An Exploratory Study of Online Equity: Differential Levels of Technological Access and Technological Efficacy Among Underserved and Underrepresented Student Populations in Higher Education

Aim/Purpose: This study aims to explore levels of Technological Access (ownership, access to, and usage of computer devices as well as access to Internet services) and levels of Technological Efficacy (technology related skills) as they pertain to underserved (UNS) and underrepresented (UNR) students. Background: There exists a positive correlation between technology related access, technology related competence, and academic outcomes. An increasing emphasis on expanding online education at the author’s institution, consistent with nationwide trends, means that it is unlikely that just an increase in online offerings alone will result in an improvement in the educational attainment of students, especially if such students lack access to technology and the technology related skills needed to take advantage of online learning. Most studies on levels of Technological Access and Technological Efficacy have dealt with either K-12 or minority populations with limited research on UNS and UNR populations who form the majority of students at the author’s institution. Methodology: This study used a cross-sectional survey research design to investigate the research questions. A web survey was sent to all students at the university except first semester new and first semester transfer students from various disciplines (n = 535). Descriptive and inferential statistics were used to analyze the survey data. Contribution: This research provides insight on a population (UNS and UNR) that is expanding in higher education. However, there is limited information related to levels of Technological Access and Technological Efficacy for this group. This paper is timely and relevant as adequate access to technology and technological competence is critical for success in the expanding field of online learning, and the research findings can be used to guide and inform subsequent actions vital to bridging any educational equity gap that might exist. Findings: A critical subset of the sample who were first generation, low income, and non-White (FGLINW) had significantly lower levels of Technological Access. In addition, nearly half of the survey sample used smartphones to access online courses. Technological Efficacy scores were significantly lower for students who dropped out of or never enrolled in an online course. Transfer students had significantly higher Technological Efficacy scores while independent students (determined by tax status for federal financial aid purposes) reflected higher Technological Efficacy, but at a marginally lower level of significance. Recommendations for Practitioners: Higher education administrators and educators should take into consideration the gaps in technology related access and skills to devise institutional interventions as well as formulate pedagogical approaches that account for such gaps in educational equity. This will help ensure pathways to sustained student success given the rapidly growing landscape of online education. Recommendation for Researchers: Similar studies need to be conducted in other institutions serving UNS and UNR students in order to bolster findings and increase awareness. Impact on Society: The digital divide with respect to Technological Access and Technological Efficacy that impacts UNS and UNR student populations must be addressed to better prepare such groups for both academic and subsequent professional success. Addressing such gaps will not only help disadvantaged students maximize their educational opportunities but will also prepare them to navigate the challenges of an increasingly technology driven society. Future Research: Given that it is more challenging to write papers and complete projects using a smartphone, is there a homework gap for UNS and UNR students that may impact their academic success? What is the impact of differing levels of Technological Efficacy on specific academic outcomes of UNS and UNR students?

Digital Innovation to Grow Quality Care Through an Interprofessional Care Team (DIG IT) Among Underserved Patients With Hypertension [Original Research]

PURPOSE

The impact of digital health on medically underserved patients is unclear. This study aimed to determine the early impact of a digital innovation to grow quality care through an interprofessional care team (DIG IT) on the blood pressure (BP) and 10-year atherosclerotic cardiovascular disease (ASCVD) risk score of medically underserved patients.

METHODS

This was a 3-month, prospective intervention study that included patients aged 40 years or more with BP of 140/90 mmHg or higher who received care from DIG IT from August through December 2021. Sociodemographic and clinical outcomes of DIG IT were compared with historical controls (controls) whose data were randomly extracted by the University of California Data Warehouse and matched 1:1 based on age, ethnicity, and baseline BP of the DIG IT arm. Multiple linear regression was performed to adjust for potential confounding factors.

RESULTS

A total of 140 patients (70 DIG IT, 70 controls) were included. Both arms were similar with an average age (SD) of 62.8 (9.7) years. The population was dominated by Latinx (79.3%) persons, with baseline mean BP of 163/81 mmHg, and mean ASCVD risk score of 23.9%. The mean (SD) reduction in systolic BP at 3 months in the DIG IT arm was twice that of the controls (30.8 [17.3] mmHg vs 15.2 [21.2] mmHg; P <.001). The mean (SD) ASCVD risk score reduction in the DIG IT arm was also twice that of the controls (6.4% [7.4%] vs 3.1% [5.1%]; P = .003).

CONCLUSIONS

The DIG IT was more effective than controls (receiving usual care). Twofold improvement in the BP readings and ASCVD scores in medically underserved patients were achieved with DIG IT.

Impact of a Multidisciplinary, Endocrinologist-Led Shared Medical Appointment Model on Diabetes-Related Outcomes in an Underserved Population

A multidisciplinary endocrinologist-led shared medical appointment (SMA) model showed statistically significant reductions in A1C from baseline over 3 years that were not significantly different from appointments with endocrinologists or primary care providers alone within a resource-poor population. Similarly, the SMA model achieved clinical outcomes on par with endocrinologist-only visits with the added benefit of improving endocrine provider productivity and specialty access for patients. Greater patient engagement with the SMA model was associated with significantly lower A1C.

Serving the underserved in workforce development: A Q&A with Beth Weigensberg


Improving data in the field of workforce development is a necessary step to evaluating programs and replicating success. What does current data tell us about the populations served? What outcomes should we measure to ensure programs are meeting America’s workforce development needs?

Earlier this month, we convened an expert group of policy makers, practitioners and scholars to address this problem, along with other challenges in workforce development. Previously, we interviewed Kate Blosveren Kreamer on the need to strengthen bridges from school to work. Next up in our Q&A series is Beth Weigensberg, a researcher at Mathematica Policy Research.

Q: What important research questions remain unanswered in the area of workforce development?

A: Although there is increasingly more rigorous research to assess effectiveness of programs, I feel a missing piece is understanding how to replicate and scale-up effective strategies. Often times workforce development programs that are deemed effective in one place do not always succeed when implemented in another. Research that evaluates effectiveness of programs should assess the role of contextual factors (including organizational, leadership, community, and political factors) to identify what is needed to successfully implement, replicate, and scale successful programs.

Q: You mentioned that you often think about the unemployed populations that are harder to serve. Who are some of these underserved populations, and what workforce development programs work for them?

A: The workforce development field has an unfortunate history of “creaming”—programs selectively work with individuals most likely to succeed at finding employment, leaving those “harder-to-serve” individuals struggling to find assistance. Individuals that are often considered “hard-to-serve” include those who are homeless, disabled, formerly incarcerated, older workers, non-English speakers, low-income, and youth who are disconnected from school and employment. Increasing efforts to focus on these “harder-to-serve” populations include specialized targeted programs and strategies to help address the complex needs of these individuals, which often extend beyond skill development and finding a job. These specialized programs often provide additional support services to help address their complex needs, which can serve as additional barriers to obtaining and retaining employment.

Q: What improvements can be made to better measure success?

A: Intermediate measures of engagement and skill development would provide interim measures of progress, while the ultimate objectives are obviously employment and educational attainment. Ongoing evaluation on interim measures allows for earlier acknowledgment of achievement and identification of those struggling to progress. Assessing outcomes in ways that control for different populations or barriers to employment, such as using risk-adjusted methodologies, can help us evaluate workforce development programs in an equitable manner.

One of the biggest challenges in the field is ensuring we have valid and reliable data to accurately estimate outcomes. The data available to assess outcomes are usually limited by what is collected in management information systems, which are often developed to be responsive to reporting requirements of publically-funded programs. But these siloed data do not allow for comprehensive assessment of workforce development outcomes within a state, locality, or even within a community-based employment and training organization that relies on numerous funding sources. Efforts are needed to integrate data and assess standardized outcome measures across program and funding silos to allow for more comprehensive assessment of outcomes within the field.
