Noninvasive diagnostic modalities and prediction models for detecting pulmonary hypertension associated with interstitial lung disease: a narrative review

Pulmonary hypertension (PH) is highly prevalent in patients with interstitial lung disease (ILD) and is associated with increased morbidity and mortality. Widely available noninvasive screening tools are warranted to identify patients at risk for PH, especially severe PH, that could be managed at expert centres. This review summarises current evidence on noninvasive diagnostic modalities and prediction models for the timely detection of PH in patients with ILD. It critically evaluates these approaches and discusses future perspectives in the field. A comprehensive literature search was carried out in PubMed and Scopus, identifying 39 articles that fulfilled inclusion criteria. There is currently no single noninvasive test capable of accurately detecting and diagnosing PH in ILD patients. Estimated right ventricular pressure (RVSP) on Doppler echocardiography remains the single most predictive factor of PH, with other indirect echocardiographic markers increasing its diagnostic accuracy. However, RVSP can be difficult to estimate in patients due to suboptimal views from extensive lung disease. The majority of existing composite scores, including variables obtained from chest computed tomography, pulmonary function tests and cardiopulmonary exercise tests, were derived from retrospective studies, whilst lacking validation in external cohorts. Only two available scores, one based on a stepwise echocardiographic approach and the other on functional parameters, predicted the presence of PH with sufficient accuracy and used a validation cohort. Although several methodological limitations prohibit their generalisability, their use may help physicians to detect PH earlier. Further research on the potential of artificial intelligence may guide a more tailored approach, for timely PH diagnosis.

MISA: Modality-Invariant and -Specific Representations for Multimodal Sentiment Analysis. (arXiv:2005.03545v1 [cs.CL])

Multimodal Sentiment Analysis is an active area of research that leverages multimodal signals for affective understanding of user-generated videos. The predominant approach, addressing this task, has been to develop sophisticated fusion techniques. However, the heterogeneous nature of the signals creates distributional modality gaps that pose significant challenges. In this paper, we aim to learn effective modality representations to aid the process of fusion. We propose a novel framework, MISA, which projects each modality to two distinct subspaces. The first subspace is modality invariant, where the representations across modalities learn their commonalities and reduce the modality gap. The second subspace is modality-specific, which is private to each modality and captures their characteristic features. These representations provide a holistic view of the multimodal data, which is used for fusion that leads to task predictions. Our experiments on popular sentiment analysis benchmarks, MOSI and MOSEI, demonstrate significant gains over state-of-the-art models. We also consider the task of Multimodal Humor Detection and experiment on the recently proposed UR_FUNNY dataset. Here too, our model fares better than strong baselines, establishing MISA as a useful multimodal framework.

Categorical Vector Space Semantics for Lambek Calculus with a Relevant Modality. (arXiv:2005.03074v1 [cs.CL])

We develop a categorical compositional distributional semantics for Lambek Calculus with a Relevant Modality !L*, which has a limited edition of the contraction and permutation rules. The categorical part of the semantics is a monoidal biclosed category with a coalgebra modality, very similar to the structure of a Differential Category. We instantiate this category to finite dimensional vector spaces and linear maps via "quantisation" functors and work with three concrete interpretations of the coalgebra modality. We apply the model to construct categorical and concrete semantic interpretations for the motivating example of !L*: the derivation of a phrase with a parasitic gap. The effectiveness of the concrete interpretations are evaluated via a disambiguation task, on an extension of a sentence disambiguation dataset to parasitic gap phrase one, using BERT, Word2Vec, and FastText vectors and Relational tensors.

Multimodality Imaging of Inflammation and Ventricular Remodeling in Pressure-Overload Heart Failure

Inflammation contributes to ventricular remodeling after myocardial ischemia, but its role in nonischemic heart failure is poorly understood. Local tissue inflammation is difficult to assess serially during pathogenesis. Although 18F-FDG accumulates in inflammatory leukocytes and thus may identify inflammation in the myocardial microenvironment, it remains unclear whether this imaging technique can isolate diffuse leukocytes in pressure-overload heart failure. We aimed to evaluate whether inflammation with 18F-FDG can be serially imaged in the early stages of pressure-overload–induced heart failure and to compare the time course with functional impairment assessed by cardiac MRI. Methods: C57Bl6/N mice underwent transverse aortic constriction (TAC) (n = 22), sham surgery (n = 12), or coronary ligation as an inflammation-positive control (n = 5). MRI assessed ventricular geometry and contractile function at 2 and 8 d after TAC. Immunostaining identified the extent of inflammatory leukocyte infiltration early in pressure overload. 18F-FDG PET scans were acquired at 3 and 7 d after TAC, under ketamine-xylazine anesthesia to suppress cardiomyocyte glucose uptake. Results: Pressure overload evoked rapid left ventricular dilation compared with sham (end-systolic volume, day 2: 40.6 ± 10.2 μL vs. 23.8 ± 1.7 μL, P < 0.001). Contractile function was similarly impaired (ejection fraction, day 2: 40.9% ± 9.7% vs. 59.2% ± 4.4%, P < 0.001). The severity of contractile impairment was proportional to histology-defined myocardial macrophage density on day 8 (r = –0.669, P = 0.010). PET imaging identified significantly higher left ventricular 18F-FDG accumulation in TAC mice than in sham mice on day 3 (10.5 ± 4.1 percentage injected dose [%ID]/g vs. 3.8 ± 0.9 %ID/g, P < 0.001) and on day 7 (7.8 ± 3.7 %ID/g vs. 3.0 ± 0.8 %ID/g, P = 0.006), though the efficiency of cardiomyocyte suppression was variable among TAC mice. The 18F-FDG signal correlated with ejection fraction (r = –0.75, P = 0.01) and ventricular volume (r = 0.75, P < 0.01). Western immunoblotting demonstrated a 60% elevation of myocardial glucose transporter 4 expression in the left ventricle at 8 d after TAC, indicating altered glucose metabolism. Conclusion: TAC induces rapid changes in left ventricular geometry and contractile function, with a parallel modest infiltration of inflammatory macrophages. Metabolic remodeling overshadows inflammatory leukocyte signal using 18F-FDG PET imaging. More selective inflammatory tracers are requisite to identify the diffuse local inflammation in pressure overload.

Abstract:
Understanding the role of vegetation fires in the Earth system is an important environmental problem. Although fire occurrence is influenced by natural factors, human activity related to land use and management has altered the temporal patterns of fire in several regions of the world. Hence, for a better insight into fires regimes it is of special interest to analyze where human activity has altered fire seasonality. For doing so, multimodality tests are a useful tool for determining the number of annual fire peaks. The periodicity of fires and their complex distributional features motivate the use of nonparametric circular statistics. The unsatisfactory performance of previous circular nonparametric proposals for testing multimodality justifies the introduction of a new approach, considering an adapted version of the excess mass statistic, jointly with a bootstrap calibration algorithm. A systematic application of the test on the Russia–Kazakhstan area is presented in order to determine how many fire peaks can be identified in this region. A False Discovery Rate correction, accounting for the spatial dependence of the data, is also required.

Punjabi Dalit rapper's offbeat style has made her a youth sensation

Ginni Mahi must have been in Std VIII or IX when she first became aware of the divisive nature of the caste system. A girl in her class had asked her which caste she belonged to and Mahi did not know what to say. The Mahis constitute nearly 40 per cent of the scheduled caste population of Jalandhar, her hometown. The 19-year-old grew up on stories about Dr BR Ambedkar, the founder of the Indian Constitution and the teachings of Guru Ravidas, who led the Bhakti movement in Punjab in the 15th century. Theirs is, as her father Rakesh Chandra Mahi calls it, an "Ambedkarite family". It's no surprise, therefore, that both Ambedkar and Guru Ravidas have become a mainstay in Mahi's songs, which, over the past year has catapulted her into a bestselling Punjabi rapper, even earning her the tag of a "Punjabi Dalit rapper".

The tag, however, doesn't agree with Mahi. Even though in her videos the singer wears a definite rapper swag, be it in a leather jacket or a patiala suit, she says her genre cannot be called that. "It is definitely a fusion between Western beats and Punjabi style, but I am not sure if you can call it rap," says the singer who performed in Mumbai for the first time yesterday at a congregation of women speakers. Mahi, who did her first live show when she was 12, knew from a very early age that she wanted to do "something to fight inequality in society". "Since I could sing, I had all these dreams, of doing live and reality shows. I was very little but I was already shooting for the stars," she smiles. While she has brought in a unique style to talk of the life and times of Dr Ambedkar, she was sure that it would work, even though no one had tried it before. "We have a team of lyricists who work hard to figure what should go, and more importantly, what should be dropped from a song. Addressing sensitive subjects like inequality and untouchability is always tricky territory. There has been an overwhelming amount of support, and some negative feedback too. But if there is no criticism, there can be no improvement," says the singer who performed in Mumbai for the first time yesterday at 'We The Women', a congregation of women speakers.

Even though this is her first visit to Mumbai, Mahi could barely make time to look around. "My second year finals are on, I have an exam on Monday," says the singer who is pursuing graduation in music, from HMV College in Jalandhar. "Studies are my priority, I want to do my masters, and then my dream is to do my PhD in music. A doctor heals people with his knowledge in medicine. I want to know music so well, that one day I am able to heal people with music," she says. In terms of her singing career, Bollywood, of course is the dream. "I want to be a playback singer here, much like my idols Lata Mangeshkar, Shreya Ghoshal and Sunidhi Chauhan."

While Mahi's music wants to break the shackles of inequality, that she herself has earned the tag of a Dalit rapper is an irony she just might have to live with. "My message is humanity, to erase caste politics. No one, especially artists, should be bound by class or caste. Such a tag, even though it has earned me fame, beats the purpose. But then, that is how the world works, in labels. We have entwined ourselves in all kinds of borders that separate us from each other."
