We investigated associations of abdominal SMI, VAT, and SAT with overall mortality among men and among women with stage I–III colorectal cancer. Methods: SMI, VAT, and SAT were assessed from abdominal CT images for 1,998 patients with stage I–III colorectal cancer diagnosed between 2006 and 2015. Restricted cubic splines (RCS) were used to investigate associations of SMI, VAT, and SAT with overall mortality. Results: Average age of the participants was 67.9 ± 10.6 years and 58% were men. During a median follow-up of 4.3 years, 546 (27%) patients died. Among men, the association of SMI and mortality was statistically significant in a nonlinear way in the RCS analyses, with lower SMI levels associated with higher mortality. SMI was not associated with mortality among women. SAT was associated with mortality in a nonlinear way for men and for women, with lower SAT levels being associated with higher mortality. VAT was not significantly associated with mortality in men or women. Conclusion: Associations of abdominal skeletal muscle mass with mortality among patients with colorectal cancer were not the same for men and for women. Impact: This study stresses the importance for more attention on sex-related differences in body composition and cancer outcomes. Full Article

Methods: We sought to quantify and assess heterogeneity of the preferences of AML patients for treatment outcomes. An AML-specific discrete choice experiment (DCE) was developed involving multiple stakeholders. Attributes included in the DCE were event-free survival (EFS), complete remission (CR), time in the hospital, short-term side effects, and long-term side effects. Continuously coded conditional, stratified, and latent-class logistic regressions were used to model preferences of 294 patients with AML. Results: Most patients were white (89.4%) and in remission (95.0%). A 10% improvement in the chance of CR was the most meaningful offered benefit (P < 0.001). Patients were willing to trade up to 22 months of EFS or endure 8.7 months in the hospital or a two-step increase in long-term side effects to gain a 10% increase in chance of CR. Patients diagnosed at 60 years or older (21.6%) more strongly preferred to avoid short-term side effects (P = 0.03). Latent class analysis showed significant differences of preferences across gender and insurance status. Conclusions: In this national sample of mostly AML survivors, patients preferred treatments that maximized chance at remission; however, significant preference heterogeneity for outcomes was identified. Age and gender may affect patients' preferences. Impact: Survivor preferences for outcomes can inform patient-focused drug development and shared decision-making. Further studies are necessary to investigate the use of DCEs to guide treatment for individual patients. Full Article

In 2018, the American Cancer Society published a guideline update recommending a reduction in the colorectal cancer screening start age for average-risk individuals from 50 to 45. Implementing these recommendations would have important implications for public health. However, the approximate number of people impacted by this change, the average-risk population ages 45–49, is not well-described in the literature. Here, we provide methodology to conservatively estimate the average-risk and screening-eligible population in the United States, including those who would be impacted by a lowered colorectal cancer screening start age. Using multiple data sources, we estimated the current average-risk population by subtracting individuals with symptomatic colorectal cancer, with a family history of colorectal cancer, and with inflammatory bowel disease and hereditary nonpolyposis colorectal cancer from the total population. Within this population, we estimated the number of screening-eligible individuals by subtracting those with previous colorectal cancer screening (45- to 49-year-old) or up to date with colorectal cancer screening (50- to 74-year-old). The total average-risk population is estimated between 102.1 and 106.5 million people, of whom 43.4–45.2 million people are eligible for colorectal cancer screening. Lowering the screening age would add roughly 19 million people to the average-risk population and increase the current number of screening-eligible individuals on immediate implementation by over 60% (from 27 to 44 million). Estimating the population size impacted by lowering the recommended colorectal cancer screening start age enables more accurate decision-making for policymakers and epidemiologists focused on cancer prevention. age Intermixed Dimethyl-Sulfoxide-Based Nonadhesive Liquid Embolic Agents Delivered Serially via the Same Microcatheter for Cerebral AVM Treatment [INTERVENTIONAL] By www.ajnr.org Published On :: 2020-04-09T06:30:48-07:00 BACKGROUND AND PURPOSE: Conventional nonadhesive liquid embolic agents currently are the criterion standard for endovascular embolization of cerebral AVMs. However, inadequate distal penetration into the nidus and unstable proximal plug formation are the major limitations of this approach and of the currently available embolic materials. The aim of this study was to evaluate the hypothetic efficacy of combining liquid embolic agents with different properties and viscosities for use in endovascular embolization of cerebral AVMs. MATERIALS AND METHODS: From March 2018 to March 2019, sixteen patients with cerebral AVMs (12 women, 4 men; age range, 33–61 years) underwent endovascular embolization with combined liquid embolic agents delivered serially via a single microcatheter. The procedure consists of initial embolization with PHIL 30%, followed by Menox 18 through the same microcatheter. According to the Spetzler-Martin scale, 11 (68.75%) AVMs were grades I–II, 4 (25%) were grade III, and 1 (6.25%) was grade IV. Angiographic, technical, and clinical outcomes were analyzed independently. RESULTS: Combined PHIL and Menox embolization through the same microcatheter via 21 pedicles was performed in these 16 patients. Once the length of the reflux reached approximately 2 cm, PHIL 30% was switched to Menox 18. Antegrade flow and distal penetration of the serially applied liquid embolic agents were observed in all 16 cases. The ability to completely control the flow of the materials and avoid any dangerous proximal reflux was noted in all performed embolizations. The estimated average size reduction of the treated AVMs was 85%, ranging from 50% to 100%. Complete embolization was achieved in 10/16 or 62.5% of the cases. There was no procedure-related complication during or after the embolization. No mortality or postprocedural clinical worsening was seen. Clinical success and complete obliteration were confirmed with at least 1 follow-up angiography in 10/16 patients. CONCLUSIONS: Serial delivery of nonadhesive liquid embolic agents via the same microcatheter was safe and effective in our study and may be a potential technique for routine AVM treatment. However, further investigations are required to validate the safety and the efficacy of the method. Full Article

This study aimed to identify the incidence of postcontrast acute kidney injury using baseline renal function when CTA and cerebral angiography were performed sequentially. MATERIALS AND METHODS: This retrospective observational study evaluated adults (18 years of age or older) with ischemic stroke who underwent CTA and cerebral angiography sequentially between 2010 and 2018. The incidence of postcontrast acute kidney injury was determined using the baseline estimated glomerular filtration rate. The value of the baseline estimated glomerular filtration rate at which the occurrence of postcontrast acute kidney injury increased was also determined. RESULTS: Postcontrast acute kidney injury occurred in 57/601 (9.5%) patients. Those with a baseline estimated glomerular filtration rate of <30 mL/min/1.73 m2 showed a higher incidence of acute kidney injury. Age, chronic kidney disease, medication (nonsteroidal anti-inflammatory drugs, angiotensin-converting enzyme inhibitors, angiotensin II receptor blockers, β blockers, statins, and insulin) use following contrast media exposure, and serum albumin affected the incidence of postcontrast acute kidney injury. The incidence of postcontrast acute kidney injury increased when the baseline estimated glomerular filtration rate was <43 mL/min/1.73 m2. CONCLUSIONS: Patients with low baseline renal function had the highest incidence of postcontrast acute kidney injury after CTA and cerebral angiography, but no fatal adverse effects were documented. Thus, patients suspected of having a stroke should be actively managed with respect to neurovascular function. Full Article

The Navajo Nation is the largest reservation in the country, larger than West Virginia, straddling Arizona, New Mexico, Colorado and Utah. Half of the over 300,000 enrolled members reside on the reservation. Navajo President Jonathan Nez has issued some of the strongest stay-at-home measures in the country, including a weekday evening curfew and a complete, stay-at-home curfew for the entire weekend. Nearby Gallup, New Mexico, with a large Diné population, has enacted a complete lockdown, with the National Guard prohibiting entry. As of May 5, despite these efforts, there were 2,559 confirmed COVID-19 cases on the Navajo Nation and 79 deaths. Among the victims, 28-year-old Valentina Blackhorse, a beloved champion of Navajo culture and a community leader. She left behind her partner, Robby Jones, and their one-year-old daughter, Poet. "She really loved her family -- her parents, her sisters, her nieces and nephews. She loved her elderly. She loved children," Jones said Tuesday on the Democracy Now! news hour. "She was a kind and hardworking lady, and she was warmhearted. She would do anything for her family." Jones is a detention officer with the Navajo Department of Corrections, and contracted COVID-19 at work. "When she was taking care of me, I guess she contracted it," he said. "She started showing symptoms -- shortness of breath, body aches, loss of taste and smell. By the time I started feeling better … that's when she started feeling sick." Valentina Blackhorse tested positive for COVID-19 on April 22. She died the next day. She had won numerous pageants, being named Miss Western Navajo and Miss Diné College, among others, and hoped to run for office in the Navajo Nation government one day. Dr. Michelle Tom, a member of the Navajo Nation, is a family physician in Winslow, Arizona, just across the Navajo reservation line. She spoke about Valentina's death on Democracy Now!: "It's a reflection of what we're going through as a people, and it correlates with what this virus can do to our young and someone who was very motivated, loved our culture, spread our rich and strong culture, and our language. That's what we're trying to fight for," she said, adding, "She was going to lead our next generation. It was a hard loss for our community." The Navajo Nation, along with the nearby Hopi, Pueblo, Zuni and Gila River Indigenous communities, have endured despite centuries of genocide, oppression and systemic racism and poverty. The novel coronavirus pandemic is afflicting them disproportionately, as it has African American and Latinx populations across the U.S. Access to water is challenging on the Navajo reservation. "That's from a long state of histories with treaties and our relationship with the [federal] government," Dr. Michelle Tom explained. "Our infrastructure for water has never been at the capacity where we can provide water for everyone on the reservation. So, you're telling people to wash your hands for 20 seconds, and yet people are trying just to get water just to drink and to cook with." "The amount of money that’s being sent to 'Indian country,' as we call it, is the largest amount in the history of the U.S. And you deserve it. And you've been through a lot," Trump said to VP Lizer. "The Navajo Nation will soon receive over $600 million. That's a lot. Should I renegotiate that? Can we renegotiate that?" (Laughter.) There was no laughter back on the Navajo Nation. "Today, the federal government announced that they intend to release a portion of funds appropriated by Congress over one month ago to tribes to help fight COVID-19, but I'll believe it when I see it," President Jonathan Nez, who himself tested positive for the virus, replied. "We couldn't sit around and wait for those dollars, so we've had boots on the ground in nearly 20 communities giving out food, water, firewood, protective masks and other supplies … We lost many of our beloved relatives and family members to this virus, but our teachings also tell us to move forward. The LinkedIn profile for the former Ubisoft employee Christophe Prelot revealed he worked on a cancelled Prince of Persia title from April 2010 to 2011 as a 3D level artist. The game was in development for the PlayStation 3, Xbox 360 and Windows PC.
