Birth defects are associated with preterm birth and are a major contributor to infant mortality. There are persistent black-white differences in overall infant mortality and infant mortality attributable to birth defects.

Among infants delivered at 37 to 44 weeks, blacks and Hispanics had significantly higher neonatal and postneonatal mortality attributable to birth defects than whites. Among infants delivered at 20 to 33 or 34 to 36 weeks, neonatal mortality attributable to birth defects was significantly lower among blacks. (Read the full article)

Childhood obesity is a public health concern. Although determinants of childhood overweight have been identified and their effect sizes have been calculated, prevention as well as treatment have had limited success.

We have calculated the population-based relevance of determinants of childhood overweight by using attributable risks, which can be interpreted as maximum success rates of preventive measures. The impact of relative age at school entry on academic progress and the risk of being diagnosed with ADHD remains controversial. Stimulants are widely used as a therapeutic option for ADHD in the United States and increasingly in Europe.

Relative age among classmates affects academic performance among boys and girls into puberty, as well as children’s risk of being prescribed stimulants for ADHD. Impaired visual development has been reported in infants born to mothers prescribed methadone in pregnancy. Immature visual evoked potentials have been reported in this population, but data were confounded by gestation, growth restriction, and illicit drug use.

Visual evoked potentials are small and immature in infants exposed to methadone and other drugs of misuse in utero. These changes are independently associated with methadone exposure and persist after controlling for gestation, socioeconomic deprivation, alcohol consumption, and cigarette smoking. Racial disparities in health care have been reported in multiple settings, but not thoroughly examined at the clinician level. The frequent occurrence of respiratory tract infections allows the evaluation of differences in the management of children seen by the same clinician.

Racial differences in the management of common pediatric infections occur among children treated by the same clinician. Given persistent concerns about nonjudicious antibiotic use, examining racial differences may inform our understanding of prescribing practices and identify opportunities for intervention. Central line–associated bloodstream infections (CLABSI) are common types of hospital-acquired infections associated with high morbidity and cost. In recent years, quality improvement initiatives have demonstrated how to reduce the incidence of CLABSI.

This study presents nationally representative estimates of the cost and length of stay attributable to pediatric CLABSI. We make the business case to justify quality improvement prevention initiatives and the adoption of strategies for cost-effective management of CLABSI. (Read the full article)

Many pediatric acute respiratory tract infections (ARTI) are viral and do not require antimicrobial treatment. Recent estimates of antimicrobial overprescribing for these infections, defined based on the published bacterial disease prevalence among all ARTI, are not available.

Based on the published bacterial prevalence rates for pediatric ARTI, antimicrobial agents are prescribed almost twice as often as expected to outpatients nationally, amounting to an estimated 11.4 million potentially preventable antimicrobial prescriptions annually. (Read the full article)

Off-label prescribing in children has been widely described. There has been growing awareness and action from regulatory bodies since 2006 to promote drug assessment in children and rational prescribing.

In comparison with a similar study done in 2000, there was no significant change in off-label prescribing in children. In contrast with the previous findings, off-label prescribing did not increase risk for adverse drug reactions. (Read the full article)

Functional capabilities in patients with spina bifida depend on the spinal level of the lesion and its type. Sociodemographic characteristics have been shown in other conditions to be an important additional influence on outcomes, making them important for risk adjustment.

Males, non-Hispanic blacks, and patients without private insurance have less favorable functional outcomes in spina bifida, and age also has an impact. These attributes need to be considered by clinicians and researchers and used in comparing care outcomes across clinic settings. (Read the full article)

Distribution of infant formula discharge packs to breastfeeding mothers is common practice in maternity care facilities in the United States. Receiving discharge packs is associated with shortened exclusive breastfeeding duration. Many efforts have been made to discourage this practice.

From 2007 to 2013, there has been a marked reduction in distribution of discharge packs containing infant formula to breastfeeding mothers in hospitals and birth centers in the United States. (Read the full article)

Seven percent of children in the United States receive mental health services each year. There are more pediatric outpatient mental health care visits to primary care physicians (PCPs) than to psychiatrists. Mental health utilization patterns regarding different conditions and medication prescribing are unknown.

One-third of children with mental health conditions see PCPs only. A greater proportion of children with attention-deficit/hyperactivity disorder see PCPs for this than do those with anxiety/mood disorders. Children seeing PCPs are prescribed psychotropic medications more often than those seeing psychiatrists. (Read the full article)

We report the first clinical Escherichia. coli strain EC3000 with concomitant chromosomal colistin and carbapenem resistance. A novel in-frame deletion, 6-11(RPISLR), in pmrB contributing to colistin resistance was verified using recombinant DNA techniques. Although decreased fitness compared to the wild-type (WT) strain or EC3000 revertant (chromosomal replacement of WT pmrB in EC3000), a portion of serially passaged EC3000 strains preserving colistin resistance without selective pressure raises the concern for further spread.

Aspergillus niger, the third species responsible for invasive aspergillosis has been considered as a homogeneous species until DNA-based identification uncovered many cryptic species. These species have been recently reclassified into the Aspergillus section Nigri. However little is yet known among the section Nigri about the species distribution and the antifungal susceptibility pattern of each cryptic species. A total of 112 clinical isolates collected from 5 teaching hospitals in France and phenotypically identified as A. niger were analyzed. Identification to the species level was carried out by nucleotide sequence analysis. The Minimum Inhibitory Concentrations (MICs) of itraconazole, voriconazole, posaconazole, isavuconazole and amphotericin B were determined by both the EUCAST and gradient concentration strips methods. Aspergillus tubingensis (n=51, 45.5%) and A. welwitschiae (n=50, 44.6%) were the most common species while A. niger accounted for only 6.3% (n=7). The MICs of azoles drugs were higher for A. tubingensis than for A. welwitschiae. The MIC of amphotericin B was 2 mg/L or less for all isolates. Importantly, MICs determined by EUCAST showed no correlation with those determined by gradient concentration strips methods, these latter being lower than the former (Spearman's rank correlation tests ranging - depending on the antifungal agent - from 0.01 to 0.25; p>0.4). In conclusion, A. niger should be considered as a minority species in the section Nigri. The differences in MICs between species for different azoles underline the importance of accurate identification. Significant divergences in the determination of MIC between EUCAST and gradient concentration strips methods require further investigation.

Linezolid has strong antimicrobial activity against the multidrug-resistant (MDR) strains of Mycobacterium tuberculosis. Little is known about the distribution of linezolid in tuberculosis (TB) lesions in patients with MDR-TB. The aim of this study is to evaluate the distribution of linezolid in TB lesions in patients with spinal MDR-TB. Nine patients with spinal MDR-TB were enrolled prospectively from August 2019 to February 2020. The patients received a linezolid-containing anti-TB treatment regimen and needed surgery for the removal of TB lesions. During the operation, nine blood samples, eight diseased bone tissue samples, seven pus samples and four granulation tissue samples were collected simultaneously and 2 h after the oral administration of 600 mg of linezolid. Linezolid concentrations in plasma, diseased bone tissue, pus, and granulation tissue samples were subjected to high-performance liquid chromatography–tandem mass spectrometry. At sample collection, the mean concentrations of linezolid in plasma, diseased bone tissue, pus, and granulation tissue samples of the nine patients were 11.14 ± 5.82, 5.94 ± 4.27, 11.09 ± 4.58, 14.08 ± 10.61 mg/L, respectively. The mean ratios of linezolid concentration in diseased bone/plasma, pus/plasma, and granulation/plasma were 53.84%, 91.69%, and 103.57%, respectively. The mean ratios of linezolid concentration in pus/plasma and granulation/plasma were higher than those in diseased bone/plasma, and the difference was statistically significant (t =-2.810, p = 0.015; t =-4.901, p = 0.001). In conclusion, linezolid had different concentration distributions in different types of TB infected tissues in patients with spinal MDR-TB.

Lima, Peru, Apr 23, 2020 / 10:02 am (CNA).- As the ongoing coronavirus lockdown in Peru leaves the working poor in a vulnerable situation, one priest has been working to ensure that thousands in need have access to food.

Fr. Omar Sánchez Portillo is the secretary general of Caritas Lurín, on metro Lima’s south side.

Sánchez has distributed more than 15,000 food baskets, with the help of donors and volunteers at the Beatitudes Association, which he founded, since the nationwide quarantine was declared March 15. Peru’s Ministry of Health has confirmed 19,000 cases of coronavirus with 530 deaths.

Recently, Sánchez also received a donation of 5,000 live chickens from a poultry farm. He found himself needing to quickly process the chickens for distribution.

Sánchez turned to his fellow priests in the diocese of Lurín with an appeal on Whatsapp. To his surprise, almost 30 priests showed up to volunteer, including Bishop-elect Cristobal Mejía, who was recently named bishop of Chulucanas.

The priests and other volunteers worked all day, plucking, cleaning and preparing the birds for distribution.

“Today has been a long day,” Sánchez commented on his Facebook page. “Thank you dear priests! Thank you for your example, your work, and your joy. I feel proud to belong to a such an active, alive diocese so full of God, and to be part of a presbyterate full of holiness and enthusiasm for our priestly mission.”

In a statement to ACI Prensa, CNA’s Spanish language news partner, Sánchez noted that in the Diocese of Lurín, which is comprised of 55 parishes, there are many poor people who do not have access to electricity or running water.

The priest estimates about 60% of the people in the diocese live in extreme poverty. Southern Lima, where his diocese is located, contains the third and fourth most COVID-infected areas in the country.

Sánchez also pointed out that most Peruvians lack the ability to save money, which leaves vulnerable populations even more at risk during the quarantine.

“A lot of people are out of work and out on the street, a lot of them are temporary workers, many of them earn a living day-to-day,” he said.

So far, volunteers have distributed 75,000 food baskets throughout the South Lima area. However, the needs remain great.

“Every day in the parishes there are people out looking, knocking on doors, that haven’t gotten any food, or what they have gotten isn’t enough and has already run out,” he explained.

 

CNA Staff, Apr 29, 2020 / 04:30 pm (CNA).- Venezuela’s prolonged social, political and economic crisis has only been compounded by the coronavirus pandemic, the archbishop emeritus of Caracas, Cardinal Jorge Urosa Savino, charged Tuesday.

Even before the coronavirus pandemic, Venezuela has been marred by violence and social upheaval under the socialist administration of Nicolas Maduro, with severe shortages of food and medicine, high unemployment, power outages, and hyperinflation. Some 4.5 million Venezuelans have emigrated since 2015.

In response to the threat of the virus, the government imposed a nationwide stay at home order March 17. According to government statistics, to date there have been 329 cases of COVID-19 with ten deaths. The country is ill prepared to handle the crisis, with chronic shortages of medical supplies, and many doctors have left the country.

“The national reality is terrible,” and the government has no answers, Urosa said in an April 28 statement to ACI Prensa, CNA’s Spanish-language news partner.

While the cardinal acknowledged the lockdown has prevented the spread of the virus, he pointed out that “the quarantine has hurt a great many people because the economic, social and logistical conditions in the country weren’t taken into account,” including “the extremely serious problem of the gasoline shortage for transport, especially for food.”

In some cases, crops are rotting in farmers’ fields due to lack of fuel to transport them to market.

Especially hard hit, the cardinal said, are “informal” workers who are paid off the books,  and who are now  “barely surviving,” and only with “the help of family members, social organizations and the Church.”

On April 25, Venezuelan vice president Delcy Rodríguez announced state intervention and oversight of several food supply companies in order to control the prices of 27 products for 180 days.

Urosa criticized the intervention, calling it “an extremely serious mistake, since it will probably result in greater shortages. Price controls are acceptable, but intervening in efficient businesses is not. The government can’t even manage to supply gasoline.” “The state-run petroleum industry has collapsed, and now Venezuelans’ food is in danger!” 

 “The current government doesn’t have any answers for such elementary things such as the extremely serious problem of the gasoline shortage” and runaway inflation. “In the last 40 days, the dollar has doubled in value, which is undoubtedly the fundamental cause of the spike in prices,” the cardinal said.

Urosa decried political persecution, which “has gotten worse since March because amid the quarantine, the government has ramped up the repression. During these weeks the government has jailed, even without due process, many political activists, especially from the inner circle of Juan Guaidó, president of the National Assembly and leader of the Venezuelan opposition.”

Guaidó declared himself the nation's interim leader Jan. 23 last year following Maduro’s inauguration for a second term. Maduro won a May 2018 presidential election, which was boycotted by the opposition and has been rejected by much of the international community. The United States was swift to recognize Guaidó as interim president, eventually followed by over 60 countries. Both the National Assembly and the Venezuelan bishops' conference declared Maduro's reelection to be invalid.

With the military firmly in support of Maduro, however, opposition protests calling for his resignation have failed to oust the leader.

On March 30, Guaidó charged that the Maduro regime had unleashed a new wave of harassment against his close collaborators. Andrea Bianchi, the wife of close associate Rafael Rico, was kidnapped, beaten and then left naked on a highway. Two others, Rómulo García and Víctor Silio were also picked up and later charged with possession of marijuana and a handgun.

The NGO Venezuelan Program for Education-Action in Human Rights reported that during the state of emergency, 34 people have been arbitrarily arrested and attacks against politicians, journalists and healthcare workers have increased.

“The bishops have always strongly criticized the political repression by the government and once again I call for the release of all political prisoners. They are even in greater physical danger because of the pandemic situation we’re going through,” Urosa stressed.

On March 26, “the Trump administration unsealed sweeping indictments against Venezuelan President Nicolás Maduro and members of his inner circle on narcoterrorism charges, a dramatic escalation in the U.S. campaign to force the authoritarian socialist from power,” even offering “a $15 million reward for information leading to his capture or conviction,” the Washington Post reported.

In response, the Maduro regime activated a plan against the Venezuelan opposition called “Operation Bolivarian Fury.”

The archbishop emeritus denounced these recent “threats of violence by the government against Venezuelans. Maduro himself has spoken of a supposed ‘Bolivarian fury’ as a threat against members of the Venezuelan opposition in case of international problems. That’s illegal, unconstitutional and unacceptable from every point of view. That threat of violence is intolerable.” 

The cardinal said the government has used the quarantine simply as an opportunity to strengthen its social and political control.

On April 25, the Maduro regime placed shipping containers on the Caracas-La Guaira highway to prevent demonstrators from other cities who have been protesting the shortages of food, water and electricity in other cities from getting to the capital.

“Why restrict the right to free transit?” the cardinal asked.

The Maduro regime also blocked the highway in February 2019 to prevent humanitarian aid from entering the country from Colombia.

Guaidó charged April 24 on Twitter that “a dictatorship of corrupt and incapable people has brought us to a crisis where farmers are losing their crops while families are starving to death in the barrios. They turned the richest country in the region into a hell. They’ll leave here, the sacrifice has been enough already.”

As signs of hope, Urosa pointed to ongoing work of Caritas Venezuela and the creative ways the clergy has reached out to the faithful through social media. “Our message is one of encouragement, trust in God, solidarity and hope in this dark hour,” he said.

 Catholics “have an unshakeable faith in God who is love,” who had died and risen and “has shown us the merciful face of God.” “We’ll come out of this,” the archbishop said, “the suffering we are experiencing has united us closer to God and opens to us the gates of heaven.”

The archbishop encouraged Venezuelans to always stand in solidarity with each other and “to be the face of God to those in need. God is love and is with us. Let us join ourselves to him and the Blessed Virgin Mary, Mother of Mercy in this painful hour.”

Award presented at Monaco ceremony

He’s been dubbed: “The Poet-Philosopher of Baseball,” “A Voice for the Ages,” “The Velvet Voice.” He’s been compared to Walter Cronkite, Mark Twain, and Garrison Keilor.  

In 1982, the Hollywood Walk of Fame honored him with a star among the Greats of stardom in the same year the National Baseball Hall of Fame enshrined his name among the Greats of baseball.

Vin Scully may be the very model of sartorial perfection, but it’s not the wardrobe that has endeared him to baseball for sixty-seven years. It’s his deep baritone voice and the power of his words.

A Catholic Education

There’s much to be said for childhood dreams.  At eight, when he wrote an assignment about his future, Vin imagined himself as a sports commentator. That dream has come true.

Vin Scully was born in the Bronx, N.Y. and received his elementary school education from the Sisters of Charity.  At Fordham Prep and Fordham University, both conducted by the Jesuits, his eager mind opened itself wide to the liberal arts, to Latin and Greek, science, literary and refining arts. He acted in plays, engaged in debate, learned to read and write well, and above all, to speak well; this is eloquentia perfecta, the hallmark of Jesuit education. When his head was not in books, he took up menial jobs to make ends meet delivering beer, pushing garment racks, and cleaning silver in the basement of the Pennsylvania Hotel in New York City. Vin Scully, the Renaissance man graduated from Fordham in 1949 with a major in communications.

With the Brooklyn Dodgers

In 1947, baseball executive Branch Rickey signed Jackie Robinson to play infield on the Brooklyn Dodgers team.  Against a tide of opposition, Rickey was determined to integrate the ball club.  Against a tide of opposition, the all-round athlete broke the color barrier in major league baseball at a great personal cost to him and his wife Rachel.  If you wanted to see electricity personified, you went to Ebbets Field where you could fix your eyes on Jackie at home plate and on the base pads tantalizing the opposing team.  There was nothing like it.  Said Rickey, “There was never a man in the game who could put mind and muscle together quicker than Jackie Robinson.”

Three years later, a twenty-two year old Vin Scully joined veteran Dodgers announcers, Red Barber and Connie Desmond to complete the broadcasting team.  As the rookie, Scully was assigned to announce only two innings.  Before long, he was announcing World Series games. He was not yet thirty.

Scully called Jackie Robinson “perhaps the most exciting, most driven player I’ve ever seen.” He spoke fondly of two other players:  “Gil Hodges was probably my all-time favorite.  He was as straight an arrow as they come.”  Duke Snider was “a true star. Subject to teasing from teammates. Great talent.”

When, in 1955, the Brooklyn ‘Bums’ won their only World Series, Scully stood and proclaimed: “Ladies and Gentlemen, the Brooklyn Dodgers are the champions of the world.”

Heartbreak in Brooklyn

In 1957, the Dodgers relocated from Brooklyn to Los Angeles.  For years, Walter O’Malley, the team’s major co-owner, had been searching for a more suitable land on which to build a new ballpark. When he and Robert Moses, the controversial construction coordinator of New York City, could not agree on a real estate price for a new Brooklyn location, O’Malley lost no time in accepting the offer of Los Angeles officials to purchase land suitable for building the ballpark he had wanted to build in Brooklyn.  Their departure triggered a virtual depression in Brooklyn.  The fans lost not only their team but also their beloved announcer.  Scully had pledged his loyalty to the team and followed them to Los Angeles.  

When, in 1959, the Los Angeles Dodgers honored Roy Campanella, one of the stars on the Brooklyn team, he was wheeled onto the field for a ceremony of lighting candles in his honor.  (He had been injured in a car accident leaving him paralyzed.) Vin Scully stood, and in tribute, spoke these memorable words:

“The lights are now starting to come out, like thousands and thousands of fireflies, starting deep in center field, glittering to left, and slowly, the entire ballpark. A sea of lights at the Coliseum. Let there be a prayer for every light, and wherever you are, maybe you, in silent tribute to Roy Campanella, can also say a prayer for his well-being. Campanella, for thousands of times, made the trip to the mound to help somebody out: a tired pitcher, a disgusted youngster, a boy perhaps who had his heart broken in the game of baseball. And tonight, on his last trip to the mound, the city of Los Angeles says hello.”

Honorary Doctorate from Alma Mater

In May, 2000, Vin Scully received an Honorary Doctorate of Humane Letters from Fordham University. In his address to the graduates, he shared some memories of his years at Fordham.  . . . “I was once you.  I walked the halls you walked.  I sat in the same classrooms.  I took the same notes and sweated out the final exams.  I played sports on your grassy fields. I hit a home run here—in Jack Coffey Field against CCNY—the only one I ever hit.”

Fordham, he said, evoked three words for him: home, love and hope. Home, because he spent eight years at Fordham both in the preparatory school and as an undergraduate. Love, because he made lifelong friends, and hope because Fordham is where his dreams thrived.

He urged all present “to take some time away from the craziness around you to foster the things that are important. Don’t let the winds blow away your dreams or your faith in God. And remember, sometimes your wildest dreams come true.”

In presenting the award, Michael T. Gillan, dean of Fordham College of Liberal Studies, noted that “when Jesuit schoolmasters developed their plan of studies in the 16th and 17th centuries, they defined “the goal of Jesuit education as eloquentia perfecta … which connotes a mastery of expression that is informed by good judgment and consistent principles. Those Jesuit schoolmasters of another age, if they had known anything about baseball, would certainly have approved the rhetorical gifts of the man who has been the voice of the Dodgers for the past fifty-one years, Vincent E. Scully.”

The Scully Anaphora

For years, every Scully broadcast has repeated the same avuncular opening:  “It’s time for Dodger baseball! Hi, everybody, and a very pleasant afternoon/evening to you, wherever you may be. Pull up a chair and relax;” The poet-philosopher announced the games painting vivid word pictures with his musical voice, tinged with Irish inflection.

Scully held himself to three rules:  Avoid criticizing managers and umpires; keep your personal opinions to yourself; avoid using clichés to describe a play. The Scully trademark, he insists, is silence—silence to allow the roar of the crowd to touch the listening audience.

Scully in His Own Words     

There are numerous quotes attributed to Mr. Scully. In describing Tom Glavine as a strike-out pitcher, he mused: “He’s like a tailor, a little off here, a little off there, and you’re done, take a seat.” The talent of Stan Musial of the St. Louis Cardinals amazed Scully:  “How good was Stan Musial? He was good enough to take your breath away.”

Vin was known to spin some philosophy out of the play at the moment.  In 1991, he remarked: “Andre Dawson has a bruised knee and is listed as day-to-day (Pause).  Aren’t we all?”  

Then there were his philosophical quips: “Good is not good enough when better is expected.” “Statistics are used much like a drunk who uses a lamp post—for support, not illumination.” “Losing feels worse than winning feels good.”     

One day, when Vin joked that Joe Torre might be apprehensive about returning from third base to catcher after getting hit by a foul tip.  “If he were apprehensive, Torre would forever be known as “Chicken Catcher Torre.”  At this, the crowd groaned.

Listed among Vin Scully’s many awards are:
1976 Most Memorable Personality in L.A. Dodger history by Dodger fans
1982 Induction into the National Baseball Hall of Fame as the Ford C. Frick Award recipient.
Four times, voted as the country’s Outstanding Sportscaster.  
Twenty-two times voted, as California Sportscaster of the Year by the National Sportscasters and Sportswriters Association.
2009  NAB Broadcasting Hall of Fame
2009  Ambassador Award of Excellence by the LA Sports & Entertainment Commission
2014 The Gabriel Personal Achievement Award from the Catholic Academy of Communication Professionals.

Personal Life

Vin Scully has not been one to wear his feelings in the open.  Yet, he has experienced two family tragedies.  In 1972, after fifteen years of married life, his wife died from an accidental medical overdose.  The next year, his thirty-three old son Michael was killed in a helicopter crash while working for the ARCO Transportation Company.  A chemist, he was inspecting oil pipelines for leaks near Fort Tejon.

Vin credits his strong Roman  Catholic  faith for helping him cope with family grief and then to resume his work as an announcer. “As long as you live,” he reflects, “keep smiling because it brightens everybody’s day.”

The Final Inning

The longevity of Vin Scully’s baseball life has drawn to a conclusion.  As part of a conference call before the Dodgers played the Giants on Sept 19th, Scully he spoke about all the attention he had received in the closing days of his long career: “First of all, I attribute it to one thing and one thing only,” Scully reflected, “God’s Grace to allow me to do what I’ve been doing for 67 years. To me, that’s really the story. Not really me, I’m just a vessel that was passed hand-to-hand, down through all those years. So I don’t take it to heart as some great compliment. I just realize that because I’ve been doing this for 67 years, that’s why everybody wants to talk about it. So I think I’ve kept it in proper perspective” (Courtesy of MLB Network).
