Opportunities for resident global health training and exposure are most often limited to electives or trainees in dedicated global health tracks.A series of short, structured, participatory global child health modules improved knowledge and were well received and integrated within academic programs. Such modules enable global health learning for all residents, including those who never intend to practice overseas. (Read the full article) Full Article
ule A Clinical Prediction Rule for the Severity of Congenital Diaphragmatic Hernias in Newborns By pediatrics.aappublications.org Published On :: 2014-07-14T00:07:16-07:00 Predicting high-risk populations in congenital diaphragmatic hernia (CDH) can help target care strategies. Prediction rules for infants with CDH often lack validation, are aimed at a prenatal population, and are of limited generalizability. We cannot currently discriminate the highest risk neonates during the crucial period shortly after birth. This clinical prediction rule was developed and validated on an international database. It discriminates patients and high, intermediate, and low risk of mortality; is easy to apply; and is generalizable to most infants with CDH. (Read the full article) Full Article

ule Physician Response to Parental Requests to Spread Out the Recommended Vaccine Schedule By pediatrics.aappublications.org Published On :: 2015-03-02T00:05:25-08:00 Some parents choose to "spread out" the recommended vaccine schedule for their child by decreasing the number of simultaneous vaccines or delaying certain vaccines until an older age. Epidemiologic studies demonstrate increasing numbers of parents are choosing to delay vaccines. We demonstrate that almost all providers encounter requests to spread out vaccines in a typical month and, despite concerns, increasing numbers are agreeing to do so. Providers report many strategies in response to requests but think few are effective. (Read the full article) Full Article
ule Derivation of Candidate Clinical Decision Rules to Identify Infants at Risk for Central Apnea By pediatrics.aappublications.org Published On :: 2015-10-19T00:07:48-07:00 Central apnea sometimes complicates bronchiolitis. Because apnea tends to occur early in the course of bronchiolitis, there is a danger that infants may be discharged from the emergency department only to subsequently develop apnea at home. This study prospectively derived clinical decision rules to help emergency physicians admit infants at risk for apnea while discharging those not at risk. ule As dating apps try to pivot to virtual events, some users are trying to get people to violate social distancing rules By feedproxy.google.com Published On :: Sat, 09 May 2020 12:50:00 GMT Source: www.businessinsider.com - Saturday, May 09, 2020 Dating apps like Tinder, Bumble, and Hinge have reported increased use amid the coronavirus pandemic, while touting virtual dating alternatives for users instead of meeting up in person. Swaths of users are still encouraging matches to break quarantine to have sex and go on dates, despite social distancing guidelines and fines to comply with them. An illustrator on Instagram has been collecting screenshots of these situations, and told Business Insider that users will brand themselves as "badasses," dispute the effectiveness of isolating, and lash out in anger and hurl abusive language when they're rejected. Spokespeople for Grindr, Tinder and Bumble told Business Insider they've informed users to adhere to social-distancing guidelines, but did not respond to inquiries about actions they're taking against users in places where violating lockdown orders can be against the law. Visit Business Insider's homepage for more stories . As millions remain confined to their homes to prevent the spread of coronavirus, the desire for human contact and connection has risen dramatically and led some to search for ways to break those social distancing rules. Popular dating apps — including Tinder , Bumble , and Hinge — have reported significant increases during the outbreak of swiping activity, matches between users, and messages exchanged. It's also led to the introduction of a breed of users who are interested in shirking lockdown orders, anAll Related Full Article
ule Sundeck BBQs, trivia nights, and strict hygiene rules: An inside look at the life of crews locked down on luxury superyachts By feedproxy.google.com Published On :: Sat, 09 May 2020 15:00:00 GMT Source: www.businessinsider.com - Saturday, May 09, 2020 Many superyacht crew members have been stuck on their vessels after lockdowns and travel bans were instated due to the coronavirus pandemic. We spoke to crew members who felt 'lucky' to be quarantined in such luxury digs — and grateful to still have jobs and secure wages. There is still plenty of work to be done, but workers also described trivia games, sundeck BBQs, and hot tub nights they're using to pass the time and bond with colleagues. Strict new hygiene and sanitation rules have also been put into place. Visit Business Insider's homepage for more stories . In late March, billionaire businessman David Geffen posted a shot on Instagram taken from his 452-foot superyacht Rising Sun. It was hastily deleted — but not before it had raised more than a few eyebrows and stirred discourse around the 1% and their self-isolation privilege. But it's not just the rare billionaire who is bunkering down on their yacht during the COVID-19 pandemic. Here is another group of people for whom this is a new reality: the yacht's crews. Before this crisis, there were around 80,000 people employed on pleasure craft around the globe, according to Laurence Lewis, CEO of YPI Crew, a yacht recruitment agency. As countries rushed to close their borders, ports were closed — and overnight, many found themselves unable to travel to get to or leave their yachts. For many, a superyacht sounds like the dream scenario to see out lockdown. But is it? "TherAll Related Full Article

ule Fenbendazole controls in vitro growth, virulence potential and animal infection in the Cryptococcus model [Experimental Therapeutics] By aac.asm.org Published On :: 2020-04-06T08:49:25-07:00 The human diseases caused by the fungal pathogens Cryptococcus neoformans and C. gattii are associated with high indices of mortality, and toxic and/or cost-prohibitive therapeutic protocols. The need for affordable antifungals to combat cryptococcal disease is unquestionable. Previous studies suggested benzimidazoles as promising anti-cryptococcal agents combining low cost and high antifungal efficacy, but their therapeutic potential has not been demonstrated so far. In this study, we investigated the antifungal potential of fenbendazole, the most effective anti-cryptococcal benzimidazole. Fenbendazole was inhibitory against 17 different isolates of C. neoformans and C. gattii at a low concentration. The mechanism of anti-cryptococcal activity of fenbendazole involved microtubule disorganization, as previously described for human parasites. In combination with fenbendazole, the concentrations of the standard antifungal amphotericin B required to control cryptococcal growth were lower than those required when this antifungal was used alone. Fenbendazole was not toxic to mammalian cells. During macrophage infection, the anti-cryptococcal effects of fenbendazole included inhibition of intracellular proliferation rates and reduced phagocytic escape through vomocytosis. Fenbendazole deeply affected the cryptococcal capsule. In a mice model of cryptococcosis, the efficacy of fenbendazole to control animal mortality was similar to that observed for amphotericin B. These results indicate that fenbendazole is a promising candidate for the future development of an efficient and affordable therapeutic tool to combat cryptococcosis. Full Article
ule A novel class of chikungunya virus small molecule inhibitors that targets the viral capping machinery [Antiviral Agents] By aac.asm.org Published On :: 2020-04-27T08:14:57-07:00 Despite the worldwide re-emergence of the chikungunya virus (CHIKV) and the high morbidity associated with CHIKV infections, there is no approved vaccine or antiviral treatment available. We here aim to identify the target of a novel class of CHIKV inhibitors i.e. CHVB series. CHVB compounds inhibit the in vitro replication of CHIKV isolates with 50% effective concentrations in the low micromolar range. A CHVB-resistant variant (CHVBres) was selected that carried two mutations in the gene encoding nsP1 (responsible for viral RNA capping), one mutation in nsP2 and one mutation in nsP3. Reverse genetics studies demonstrated that both nsP1 mutations were necessary and sufficient to achieve ~18-fold resistance, suggesting that CHVB targets viral mRNA capping. Interestingly, CHVBres was cross-resistant to the previously described CHIKV capping inhibitors from the MADTP series, suggesting they share a similar mechanism of action. In enzymatic assays, CHVB inhibited the methyltransferase and guanylyltransferase activities of alphavirus nsP1 proteins. To conclude, we identified a class of CHIKV inhibitors that targets the viral capping machinery. The potent anti-CHIKV activity makes this chemical scaffold a potential candidate for CHIKV drug development. Full Article
ule Validation of a Prediction Rule for Mortality in Congenital Diaphragmatic Hernia By pediatrics.aappublications.org Published On :: 2020-04-01T01:00:57-07:00 BACKGROUND: Congenital diaphragmatic hernia (CDH) is a rare congenital anomaly with a mortality of ~27%. The Congenital Diaphragmatic Hernia Study Group (CDHSG) developed a simple postnatal clinical prediction rule to predict mortality in newborns with CDH. Our aim for this study is to externally validate the CDHSG rule in the European population and to improve its prediction of mortality by adding prenatal variables. METHODS: We performed a European multicenter retrospective cohort study and included all newborns diagnosed with unilateral CDH who were born between 2008 and 2015. Newborns born from November 2011 onward were included for the external validation of the rule (n = 343). To improve the prediction rule, we included all patients born between 2008 and 2015 (n = 620) with prenatally diagnosed CDH and collected pre- and postnatal variables. We build a logistic regression model and performed bootstrap resampling and computed calibration plots. RESULTS: With our validation data set, the CDHSG rule had an area under the curve of 79.0%, revealing a fair predictive performance. For the new prediction rule, prenatal herniation of the liver was added, and absent 5-minute Apgar score was taken out. The new prediction rule revealed good calibration, and with an area under the curve of 84.6%, it had good discriminative abilities. CONCLUSIONS: In this study, we externally validated the CDHSG rule for the European population, which revealed fair predictive performance. The modified rule, with prenatal liver herniation as an additional variable, appears to further improve the model’s ability to predict mortality in a population of patients with prenatally diagnosed CDH.
