Tumoral and immune heterogeneity in an anti-PD-1-responsive glioblastoma: a case study [RESEARCH REPORT]

Clinical benefit of immune checkpoint blockade in glioblastoma (GBM) is rare, and we hypothesize that tumor clonal evolution and the immune microenvironment are key determinants of response. Here, we present a detailed molecular characterization of the intratumoral and immune heterogeneity in an IDH wild-type, MGMT-negative GBM patient who plausibly benefited from anti-PD-1 therapy with an unusually long 25-mo overall survival time. We leveraged multiplex immunohistochemistry, RNA-seq, and whole-exome data from the primary tumor and three resected regions of recurrent disease to survey regional tumor-immune interactions, genomic instability, mutation burden, and expression profiles. We found significant regional heterogeneity in the neoantigenic and immune landscape, with a differential T-cell signature among recurrent sectors, a uniform loss of focal amplifications in EGFR, and a novel subclonal EGFR mutation. Comparisons with recently reported correlates of checkpoint blockade in GBM and with TCGA-GBM revealed appreciable intratumoral heterogeneity that may have contributed to a differential PD-1 blockade response.

Lipid Trait Variants and the Risk of Non-Hodgkin Lymphoma Subtypes: A Mendelian Randomization Study

Background:

Lipid traits have been inconsistently linked to risk of non-Hodgkin lymphoma (NHL). We examined the association of genetically predicted lipid traits with risk of diffuse large B-cell lymphoma (DLBCL), chronic lymphocytic leukemia (CLL), follicular lymphoma (FL), and marginal zone lymphoma (MZL) using Mendelian randomization (MR) analysis.

Methods:

Genome-wide association study data from the InterLymph Consortium were available for 2,661 DLBCLs, 2,179 CLLs, 2,142 FLs, 824 MZLs, and 6,221 controls. SNPs associated (P < 5 x 10–8) with high-density lipoprotein (HDL, n = 164), low-density lipoprotein (LDL, n = 137), total cholesterol (TC, n = 161), and triglycerides (TG, n = 123) were used as instrumental variables (IV), explaining 14.6%, 27.7%, 16.8%, and 12.8% of phenotypic variation, respectively. Associations between each lipid trait and NHL subtype were calculated using the MR inverse variance–weighted method, estimating odds ratios (OR) per standard deviation and 95% confidence intervals (CI).

Results:

HDL was positively associated with DLBCL (OR = 1.14; 95% CI, 1.00–1.30) and MZL (OR = 1.09; 95% CI, 1.01–1.18), while TG was inversely associated with MZL risk (OR = 0.90; 95% CI, 0.83–0.99), all at nominal significance (P < 0.05). A positive trend was observed for HDL with FL risk (OR = 1.08; 95% CI, 0.99–1.19; P = 0.087). No associations were noteworthy after adjusting for multiple testing.

Conclusions:

We did not find evidence of a clear or strong association of these lipid traits with the most common NHL subtypes. While these IVs have been previously linked to other cancers, our findings do not support any causal associations with these NHL subtypes.

Impact:

Our results suggest that prior reported inverse associations of lipid traits are not likely to be causal and could represent reverse causality or confounding.

Total Antioxidant Capacity and Pancreatic Cancer Incidence and Mortality in the Prostate, Lung, Colorectal, and Ovarian Cancer Screening Trial

Background:

Total antioxidant capacity (TAC) reflects an individual's overall antioxidant intake. We sought to clarify whether higher TAC is associated with lower risks of pancreatic cancer incidence and mortality in the U.S. general population.

Methods:

A total of 96,018 American adults were identified from the Prostate, Lung, Colorectal, and Ovarian Cancer Screening Trial. A ferric-reducing ability of plasma score was used to reflect an individual's TAC intake from diet and/or supplements. Cox regression was used to calculate hazard ratios (HR) for pancreatic cancer incidence, and competing risk regression was used to calculate subdistribution HRs for pancreatic cancer mortality. Restricted cubic spline regression was used to test nonlinearity.

Results:

A total of 393 pancreatic cancer cases and 353 pancreatic cancer–related deaths were documented. Total (diet + supplements) TAC was found to be inversely associated with pancreatic cancer incidence (HR quartile 4 vs. quartile 1 = 0.53; 95% confidence interval, 0.39–0.72; Ptrend = 0.0002) and mortality (subdistribution HR quartile 4 vs. quartile 1 = 0.52; 95% confidence interval 0.38–0.72; Ptrend = 0.0003) in a nonlinear dose–response manner (all Pnonlinearity < 0.01). Similar results were observed for dietary TAC. No association of supplemental TAC with pancreatic cancer incidence and mortality was found.

Conclusions:

In the U.S. general population, dietary but not supplemental TAC level is inversely associated with risks of pancreatic cancer incidence and mortality in a nonlinear dose–response pattern.

Impact:

This is the first prospective study indicating that a diet rich in antioxidants may be beneficial in decreasing pancreatic cancer incidence and mortality.

Adeno-associated virus (AAV) recombinants are currently the vector of choice for many gene therapy applications. As experimental therapies progress to clinical trials, the need to characterize recombinant adeno-associated viruses (rAAVs) accurately and reproducibly increases. Accurate determination of rAAV infectious titer is important for determining the activity of each vector lot and for ensuring lot-to-lot consistency. The following protocol developed in our laboratory uses a 96-well TCID50 format and quantitative polymerase chain reaction (qPCR) detection for the determination of rAAV infectious titer.

Titration of Recombinant Adeno-Associated Virus (rAAV) Genome Copy Number Using Real-Time Quantitative Polymerase Chain Reaction (qPCR)

This protocol is used to determine the concentration of DNase-resistant vector genomes (i.e., packaged in the capsid) in purified recombinant adeno-associated virus (rAAV) preparations. The protocol begins with treatment of the vector stock with DNase I to eliminate unencapsidated AAV DNA or contaminating plasmid DNA. This is followed by a heat treatment to heat-inactivate DNase I, to disrupt the viral capsid, and to release the packaged vector genomes for quantification by real-time polymerase chain reaction (PCR) using a set of standards (linearized plasmid used for vector production) containing known copy numbers. To accomplish high-throughput titration, the primer and probe sets used in real-time PCR are usually designed to target common elements present in most rAAV genomes, such as promoters and poly(A) signals. This strategy significantly reduces the number of PCRs, controls, and turnaround time. Several important controls should be included in the assay as follows: The first two controls should have a known copy number of the rAAV genome plasmid treated or not treated with DNase I. This control tests the effectiveness of DNase treatment. To control for potential cross-contamination between samples during the preparation process, a blank control containing nuclease-free water only should be processed and tested in parallel. A validation vector sample with a known titer should be included in every assay to monitor interassay variability. Finally, for the PCR run, a no-template control (NTC) is included to indicate cross-contamination during PCR setup.

[PERSPECTIVES] Regulating Preimplantation Genetic Testing across the World: A Comparison of International Policy and Ethical Perspectives

Preimplantation genetic testing (PGT) is a reproductive technology that, in the course of in vitro fertilization (IVF), allows prospective parents to select their future offspring based on genetic characteristics. PGT could be seen as an exercise of reproductive liberty, thus potentially raising significant socioethical and legal controversy. In this review, we examine—from a comparative perspective—variations in policy approaches to the regulation of PGT. We draw on a sample of 19 countries (Australia, Austria, Belgium, Brazil, Canada, China, France, Germany, India, Israel, Italy, Japan, Mexico, Netherlands, Singapore, South Korea, Switzerland, United Kingdom, and the United States) to provide a global landscape of the spectrum of policy and legislative approaches (e.g., restrictive to permissive, public vs. private models). We also explore central socioethical and policy issues and contentious applications, including permissibility criteria (e.g., medical necessity), nonmedical sex selection, and reproductive tourism. Finally, we further outline genetic counseling requirements across policy approaches.

Comparison of Enhancement of the Vestibular Perilymph between Variable and Constant Flip Angle-Delayed 3D-FLAIR Sequences in Meniere Disease [HEAD & NECK]

BACKGROUND AND PURPOSE:

Endolymphatic hydrops in patients with Menière disease relies on delayed postcontrast 3D-FLAIR sequences. The purpose of this study was to compare the degree of perilymphatic enhancement and the detection rate of endolymphatic hydrops using constant and variable flip angles sequences.

MATERIALS AND METHODS:

This was a retrospective study performed in 16 patients with Menière disease who underwent 3T MR imaging 4 hours after gadolinium injection using two 3D-FLAIR sequences with a constant flip angle at 140° for the first and a heavily-T2 variable flip angle for the second. The signal intensity ratio was measured using the ROI method. We graded endolymphatic hydrops and evaluated the cochlear blood-labyrinth barrier impairment.

RESULTS:

Both for symptomatic and asymptomatic ears, the median signal intensity ratio was significantly higher with the constant flip angle than with the heavily-T2 variable flip angle (7.16 versus 1.54 and 7.00 versus 1.45, P < .001). Cochlear blood-labyrinth barrier impairment was observed in 4/18 symptomatic ears with the heavily-T2 variable flip angle versus 8/19 with constant flip angle sequences. With heavily-T2 variable flip angle sequences, endolymphatic hydrops was observed in 7–10/19 symptomatic ears versus 12/19 ears with constant flip angle sequences. We found a significant association between the clinical symptomatology and the presence of endolymphatic hydrops with constant flip angle but not with heavily-T2 variable flip angle sequences. Interreader agreement was always perfect with constant flip angle sequences while it was fair-to-moderate with heavily-T2 variable flip angle sequences.

CONCLUSIONS:

3D-FLAIR constant flip angle sequences provide a higher signal intensity ratio and are superior to heavily-T2 variable flip angle sequences in reliably evaluating the cochlear blood-labyrinth barrier impairment and the endolymphatic space.

CT Angiography in Evaluating Large-Vessel Occlusion in Acute Anterior Circulation Ischemic Stroke: Factors Associated with Diagnostic Error in Clinical Practice [INTERVENTIONAL]

BACKGROUND AND PURPOSE:

It is currently not completely clear how well radiologists perform in evaluating large-vessel occlusion on CTA in acute ischemic stroke. The purpose of this study was to investigate potential factors associated with diagnostic error.

MATERIALS AND METHODS:

Five hundred twenty consecutive patients with a clinical diagnosis of acute ischemic stroke (49.4% men; mean age, 72 years) who underwent CTA to evaluate large-vessel occlusion of the proximal anterior circulation were included. CTA scans were retrospectively reviewed by a consensus panel of 2 neuroradiologists. Logistic regression analysis was performed to investigate the association between several variables and missed large-vessel occlusion at the initial CTA interpretation.

RESULTS:

The prevalence of large-vessel occlusion was 16% (84/520 patients); 20% (17/84) of large-vessel occlusions were missed at the initial CTA evaluation. In multivariate analysis, non-neuroradiologists were more likely to miss large-vessel occlusion compared with neuroradiologists (OR = 5.62; 95% CI, 1.06–29.85; P = .04), and occlusions of the M2 segment were more likely to be missed compared with occlusions of the distal internal carotid artery and/or M1 segment (OR = 5.69; 95% CI, 1.44–22.57; P = .01). There were no calcified emboli in initially correctly identified large-vessel occlusions. However, calcified emboli were present in 4 of 17 (24%) initially missed or misinterpreted large-vessel occlusions.

CONCLUSIONS:

Several factors may have an association with missing a large-vessel occlusion on CTA, including the CTA interpreter (non-neuroradiologists versus neuroradiologists), large-vessel occlusion location (M2 segment versus the distal internal carotid artery and/or M1 segment), and large-vessel occlusion caused by calcified emboli. Awareness of these factors may improve the accuracy in interpreting CTA and eventually improve stroke outcome.

Hippocampal Sclerosis Detection with NeuroQuant Compared with Neuroradiologists [FUNCTIONAL]

BACKGROUND AND PURPOSE:

NeuroQuant is an FDA-approved software that performs automated MR imaging quantitative volumetric analysis. This study aimed to compare the accuracy of NeuroQuant analysis with visual MR imaging analysis by neuroradiologists with expertise in epilepsy in identifying hippocampal sclerosis.

MATERIALS AND METHODS:

We reviewed 144 adult patients who underwent presurgical evaluation for temporal lobe epilepsy. The reference standard for hippocampal sclerosis was defined by having hippocampal sclerosis on pathology (n = 61) or not having hippocampal sclerosis on pathology (n = 83). Sensitivities, specificities, positive predictive values, and negative predictive values were compared between NeuroQuant analysis and visual MR imaging analysis by using a McNemar paired test of proportions and the Bayes theorem.

RESULTS:

NeuroQuant analysis had a similar specificity to neuroradiologist visual MR imaging analysis (90.4% versus 91.6%; P = .99) but a lower sensitivity (69.0% versus 93.0%, P < .001). The positive predictive value of NeuroQuant analysis was comparable with visual MR imaging analysis (84.0% versus 89.1%), whereas the negative predictive value was not comparable (79.8% versus 95.0%).

CONCLUSIONS:

Visual MR imaging analysis by a neuroradiologist with expertise in epilepsy had a higher sensitivity than did NeuroQuant analysis, likely due to the inability of NeuroQuant to evaluate changes in hippocampal T2 signal or architecture. Given that there was no significant difference in specificity between NeuroQuant analysis and visual MR imaging analysis, NeuroQuant can be a valuable tool when the results are positive, particularly in centers that lack neuroradiologists with expertise in epilepsy, to help identify and refer candidates for temporal lobe epilepsy resection. In contrast, a negative test could justify a case referral for further evaluation to ensure that false-negatives are detected.

In this country, however, the High River plant has an extremely high profile. It is one of the epicentres of COVID-19 in Canada -- in all of North America, in fact -- with over 900 reported cases out of 2,000 employees. That's almost half the workforce.  

Two people have died in connection with the Cargill outbreak -- one, a plant worker originally from Vietnam; the other, an infected plant worker's father, who had been visiting from the Philippines.

Cargill initially resisted pleas from workers and their union to close the plant, but finally relented, in late April. After only two weeks, it hastily reopened, on Monday, May 4, giving the largely immigrant workforce the Hobson's choice of either going back to a potentially fatal workplace or losing their jobs. 

Neither the workers, nor their union think the plant has become safe. 

The union, the United Food and Commercial Workers (UFCW), has gone to court to force a shutdown, until Cargill can absolutely guarantee safe and healthy conditions for all employees. 

The UFCW does not think the notoriously low-paid plant workers should have to risk their lives to fatten the balance sheet of a U.S.-based transnational corporation that ranks number 15 on the Fortune 500. 

Kenney and Trump on the same wavelength

Alberta Premier Jason Kenney has a different view from that of the union and the workers it represents. 

The premier, and former Harper Conservative government cabinet minister, appropriates a concept meant to describe access to necessary basic foodstuffs we all need for sustenance – food security – and applies it to the much different situation of the High River plant. The Cargill workers have to do their part, the Alberta premier argues, to ensure food security for Canadians. 

The truth is that Canada's food security does not depend on meat from Cargill or any other commercial operation. 

If our local butcher runs out of hamburger for the barbecue, we all have other nutritious options. There are, for instance, the protein-packed pulses -- chickpeas, lentils and the like -- that farmers in Saskatchewan grow in great quantity. 

In the U.S., as in Canada, COVID-19 has been particularly hard on the meat-packing industry, forcing more than 20 plant closures, and causing meat shortages on grocery shelves. Some fast food chains have even had to take hamburgers off the menu. 

Corporate executives in the meat industry told U.S. President Trump that they were reluctant to reopen their U.S.-based plants for fear of lawsuits. The U.S. is a far more litigious country than Canada. 

The president's response was to give the corporations cover, by invoking the U.S. Defense Protection Act (DPA). In effect, the president is forcing the corporations to reopen their plants. 

The purpose of the DPA is to allow a president to harness the resources of private industry to serve public needs in time of war or national emergency. Many have urged Trump to invoke the act to assure production of personal protective equipment for front-line workers during the pandemic, but he has refused. 

Now, Trump is using the extraordinary powers of the DPA to force workers back to dangerous plants, while shielding their bosses from responsibility.

As for the High River Cargill plant workers, they fall under provincial labour jurisdiction. And the Alberta premier has already indicated he will not lift a finger to protect them. But there might be a way that federal authorities could step in.

Jagmeet Singh urges Trudeau government to act

In Canada, it is the federal government that has authority over food safety, and NDP Leader Jagmeet Singh believes the Trudeau team should assertively use that power to protect the Cargill workers.

Singh put the question to Deputy Prime Minister Chrystia Freeland during the House of Commons' weekly face-to-face session on Wednesday, May 6.

"Food safety and worker safety cannot be divorced," Singh told the House. "Will the government ensure that the Cargill workers are in safe work conditions?"

Freeland, in a manner all-too-typical of Liberal politicians, dissimulated, offering sympathy but no action.

"The member opposite is quite right that where the federal government has particular authority in food processing is to guarantee the safety of the foods processed there for Canadians to eat," she said, and then expressed some vague sentiments of concern. "When it comes to Cargill and food processing, I agree with the member opposite that it's something we all need to be particularly concerned about, and we have been."

The NDP leader was not satisfied. 

"Will the government commit to using the authority that it has under food safety to ensure that workers are also safe, because there's no way that food can truly be safe if workers are in dangerous conditions and if workers are contracting COVID-19?" Singh asked, adding: "If workers are dying, the food can't be safe."

Freeland would not budge. The Trudeau government wants to get credit for caring, without pushing the envelope in dealing with the most prickly and confrontational provincial government in the country, Alberta's. 

"I think we all understand there is a very clear difference between the duty to inspect food which is produced and to ensure that that food is safe for Canadians, and even more sacred duty to ensure that workers are working in safe conditions," Freeland answered. "We take both of those extremely seriously and we are aware what falls specifically in our jurisdictions. Having said that, we care very much about all Canadian workers." 

Freeland's assertion that responsibility for the safety of a product that consumers eat does not include making sure a processing plant is not an active breeder of a deadly virus reflects a narrow and limited understanding of the federal role. 

There is no evidence of food borne transmission of COVID-19, or of food packaging carrying the virus, according to authorities in both the U.S. and Canada.

But experts have not always got it right about COVID-19 since the outbreak at the beginning of this year. At this stage, all we know for sure is that there remain many unanswered questions about it.

'The worst company in the world'

What is not in doubt is the kind of company we're dealing with. 

Not too long ago the U.S. environmental organization Mighty Earth undertook a study of the social and environmental impact of Cargill's operations and issued a report they called "The Worst Company in the World."

The report opens by stating "when it comes to addressing the most important problems facing our world, including the destruction of the natural environment, the pollution of our air and water, the warming of the globe, the displacement of Indigenous peoples, child labor, and global poverty, Cargill is not only consistently in last place, but is driving these problems at a scale that dwarfs their closest competitors."

The report details how Cargill has become more powerful than governments and has betrayed repeated promises to adhere to high environmental standards. 

"Nowhere is Cargill's pattern of deception and destruction more apparent than in its participation in the destruction of the lungs of the planet, the world's forests. Despite repeated and highly publicized promises to the contrary, Cargill has continued to bulldoze ancient ecosystems, sometimes within the bounds of lax laws -- and, too often, outside those bounds as well."

With the advent to power of virulently anti-environmental Trump in the U.S. and Jair Bolsonaro in Brazil, there is now virtually no limit, Mighty Earth says, to Cargill's capacity to ravage rainforests, savannahs and other vital habitats. 

Mighty Earth cites many examples. 

One of those is that of "the Gran Chaco, a 110-million-hectare ecosystem spanning Argentina, Bolivia, and Paraguay." 

This ecosystem "is one of the largest remaining continuous tracts of native vegetation in South America, second in size only to the Amazon rainforest. These forests are home to vibrant communities of Indigenous Peoples … who have depended on and coexisted with the Chaco forest for millennia."

Cargill, the report tells us, is now actively endangering both the people and other inhabitants of the Gran Chaco to produce a cash crop -- soy -- that feeds the animals which become Big Macs and Whoppers.

"Once the impenetrable stronghold of creatures like the screaming hairy armadillo, the jaguar, and the giant anteater, Cargill has infiltrated the Gran Chaco, bulldozing and burning to make way for vast fields of genetically modified soy."

Mighty Earth also documents Cargill's use of violence to subdue Indigenous peoples, its exploitative labour practices, including child labour, and its predatory practices that have driven competitors out of certain businesses. 

This is the company that Jason Kenney says must be allowed to operate, uninhibited by health concerns, to assure our food security. 

If you believe that, you might also believe that injecting bleach into your veins can cure COVID-19, or that, as many opinion leaders in the U.S. say, it is necessary to accept that thousands must die in the interests of what they call the economy. 

The owners of Cargill are not personally offering to sacrifice their lives. They are offering their employees' lives instead.

GTAV Tops the EU PlayStation Store Downloads in April 2020, Final Fantasy VII Remake Debuts in 4th
