum inhibitory concentrations(MICs). The results showed that there may be insufficient drug use in the clinical routine drug dose regimen, and 750mg Q6h could achieve a higher treatment success rate. The blood concentrations of imipenem in ECMO patients were lower than that of non-ECMO patients, therefore dosage may need to be increased. The dosage may need adjustment for patients with CrCL ≤ 70ml/min, but dose should be lowered carefully to avoid the insufficient drug exposure. Dose adjustment is not necessary for patients within the WT ranging from 50-80 kg. Due to the large variation in PK profile of imipenem in critically ill patients, TDM should be carried out to optimize drug regimens. Full Article

mip Activity of Imipenem-Relebactam against Carbapenem-Resistant Escherichia coli Isolates from the United States in Relation to Clonal Background, Resistance Genes, Coresistance, and Region [Epidemiology and Surveillance] By aac.asm.org Published On :: 2020-04-21T08:01:10-07:00 Imipenem-relebactam (I-R) is a recently developed carbapenem–beta-lactamase inhibitor combination agent that can overcome carbapenem resistance, which has now emerged in Escherichia coli, including sequence type 131 (ST131) and its fluoroquinolone-resistant H30R subclone, the leading cause of extraintestinal E. coli infections globally. To clarify the likely utility of I-R for carbapenem-resistant (CR) E. coli infections in the United States, we characterized 203 recent CR clinical E. coli isolates from across the United States (years 2002 to 2017) for phylogroup, clonal group (including ST131, H30R, and the CTX-M-15-associated H30Rx subset within H30R), relevant beta-lactamase genes, and broth microdilution MICs for I-R and 11 comparator agents. Overall, I-R was highly active (89% susceptible), more so than all comparators except tigecycline and colistin (both 99% susceptible). I-R’s activity varied significantly in relation to phylogroup, clonal background, resistance genotype, and region. It was greatest among phylogroup B2, ST131-H30R, H30Rx, Klebsiella pneumoniae carbapenemase (KPC)-positive, and northeast U.S. isolates and lowest among phylogroup C, New Delhi metallo-β-lactamase (NDM)-positive, and southeast U.S. isolates. Relebactam improved imipenem’s activity against CR isolates within each phylogroup—especially groups A, B1, and B2—and particularly against isolates containing KPC. I-R remained substantially active against isolates coresistant to comparator agents, albeit somewhat less so than against the corresponding susceptible isolates. These findings suggest that I-R should be useful for treating most CR E. coli infections in the United States, largely independent of coresistance, although this likely will vary in relation to the local prevalence of specific E. coli lineages and carbapenem resistance mechanisms. The objective of this analysis was to compare the outcomes among patients meeting eligibility requirements based on central laboratory susceptibility versus local laboratory susceptibility. Patients with serious infections caused by imipenem-nonsusceptible, colistin-susceptible, and imipenem-REL-susceptible pathogens were randomized 2:1 to IMI-REL plus placebo or colistin plus IMI for 5 to 21 days. The primary endpoint was a favorable overall response. Key endpoints included the clinical response and all-cause mortality. We compared outcomes between the primary microbiological modified intent-to-treat (mMITT) population, where eligibility was based on central laboratory susceptibility testing, and the supplemental mMITT (SmMITT) population, where eligibility was based on local, site-level testing. The SmMITT (n = 41) and MITT (n = 31) populations had similar baseline characteristics, including sex, age, illness severity, and renal function. In both analysis populations, favorable overall response rates in the IMI-REL treatment group were >70%. Favorable clinical response rates at day 28 were 71.4% for IMI-REL and 40.0% for colistin plus IMI in the mMITT population, whereas they were 75.0% for IMI-REL and 53.8% for colistin plus IMI in the SmMITT population. Day 28 all-cause mortality rates were 9.5% for IMI-REL and 30.0% for colistin plus IMI in the mMITT population, whereas they were 10.7% for IMI-REL and 23.1% for colistin plus IMI in the SmMITT population. The outcomes in the SmMITT population were generally consistent with those in the mMITT population, suggesting that outcomes may be applicable to the real-world use of IMI-REL for treating infections caused by imipenem-nonsusceptible Gram-negative pathogens.
