Capturing insights into Ageing Singapore

Senior Research Associate Stephen Hoskins CREA The issues facing a growing silver population are gaining ground. Singapore’s baby boomers are now exiting the workforce. Would those approaching retirement have enough resources to maintain their current standard of living? Would future policy changes in healthcare, housing and pension impact their retirement adequacy? Facing a rapidly ageing population, Singapore is presented with urgent policy challenges. Yet there is very little data on the economic, health and family circumstances of older Singaporeans. In response, the Centre for Research on the Economics of Ageing, or CREA, at the Singapore Management University has been collecting monthly data on a panel of Singaporeans aged between 50 and 70 years, resulting in the Singapore Life Panel®, or SLP. SMU’s Prof Bryce Hool and Prof Rhema Vaithianathan detail the methodology by which the SLP was constructed, in a published paper titled High-Frequency Internet Survey of a Probability Sample of Older Singaporeans: The Singapore Life Panel®. In this podcast, Stephen Hoskins, Senior Research Associate at CREA introduces the SLP and how it yields rich and remarkable insights to Singapore’s ageing situation. Old age doesn’t come alone a case study on the impact of the ageing population on a Scottish local authority’s care at home service.

This research was undertaken by Stuart Fordyce as part of an MSc in Integrated Service Improvement (Health and Social Care) at the University of Edinburgh. It considers the impact of a rapidly ageing population on a Scottish local authority and its attempt to shift the focus to a more contemporary service provision. The aim is to explore what factors are inhibiting the effectiveness of enablement. Using a case study approach the research explored whether: (i) enablement is wholly effective in addressing the increase in current service demands; (ii) the approach adopted by the local authority is undermined as services are now over extended and are attempting to address competing objectives, and; (iii) the paradigmatic change across the sector in the future will materialise if organisations cannot create the capacity and infrastructure to enable change to occur

The ageing of people living with cystic fibrosis: what to expect now?

The prognosis of people with cystic fibrosis (pwCF) has improved dramatically with the introduction of cystic fibrosis transmembrane conductance regulator (CFTR) modulators (CFTRm). The ageing of the cystic fibrosis (CF) population is changing the disease landscape with the emergence of different needs and increasing comorbidities related to both age and long-term exposure to multiple treatments including CFTRm. Although the number of pwCF eligible for this treatment is expected to increase, major disparities in care and outcomes still exist in this population. Moreover, the long-term impact of the use of CFTRm is still partly unknown due to the current short follow-up and experience with their use, thus generating some uncertainties. The future spread and initiation of these drugs at an earlier stage of the disease is expected to reduce the systemic burden of systemic inflammation and its consequences on health. However, the prolonged life expectancy is accompanied by an increasing burden of age-related comorbidities, especially in the context of chronic disease. The clinical manifestations of the comorbidities directly or indirectly associated with CFTR dysfunction are changing, along with the disease dynamics and outcomes. Current protocols used to monitor slow disease progression will need continuous updates, including the composition of the multidisciplinary team for CF care, with a greater focus on the needs of the adult population.

Active ageing with technology

Seventy-year-old Theresa Chan suffers from joint degeneration, which is why she needs a walking stick.

“My joints are severely abraded. They are painful when I walk or stand. My doctor suggested I replace some joints with implants, but I don't want surgery,” she said.

Smart seniors

Interested in how gerontechnology can help improve the quality of life for older adults, Ms Chan joined the Gerontechnology Practitioner Training Course under Lingnan University’s Gerontechnology & Smart Ageing Project.

The course targets elderly people and caregivers. It introduces them to devices that help seniors get around with ease and live more independent lives.

Ms Chan was impressed by a smart walking stick that is equipped with senior-worthy features like a flashlight with an adjustable safety light, a siren for emergency use and a radio.

“I am grateful for the course because I can learn more about gerontechnology in my free time. After class, I can share my knowledge with others who have no idea about this new technology. It is modern and I want to keep up with the times and progress,” she said.

Youth opportunities

Gerontechnology has also opened up career opportunities for young people.

Cecilia Auyeung recently graduated from Lingnan University with a major in marketing. Last year she set up a social enterprise called Gatherly. It provides a platform for the elderly to teach other people handicraft skills and helps them to sell their products online.

Ms Auyeung joined the Socialpreneur Incubation Course under the Lingnan University project. It encourages the development of social enterprises in Hong Kong, gerontechnology and marketing. She quickly drew inspiration from other socialpreneurs.

“I discovered that my social enterprise has to change every two years according to market demands. For example, fabric bags are popular among youngsters. While the elderly at Gatherly are equipped with blue-dye techniques, I make use of their skills to create products that match the market situation.”

University contribution

According to the Census & Statistics Department, in 2036, the proportion of the population aged 65 and over is projected to be 31%. To meet the needs of an ageing population, the Government is promoting gerontechnology, which combines gerontology and technology. It focuses on providing effective solutions to increase vitality and quality of life.

The Lingnan University’s three-year project aims to enhance public understanding of gerontechnology and support smart ageing socialpreneurship development. The project also covers studies with the goal of building a database for gerontech products and services as well as social innovation and startups. The overall vision is to offer policy recommendations to the industry and Government.

Lingnan University Asia-Pacific Institute of Ageing Studies Project Manager Chloe Siu said: “Most of the participating students are willing to think out-of-the-box, using their creativity and knowledge learnt from their courses and to echo the needs of the society. We are trying to facilitate different stakeholders and community partners to create a co-working atmosphere.”

The university has also built a 2,000 sq ft Gerontech-X Lab to display over 40 gerontech products catered to healthcare, dining, living and transport. Anyone interested can book an appointment for a free visit from November 28.

Exercise four to five times a day to slow down ageing

Want to stay young for long? If so, start exercising four to five times a day as it may help keep your heart stay healthy and slow down ageing, according to researchers. Research showed that different sizes of arteries are affected differently by varying amounts of exercise.

While exercising for about two to three days a week for about 30 minutes may be sufficient to minimise stiffening of middle-sized arteries, exercising for about four to five days a week is required to keep the larger central arteries youthful.

The study would help "develop exercise programmes to keep the heart youthful and even turn back time on older hearts and blood vessels", said one of the study authors, Benjamin Levine from the University of Texas.

With age, arteries -- which transport blood in and out of the heart -- become prone to stiffening, increasing the risk of heart diseases.

For the study, published in The Journal of Physiology, the team examined 102 people over 60 years old, with a consistent lifelong exercise history.

The participants were divided into four groups depending on their exercise history -- Sedentary: less than 2 exercise sessions per week; Casual Exercisers: 2-3 exercise sessions per week; Committed Exercisers: 4-5 exercise sessions per week and Masters Athletes: 6-7 exercise sessions per week.

A lifelong history of casual exercise (two-three times a week) resulted in more youthful middle-sized arteries, which supply oxygenated blood to the head and neck.

However, committed exercisers (4-5 times per week) also had more youthful large central arteries, which provide blood to the chest and abdomen, in addition to healthier middle-sized ones.

Larger arteries need more frequent exercise to slow down ageing, the researchers said.

The findings will help see "if we can reverse the ageing of a heart and blood vessels by using the right amount of exercise at the right time", Levine explained.
