A review of respite / short break provision for adult carers of adults in the Highland Partnership area

As part of the implementation of the Equal Partners in Care (EPiC) Highland Carer’s Strategy 2014-2017 it was agreed to undertake a review of respite for Adult Carers of Adults (aged 16+). Independent consultants were commissioned by NHS Highland through Connecting Carers to undertake this work. There are four groups of people – totalling an estimated 200 people - with whom conversations have taken place during the review: Carers and staff from carer support organisations – more than 75 carers have given their views Health and social care workforce – we have met with just over 50 people who have given their views and shared our initial findings with more than 60 others Respite providers – we have met with staff from 15 organisations that are providers of respite Those staff responsible for overseeing the commissioning, planning and administration of respite.

A research agenda for respite care. Deliberations of an expert panel of researchers, advocates and funders

ARCH, the National Respite Network and Resource Center in the United States of America identified that evidence-based research on respite care has, to large extent, been lacking. Across ages, needs and settings, respite is based upon the premise that providing caregivers periodic relief from daily, ongoing caregiving responsibilities will directly benefit them in terms of their physical health, immediate and long-term psychological health, and social-emotional relationships with family members. These benefits are assumed to result in secondary benefits for care receivers and even larger societal benefits in the form of cost benefits or improved employee productivity. Some research studies point to the merits of these assumptions. However, evidence-based research supporting this premise - or going beyond it to demonstrate how to best provide respite care that results in maximum benefits - has not been available. This report presents the findings of an expert panel composed of academics, researchers, service providers, advocates, policymakers and administrators representing a range of age groups, disabilities and professional disciplines. Over a period of 18 months the panel explored the current status of respite research, proposed strategies to overcome barriers to research, and developed a plan to encourage rigorous research in key areas.

Rethinking Respite for People Affected by Dementia

The ‘Dementia: More Than Just Memory Loss’ report, was published in 2016, and set out some of the key issues affecting people with dementia in Wales, in particular: • A widespread lack of knowledge and understanding of dementia amongst professionals and the wider public. • A lack of flexibility to effectively meet the needs of people living with dementia and their carers. • A lack of co-operation between services creates unnecessary difficulties and barriers for people living with dementia and their carers. The authors of the report called for a range of actions to address this, and there has been some progress, however, despite a range of changes across society at a policy, practice and community level, there is still a long way to go to transform services and drive the cultural change needed to effectively meet the needs of people affected by dementia.  The author of this report has consistently focused on the importance of meaningful outcomes for people with dementia and their carers, to ensure that their lives have value, meaning and purpose. This is fundamental to ‘Rethinking Respite’ and to delivering the Welsh Government’s vision of ‘a dementia friendly nation that recognises the rights of people with dementia to feel valued and to live as independently as possible in their communities as outlined in the new Dementia Action Plan for Wales.

I met Nadia Salomon through the SCBWI and am also familiar with her online. I love her enthusiasm, positive outlook, and the support she has shown fellow members of the kidlit community over the years! I was excited to get a sneak peek of GOODNIGHT GANESHA, her debut as a picture book author. The bedtime story celebrates nigthtime rituals as two young children visit their grandparents in India. SUCH gorgeous illustrations by Poonam Mistry. Thanks to both Nadia and Poonam for answering some questions about the book as well as their advice for young writers and illustrators!

"As nighttime falls over the city, two children visiting their grandparents in India find there’s so much fun to be had! Whether it’s listening to epic stories or observing rituals in the puja room, there are many moments that make this time together special. In this beautiful, rhyming ode to bedtime, the only thing more universal than getting ready for bed and saying goodnight is the love between children and their grandparents."

Q. What was your path to publication?

Nadia: I'm a #PitMad success story. I landed my amazing agent, Ammi-Joan Paquette, through twitter pitching! That's about three years after joining SCBWI. But my debut, GOODNIGHT GANESHA, is not the story she faved, but instead what I call a 'hot potato' idea inspired by my child. I wrote the draft in October of 2017. I sent it to several editors and agents I had connected with through conferences, but received lots of champagne rejections before shelving it. It was one of 21 manuscripts I shared with my agent in March of 2018. She loved the concept, but asked me to write the story in lyrical prose. After rounds of revisions, in January of 2019, she finally accepted the manuscript. She sent it out on sub. Six months later, in July of 2019, we sold the manuscript to Liza Kaplan, a senior editor at Philomel Books. I am now traditionally published. From concept to release will be 3 years, 10 months, 14 days to be exact.

Q. What interaction did you have with the illustrator, if any, during the process?

Nadia: Poonam and I interacted behind the scenes, but we NEVER discussed GOODNIGHT GANESHA or the art for the book. I trusted her whole-heartedly. All work related communications went through our editor. We became friends behind the scenes - coping through the pandemic, bonding over shared culture, family, and everything else in between, but we NEVER had convos as she worked on the book. The one time was at the end, when Poonam completed the art and asked me, "DID YOU SEE THE FINAL ART YET?" And I was like, "Uhm...NO." She was so excited. She said she put a lot of extra details and hoped I liked it. I was so antsy, but stayed calm. A few days later I received the final art. It was just incredible! We had a good cry messaging back and forth over the final art and what this project meant to each of us. We're about to contribute a piece of Telugu Culture and Kalamkari art into kidlit; we're ecstatic and proud of what we created together.

Q. What was your illustration process for GOODNIGHT GANESHA?

Poonam: For Goodnight Ganesha, I began by actually taking photographs around my parent's house. They have a lot of decorative ornaments and wall hangings all around their home so this was the perfect starting point for the book. Nadia's text is so poetic and beautiful so I sketched the imagery I felt best represented each spread and would really showcase her words- usually I have several versions for each part of the text and pick the ones I like the most. Once this was approved, I sketched out larger versions on thick cartridge paper and used black ink to draw in the designs in neat. This is where I added all the patterns and details. Mostly I took inspiration from traditional Indian art such Kalamkari textiles and Madhubani art. I layered the patterned very much like you would see on sarees and fabrics. After completing the drawing, I used Photoshop to add colour and additional patterns to finish it off. Some of the spreads have a patterned border- this was because I wanted them to look like wall hangings and Kalamkari textile pieces- big pieces of art!

Q. What advice do you have for young writers?

Nadia: Read, read, read. Write your stories. Make writing a habit. Write, write, write. Use the power of storytelling to change the world through joy. Explore your emotions, your memories, and create new classics. There's someone out there, an adult, another child, a pair of hands waiting to read your story that may impact or influence their life. No matter your age, your size, your voice - your words have value.

Q. What advice do you have for young illustrators?

Poonam: My biggest piece of advice is to do art that is true to yourself. My style is a real reflection of the things I love and what makes me who I am. It is my visual voice. Also always keep drawing and be experimental in the media you use. Although my work is mostly digital, I love painting in my style!

Across the Middle East and North Africa young people between the ages of 15 and 29 comprise around 24 per cent of the population in the region. Mapping invisible epitopes by NMR spectroscopy [Molecular Biophysics]

Defining discontinuous antigenic epitopes remains a substantial challenge, as exemplified by the case of lipid transfer polyproteins, which are common pollen allergens. Hydrogen/deuterium exchange monitored by NMR can be used to map epitopes onto folded protein surfaces, but only if the complex rapidly dissociates. Modifying the standard NMR-exchange measurement to detect substoichiometric complexes overcomes this time scale limitation and provides new insights into recognition of lipid transfer polyprotein by antibodies. In the future, this new and exciting development should see broad application to a range of tight macromolecular interactions.

On the Capital planet the rebellious droids had followed maily the Bat-Bot, but as time progressed his circuits had gone all mushy at 780 years or so without maintenance…

Several splinter groups all with their local bot leaders emerged such as the Che-bot, the traffic-light-robot and the Butt-bot, but none of these collected enough sentient circuits to call themselves a popular (or Animata) mass movement!

That was until a cyborg came along, one known as Jones, a long time prisoner and terrorist, his easy solutions to every problem rang well in the masses' auditory circuits!!!

His slogans and simple rhetoric were simple enough for the simple traffic-light to comprehend and cheer!

His language was full of hate towards the organics and especially the humans who were the most common races among the ruling class of the federation!!!

Despite being a “Fleshie” himself his message collected the angry enslaved
bot community by only weeks all rebellious robots except for a few fringe loonies had forgotten the old leaders…

One morning at Jones gave the signal…

All over the capital planet hordes and swarms of any form of mechanical sentient beings attacked first the police stations, then the Company boards running the planet and the federation as well as their starfleet…

Many died, especially the low level police and army! Many mechanicals died too, but their ranks were soon filled by Mutant fleshie allies of the lower levels who hated the Federation feudal society and upper classes as much as their technological allies…

The Federation state apparatus and ruling class, most of their fleet army fled when they knew the game was up, they activated the emergency escape plan and whole city blocks with important factories, administrational units, valuable assets and so on separated from the capital by hidden rocket engines and homed in their course to Mars…

On Mars the federation regrouped and formed their new society…

On the Capital planet, the robots proclaimed the first Techno-republic of the advanced inorganic civilization, the low level fleshies left behind, became slaves and their mutant allies got to rule their own minute chiefdoms as protectorates under the Techno-republic…

Jones was now the undisputed ruler of the capital planet, but the victory was a pyrros one since, all important buildings, all of value was now one Mars!

But as Jones put it:

Our proud race the Techno-species didn’t need the Fleshies administration, their infrastructure, their spaceships…

We shall start from scratch, with a new administration, a new order, every droid shall work at 4x speed than they did during human oppression since now we are free and the fleshies shall work twice as hard than the Techno-Race, until we have breed enough new fleshies so they can do all work!

Our future is bright and shiny like glistering shiny metal!

Designers note:

I am sad to say that this is the last episode in this years-spanning space series… At least for a while, I will still post LEGO hobby stuff here but without a storyline, perhaps small designs and builds… and occasionally a story when I feel like it!!!

I would like to thank all who had been in this journey of our heros, but it has taken far to much time and effort and since the state of the world is as it is, I am spiraling down in another depression, I must stop it before I reach the abyss, so I have remove some stress out of my equation… I ended it in a cliffhanger so I can easily restart it when my mental health improves… I hope that won’t be forever???

Hydrogen/deuterium exchange memory NMR reveals structural epitopes involved in IgE cross-reactivity of allergenic lipid transfer proteins [Protein Structure and Folding]

Identification of antibody-binding epitopes is crucial to understand immunological mechanisms. It is of particular interest for allergenic proteins with high cross-reactivity as observed in the lipid transfer protein (LTP) syndrome, which is characterized by severe allergic reactions. Art v 3, a pollen LTP from mugwort, is frequently involved in this cross-reactivity, but no antibody-binding epitopes have been determined so far. To reveal human IgE-binding regions of Art v 3, we produced three murine high-affinity mAbs, which showed 70–90% coverage of the allergenic epitopes from mugwort pollen–allergic patients. As reliable methods to determine structural epitopes with tightly interacting intact antibodies under native conditions are lacking, we developed a straightforward NMR approach termed hydrogen/deuterium exchange memory (HDXMEM). It relies on the slow exchange between the invisible antigen-mAb complex and the free 15N-labeled antigen whose 1H-15N correlations are detected. Due to a memory effect, changes of NH protection during antibody binding are measured. Differences in H/D exchange rates and analyses of mAb reactivity to homologous LTPs revealed three structural epitopes: two partially cross-reactive regions around α-helices 2 and 4 as well as a novel Art v 3–specific epitope at the C terminus. Protein variants with exchanged epitope residues confirmed the antibody-binding sites and revealed strongly reduced IgE reactivity. Using the novel HDXMEM for NMR epitope mapping allowed identification of the first structural epitopes of an allergenic pollen LTP. This knowledge enables improved cross-reactivity prediction for patients suffering from LTP allergy and facilitates design of therapeutics.

Overview of how N32 and N34 elovanoids sustain sight by protecting retinal pigment epithelial cells and photoreceptors [Thematic Reviews]

The essential fatty acid DHA (22:6, omega-3 or n-3) is enriched in and required for the membrane biogenesis and function of photoreceptor cells (PRC), synapses, mitochondria, etc. of the CNS. PRC DHA becomes an acyl chain at the sn-2 of phosphatidylcholine (PC), amounting to more than 50% of the PRC outer segment phospholipids, where phototransduction takes place. Very long chain PUFAs (VLC-PUFAs,n-3, ≥ 28 carbons) are at the sn-1 of this PC molecular species and interact with rhodopsin. PRC shed their tips (DHA-rich membrane disks) daily, which in turn are phagocytized by the retinal pigment epithelium (RPE), where DHA is recycled back to PRC inner segments to be used for the biogenesis of new photoreceptor membranes. Here, we review the structures and stereochemistry of novel elovanoid (ELV)-N32 and ELV-N34 to be ELV-N32: (14Z,17Z,20R,21E,23E,25Z,27S,29Z)-20,27-dihydroxydo-triaconta-14,17,21,23,25,29-hexaenoic acid; ELV-N34: (16Z,19Z,22R,23E,25E,27Z,29S,31Z)-22,29-dihydroxytetra-triaconta-16,19,23,25,27,31-hexaenoic acid. ELVs are low-abundance, high-potency, protective mediators. Their bioactivity includes enhancing of anti-apoptotic and pro-survival protein expression with concomitant downregulation of pro-apoptotic proteins when RPE is confronted with uncompensated oxidative stress (UOS). ELVs also target PRC/RPE senescence gene programming, the senescence secretory phenotype in the interphotoreceptor matrix (IPM), as well as inflammaging (chronic, sterile, low-grade inflammation). An important lesson on neuroprotection is highlighted by the ELV mediators that target the terminally differentiated PRC and RPE, sustaining a beautifully synchronized renewal process. The role of ELVs in PRC and RPE viability and function uncovers insights on disease mechanisms and the development of therapeutics for age-related macular degeneration (AMD), Alzheimer’s disease (AD), and other pathologies.

Generation and validation of a conditional knockout mouse model for the study of the Smith-Lemli-Opitz Syndrome [Research Articles]

Smith-Lemli-Opitz Syndrome (SLOS) is a developmental disorder (OMIM #270400) caused by autosomal recessive mutations in the Dhcr7 gene, which encodes the enzyme 3β-hydroxysterol-7 reductase. SLOS patients present clinically with dysmorphology and neurological, behavioral and cognitive defects, with characteristically elevated levels of 7-dehydrocholesterol (7-DHC) in all bodily tissues and fluids. Previous mouse models of SLOS have been hampered by postnatal lethality when Dhcr7 is knocked out globally, while a hypomorphic mouse model showed improvement in the biochemical phenotype with ageing, and did not manifest most other characteristic features of SLOS. We report the generation of a conditional knockout of Dhcr7 (Dhcr7flx/flx), validated by generating a mouse with a liver-specific deletion (Dhcr7L-KO). Phenotypic characterization of liver-specific knockout mice revealed no significant changes in viability, fertility, growth curves, liver architecture, hepatic triglyceride secretion, or parameters of systemic glucose homeostasis. Furthermore, qPCR and RNA-Seq analyses of livers revealed no perturbations in pathways responsible for cholesterol synthesis, either in male or female Dhcr7L-KO mice, suggesting hepatic disruption of post-squalene cholesterol synthesis leads to minimal impact on sterol metabolism in the liver. This validated conditional Dhcr7 knockout model may now allow us to systematically explore the pathophysiology of SLOS, by allowing for temporal, cell and tissue-specific loss of DHCR7.

Antibody binding epitope Mapping (AbMap) of hundred antibodies in a single run [Research]

Antibodies play essential roles in both diagnostics and therapeutics. Epitope mapping is essential to understand how an antibody works and to protect intellectual property. Given the millions of antibodies for which epitope information is lacking, there is a need for high-throughput epitope mapping. To address this, we developed a strategy, Antibody binding epitope Mapping (AbMap), by combining a phage displayed peptide library with next generation sequencing. Using AbMap, profiles of the peptides bound by 202 antibodies were determined in a single test, and linear epitopes were identified for >50% of the antibodies. Using spike protein (S1 and S2)-enriched antibodies from the convalescent serum of one COVID-19 patient as the input, both linear and conformational epitopes of spike protein specific antibodies were identified. We defined peptide-binding profile of an antibody as the Binding Capacity (BiC). Conceptually, the BiC could serve as a systematic and functional descriptor of any antibody. Requiring at least one order of magnitude less time and money to map linear epitopes than traditional technologies, AbMap allows for high-throughput epitope mapping and creates many possibilities.

Association Between CA 15-3 and 18F-FDG PET/CT Findings in Recurrent Breast Cancer Patients at a Tertiary Referral Hospital in Kenya

The tumor marker cancer antigen 15-3 (CA 15-3) is that most commonly used to monitor metastatic breast cancer during active therapy and surveillance for disease recurrence after treatment. The association of CA 15-3 and 18F-FDG PET/CT findings can be considered complementary, since any significant rise may indicate the presence of disease and imaging is able to map the tumor sites. Although current guidelines do not recommend the routine performance of CA 15-3 in asymptomatic patients being followed up after definitive breast cancer treatment, most oncologists perform serial assessment of the tumor markers as part of routine follow-up of patients. The aim of this study was to evaluate the correlation between CA 15-3 levels and 18F-FDG PET/CT scan findings in patients with recurrent breast cancer. Methods: This was a cross-sectional study with data collected retrospectively. Patients being evaluated for breast cancer recurrence with 18F-FDG PET/CT imaging and CA 15-3 level were included. Evaluation of the association between CA 15-3 levels and 18F-FDG PET/CT scan findings was then done. Results: In total, 154 cases were included in this study; 62 patients had recurrence (positive) on the 18F-FDG PET/CT scans, whereas 92 patients had normal (negative) findings on follow-up 18F-FDG PET/CT scans. There was an association between CA 15-3 levels and the presence or absence of recurrence on 18F-FDG PET/CT scans, with 84.4% (27/32) of patients who had elevated CA 15-3 levels having disease recurrence on 18F-FDG PET/CT and 84.4% (27/32) of patients who had elevated CA 15-3 levels having disease recurrence on 18F-FDG PET/CT as well as a correlation with the burden of metastases. Most patients with disease recurrence on 18F-FDG PET/CT, however, had normal CA 15-3 levels. Conclusion: Higher CA 15-3 levels correlate with breast cancer recurrence on 18F-FDG PET/CT as well as with burden of metastasis.
