To perform focused surgical approaches, it is necessary to localize all hyperfunctioning glands. The aim of the study was to compare the efficiency of established conventional scintigraphic imaging modalities with emerging 18F-fluorocholine PET/CT imaging in preoperative localization of hyperfunctioning parathyroid glands in a larger series of PHPT patients. Methods: In total, 103 patients with PHPT were imaged preoperatively with 18F-fluorocholine PET/CT and conventional scintigraphic imaging methods, consisting of 99mTc-sestamibi SPECT/CT, 99mTc-sestamibi/pertechnetate subtraction imaging, and 99mTc-sestamibi dual-phase imaging. The results of histologic analysis, as well as intact parathyroid hormone and serum calcium values obtained 1 d after surgery and on follow-up, served as the standard of truth for evaluation of imaging results. Results: Diagnostic performance of 18F-fluorocholine PET/CT surpassed conventional scintigraphic methods (separately or combined), with calculated sensitivity of 92% for PET/CT and 39%–56% for conventional imaging (65% for conventional methods combined) in the entire patient group. Subgroup analysis, differentiating single and multiple hyperfunctioning parathyroid glands, showed PET/CT to be most valuable in the group with multiple hyperfunctioning glands, with sensitivity of 88%, whereas conventional imaging was significantly inferior, with sensitivity of 22%–34% (44% combined). Conclusion: 18F-fluorocholine PET/CT is a diagnostic modality superior to conventional imaging methods in patients with PHPT, allowing for accurate preoperative localization. Full Article
super Superior Long-term Survival for Simultaneous Pancreas-Kidney Transplantation as Renal Replacement Therapy: 30-Year Follow-up of a Nationwide Cohort By care.diabetesjournals.org Published On :: 2020-01-20T12:00:30-08:00 OBJECTIVE In patients with type 1 diabetes and end-stage renal disease, it is controversial whether a simultaneous pancreas-kidney (SPK) transplantation improves survival compared with kidney transplantation alone. We compared long-term survival in SPK and living- or deceased-donor kidney transplant recipients. RESEARCH DESIGN AND METHODS We included all 2,796 patients with type 1 diabetes in the Netherlands who started renal replacement therapy between 1986 and 2016. We used multivariable Cox regression analyses adjusted for recipient age and sex, dialysis modality and vintage, transplantation era, and donor age to compare all-cause mortality between deceased- or living-donor kidney and SPK transplant recipients. Separately, we analyzed mortality between regions where SPK transplant was the preferred intervention (80% SPK) versus regions where a kidney transplant alone was favored (30% SPK). RESULTS Of 996 transplanted patients, 42%, 16%, and 42% received a deceased- or living-donor kidney or SPK transplant, respectively. Mean (SD) age at transplantation was 50 (11), 48 (11), and 42 (8) years, respectively. Median (95% CI) survival time was 7.3 (6.2; 8.3), 10.5 (7.2; 13.7), and 16.5 (15.1; 17.9) years, respectively. SPK recipients with a functioning pancreas graft at 1 year (91%) had the highest survival (median 17.4 years). Compared with deceased-donor kidney transplant recipients, adjusted hazard ratios (95% CI) for 10- and 20-year all-cause mortality were 0.79 (0.49; 1.29) and 0.98 (0.69; 1.39) for living-donor kidney and 0.67 (0.46; 0.98) and 0.79 (0.60; 1.05) for SPK recipients, respectively. A treatment strategy favoring SPK over kidney transplantation alone showed 10- and 20-year mortality hazard ratios of 0.56 (0.40; 0.78) and 0.69 (0.52; 0.90), respectively. CONCLUSIONS Compared with living- or deceased-donor kidney transplantation, SPK transplant was associated with improved patient survival, especially in recipients with a long-term functioning pancreatic graft, and resulted in an almost twofold lower 10-year mortality rate. He views the accident asA warning, I thought superstitiously, had been issued to me: that the going would not be easy and that romanticism would be quickly punished. It was only a few miles later that I remembered the letter a friend had sent me when I told him about my plan to walk the Icknield Way. Take care as you pass the ring-fort, he had written back. When I mentioned the fall later, he was unamazed."This was an entry fee to the old ways, charged at one of the usual tollbooths, " he said. "Now you can proceed. You're in. Bone for chalk: you've paid your due." It was the first of several incidents along the old ways that I still find hard to explain away rationally." (p.43).Throughout the book, Macfarlane risks meeting the supernatural. He spends the nights camping near Iron Age barrows. He sleeps in circular Pictish shielings. Finally he decides to sleep in Chanctonbury RIng in Sussex because author Laurie Lee had slept there while walking over England in the 1930's.I first learned about Chanctonbury Ring when I read about called Sussex Cottage written by Esther Meynell in 1936. The ring contains a temple built by the Romans on a previously inhabited Bronze and Iron Age fort site. According to legend, Julius Caesar and his legions ride around the ring. It is also possible to summon the devil by running around it a certain number of times. First he walks around the ring, then beds down for the night. He is awoken by human-sounding voices moving around the ring until two voices meet directly over his head. Eventually the voices go away and he is able to go back to sleep although he does not feel rested in the morning. Later in the day Macfarlane meets up up with an archaeologist friend and they discuss why it was a bad idea to sleep in the ring. However, it is not until he gets home that Macfarlane researches the folklore of Chanctonbury Ring and realizes that it is one of the most malevolently haunted spots in England.What's interesting is that while Macfarlane is aware of the many centuries of human history that his paths have run through, and of the theories that the paths exist throughout time, he does little research on the supernatural history of the places where he travels. These places seem to do their best to make him aware of their history and their special qualities.
