stf Evidence-Based Updates on the First Week of Exclusive Breastfeeding Among Infants >=35 Weeks By pediatrics.aappublications.org Published On :: 2020-04-01T01:00:57-07:00 The nutritional and immunologic properties of human milk, along with clear evidence of dose-dependent optimal health outcomes for both mothers and infants, provide a compelling rationale to support exclusive breastfeeding. US women increasingly intend to breastfeed exclusively for 6 months. Because establishing lactation can be challenging, exclusivity is often compromised in hopes of preventing feeding-related neonatal complications, potentially affecting the continuation and duration of breastfeeding. Risk factors for impaired lactogenesis are identifiable and common. Clinicians must be able to recognize normative patterns of exclusive breastfeeding in the first week while proactively identifying potential challenges. In this review, we provide new evidence from the past 10 years on the following topics relevant to exclusive breastfeeding: milk production and transfer, neonatal weight and output assessment, management of glucose and bilirubin, immune development and the microbiome, supplementation, and health system factors. We focus on the early days of exclusive breastfeeding in healthy newborns ≥35 weeks’ gestation managed in the routine postpartum unit. With this evidence-based clinical review, we provide detailed guidance in identifying medical indications for early supplementation and can inform best practices for both birthing facilities and providers. stf Baby-Friendly Hospital Practices and Meeting Exclusive Breastfeeding Intention By pediatrics.aappublications.org Published On :: 2012-06-04T00:07:39-07:00 Most mothers in the United States do not meet recommendations for exclusive breastfeeding; however, little is known about how long mothers intend to exclusively breastfeed or how hospital practices affect achieving these intentions. Most mothers who want to exclusively breastfeed intend to do so for ≥3 months, but the majority are not meeting their intended duration. Mothers are more likely to achieve their intended duration when their infant is not supplemented in the hospital. (Read the full article) Full Article
stf Breastfeeding, Childhood Milk Consumption, and Onset of Puberty By pediatrics.aappublications.org Published On :: 2012-08-20T00:07:42-07:00 Early life nutrition may program pubertal timing. Limited evidence suggests breastfeeding is associated with later puberty and childhood milk consumption with earlier puberty; whether these observations are biologically mediated or confounded by socioeconomic position is unclear. In a developed non-Western setting with little socioeconomic patterning of pubertal timing, neither breastfeeding nor childhood milk consumption was associated with pubertal timing, suggesting nutritional exposures during potentially critical periods may not have long-term effects on rates of maturation. stf Vitamin D Status of Exclusively Breastfed 4-Month-Old Infants Supplemented During Different Seasons By pediatrics.aappublications.org Published On :: 2012-09-24T00:07:05-07:00 Despite numerous preventive strategies including prophylaxis with 400 IU/day of vitamin D in recent years, the deficiency of vitamin D in infants is still a global health problem. This study reveals that the risk of vitamin D deficiency is high in exclusively breastfed infants, especially in winter, despite vitamin D supplementation. Therefore, it is suggested that an adjustment of vitamin D dosage for seasonal variation might be necessary. (Read the full article) Full Article
stf Breastfeeding Education and Support Trial for Overweight and Obese Women: A Randomized Trial By pediatrics.aappublications.org Published On :: 2012-12-03T00:07:40-08:00 Obesity is a risk factor for failure to initiate breastfeeding, formula supplementation, and short breastfeeding duration. There is a need for interventions that can improve the breastfeeding outcomes of overweight and obese women.Breastfeeding peer counseling targeting overweight/obese women did not affect exclusive breastfeeding rates or breastfeeding continuation beyond 2 weeks. However, the intervention was associated with improvements in early breastfeeding intensity and fewer infant hospitalizations in the first 6 months after birth. (Read the full article) Full Article
stf Reasons for Earlier Than Desired Cessation of Breastfeeding By pediatrics.aappublications.org Published On :: 2013-02-18T00:06:42-08:00 Reasons mothers cite for breastfeeding cessation vary across an infant's first year of life; however, once women stop breastfeeding, little is known about whether they breastfed as long as they desired or reasons why they did not meet their desired duration.About 60% of mothers do not meet their desired breastfeeding duration. Mothers who do not breastfeed as long as they desire primarily cite concerns about maternal or child health and processes associated with breastfeeding as their reason to stop breastfeeding. stf Baby-Friendly Hospital Accreditation, In-Hospital Care Practices, and Breastfeeding By pediatrics.aappublications.org Published On :: 2013-03-11T00:06:31-07:00 Baby-Friendly Hospital Initiative (BFHI) accreditation can have a positive effect on breastfeeding initiation and continuation rates; however, little is known about the effect of BFHI accreditation in populations with high breastfeeding-initiation rates and where infant-friendly practices are common.BFHI accreditation per se does not improve breastfeeding rates at 1 and 4 months when breastfeeding-initiation rates are high and accredited and nonaccredited hospitals have infant-friendly practices. Baby-friendly practices are more important than accreditation. (Read the full article) Full Article
stf Pacifier Restriction and Exclusive Breastfeeding By pediatrics.aappublications.org Published On :: 2013-03-18T00:07:05-07:00 Pacifiers may interfere with breastfeeding and thus are discouraged until 3 to 4 weeks of life, when they are recommended for sudden infant death syndrome risk reduction. Hospitals are restricting pacifier distribution as part of the Baby-Friendly Hospital Initiative. We describe a temporal association between reduced exclusive breastfeeding and pacifier restriction. This observation encourages research on breastfeeding promotion and the effects of pacifiers and pacifier restriction on breastfeeding. (Read the full article) Full Article
stf Evaluation of an Office Protocol to Increase Exclusivity of Breastfeeding By pediatrics.aappublications.org Published On :: 2013-04-01T01:06:21-07:00 A gap exists with lack of programs to help mothers breastfeed. The 2012 American Academy of Pediatrics' "Policy Statement on Breastfeeding and the Use of Human Milk" re-emphasized breastfeeding as an important public health initiative rather than a lifestyle choice.Families who receive care in a primary care setting that has implemented a "breastfeeding-friendly" office protocol may have increased rates of exclusive breastfeeding. This study evaluated an accepted clinical protocol in a large, diverse pediatric primary care setting. (Read the full article) Full Article
stf Association Between Total Duration of Breastfeeding and Iron Deficiency By pediatrics.aappublications.org Published On :: 2013-04-15T00:06:42-07:00 Previous studies have found a relationship between exclusive breastfeeding for ≥6 months and iron deficiency. Little is known about the relationship between total breastfeeding duration, including the period after the introduction of complementary foods, and infant iron status. Our results suggest that infants with longer total breastfeeding duration may be at risk for iron deficiency. Our findings highlight a clinically important association warranting additional investigation that may inform future guideline updates regarding assessment of risk for iron deficiency in young infants. stf Effect of Early Limited Formula on Duration and Exclusivity of Breastfeeding in At-Risk Infants: An RCT By pediatrics.aappublications.org Published On :: 2013-05-13T00:06:59-07:00 Public health policy focuses on reducing formula use for breastfed infants during the birth hospitalization. Observational evidence supports this approach, but no previous studies have examined the effect of early use of small volumes of formula on eventual breastfeeding duration.Use of limited volumes of formula during the birth hospitalization may improve breastfeeding duration for newborns with high early weight loss. Reducing the use of formula during the birth hospitalization could be detrimental for some subpopulations of healthy term newborns. (Read the full article) Full Article
stf Breastfeeding Concerns at 3 and 7 Days Postpartum and Feeding Status at 2 Months By pediatrics.aappublications.org Published On :: 2013-09-23T08:26:04-07:00 Although most US mothers initiate breastfeeding, half fail to achieve their breastfeeding intentions. In cross-sectional and retrospective surveys, early breastfeeding difficulties are often cited as reasons for stopping breastfeeding earlier than intended. We characterized 4179 breastfeeding concerns/problems as reported by primiparas interviewed prospectively. Concerns were highly prevalent and associated with up to ninefold greater risk of stopping breastfeeding earlier than intended. Concerns at 3 to 7 days posed the greatest risk. stf The Natural History of Jaundice in Predominantly Breastfed Infants By pediatrics.aappublications.org Published On :: 2014-07-21T00:07:03-07:00 Newborn infants who are predominantly breastfed are much more likely to develop prolonged hyperbilirubinemia than those fed formula, but the prevalence of prolonged hyperbilirubinemia in a largely white, North American, breastfed population is unknown.Practitioners can be reassured that it is normal for 20% to 30% of predominantly breastfed infants to be jaundiced at age 3 to 4 weeks and for 30% to 40% of these infants to have bilirubin levels ≥5 mg/dL. stf Telephone Peer Counseling of Breastfeeding Among WIC Participants: A Randomized Controlled Trial By pediatrics.aappublications.org Published On :: 2014-08-04T00:06:57-07:00 In-person peer counseling to pregnant and new mothers has been shown to improve breastfeeding modestly in three US RCTs. But this level of support for WIC is unlikely to be scaled up nationally in the current fiscal environment. We randomly assigned WIC clients to a telephone peer counseling program relative to standard WIC support for breastfeeding. Nonexclusive breastfeeding among Spanish-speakers increased at 1, 3, and 6 months, but the program had much less of an effect on English-speaking clients. (Read the full article) Full Article
stf Coparenting Breastfeeding Support and Exclusive Breastfeeding: A Randomized Controlled Trial By pediatrics.aappublications.org Published On :: 2014-12-01T00:06:28-08:00 Fathers’ attitude and support affects breastfeeding outcomes. Fathers are currently not targeted in breastfeeding support and care provided by health care professionals. Breastfeeding interventions delivered to fathers have been shown to increase breastfeeding exclusivity and duration.A coparenting breastfeeding support intervention delivered to mothers and fathers in the postpartum period showed beneficial effects on breastfeeding duration, paternal breastfeeding confidence, breastfeeding help provided by fathers, and mothers’ satisfaction with fathers’ involvement with breastfeeding. stf Early Weight Loss Nomograms for Exclusively Breastfed Newborns By pediatrics.aappublications.org Published On :: 2014-12-01T00:06:28-08:00 Exclusively breastfed newborns lose weight daily in the first few days after birth. The amount of weight lost varies substantially between newborns, with higher amounts of weight loss increasing risk for morbidity. This study presents nomograms demonstrating percentiles for weight loss by delivery mode for those who are exclusively breastfed. The nomograms have potential to be used for early identification of neonates on a trajectory for greater weight loss and related morbidities. (Read the full article) Full Article
stf Breastfeeding Duration and Weight Gain Trajectory in Infancy By pediatrics.aappublications.org Published On :: 2014-12-01T00:06:27-08:00 Although we know breastfeeding is beneficial and infant weight gain can predict obesity later in life, the relationship between breastfeeding duration and infant weight gain patterns among populations exhibiting high risk for obesity is unexplored. This study demonstrates the greater odds of increased infant weight gain for infants who breastfed for shorter durations among those exposed to a high number of maternal biopsychosocial risk factors for obesity.
