Association of BMI, Fitness, and Mortality in Patients With Diabetes: Evaluating the Obesity Paradox in the Henry Ford Exercise Testing Project (FIT Project) Cohort

OBJECTIVE

To determine the effect of fitness on the association between BMI and mortality among patients with diabetes.

RESEARCH DESIGN AND METHODS

We identified 8,528 patients with diabetes (self-report, medication use, or electronic medical record diagnosis) from the Henry Ford Exercise Testing Project (FIT Project). Patients with a BMI <18.5 kg/m2 or cancer were excluded. Fitness was measured as the METs achieved during a physician-referred treadmill stress test and categorized as low (<6), moderate (6–9.9), or high (≥10). Adjusted hazard ratios for mortality were calculated using standard BMI (kilograms per meter squared) cutoffs of normal (18.5–24.9), overweight (25–29.9), and obese (≥30). Adjusted splines centered at 22.5 kg/m2 were used to examine BMI as a continuous variable.

RESULTS

Patients had a mean age of 58 ± 11 years (49% women) with 1,319 deaths over a mean follow-up of 10.0 ± 4.1 years. Overall, obese patients had a 30% lower mortality hazard (P < 0.001) compared with normal-weight patients. In adjusted spline modeling, higher BMI as a continuous variable was predominantly associated with a lower mortality risk in the lowest fitness group and among patients with moderate fitness and BMI ≥30 kg/m2. Compared with the lowest fitness group, patients with higher fitness had an ~50% (6–9.9 METs) and 70% (≥10 METs) lower mortality hazard regardless of BMI (P < 0.001).

CONCLUSIONS

Among patients with diabetes, the obesity paradox was less pronounced for patients with the highest fitness level, and these patients also had the lowest risk of mortality.

The Henry Buckley Collection of Planktonic Foraminifera at the Natural History Museum in London (NHMUK) consists of 1665 single-taxon slides housing 23 897 individuals from 203 sites in all the major ocean basins, as well as a vast research library of Scanning Electron Microscope (SEM) photomicrographs. Buckley picked the material from the NHMUK Ocean-Bottom Deposit Collection and also from fresh tow samples. However, his collection remains largely unused as he was discouraged by his managers in the Mineralogy Department from working on or publicizing the collection. Nevertheless, Buckley published pioneering papers on isotopic interpretation of oceanographic and climatic change and was one of the first workers to investigate foraminiferal wall structure using the SEM technique. Details of the collection and images of each slide are available via the NHMUK Data Portal (http://dx.doi.org/10.5519/0035055). The Buckley Collection and its associated Ocean-Bottom Deposit Collection have great potential for taxon-specific studies as well as geochemical work, and both collections are available on request.

How did Mark Henry first discover you? And what is the most useful piece of advice he’s given you from your tryout and beyond?
“After my track career I wanted to find something I could still be competitive in, so I was doing Crossfit and I really stood out…all the other girls were just wearing regular shorts and t-shirts and I was coming out in tutu’s and big bows and just really standing out. At a lot of Crossfit competitions I was getting on the mic and, without even knowing it, cutting promos. Mark Henry came across a video of me at a Crossfit event on the internet and he contacted me and asked me if I’d ever thought of being in the WWE… He got me in the door but he made it very clear from the very beginning that “this is all going to be on you”… It gave me a lot of confidence and it really pushed me. Ever since then he has always been someone I can go to for advice”

You famously declared yourself the ‘EST of NXT’, and now the ‘EST of WWE’. Can you explain to people who don’t know, what that means?
“The ‘EST of WWE’ means that I am a hybrid athlete. I’m not just good in one area, I’m not just good at one thing, I’m not just strong, I’m not just someone who can go in there and do flips and be a showcase athlete. I’m someone who can do it all. I’m not just average in different areas, I’m the best in every single area. I’m the strongEST, the fastEST, the quickEST, the roughEST, the greatEST, the smartEST, I’m the bEST. All those end in EST. I just see myself as being the absolute best in every single area that you can think of.”

You’re a Superstar with a host of skills, one of which is making your own gear. Has that always been a passion for you? Which ring gear have you designed that means the most to you, and why?
“I have been pretty creative and pretty hands on my whole life. I get it from my dad who is always doing something, always making something. My mum is the same way. They are super creative and I just grew up in that environment. I used to sew when I was younger, my mum had this little Christmas cookie jar that had everything in it but cookies. At first I started sewing pillow cases together, I didn’t even know what I was doing, but it carried over into High School and I used to sew and make my own outfits… It carried over into WWE because I do everything the best and I do everything myself, it’s a huge part of my character. Probably my favourite gear is my Black History Month gear that I wore at NXT TakeOver: Portland. Black History Month is really important to me because I really feel that Black history is American history is World history, and I’m very passionate about that. So that’s probably my favourite gear that I’ve made and it has the most sentimental value to me.”

Do you see your position as a WWE Superstar carrying a responsibility to set an example for young girls and women around the world?
“The further along I get in my career the more I see that I do have a responsibility and I take that very seriously. Especially with women and young girls we are often taught to shrink ourselves, and that is a big part of my character is to never shrink yourself to soothe anyone’s insecurities, you never dim your light for anyone. You go out there and you shine bright, it’s even in my theme song: “watch me shine now”. It’s something that I really want to stress to young girls, especially within the community that I come from… Go out there and show them who you are and don’t hold back. I hold that responsibility very close to my heart and it’s not just for young girls or for women, it’s for everyone. You have to be your biggest supporter, you have to be your biggest cheerleader.”

What are Bianca Belair’s future WWE ambitions?
“The cliché answer is to be RAW Women’s Champion, or if I’m on SmackDown to be SmackDown Women’s Champion, or to eventually be NXT Women’s Champion. Of course, I want to be the Champion on all three brands at some particular time but, honestly, I just want to go out there and leave an impact no matter where I’m at. I feel like I did that with NXT, I left an impact. When people think back to this era in WWE history I want to be one of the names that pops up as one of the key players regardless of if I’ve won one match or one hundred matches. I want to leave a legacy, I want to leave an impact.”

Matt Henry's contract with Kent cancelled due to coronavirus crisis

New Zealand pacer Matt Henry will not be playing for England's county Kent Cricket due to the ongoing COVID-19 pandemic and disruption to the 2020 county cricket season.

"Kent Cricket can confirm that New Zealand International Matt Henry will no longer be joining the Club for the first seven County Championship matches of 2020," the club said in a statement.

Henry was due to join Kent - for whom he starred in 2018 - for seven County Championship games at the start of the season, but with all professional cricket cancelled in the UK until May 28 at the earliest, it became clear that he would be unable to play for the club.

The Kiwi player becomes the fourth player - after Nathan Lyon, Cheteshwar Pujara and Michael Neser - to miss out on a stint in county cricket due to the virus.

Paul Downton, Kent's director of cricket, said: "Once the decision was announced by the ECB that there would be no cricket until at least the end of May it became inevitable the Matt would not be returning to Kent this season."

"The club continues to enjoy a good relationship with Matt. He has many friends here and the overwhelmingly positive feedback we received when we announced that he would return in 2020 is a measure of Matt himself: he's a fantastic person on and off the cricket field," he added.

COVID-19: Henry Nicholls donates World Cup 2019 final shirt to UNICEF to raise funds

New Zealand batsman Henry Nicholls has donated his ICC 2019 World Cup shirt to UNICEF to help raise funds for the battle against COVID-19. The 28-year-old has offered his half-sleeve shirt signed by his teammates to facilitate meals for Kiwi families. The official Twitter handle of UNICEF New Zealand wrote, "NZ Cricketer @HenryNicholls27 has offered up one of his prized 2019 Cricket World Cup jerseys, signed by the whole team! One lucky donor who has supported our #FoodForKiwiFamilies appeal will receive it. You've got to be in it, to win it so donate now!"

Anyone who donates by Monday, irrespective of the amount, will enter a draw and one lucky person will get the shirt. The Blackcaps played their second consecutive final but failed to bring the trophy home as they lost to England on the basis of the boundary-countback rule. As a result, the Three Lions went on to lift their first-ever 50-over title.

The normal 50-over match action and super over had ended as a tie, and in the end, England was announced as the winners after scoring more boundaries in the match. As all the sporting activities including cricket have been suspended due to the global surge of the coronavirus pandemic, the left-handed batsman was last seen in action against Australia in an ODI on March 13.

He has played 33 Tests, 49 ODIs, and 5 shortest format games for Kiwis. The player has scored 3,095 runs across all formats including six centuries.
