Autism spectrum disorder (ASD) is characterized partly by atypical attentional engagement, reflected in exaggerated and variable responses to sensory stimuli. Attentional engagement is known to be regulated by the locus ceruleus (LC). Moderate baseline LC activity globally dampens neural responsivity and is associated with adaptive deployment and narrowing of attention to task-relevant stimuli. In contrast, increased baseline LC activity enhances neural responsivity across cortex and widening of attention to environmental stimuli regardless of their task relevance. Given attentional atypicalities in ASD, this study is the first to evaluate whether, under different attentional task demands, individuals with ASD exhibit a different profile of LC activity compared with typically developing controls. Males and females with ASD and age- and gender-matched controls participated in a one-back letter detection test while task-evoked pupillary responses, an established correlate for LC activity, were recorded. Participants completed this task in two conditions, either in the absence or presence of distractor auditory tones. Compared with controls, individuals with ASD evinced atypical pupillary responses in the presence versus absence of distractors. Notably, this atypical pupillary profile was evident despite the fact that both groups exhibited equivalent task performance. Moreover, between-group differences in pupillary responses were observed specifically in response to task-relevant events, providing confirmation that the group differences most likely were specifically associated with distinctions in LC activity. These findings suggest that individuals with ASD show atypical modulation of LC activity with changes in attentional demands, offering a possible mechanistic and neurobiological account for attentional atypicalities in ASD.

SIGNIFICANCE STATEMENT Individuals with autism spectrum disorder (ASD) exhibit atypical attentional behaviors, including altered sensory responses and atypical fixedness, but the neural mechanism underlying these behaviors remains elusive. One candidate mechanism is atypical locus ceruleus (LC) activity, as the LC plays a critical role in attentional modulation. Specifically, LC activity is involved in regulating the trade-off between environmental exploration and focused attention. This study shows that, under tightly controlled conditions, task-evoked pupil responses, an LC activity proxy, are lower in individuals with ASD than in controls, but only in the presence of task-irrelevant stimuli. This suggests that individuals with ASD evince atypical modulation of LC activity in accordance with changes in attentional demands, offering a mechanistic account for attentional atypicalities in ASD.

Denver Newsroom, May 7, 2020 / 03:48 pm (CNA).- For the first time in over a century, the annual May 13 celebrations at the Fatima shrine will take place without the physical presence of pilgrims, the bishop of Fatima confirmed this week.

Cardinal Antonio Marto said in a May 3 statement that the celebrations for the Feast of Our Lady of Fatima “will take place as was announced April 6, without the physical presence of pilgrims, in the name of prudence to avoid the risk of spreading the novel coronavirus.”

“As planned in conjunction with the civil authorities, the May 12 and 13 celebrations for this year cannot have the physical presence of the pilgrims and will be transmitted by broadcast and digital media,” he continued.

The cardinal explained that hosting “an unpredictable multitude of people” gathering at a time when the coronavirus pandemic is still a serious threat would go against the efforts of health authorities to gradually lift restrictions imposed to slow the spread of the virus.

“We therefore respect, in an attitude of collaboration with the competent civil authorities, the guidelines for these celebrations to be held with a symbolic presence of participants,” Marto said.

The clarification over the Fatima feast day came as the Portuguese government gradually begins to ease restrictions that had been put in place in response to the pandemic, prompting speculation over whether the May 13 celebrations would be able to take place as normal. The annual event typically draws hundreds of thousands from around the world to the shrine.

The Portuguese Bishops’ Conference had announced earlier this month that public Masses could tentatively resume the weekend of Pentecost, May 30-31, in accordance with guidelines to be established by the country’s health department and the Catholic Church.

Cardinal Marto stressed that the decision to suspend public religious celebrations was based in a sense of responsibility toward the public health, as a way of loving one’s neighbor.

“[A]s much as our hearts would like to be in Fatima celebrating together in the same place, as has been the case since 1917, prudence counsels us not to do so this time,” he said, adding that Catholics can look forward to an end to the pandemic and an opportunity to gather together joyfully in the future.

Fr. Carlos Cabecinhas, rector of the Shrine of Our Lady of Fatima, encouraged pilgrims who are unable to physically attend the events to make an interior pilgrimage, participating in the events of the Solemnity of Our Lady of Fatima through the internet or social media.

“This is a painful time: the shrine exists to welcome the pilgrims and we cannot do so, this is a cause for great sadness; but this decision is also an act of responsibility toward the pilgrims, protecting their health and welfare,” the priest said.

The shrine’s website offers four steps to guide people on this interior pilgrimage.

According to the website, “the celebrations of May 12 and 13 will maintain the usual schedule with recitation of the Rosary at 9:30 p.m. followed by the Candlelight Procession. On the 13th the Rosary will be prayed at 9:00 a.m. followed by the International Mass and the Farewell Procession.”

In March, 24 countries were consecrated to the Sacred Heart of Jesus and the Immaculate Heart of Mary at the Shrine of Our Lady of Fatima.

At that ceremony, Cardinal Marto recalled that Saints Francisco and Jacinto Marto, shepherd children to whom an angel, and then the Blessed Virgin Mary appeared in 1916 and 1917 at Fatima, both died amid the victims of the Spanish flu pandemic.

Epilepsy/seizure disorder is known to be associated with a range of mental health and neurodevelopmental comorbidities, based on clinical studies, and on population studies largely conducted outside the United States.

In a nationally representative sample of US children, estimated prevalence of reported lifetime epilepsy/seizure disorder was 1%, and of current epilepsy/seizure disorder was 6.3/1000. Developmental, mental health, and physical comorbidities are common, warranting enhanced surveillance, and an integrated service approach. (Read the full article)

Nonconvulsive status epilepticus (NCSE) is the diagnosis for encephalopathy caused by continuous epileptic activity on EEG. It is a well-known cause of morbidity and mortality in critically ill adults and neonates. NCSE is increasingly reported in critically ill children.

We show that NCSE is common in all inpatient settings, not only in the critically ill. Key risk factors that should dramatically increase suspicion of NCSE in clinical practice include a history of convulsive seizure and acute imaging abnormalities. (Read the full article)

Survival takes precedence for adolescent patients with cancer and their families. Patients may not discuss their treatments’ potential to damage their reproductive capacity, which has significant psychological late effects in survivorship.

Strong reproductive concerns of adolescents with cancer may not be captured on current health-related quality of life instruments and may be neglected by parents’ unawareness. Prevalence estimates for neurologic and neurodevelopmental disorders in children vary widely, and there is uncertainty as to what extent the individual disorders overlap. Most previous prevalence studies have been based on survey data and not on specialist-confirmed diagnoses.

This study used nationwide register data to determine the proportions of Norwegian children diagnosed with autism spectrum disorder, attention-deficit/hyperactivity disorder, epilepsy, and cerebral palsy and to study how the disorders overlap. Clinical trials have demonstrated that prophylactic human papillomavirus (HPV) vaccines are highly effective in preventing HPV infection, but the impact of vaccination on HPV prevalence rates in real-world, community settings is uncertain.

The outcome of status epilepticus in children depends on the etiology. In otherwise normal children who have ≥1 episodes of unprovoked status epilepticus as part of the evolution of their epilepsy, the seizure and intellectual outcome is unclear.

Based on population-based data and 20 to 30 years’ follow-up of normal children with focal epilepsy, one-third with status epilepticus had recurrence of status. Concerns persist about sexual disinhibition after human papillomavirus (HPV) vaccination of preteenage girls. Self-reported surveys have indicated few anticipated behavior changes after HPV vaccination. Little is known about sexual activity–related clinical outcomes after HPV vaccination.

Utilizing managed care organization electronic data, we evaluated the incidence of adverse outcomes of sexual activity among vaccinated preteenage girls and found little difference between those who received HPV vaccine and those who did not. (Read the full article)

Seizure-related death, including sudden death, is a frightening prospect. In part because risk and prevention are poorly understood, neurologists tend to avoid discussions of sudden death with families and young patients.

Most deaths in children with epilepsy are not seizure related. Relative to the population, however, sudden and seizure-related deaths alone double overall mortality. In uncomplicated epilepsy, such deaths occur at rates comparable to individual leading causes of death in young people. (Read the full article)

Injuries in children and young adults commonly cause morbidity and mortality. Epilepsy is common among children. Injury risk may be greater among those with epilepsy, but there are few large, population-based studies, making it difficult to estimate risk.

Children and young adults with epilepsy are at a greater risk of medicinal poisonings, thermal injuries, and fractures than those without epilepsy. Young adults with epilepsy are at particularly high risk of medicinal poisonings. (Read the full article)

The short-term immunogenicity and safety of a HPV4 vaccine have been previously evaluated in preadolescents and adolescents. To date, no long-term studies of the safety, effectiveness, and immunogenicity of the HPV4 vaccine have been reported in this age group.

The HPV4 vaccine administered to adolescents demonstrated durability in clinically effective protection and sustained antibody titers over 8 years. These data, along with extensive postapproval safety surveillance data, should help reinforce national recommendations for HPV vaccination of preadolescents and adolescents. (Read the full article)

Clinical trials of the quadrivalent human papillomavirus vaccine show it to be highly efficacious in preventing vaccine-type–specific cervical dysplasia and anogenital warts, but few studies have assessed its effects in the real world and none have done so at the program/population level.

This study provides strong evidence of the early benefits of quadrivalent human papillomavirus vaccination on reductions in cervical dysplasia and possible reductions in anogenital warts among girls aged 14 to 17 years, offering additional justification for not delaying vaccination until girls are older. Previous studies have shown that concomitant administration of the quadrivalent human papillomavirus vaccine with MCV4 and Tdap was generally well tolerated and did not interfere with the immune responses to the respective vaccines.

Concomitant administration of the novel 9-valent human papillomavirus vaccine with MCV4 and Tdap, 2 vaccines that are currently recommended for routine vaccination of adolescents, did not compromise the safety, tolerability, and immunogenicity of the individual vaccines. (Read the full article)
